Dear Sir/Madam

At Leeds Teaching Hospitals Trust our aim is to provide the highest quality and safest care for our patients at all times throughout their stay with us.

Your medical team have decided that you no longer need to be in hospital but a period of recuperation and/or further assessment is required for you. We are working in partnership with Villa Care Ltd to provide this service.

You will shortly be transferred to a Villa Care Ward which will provide the supportive nursing environment you need during your recuperation. During your stay there you will continue to be assessed to determine the type of care needs you require, either to prepare you for your discharge home or for your transfer to another care setting.

This patient leaflet will give you more information about the units and services provided.

We would like to welcome you to our Villa Care Ward. It is a care environment where you will be looked after until your home, care home or other service in the community, is ready for you to be transferred to.

The TeamWe are all here to help you so don’t hesitate to ask for anything you need.

Uniforms

The Unit Matron wears navy tunic with a red trim.

Nurses wear navy and lilac, carers lilac with a navy trim. More information is available on the wards.

Keeping WellOur nurses and care staff will work with you to enable you to leave hospital. Access to a doctor is available should you need it however, you may not see a team of doctors on a daily basis as you may have experienced on your previous ward.

A consultant in Elderly Medicine will make a weekly visit.

Physiotherapists and Occupational Therapists may be available to provide some therapy whilst you await your placement or service in the community.

Other members of the care team may also help to provide this.

If you require support in the community or a more long term plan, a social worker may be allocated to you and will work with you to arrange your discharge.

Your Plan of CareWhen completing your care plan with you, as well as discussing what is important for you now, we will also consider your future requirements for your healthcare, particularly if you are unwell. Although all staff are fully trained and experienced, it is important to remember that the Villa Care wards are here to care for patients who are medically ready to leave hospital. Although we do have access to doctors, should you become unwell you may need to be transferred back into main site for further care. Although we aim to transfer unwell patients back into a ward bed, on occasion, this may mean transferring via ambulance to the A&E department.

Meal Times

We do provide meals, drinks and snacks at various times throughout the day. If you require any assistance with your meals please let one of the team know.

We do have facilities to enable you to have your meal in a dining room. You will be encouraged to use this if you feel able. Please ask the staff for more details and/or assistance.

Times are below, snacks and drinks are always available:

• Breakfast at your convenience in the morning

• Morning coffee at 11am

• Lunch midday

• Afternoon tea at 3pm

• Evening meal at 5pm

• Supper at your convenience in the evening

What to bring with youIt would be useful if your family could supply some night and day clothes if you wish to change during the day however, please note there is no laundry service available and therefore all laundry needs to be taken home or given to your family

Keeping in touch and keeping busyThere are large day rooms available on all of the Villa Care wards which can be used day and night. All have televisions and a selection of books, jigsaws etc. please feel free to use them. If you would like to eat in the day room, please inform one of the care staff and your meals can be brought to you. Activities are planned at various times on the wards and may happen during your stay. A hairdresser also visits the wards. For more information please ask one of the care team.Please note: Although your stay is funded by the NHS, you will be responsible for personal expenses such as hairdressing.

VisitingVisiting hours are 11:00 am until 19:00 pm. However, we would ask that you bear in mind others when thinking about the numbers of people visiting, the times of visits and supervising young children.

Please ask your visitors not to come if they are unwell with coughs, colds or have symptoms of vomiting and diarrhoea, as these are all easily spread.

Thinking AheadPreparing and planning for your discharge is an essential part of your recuperation and there is an expectation that you and/or your family or representative will participate in your assessments and any necessary care planning meetings along with our hospital team.

Working Together We believe in working in partnership. If you have any concerns at all please raise them at the earliest opportunity with the nurse in charge. If you do not feel your concerns have been addressed sufficiently please contact the matron or speak to the Patient Advice and Liaison Service (PALS) Tel: (0113) 206 6261 - Available during normal working hours only (9:00am to 4:30pm Monday to Friday).

My uncle spent two months on Bilberry ward until residential care was obtained. The staff are caring, kind, considerate and patient. My uncle was well cared for with dignity and respect.

Comments from our patients

The staff have all been so excellent and very kind and caring towards my husband. The surroundings are so peaceful which help the patients to feel well. The staff are compassionate and are always there when needed. Thank you so much.

The staff, all of them, from the nurses and the staff serving the meals and assistants to the cleaners. All top staff. 5 star treatment. All great staff very caring.

I have enjoyed the care and happiness there is here. The meals and service from all the staff has been first class. Thank you very much.

If you would like to comment on your experience, either to give praise or to suggest an improvement, then please complete a comment card which are available from the nurses station.

The staff have been extremely kind, patient and nothing has been too difficult for them in meeting my care and emotional needs. The ward provided a Caribbean diet in keeping with my cultural needs which made my unexpected stay much smoother to cope with. The staff couldn’t do enough for me.
