Good morning/afternoon everyone and thank you for taking time out of your day to listen to a short talk about the importance of vision benefits. As a highly valued group benefit, your vision benefits are an important part of your personal benefits plan that helps to protect you and your family, and I am pleased to present this valuable benefits plan that is available to you during your open enrollment period. During the presentation, I’ll explain how this plan works and then I’ll give you important details on how to enroll. Now, let’s talk specifically about your company’s plan for your vision coverage. This simple chart gives you the facts and shows how you receive benefits, whether you visit a in- or out-of-network vision provider. Your employer has designed a plan that provides a level of coverage for routine eye exams, whether you choose to go in- or out-of-network. This way, your coverage benefits every member of your family – even those who don’t wear glasses or contacts at all. As you can see, you will pay a nominal copay for in-network services including exams and eyewear. When you visit an out-of-network provider, you are responsible for any copay amounts and a portion of the costs for these services. You will typically pay for the full cost of the treatment at the time of service, then submit a claim form for reimbursement from MetLife.

Here’s the breakdown of coverage for lens options. Again, you will pay a nominal copay for in-network services including exams and eyewear. When you visit an out-of-network provider, you are responsible for any amounts that exceed the applicable allowances for the costs of these services. You will typically pay for the full cost of the treatment at the time of service, then submit a claim form for reimbursement from MetLife. Your coverage includes a frame allowance of up to $150 after the eyewear copay. Your allowance is up to $70 at out-of-network providers and $85 if you visit a Costco retailer. Plus, when you visit an in-network private practice provider and the frames you choose exceed the benefit allowance, you get a 20% discount off the amount that exceeds your allowance. Walmart and Sam’s Club is now in-network, which means you will still be able to take advantage of the lowest out-of-pocket costs, convenience and a wide-range of services.

Frequently Asked QuestionsQ. Why should I enroll?A. A vision plan is a competitively-priced way to help protect the eyesight of everyone in your family. Even if

you don’t wear glasses or contacts, regular visits to your eye doctor are important to your overall health. Routine vision exams do more than protect eyes. They can also help protect overall health by catching serious problems, such as diabetes and high blood pressure.

Q. How can a vision plan help me save money?A. Eyeglasses and routine eye exams can be more expensive than you may think. With MetLife, through low to

no copays, you can save up to 60% on vision wear and services. Lens options like polycarbonate (shatter-resistant) lenses for children up to age 18 and ultraviolet (UV) coating are covered in full. You also enjoy fixed copays for scratch-resistant and anti-reflective coatings, progressive lenses, and more.

Q. Can I choose my own eye care professional?A. You can go to any licensed eye care professional. Choose from the thousands of ophthalmologists, optometrists

and opticians at private practices or popular retail locations like Costco® Optical, America’s Best, Cohen’s Fashion Optical, Eyeglass World, For Eyes Optical, Pearle Vision*, Walmart and Sam’s Club, Visionworks and more.
