Philadelphia Infant Toddler Early Intervention is requesting your response to the survey linked here below at your earliest convenience. This is a follow up to the first survey sent last summer evaluating knowledge and use of the Release of Information Form. Your supervisors are aware that this survey is being sent out and we will be sending them aggregate (not individual) data. The estimated completion time is 4 minutes and the deadline for completion is Friday July 3rd. We certainly understand that circumstances have more recently changed due to COVID-19, and are most interested in collecting general information on your knowledge and use of these materials. Philadelphia Infant Toddler Early Intervention 2019-20 requirements
As per Mayor Kenney’s announcement on Tuesday, “Juneteenth” has been declared an Official City Holiday. On Friday, June 19th, City Offices will be closed. Child Link & Partnership Office will also be closed thus, no referrals will be picked up. Since this Holiday is not incorporated into the existing MRL Phone Round Robin calendar, June 19th will need to follow the “Act of Nature” protocol in which “B” Day Providers will be skipped. Referral pick up will resume on Monday, 6/22 with “A” Day Providers. Thank you and Be Safe! Sharon
