HOSPICE WHAT IS HOSPICE?

Hospice is considered to be the model for quality, compassionate care for people facing a life-limiting illness or injury. Hospice care involves a team-oriented approach to expert medical care, pain management, and emotional and spiritual support expressly tailored to the patient’s needs and wishes.

The emphasis is on comfort and quality of life, rather than aggressive treatment and finding a cure. Hospice care enables patients to live each day to the fullest as comfortably and dignified as possible.

At Memorial Healthcare, both the patient and family are included in the plan of care, with special attention given to emotional, spiritual and practical support as expressed by the patient or family.

HOSPICE SERVICE

Memorial Healthcare Hospice provides care for the mind, body and spirit. Our team approach includes the patient, their loved ones and other health care professionals. Our services include:

• Comforting care provided by an interdisciplinary team

• Pain and symptom management

• Grief and spiritual support

• Medications related to the terminal diagnosis or prognosis

• Durable medical equipment and supplies

• Patient and family education on end of life care

• Therapies to improve quality

OUR HOSPICE TEAM

At Memorial Healthcare Hospice, we provide care wherever our patients call home. This could be in the home they've lived in for years, a skilled nursing center, an assisted or independent living facility or a hospice house.

Memorial Healthcare Hospice provides hospice care through an interdisciplinary, medically directed team approach. The Hospice team develops a plan of care that meets each of our patient's individual needs for pain management and symptom control.

The team consists of:

• The patient’s chosen personal physician

• Hospice Physician (Medical Director)

• Nurses

• Home Health Aides

• Social Workers

• Clergy or Counselors

• Trained Volunteers

• Therapists

Physical

Occupational

Speech

• Grief Support

Throughout the care process, Memorial Healthcare Hospice nurses continue to keep the patient's chosen physician informed of their health care needs.
