ABOUT THIS GUIDE: The 2019 New Hire Guide describes the coverage offered to the majority of benefits-eligible employees under the Donnelley Financial Group Benefits Plan (“the Plan”). Your benefits eligibility will determine the coverage that is offered to you, your spouse, domestic partner and/or your dependent child(ren). More details on benefits eligibility are available in the Summary Plan Descriptions (SPDs) and Summaries of Material Modifications (SMMs) online at mybenefitsdfsco.com.

Welcome!This New Hire Guide describes the Health & Welfare benefits that are available to you, provided you meet the eligibility and waiting period requirements as an employee of Donnelley Financial Solutions (DFIN). Some benefits you receive automatically, while others you receive after you choose the appropriate option and coverage category.

It is your responsibility to make sure you have the benefits information you need to enroll in a timely manner. Learn more about your benefit options at our all-in-one benefits information website: mybenefitsdfsco.com. There, you’ll find:

• An easy-to-use online resource that you and your family can access quickly and easily

• A one-stop shop where you can find the most up-to-date benefits information, plan documents, Summaries of Benefits & Coverage (SBCs) and links to key resources

• How to contact the vendors for each of your benefit programs

Through the Benefits Center website dfsco.benefitsnow.com you can enroll in your benefits and access health-related tools.

If you don’t have online access or if you have additional questions, call the Benefits Center at 1-844-44-DFSCO (1-844-443-3726) for assistance Monday through Friday (except holidays), from 8:00 a.m. to 5:00 p.m. Central Time.

The elections you make remain in effect through December 31 of each year. You cannot change your benefit elections during the year unless you experience a qualified status change (e.g., marriage, divorce, birth of a child). Go to mybenefitsdfsco.com and refer to your SPDs (and any related SMMs) for more information about qualified status changes.

If you experience a qualified status change during the year, you can make a new election, consistent with the status change, within 30 calendar days (or 60 days following eligibility for premium assistance for Medicaid or CHIP or birth of a child) through the Benefits Center. Refer to your Qualified Status Changes SPD available at mybenefitsdfsco.com for more details.

Register to Enroll OnlineRegister as a new user on dfsco.benefitsnow.com. This will enable you to enroll in your benefits by clicking on the Your Benefits Resources™ link.

NOTE: If you don’t have online access, call the Benefits Center to set up your user ID and password and to enroll in your benefits.

Set Up a Personal Email and Go Paperless!If you want to set your email address as your preferred communication method, go to dfsco.benefitsnow.com and follow the steps to set your mailing preference.

To simplify your life, you can elect to review Plan information online — such as Summary Plan Descriptions (SPDs) and related Summaries of Material Modifications (SMMs) — rather than to receive paper copies. Log on to dfsco.benefitsnow.com and select the “Personal

Information” link under the “Your Profile” tab. There you will choose a Preferred Email Address so you can be notified when new documents are available.

As a reminder, you also have access to information about your benefits, SPDs and SMMs online at mybenefitsdfsco.com. You may refer to the SPDs and related SMMs whenever you have questions about your health and welfare coverage.

Your Benefit ChoicesMedical

You have two medical options that offer different levels of medical coverage to help meet different needs. Coverage is through Blue Cross and Blue Shield of Illinois (BCBSIL).

Coverage OptionsYour medical coverage options are:

HSA medical plan Copay medical plan

The HSA medical plan includes a Health Savings Account (HSA) and the company HSA contribution.

PremiumsYou must pay the premium of the coverage you elect for you and your enrolled, eligible dependents. Your Monthly Medical, Dental and Vision Premiums sheet lists the monthly costs associated with each option and coverage category. Premiums are based on your pay band, which is generally your base pay when you are first hired until the next September 1. The amount you pay is based on several factors as outlined in the SPD (and any related SMM).

Virtual Visits — Health Care on Your ScheduleYou and your covered dependents can get care for many non-emergency issues from a board-certified doctor when it’s convenient for you, 24/7 through a virtual visit right from your own home! Go to MDLIVE.com/bcbsil to register today.

Tobacco-Free PledgeDFIN encourages you to lead a healthy lifestyle. That’s why the company offers a discount on medical premiums if you and your covered spouse, domestic partner or child(ren) don’t use tobacco products. If you or any covered dependents are tobacco users and will not participate in the tobacco-cessation program in 2019, each of you will pay an annual surcharge (added to your medical premium) as follows:

• Employee Only: $500

• Spouse/Domestic Partner Only: $500

• Employee + Spouse/Domestic Partner: $1,000

• Dependent Child(ren) Only: $250

• Employee + Child(ren): $750

• Spouse/Domestic Partner + Child(ren): $750

Comparing Plan OptionsYour medical options generally cover the same services and prescription drugs, but the way you pay for them differs. Consider your typical health care use and your ability to cover unexpected health care costs to determine which option will work best for your situation.

Get Help to Make Informed Medical Decisions

Support for individuals with diabetes, coronary artery disease and congestive heart failure may be provided through BCBSIL. To get information about the programs available to you, contact a Benefits Value Advisor at 1-888-895-6981.

Also, if you are undergoing chemotherapy or later need such treatment, ask your doctor to contact BCBSIL for prior authorization. Our cancer management program aligns your chemotherapy treatment to national guidelines developed by the top 25 cancer facilities to promote the use of evidence-based guidelines for different types and stages of cancer.

Lower Reimbursement for Out-of-Network Care

• Many health plans cover services from out-of-network providers at a specific percentage of an amount referred to as “reasonable and customary.” The DFIN Medical Program pays out-of-network claims at the Medicare reimbursement level.

• This means you’ll pay significantly more to see an out-of-network doctor or other provider.

Prescription Drug Coverage

Both medical options include prescription drug coverage through CVS Caremark.

Save on Prescription Drug CostsTip Why This Matters

Use generics and formulary drugs when available

The use of generic drugs helps to lower prescription drug costs for both you and the company. The FDA requires generic drugs to be as safe and effective as brand-name drugs. Talk to your doctor about choosing a generic or brand formulary option.

BONUS: The Medical Program pays 100% of the cost of generic cholesterol and blood pressure medication.

Use the CVS Caremark Maintenance Choice Program or CVS Caremark Mail Service for maintenance drugs

Save time and money by getting a 90-day supply of your maintenance medication filled at your local CVS pharmacy, including Target stores with a CVS pharmacy, OR mailed directly to your home. Pay the same price either way. Plus, if you use the CVS Caremark Mail Service, you can choose to spread your payments over three months. See below for prescribing rules.

Use your HSA or FSA Save money by paying for your prescriptions with tax-free dollars through a Health Savings Account (HSA) or the Health Care Flexible Spending Account (FSA). Just be sure to understand IRS reimbursement rules and how your medical option can affect your Health Care FSA.

Prescribing Rule Why This Matters

Dispense as Written penalty If a generic drug is available, but your doctor notes “Dispense as Written/No Substitutions” on the prescription for the brand-name drug, you will pay the coinsurance plus the difference between the generic and the brand price.

Quantity limits on certain drug classes (e.g., steroid inhalants, nasal steroids)

Helps ensure appropriate drug use.

Prior authorization for certain drug classes (e.g., androgens, antipsoriatics, ADHD agents)

Protects against off-label use.

Generic step therapy Encourages use of less-costly generics before brand-name medications; please consult with your health care provider.

Maintenance Choice Program (Maintenance Choice does not apply to one-time or short-term medications, which are taken for 90 days or less)

• First maintenance medication: May fill a 30-day supply at any pharmacy in the CVS network.• After one fill: Must fill maintenance medications in a 90-day supply through the Mail

Service or at a CVS pharmacy, including those inside Target stores.

NOTE: If you continue to order 30-day supplies of your maintenance medications without using the CVS pharmacy Maintenance Choice Program or CVS Caremark Mail Service, you may be asked to pay the full cost of your prescriptions. If you prefer to obtain long-term medications through any network pharmacy, you must contact CVS Caremark to opt out of the Maintenance Choice Program.

No coverage for brand-name proton pump inhibitors or any non-sedating antihistamines

These are available over the counter with no prescription.

Health Savings Account (HSA)

Only for the HSA Medical PlanIf you enroll in the HSA medical plan, you’ll receive a Health Savings Account (HSA). An HSA lets you save tax-free money to pay for eligible health care expenses now and in the future.

DFIN Contributes, and You Can, Too!DFIN will contribute to your HSA based on your medical coverage level. You can also choose to make contributions to your account, up to IRS limits. See below for the maximum contributions for 2019.

You Can Use It Now …Use your HSA now to help you meet your deductible and to pay for current eligible health care expenses.

TIP: If you participate in the Limited-use FSA, you can use your FSA to pay for eligible dental and vision expenses at any time. However, you can use it to pay for eligible medical/Rx expenses only after you meet your deductible. And there isn’t a deadline to use your HSA — any unused funds roll over at the end of each year and continue to build.

… Or Invest It for the FutureYou earn interest on money in your HSA. If you choose to let your HSA grow (instead of using it now to pay for eligible health care expenses), you may invest the money in a choice of investment options once you meet the investment threshold. Mutual fund investment options are made available through the services of an independent investment advisor. Be sure to discuss if investing the money in your HSA is right for you with your financial advisor.

DentalYour dental options include the Dental PPO and Dental PPO Plus (both provided by MetLife). Both options allow you to choose any dentist, but you receive a higher level of coverage with lower out-of-pocket costs if you use participating network dentists.

To find an in-network dentist, go to www.metlife.com/mybenefits, select Employee Benefits and then Dental. Look for the PDP Plus Network under Find a Dentist. If you use an out-of-network provider, the Program pays benefits in accordance with the two options’ usual and customary (U&C) limits.

Both EyeMed options provide comprehensive coverage for exams, lenses, frames and contact lenses through a network of providers, as well as discounts on laser vision correction.

Flexible Spending Accounts (FSAs)

FSAs allow you to pay for eligible out-of-pocket health care and dependent day care expenses using pre-tax dollars. Taking the time to understand how they work can help you save up to 40% or more on eligible expenses, depending on your tax bracket.

Life and Accident InsuranceThe Life and Accident Insurance Program provides important financial protection in the event something happens to you or your spouse, domestic partner or child(ren).

IMPORTANT: You cannot cover another employee as a spouse/domestic partner or child under the Life and Accident Insurance Program.

Life Insurance Type Coverage

Basic Life Insurance

• Pays a benefit to your designated beneficiary if you die in accordance with Plan provisions

1x annual base pay up to $125,000

Business Travel Accident The Business Travel Accident (BTA) policy is a global policy that provides death and disability coverage for any eligible employee traveling on company business — domestically or internationally. This benefit is administered by ACE American Insurance Company, a member of the Chubb Group of Companies. Benefits vary depending on the type of covered activities.

International SOS AssistanceDFIN has partnered with the world’s leading medical and security assistance company, International SOS, for worldwide medical, travel and emergency assistance when traveling on company business.

Optional Life Insurance — Evidence of Insurability (EOI) • If you enroll when initially eligible as a new hire:

— If you enroll for Optional Life Insurance, you can elect up to three times your annual base pay without providing EOI if the benefit remains under $500,000.*

— You can elect optional spouse/domestic partner life insurance up to $50,000* without your spouse/domestic partner needing to provide EOI.

— EOI is not required for optional AD&D insurance or optional child life insurance.

DisabilityThe Disability Benefit Program provides protection against the loss of your regular pay if you’re unable to work because of a covered illness or injury, subject to the claim administrator’s approval. Aetna Life Insurance Company is the claims administrator for the program.

Disability Type Coverage

Short-term Disability (STD)

• This company-provided benefit is provided to you automatically

60% of your pre-disability earnings for up to 26 weeks

• Benefits begin on the 8th calendar day even if you are hospitalized.

Basic Long-term Disability (LTD)

• This company-provided benefit is provided to you automatically

50% of your earnings (up to $10,000 a month combined with LTD Buy-up)

• Monthly LTD benefits continue until the earlier of age 65 or the date you are no longer disabled according to the Program.

• If you become disabled after age 60, your benefits duration schedule may vary.

LTD Buy-up

• You purchase this coverage

Additional 10% of LTD coverage, which would provide a total LTD benefit of 60% of your pre-disability earnings (up to $10,000 a month combined with basic LTD)

NOTE: Evidence of insurability (EOI) is not required for LTD Buy-up if you enroll by your benefit enrollment date when first eligible as a new hire.

LTD Pre-Existing Condition Limitations

These pre-existing condition limitations apply if you elect LTD Buy-up for the first time. For long-term disability (LTD) if the disability starts within the first 12 months after your coverage goes into effect, your LTD coverage does not pay benefits for a disability that is caused, or contributed to, by a pre-existing condition that you have been diagnosed, treated or received medical services for in the prior 15 months (i.e., this includes the three months prior to your coverage effective date).
