Welcome to the Practice We offer a full family doctor practice service and have specialist staff in chronic disease management. We also offer an in house Pharmacy at both sites. We aim to treat all our patients courteously and in confidence. We feel it is important you know who you are speaking to, so our Practice staff wear a name badge and identify themselves on the telephone. The Practice undertakes training of medical students and doctors they spend 4-12 months with the practice gaining experience and training. As we mentor medical students, your medical records may be used for educational purposes with your consent. As a consequence we ask patients registering with us to sign a consent form giving permission for their records to be used in this way. You may also occasionally be asked if you would allow a medical student to be present during your consultation with your GP or you may be asked for your consent in taking a video of your consultation in order to assist them with training. You may refuse without affecting your care at the practice.

Your Opening Times The surgery is open from 8:00am to 7:00pm Monday to Friday

How to make a routine appointment If you require an urgent or routine appointment, please telephone the appointments line on 01253 953500 option 1. If you have complex or multiple issues to discuss, please ask for a longer appointment (for example a double appointment).

Your Practice offers same day urgent appointments, routine appointments which can be pre-booked up to four weeks in advance.

You can also book appointments over the internet once you have registered with EMIS Access. If you are interested in using EMIS Access, please ask our receptionist for further details.

If you are more than 5 minutes late for your appointment please report to reception who will check with the clinician to see if you can still be seen for your appointment.

How to request a home visit and/or a telephone consultation If you are unable to attend the surgery and are requesting a home visit you need to contact the Practice before 11 am on:

When you make a request for the Doctor to visit you at home, the receptionist will ask you your details and a contact telephone number. Your details will be given to the Doctor who will ring you back to speak to you about your request for a home visit and assess your problem. In some cases the doctor may be able to deal with your request by telephone consultation or you may be asked to attend the surgery. If you feel that your problem could be dealt with by the Doctor or Nurse Practitioner by telephone, rather than making an appointment, ask the receptionist for a ‘telephone consultation’. The Doctor or Nurse Practitioner will contact you as soon as practical.

Repeat Prescriptions Repeat prescriptions can be ordered online, at the surgery or by post. Prescription will be ready for collection after 3pm two working days after the order is placed. To order your prescription online use ‘EMIS Access’ (see next page). Alternatively you can order your prescription using the printed order slip provided with your last prescription, tick the items you require and post your request in the prescription box situated next to the entrance at Moor Park Health and Leisure and in the entrance at Whitegate Drive or you may hand your request to a member of staff. When ordering by post, please allow an additional two working days for first class carriage via the postal service. If you enclose a stamped addressed envelope, your prescription will be returned to you, again please allow an additional two working days for return.

When the Practice is closed call the Urgent Care Centre If it is not a 999 emergency but you need medical help fast, please call 111. When the surgery is closed, please telephone 111. You will be assessed by a team of highly trained advisors who are supported by experienced nurses. They will give you health care advice, book you an appointment if needed or transfer you to the people you need to speak to. NHS Direct ‘Mobile App’ You can now access medical advice wherever you are from the NHS Direct Mobile App.

Your Services offered within the Practice Minor Illness Clinic The Practice has a Nurse Practitioner, two Clinical Pharmacists and a Specialist Nurse. Their role is to compliment the role of the GP’s. They have advanced skills to take a health history, examine, diagnose and treat patients. The Nurse Practitioner can see patients in the minor illness clinic for care including:- acne, asthma, acute back pain, bites, boils, non-acute breathing difficulties, colds/flu, emergency contraception, menstrual problems, vomiting, sunburn, chicken pox, conjunctivitis, diabetes, earache/discharge, hay-fever, oedema/swollen ankles. The Nurse Practitioner routinely sees patients for:

Cervical cytology

Pill Checks/coil fitting/ring pessaries

Implanon fitting and removals

Zoladex injections

Cryotherapy

Sexual Health

All our GP’s, Nurse Practitioner, Nurse and Specialist Nurse are able to provide advice on all methods of contraception including pills, implanon (contraceptive implant), injections, coils, caps and condoms. The morning after pill must be taken within 72 hours of unprotected intercourse or a coil (IUCD) fitted within 5 days. If you require emergency contraception, please contact the surgery where you will be directed to a participating pharmacy or alternatively you can attend the Walk in Centre.

Minor Surgery

The practice undertakes minor surgical procedures which include joint and soft tissue (muscle and tendon) injections, removal of skin tags, and liquid nitrogen (freezing) treatment of warts.

Vaccinations and Travel Advice

The Practice Nurses are available by appointment to give advice on routine immunisation and travel vaccinations. Please allow at least 8 weeks before your impending travel to ensure that you are fully vaccinated.

Cervical Cancer Smear Tests

There is a “call and recall” system to remind you about your “smear test” which are carried out by appointment with the Practice Nurse. Women aged 25-49 years will be recalled every 3 years for routine screening or as directed by the laboratory analysing the tests. Women aged between 50-64 years will be recalled every 5 years.

Smoking Cessation

The Practice offers smoking cessation advice with our Health Care Assistants and Pharmacists. If you would like to stop smoking, please make an appointment for smoking cessation advice.

Flu and Pneumococcal Vaccination

In accordance with Department Of Health Guideline we recommend an annual flu vaccination for patients aged 65 years and over and patients with chronic heart, lung or kidney disease, diabetes and residents of nursing and

residential homes. Annual vaccinations start early October. All patients who have chronic heart, lung, liver, renal (kidney) disease, on immune suppressing therapy (steroids and chemotherapy), diabetes, sickle cell disease sufferers, HIV positive patients, organ (inc bone marrow) transplant recipients and those have no spleen are recommended to receive vaccination to protect against infection with the “pneumococcus”, the commonest bacteria involved in pneumonia and other respiratory infections. All patients over 65 years of age are strongly advised to have pneumococcal vaccinations

Diabetic Reviews

The Practice has specialist Practitioners in Diabetes, and Practice Nurses who are able to offer advice and education on the management of your diabetes. They will assess your diabetes and assess any problems in your glycaemic control and address them to improve it. The Specialist Practitioner and Practice Nurse are there to provide support and detect any complications which may affect your overall health.

Coronary Heart Disease and Stroke Reviews The Practice has specialist Practitioners and Practice Nurses who are able to offer advice and education on the management of your condition. The clinic has been designed to review your medication and lifestyle patterns. The overall aim of the clinic is to provide optimum advice, support and treatment for our patients who have coronary heart disease or had a stroke

Asthma/COPD reviews The Practice has staff who are qualified in managing patients with asthma and COPD. They will assess and manage your respiratory condition to improve your overall

health and well-being.

Community Nurses (‘District Nurses’)

The Practice has a Community Nursing team attached to the

Practice. The Community Nurses play a crucial role within the

Primary Health Care Team. They attend patients in their own

homes or in residential care homes providing care and

supporting family members.

Antenatal Care

The practice have attached midwifes whose role is to provide antenatal care. The clinics are held on at Tuesday afternoon at the Whitegate Drive site and Friday afternoon at the Moor Park site. You will need to book an appointment to attend the clinic. Child Health Surveillance and Childhood Immunisations

With effect from 5th September 2012 your practice will undertake routine child health checks and the childhood immunisation programme. These are performed by the GP’s and Practice Nurses on a Thursday afternoon at Moor Park and a Wednesday afternoon at Whitegate Drive.

Health Visitors

The Practice has attached Health Visitors whose role involves promoting health in the whole community. The Health Visitors are particularly involved with families who have children under five and the elderly population.

A Health Visitors role is a varied one and is an integral part of the NHS Community Health Service. The main focus of their work is prevention, helping people to stay healthy and avoid

illness.

The Health Visitor is a Qualified Registered Nurse who can offer help and advice to parents on the following:

Child’s growth & development

Common infections in childhood

Common skin problems

Behaviour difficulties, sleeping, eating, potty training, temper tantrums and teething

Breastfeeding, weaning, healthy eating, hygiene, safety and exercise

Postnatal depression, bereavement and violence in the family They are also involved in:

Working in partnership with families to tailor health plans to their needs

Health promotion groups, breast feeding support groups, parent support groups, parenting courses

Change of Personal Details

Documentary Proof

We will require proof of name or address changes so please bring this with you on your next visit to the practice

A change of address may mean that you are outside the Practice area, so do not assume that we are able to continue to care for you. The receptionist can advise you.

Access to Health Records

The Data Protection Act 1998 now allows access to medical records of living patients. All applications for access to records whether paper based or electronic are now made under the

Data Protection Act 1998. To have access to your medical records, you must apply in writing to the Practice Secretary who on receipt of your request will respond to you within 21 days. Please note there will be a charge of £10.00 for copies of electronic health records and for copies of manual and electronic records a charge of £50.00 is applied. We will process applications for access received from patients, the parent/guardian of a child, a personal representative of a deceased patient or a person appointed to look after the affairs of a patient incapable of doing so personally.

Freedom of information- Publication Scheme

The Freedom of Information Act 2000 obliges the practice to produce a Publication Scheme. A publication scheme is a guide to the ‘classes’ of information the practice intends to routinely make available.

Medical Education and Training at Your Practice This Practice is a ‘training practice’. All training practices must be accredited in order to ensure that the highest standards are maintained in the training of junior doctors and students. This accreditation is carried out by the North Western Deanery and, on occasion, by members of the Joint Committee of Postgraduate Education for General Practice (JCPTGP). The JCPTGP sets the standards for all GP training posts in the UK and carries out a programme of accreditation visits to all areas in the UK, to ensure that its standards are being met.

During an accreditation visit, samples of patient medical records are inspected. Doctors inspecting the records are bound by the strictest rules of confidentiality, and the inspection is limited to a verification process to ensure the practice is compliant to the required standards. Any patient may object to the whole, or any part, of the inspection of their own records and their objection will be respected.

Complaints/Suggestions/Compliments Glenroyd Medical aims to give a friendly and professional service to all our patients. If you have had a problem with the service you have received in the Practice, please let us know. The NHS Complaints Regulations were amended in April 2009 and our current Practice complaints policy and procedures are in line with contract regulations. We hope that most problems can resolved at the time they arise and with the person concerned. If your problem cannot be resolved to your satisfaction and you wish to make a formal complaint, this should be within the following timescales:

Within 12 months of the incident that caused the problem or

within 12 months of identifying an incident giving cause for complaint

All complaints are dealt with by Mrs Sharon Orr, Practice Manager. We shall acknowledge your complaint either verbally or by letter. We endeavour to resolve complaints as soon as is practically possible.

Zero Tolerance

The Practice believes that its staff, patients and others have a right not to be subjected to any kind of harassment or abuse. In order to protect and support staff, patients or visitors from any unacceptable behaviour, the Practice reserves the right to take appropriate action. This can include:

Asking patients or relatives or others to modify their Behaviour

Requiring relatives or others to leave the premises if their behaviour compromises staff ability to provide an appropriate level of care to the patients in their charge

Reporting acts of harassment or abuse to the police

Removal from Practice register

Fees

The practice provides additional services that are not covered by the NHS and you may be asked to pay a fee for e.g. insurance medicals, private certificates, and holiday cancellation forms. Please ask at reception for further details.

How information about you helps us to provide better care Health & Social Care Information Centre Benefits of sharing information

Sharing information can help improve understanding, locally and nationally, of the most important health needs and the quality of the treatment and care provided by local health services. It may help researchers by supporting studies that identify patterns in diseases, responses to different treatments and potential solutions. Information will also help to:

What will we do with the information? We will only use the minimum amount of information we need to help us improve patient care and the services we provide. We have developed a thorough process that must be followed before any information can be shared. We sometimes release information to approved researches, if this is allowed under the strict rules in place to protect your privacy. We are very careful with the information and we follow strict rules about how it is stored and used. Yes. You have the right to prevent confidential information about you from being shared or used for any purpose other than providing your care, except in special circumstances. If you do not want information that identifies you to be shared outside your GP practice, ask us to make a note of this in your medical records. This will prevent your confidential information being used other than where necessary by law, (for example if there is a public health emergency).

Do I need to do anything?

If you are happy for your information to be shared you do not need to do anything. There is no form to fill in and nothing to sign and you can change your mind at any time. If you have any concerns or are not happy for your information to be shared, speak to your GP.

Statement of Intent

New contractual requirements came into force from 1st April 2014 requiring that GP Practices should make available a statement of intent in relation to the following IT developments:

1. Summary Care Record (SCR) 2. GP to GP Record Transfers 3. Patient Online Access to Their GP Record 4. Data for commissioning and other secondary users 5. Referral Management

Please see below details of Dr Shearer & Partners statement of intent. Summary Care Record (SCR) NHS England require practices to enable successful automated uploads of any changes to patient’s summary information, at least on a daily basis, to the summary care record (SCR) or have published plans in place to achieve this by 31st March 2015. Having your Summary Care Record (SCR) available to other service providers, such as hospitals, will help those treating you, by allowing them to see a small part of your GP medical record. They will have access to information about any medication you may be taking and any allergies you may have. If you do not wish your medical records to be available in this way then you will need to let us know so that we can update your record. If you have already advised us that you do not wish a SCR available outside the Practice, you do not need to do so again, your record will have been excluded. If you have

not already done so, you can do this via the opt out form on our website www.glenroydmedical.co.uk Dr Shearer & Partners confirm that your SCR is automatically updated on a daily basis to ensure that your information is as up to date as possible. GP to GP Record Transfers NHS England requires practices to utilise the GP2GP facility for the transfer of patient records between practices, when a patient registers or de-registers, this does not apply to temporary registration. It is very important that you are registered with a doctor at all times. If you leave your GP and register with a new GP, your medical records are forwarded onto your new GP. It can take your paper records several weeks to reach your new surgery. With GP2GP record transfers, your electronic medical record is transferred to your new practice much sooner. Dr Shearer & Partners confirm that GP2GP transfers are already active and we send and receive patient records via this system. Patient Online Access to Their GP Record NHS England require practices to promote and offer the facility to enable patients online access to appointments, prescriptions, allergies and adverse reactions or have published plans in place to achieve this by 31st March 2015. We currently offer the facility for booking and cancelling appointments and ordering your repeat prescriptions on-line via Patient Services. If you do not have a user name and password for this system, please ask at reception for further details or register for Patient Services on our website

Dr Shearer & Partners advise you that from 1st January 2015 you shall be able to access summary information, which means data relating to medications, allergies, immunisations or any adverse reactions. Data for commissioning and other secondary users

It is already a requirement of the Health and Social Care Act that practices must meet the reasonable data requirements of commissioners and other health and social care organisations through appropriate and safe data sharing for secondary uses, as specified in the technical specification for care data. Glenroyd Medical has specific arrangements in place to allow patients to “opt out” of care data if they so wish. Care data allows patient data to be used outside of the Practice. From 1st April 2014, Practices must include the NHS Number as the primary identifier in all NHS clinical correspondence issued by the Practice whether in an electronic form or otherwise, except in exceptional circumstances where the number cannot be ascertained. Dr Shearer & Partners confirm these arrangements are in place and that our Practice undertakes annual training and audits to ensure that all our data is handled correctly and safely via the Information Governance Toolkit.
