1 INTRODUCTION

ABC Corporation vision is to give the best performance in the delivery of services to our clients while maintaining a Zero Harm philosophy. We pride ourselves on a culture that fosters the value of our people and our clients through a spirit of internal cooperation. To enforce best and safest approach in everything we do.

This WHSMS covers the provisions of services and work activities identified within the scope of works. The WHSMS outlines the safety activities that must occur as directed by regulations, procedures and safe work practices designed to identify and control hazards, to ensure the health, safety and welfare of all personnel and to prevent harm to the environment. It also describes the roles and responsibilities for all personnel that contribute to the prevention of injury and the reduction of potential hazards and incidents within the workplace.

The WHSMS is designed to provide a reference guide to the safety systems applicable to ABC Corporation employees and sub-contractors working on all external commercial and industrial client sites.

Our WHSMS is enforced and culture driven to be a comprehensive Health, Safety and Environment Management System that detail requirements and standards that must be met on a daily basis. The WHSMS will provide the guidance to ensure that these requirements will continue to be satisfied during the works period and monitored/audited for compliance. The WHSMS shall be adopted and fully implemented by all ABC Corporation Personnel. Implementation of the WHSMS and associated documentation shall be verified by periodic audits performed by the PCBU and CEO of ABC Corporation. An updated WHSMS will be prepared for each contracted works project and hard copies will be available to all ABC Corporation employees and contractors working on site. Hard copies will be considered “Uncontrolled Copies” and will be available through supervisors, project manager and project office.

1.3 HSE Management PlanABC Corporation is dedicated to delivering and achieving Zero Harm while respecting the community and environment. To achieve our goals ABC Corporation has developed a HSE Management System that is underpinned by a rigorous Hazard Identification and Risk Assessment Process.

The management system revolves around the four steps of Planning, Implementation, Review and Improvement of our systems.

1.3.1 PlanningPrior to undertaking any work, ABC Corporation will conduct a Risk Assessment to identify contract level risks such as safety, statutory, regulatory, emergency and community risks.

Further planning is put in place through development of the Project Risk Analysis that defines specific work activities, and identifies the associated HSE hazards and risks. The purpose of this process is to develop a management system that is focussed on site-specific risks.

ABC Corporation have in place Operational Procedures developed to ensure that hazards identified during the Risk Assessment process are appropriately managed. These Operational Procedures shall be communicated to the workforce during the Induction process prior to the commencement of any work.

1.3.2 ImplementationThe effective implementation of the systems and procedures identified during the planning phase is the key to achieving No Injuries to Anyone, Anytime. Communication and Consultation is seen as a critical process for the implementation of ABC Corporation HSE Management System.

Training is one of the primary methods for communicating the requirements of the HSE Management System to the workforce. Risk Analysis is also used to assist in the development of induction programs and the identification of training needs.

This structured training program is further supported throughout the project by toolbox talks that help maintain awareness of HSE procedures and issues. The toolbox meetings, in conjunction with HSE Committees, are also used to obtain feedback and suggestions from the workforce.

1.3.3 ReviewThe next step in the HSE management process is to review the effectiveness of the planning and implementation of the system by checking how well it is working in practice. ABC Corporation uses a range of tools and metrics to do this, including regular formal workplace Job Observations (normally carried out by direct supervision), audits, incident investigations and monitoring processes.

1.3.4 ImprovementThe final step in the HSE Management System process is to collate and interpret the feedback generated from the review processes and use it to develop objectives and actions to ensure that the HSE Management System and performance is continually improving. Learning from our activities is seen as critical in shaping the organisational culture.

1.4 HSE OutcomesABC Corporation recognise that we cannot rely on management systems alone to deliver a safe workplace. In particular, there is a risk that systems may be seen as replacing the human element in workplace safety, rather than complementing and supporting it. To counter this risk, we continue to develop and fine tune initiatives designed to strengthen and reinforce HSE leadership at all levels of management and safe work behaviours among all employees.

2 PLANNING

2.1 Responsibilities and AccountabilitiesResponsibilities and accountabilities for all personnel are recorded in Position Descriptions and Procedures that are located within the ABC Corporation Health & Safety Management system. Each person involved in the team has equal responsibility to strive to achieve:• No injuries to anyone, anytime• Respect for the community and environment

A summary of key individual responsibilities are provided below:

2.1.1 Company Director The Company Director is responsible for the planning, preparation and overall coordination of the

execution of the works. The Company Director has an obligation towards maintaining safe work practices and a safe and

healthy work environment for themselves as well as others in the workplace. The Company Director shall foster a culture that requires and encourages employees to report

hazards and incidents. The Project Manager is to lead safety and improvement initiatives by personal example and

actively participate in the development and application of Company Safety, Environment and Quality initiatives.

The Company Director is responsible for ensuring the requirements of the HSE Management Plan are communicated and complied with.

2.1.2 Site ManagerSupervisory Staff are to: Provide Supervisory support and direction to employees and personnel. Communicate Quality, Safety and Job requirements to the employeesSupervisory Staff are accountable to the Company Director for the safe conduct of all work activities under their control.

Supervisor must: Fully understand the requirements of the relevant safety legislation, safety management

plan/project risk analysis, in particular, their duties and responsibilities. On all works carry out in conjunction with employees, documented Job Hazard Analysis (SWMS)

and Work Permits and explain those to their workforce prior to starting, ensuring they are fully completed and complied with.

Instruct all their employees (including those on hourly hire from subcontractors) in the safety standards that affect their work and the safe work methods that are to be adhered to.

Ensure that accurate records are taken of all safety concerns and forwarded to the Company Director Ensure that all employees in the workforce (including those of subcontractors) comply with the projects safety requirements and take immediate action with those who do not.

Encourage employees to report safe acts, hazards, unsafe work practices, near misses and all injuries, no matter how insignificant they may seem.

Participate in regular safety audits / inspections. Report all injury and accidents and dangerous occurrences immediately and investigate and

formally report within 24 hours to the Company Director. Run daily pre-start meetings and conduct daily job observations. Ensure workers with less experience are interspersed throughout the work group and that all

workers are encouraged to stop and ask questions if unsure of anything at all.

2.1.3 EmployeesEmployees are to:

Provide support to all working activities in collaboration with ABC Corporation in a safe, effective and efficient manner.

Follow all reasonable instructions and work in compliance with all ABC Corporation Quality, Safety and Environmental plans and procedures.

Report all safe acts, hazards, near misses, and incidents immediately to an appointed supervisor and participate in investigations where required. Abide by the ABC Corporation Policy and present for duties fit for work.

Ensure all equipment is tagged correctly, fit for purpose and in good working condition. Take responsibility for the safe working practices of others and ensure all personnel work in a

safe manner. Actively participate in Pre-start Meetings, Job Observations and other safety processes where

required. Only operate equipment they have been fully trained, certified and deemed competent in which

to operate. Quarantine defective tools and equipment that require repairs and advise the appropriate

persons who will arrange repairs or replacement. Stop and ask questions if unsure of anything at all.

2.2 Risk ManagementABC Corporation shall implement a risk management process that ensures the systematic identification, assessment, treatment and ongoing monitoring of risk throughout the life of the contract. ABC Corporation shall control and manage identified hazards and risk through implementation the HSE Hazards Procedure and Risk Analysis.

The general nature and content of the Risk Analysis shall be communicated to new personnel at induction. Refresher information on the general nature and content of the Risk Analysis shall be communicated via Toolbox Meetings, noticeboards and other appropriate forum.

2.3 Legal RequirementsABC Corporation maintains systems for identifying, accessing, communicating and maintaining up-to-date legal and other requirements as applicable to the effective functioning and compliancy.ABC Corporation shall, throughout the duration of operations, ensure compliance with the following appropriate Queensland Government Regulatory requirements:

• Work Health Safety Act 2011• Work Health Safety Regulation 2011• Environmental Protection Act 1994• Environmental Protection Regulation 1998

2.4 Training & AuthorisationsAll personnel commencing employment with ABC Corporation will be competent in all areas required as depicted from the company director in accordance with Aust. Training regulations as per Aust. Government standards.

The familiarisation and orientation will cover: • Site safety objectives and requirements• Fitness for Work• Responsibility to report injuries, incidents and potential hazards• Risk assessments, job procedures• Equipment, tools and vehicles• Site Environmental Requirements

2.5 Induction RecordsOnce personnel have completed induction requirements, copies of the documents issued upon completion (including a scanned image of issued documentation, expiry dates and personnel details) will be held on the employees personnel file.

A record of attendance at internal and external training (as well as any training certification issued) will be documented and retained and kept as a ready reference for all employees.

2.6 Competence, Awareness & TrainingNo person is to work on the project unless the Company Director is satisfied that the person is adequately trained for the work to be performed and relevant authorisation is provided. All employees are to be provided with such information, instruction, training and supervision to enable them to perform their work in a manner that is safe and without risks to their health and safety.

No employee or sub contract employee will be permitted to perform a designated task unless that person holds the required competency and those procedures are designed to support this objective.

ABC Corporation shall ensure site personnel are competent on the basis of applicable education, training, skills and experience in relation to the assigned work activities, responsibilities and authorities as defined in the relevant position descriptions.

3 IMPLEMENTATION

3.1 Operational ControlsOperational controls are based on the effective application of system requirements, along with task related risk control mechanisms such as the Job Hazard Analysis process. The operational controls listed below include Take 5 checklists and Job Hazard Analysis, which form part of the control measures for almost all hazards.

3.1.1 Take 5Take 5 is the primary personal risk assessment method for ABC Corporation work on sites. Where off-lease or other work is required the ABC Corporation Take 5 booklet may be used.

ABC Corporation is a personal risk assessment that must be completed and recorded prior to the commencement of any task. The Take 5 process asks all personnel to stand back 5 paces and take 5 minutes to identify all hazards.

All ABC Corporation employees and contractors must be trained in the Take 5 process.

There are 5 steps involved in completing a Take 5 checklist. They are as follows:

3.1.2 Safe Work Method Statement (SWMS)The requirement for a SWMS will typically arise when a task or activity meets some or all of the following broad criteria: Cannot be controlled using a Take 5. A history of significant injuries, incidents, or near misses. Significant or unusual hazards are present. A high degree of task complexity. A variable, unpredictable or potentially hazardous working environment such as confined space

or work at heights. The potential to impact others eg. Other workgroups, neighbours. Work performed on critical equipment eg. Fire systems, safety relief valves.

Existing SWMS may be accessed via the Job Hazard Analysis Register. All aspects of an existing SWMS must be applicable to the proposed task for it to be used in relation to that task. The work crew are required to review existing SWMS’s prior to and during the job to ensure appropriate measures are adopted.

New or revised Job Hazard Analyses shall be returned to the Supervisor following review and subsequent comments by the work team. Once agreement is received on any changes that may be required the SWMS shall be recorded in the register for future use.

Where work tasks are identified as being infrequent or a ‘one – off’ task and the hazard associated with the work task has a low frequency / likelihood of occurring but has a high consequence then a risk assessment shall be conducted. The Risk Assessment shall employ the principle of ALARP whereby the existing mitigating controls (if any) shall be risk ranked and any further identified mitigating controls also being subject to an additional risk ranking. Any residual risks shall be carefully monitored.

3.1.3 Confined SpacesFor the purposes of this plan the definition of a confined space shall be in accordance with AS2865.

All persons required to enter a confined space will have successfully completed nationally recognised training in “Safe Work in a Confined Space”. No work is to be carried out in a confined space unless authorised by a Responsible Person, unless, a Confined Space / Hot Work Permit has been issued for the work. It is mandatory that all Confined Space identified a comprehensive SWMS and have an ‘Emergency action plan’ attached to the permit.

Personnel working in a confined space shall require an observer located outside of the space and whose job is to monitor the work being undertaken.

3.1.4 Hot WorkHot Work is defined as two types, Hot Work and Hazardous Hot Work. Hot work is work conducted in a non-hazardous environment such as a purpose built workshop or open area free of combustibles. Hot work conducted in this type of environment requires no special permitting or specialist equipment; however, Take 5 and SWMS do apply.

Hazardous Hot Work is hot work conducted inside confined spaces, on or near storage tanks, on or near combustible materials or on or near structures and plant. This type of work does require a permit as well as all standard risk assessments.

3.1.5 Work at HeightsABC Corporation has in place specific systems and specialised equipment for managing hazards when working at height. Risk control measures shall be implemented wherever there is potential for any person to fall 1.8 metres or more, or to gain access to within 2 metres of an open edge from where there is the potential to fall 1.8 metres or more, including working from elevated work platforms, cages, ladders, scaffolding and where objects could fall and cause injuries. Any hole or opening of 300mm diameter or greater must be covered or have hard barricade erected. A documented risk assessment / Job Hazard Analysis shall be conducted before the commencement of work and at any time the scope of work changes or the risk of a fall increases. Any opening that has the potential to cause harm must be managed by risk assessment and the implementation of necessary controls.

EquipmentAll scaffolds, elevated work platforms and suspended work cages shall conform to relevant approved design standards. People in the work platform basket shall wear a correctly fitted harness attached by a lanyard to a suitable anchor point in the basket. This does not apply to people working from a properly constructed and certified scaffold with the requisite handrails and toe boards.

Work PermitsDue to the risks involved with working at heights a Working at Heights Permit will be completed prior to work commencing. No work is to be carried out at heights unless it is in accordance with the Working at Heights procedure.

3.1.6 First Aid / Electrical First AidAdequate first aid supplies and equipment, which complies with the relevant Australian Standards, shall be made available. First aid supplies are to be in accordance with the Workplace Health and Safety First Aid Code of Practice and the site risk assessment.Sites shall have appropriate signage and appropriately marked location for first aid supplies and equipment. All sites shall have access to emergency services. Records shall be kept on all first aid injuries.

Registers & MSDS’sABC Corporation register shall be in place for all hazardous materials. MSDS’s are available to all personnel (including first aiders and medical personnel) involved in the transportation, storage, handling, use, decanting and disposal of hazardous materials on site.

A register including all MSDS will be nominally sited at the following location/s during a works• All work vehicles

TrainingThe company director shall ensure up-to-date information and training material is made available to ensure compliance by all persons required to transport, store, handle, use, decant or dispose of hazardous substances. Familiarisation with this procedure shall be included in the induction of new starters and refresher training programs.

3.1.7 Heat StressHeat stress can be a factor in activities within ABC Corporation. Workload and environmental conditions such as ambient temperatures may have an effect on an employee. This may be applicable in instances where the need for personal protection can restrict the body’s ability to dissipate heat.

In order to effectively manage exposure to hot and cold conditions, the following elements shall be taken into consideration:• Identify work areas/tasks that expose workers to undue hot or cold conditions.• Include work rotation that limit the level of time to exposure or provide a shade barrier over the

work area.• Implement into place a monitoring process to ensure the effectiveness of such controls.• Educate the workforce of the effects of extremes of heat and cold.

UV RadiationAll work areas and tasks which involve UV radiation exposure shall be assessed to determine the level of risk presented to workers. With reference to the ‘Hierarchy of Controls’, control measures shall be implemented to eliminate or minimise risk wherever practicable.

3.1.8 Manual HandlingABC Corporation recognises the risks associated with manual handling and through the risk management program shall implement appropriate control where practicable. Manual handling shall be managed through the use of mechanical lifting equipment such as cranes and forklifts.

TrainingTraining and information shall be provided to workers to address safe manual handling associated with the type of work being undertaken.

3.1.9 NoiseABC Corporation recognises the risks involved with noise. Risk Analysis shall identify tasks and processes likely to produce a noise hazard and identify appropriate control measures. In addition to this workers shall undertake a Job Hazard Analyses and Tae 5 for all tasks; workers are expected to mitigate noise hazards as part of the risk management process.

3.1.10 Emergency PreparednessAn emergency plan has been developed to ensure that a state of readiness is maintained in order to effectively deal with foreseeable emergencies and to minimise the impact on health, safety or environment.The procedure will also be communicated to workers via induction and toolbox meetings.

3.1.11 ElectricityAll electrical work shall be conducted by appropriately trained and qualified personnel. The design, installation and maintenance of electrical work shall be conducted in accordance with applicable standard operating procedures.

3.1.12 Hand Tools – Electrical, Pneumatic & Hydraulic PressureAll electrical powered hand tools shall be inspected and tagged. Powered tools should be inspected prior to use to ensure test tag is current and tool is free of obvious faults.

An inspection of air equipment shall be conducted to ensure the equipment is safe for use All air lines must have safety pins or chains fitted prior to operation of the equipment to reduce the potential of “whiplash” should the equipment and airline disconnect.

An inspection of hydraulic equipment shall be conducted to ensure the equipment is safe for use. Items to take into consideration include, but not limited to, the condition and cleanliness of the fittings, body condition, hydraulic hose condition, pump condition.

If faults are identified the equipment shall not be operated, an Out Of Service Tag shall be attached and the tool shall be sent for repair.

3.1.13 Access & EgressThe means of access shall comply with the requirements of AS1657 - Fixed platforms, walkways, stairways and ladders, design, construction and installation (S.A.A. Code for fixed platforms, walkways, stairways and ladders).

The means of access shall be maintained in a clear condition, free from accumulation of debris, equipment and other matter so that access is readily available at all times.

3.1.14 Fitness for WorkABC Corporation is committed to the health and safety of all its employees and contractors. This includes ensuring that no person commences or continues to work if it is known that they are not fit for work.

We recognise that health and a range of factors, including fatigue, psychological stress, illness and the use of medication, illicit drugs and alcohol, may influence an individual’s safety performance.

FatigueTo ensure that fatigue is appropriately managed ABC Corporation employees, supervisors and managers will be informed of the ‘elements contributing to fatigue’ so as to manage fatigue related issues.

3.1.15 Personal Protective Equipment (PPE)ABC Corporation shall provide an adequate supply of PPE that shall comply with relevant standards. All sub-contractors shall supply adequate and applicable PPE to their employees and ensure that the PPE is utilised. Standard PPEAll personnel shall wear the standard PPE, which includes high visibility long sleeve shirts, long trousers, helmet, safety glasses and safety footwear in all areas of the site except for those items exempt by the Risk Assessment. Gloves shall be carried at all times and worn when performing a task, except where they are identified as a hazard on the Tae 5 / Job Hazard Analysis.

3.1.16 Subcontractor ManagementSubcontractor AssessmentsPrior to appointing any subcontractor, ABC Corporation will conduct a review of the subcontractors Health & Safety Management Systems. Where a subcontractor’s management system has been assessed and approved by ABC Corporation, that subcontractor will be permitted to commence work using their own management system or the subcontractor may elect to work directly under the ABC Corporation WHSMS.

GeneralABC Corporation acknowledges their responsibility to ensure that all subcontractors understand and comply, as a minimum to the safety management plan. To this end ABC Corporation will ensure that

all subcontractors are briefed on the requirements of the ABC Corporation Safety Management system, prior to commencing work.

3.2 HSE Communication & ConsultationABC Corporation shall ensure that there is regular and effective communication and consultation between management and employees on HSE issues.

3.2.1 HSE CommitteeABC Corporation shall establish and maintain an appropriately convened HSE Committee. The HSE Committee will meet on a 6 monthly basis, and additional meetings may be convened if the need arises due to special circumstances.

The HSE Committee is attended by both site management and worker representatives with representatives from other sub-contractors contributing to the meeting process.

As a minimum the following agenda Items shall be reviewed and actioned as part of the HSE Committee meetings. Actions required and completed shall be recorded in meeting minutes: HSE related communications (internally through Incident Alerts or other), Review of HSE related initiatives (e.g. inspections, Job Observations, audits, proposed

new/revised procedures), Incidents (incl. reports, investigations, recommendations, corrective actions, etc). Minutes of the meeting including agreed outcomes shall be posted within 24 hours of the

completion of the meeting.

3.2.2 Pre-Start Meetings Pre Start Meetings, also known as Toolbox Meetings, are to be run by supervisors at the start of each shift and are designed to convey and exchange important information prior to the start of shift. The topics discussed are to include reviews of recent incident reports, discussion of any incident or safety alerts from within ABC Corporation or from external sources, hazards identified, refresher or awareness training on specific OHS procedure matters, and so on. Every member of the group should be given the opportunity to express concerns and make recommendations.

The Pre-Start Meetings can also be a forum in which the tasks for the shift are assigned, risk assessments reviewed, safe work procedures, other contractor activities, proposed work areas and other general business matters can also be discussed.

4.1 Workplace Inspection and AuditsSupervisors in each area will be responsible for all inspections being conducted in their areas.. Supervisors shall include Workers in the audit and encourage active participation in the audit process. All completed inspections and associated corrective actions shall be filed and maintained by ABC Corporation.

4.2 Incident ManagementIt is a requirement of ABC Corporation that all incidents are reported, recorded, investigated and all resulting corrective actions implemented.

All incidents shall be recorded and managed. The respective supervisor shall be responsible for ensuring all data is reported and a summary is to be sent to the Company director

4.3 Safe Acts, Hazards, Near Miss ReportingThe Safe Acts, Hazards, Near Miss Reporting Initiative uses a workforce report book and regular workforce updates to promote a proactive safety culture. All employees are encouraged to report any such issues. The reports are collated and feedback to the workforce is via a regular report sheet that also includes photos of personnel performing work in a safe manner, providing recognition of safe acts, thereby promoting discussion and further interest in positive safety culture.

4.5 Conclusion

This WHSMS overview document has detailed the intent in which ABC Corporation intends to implement and monitor the health and safety of its employees.
