But what is the best way to manage these referrals? Using the Referrals module in Centricity? Using another order type in Centricity? Other tracking mechanisms? Improving Referral Tracking and Completion Claudia Burchinal — Erie Family Health Center

This session describes Erie’s successful initiative to ensure that patients were completing hospital referrals on a regular and timely basis. Based on provider feedback, Erie recognized that patient test results were often not available in the medical records, reflecting that either hospitals were not providing results or referrals were not being completed. Consequently, Erie engaged a multi-disciplinary team and an external Six Sigma expert from Northwestern Medicine in an organization-wide process improvement project to improve referral tracking and completion. Patient focus groups revealed that patients did not fully understand what to expect when given a referral and did not know who to contact with questions. Providers and referral staff identified other systemic challenges. As a result of these findings, Erie undertook several activities and was able to double the referral completion rate for both diagnostic and specialty referrals, and both staff and patients are much more satisfied with the process. This session will describe key strategies to ensure these successful efforts and next steps toward continued improvement.

Nearly half of smokers fail to receive smoking cessation treatment during primary care visits. Increased access is critical for vulnerable populations who carry a high burden of tobacco-related disease but are especially likely to lack motivation to quit. Our organization, North Country HealthCare, is a large Federally Qualified Health Center serving 14 communities across Northern Arizona. In our mission to be a premier Patient Centered Medical Home, we have developed a robust Care Management/Health Coaching program. Northern Arizona University (NAU) has created Fitness Wellness and Health and Wellness Coaching minor programs within its School of Health Professions. North Country and NAU have partnered to pilot a Health Coaching Internship with students from this innovative program. The goals of the internship are to train Health Coaching students in real-life situations and to educate our organization’s Health Coaching/Care Management Team in evidence-based practice for the betterment of our patients. Participants at the AllianceChicago User Conference will learn about North Country HealthCare’s processes in creating this internship, the goals of the pilot program and the responsibilities of the stakeholders. The burgeoning Health Coaching field has yet to establish standardized practices for training and certification. As Health Coaching becomes a more standard part of Community Health Center care, internships of this type will be useful in assuring students have necessary field experience and Health Centers are providing best practice care. The goal of the presentation is to provide tools to other AllianceChicago members who wish to explore similar internship programs.

Session Objectives: • Understand the benefit the Health Coach provides to the Care Team

and to Community Health Centers (CHC)  in reaching population health and Patient Centered Medical Home (PCMH) goals

• Be able to articulate the value of standardized Health Coach training within a CHC

• Understand the value a Health Coach intern brings to the team and the organization 

• Identify steps necessary to be able to start a Health Coaching internship program.

At the beginning of 2017, CMS leadership pronounced Meaningful Use dead—and then introduced the Quality Payment Program (QPP) as part of the Medicare Access and CHIP Reauthoriziation Act. Meaningful Use for those who provide care for Medicaid beneficiaries is very much alive and well and the QPP and its associated Merit-based Incentive Payment Program adopts many of the key components of MU as CMS continues to move towards a stronger value based payment model. This discussion will provide an overview of MU and QPP in 2017 and how these programs could impact and benefit community health centers as well as information about programs that provide technical support for MU and practice transformation and help clinical teams effectively participate in these programs.

Session Objectives: • Receive up-to-date information about participation in Medicaid

Meaningful Use programs during 2017.

• Receive an overview of the new Quality Payment Program, its components, and the pros and cons of voluntary participation by those who do not focus on Medicare beneficiaries.
