Weekly Epidemiological Bulletin This weekly Epidemiological Bulletin is published jointly by the Federal Ministry of Health, Government of Pakistan, National Institute of Health, Islamabad and World Health Organization (WHO), Pakistan . Epidemiological week no. 6 (4 to 10 February 2011) 63 districts in 4 provinces provided surveillance data to the DEWS. This system is gradually expand- ing to provide information about communicable disease threats in all parts of Pakistan as the country goes through the recovery process. 2,251 fixed health and 8 mobile medical outreach centres provided surveillance data for this week. A total of 845,328 consultations were reported through DEWS of which 31% were acute respiratory infections (ARI), 9% skin disease, 6% acute diarrhoea, and 6% were suspected Malaria. Two follow up alerts highlighted from last week are: 1. The hospitalized case from Haripur with Influenza-like Illness (ILI) was the only one out of nine samples from KP last week which was lab confirmed as positive for Influenza H1N1. This indicates that KP may be past the seasonal peak for H1N1 noted in January. 2. Out of 8 suspected measles cases in Nasirabad, Balochistan, two samples were collected and both were positive for Rubella. This may indicate there is an outbreak of Rubella instead of Measles. A total of 46 alerts are reported in this bulletin: 2 from week five, 39 from week six, and 5 from week seven. Altogether 25 were for Measles; 4 were for each Chickenpox and influenza; 3 were for each Neonatal Tetanus, and Pertussis; 2 were for each ARI and Meningitis; and 1 each for Tetanus, Mumps, and Rabies. Epidemiological week no. 6 (4 to 10 February 2011) • 63 districts in 4 provinces provided surveillance data to the DEWS. This system is gradually expand-

ing to provide information about communicable disease threats in all parts of Pakistan as the country goes through the recovery process.

• 2,251 fixed health and 8 mobile medical outreach centres provided surveillance data for this week.

• A total of 845,328 consultations were reported through DEWS of which 31% were acute respiratory infections (ARI), 9% skin disease, 6% acute diarrhoea, and 6% were suspected Malaria.

• Two follow up alerts highlighted from last week are: 1. Since July 29, 2010, a total of 1,533,814 acute diarrhea patient consultations have been reported to DEWS from the disaster affected districts of 4 provinces in Pakistan.   

In the graph at right, DEWS reports receiving in 2011 are compared to the reports of 2010 and 2009 from disaster‐affected districts.  This week, DEWS received 48,938 reports of acute diarrhea, 6% of total con‐sultations. This is the same proportional morbidity (6%) reported by DEWS in week‐6/2010 and in week‐6/2009.

Since July 29, 2010, approximately 3,879,204 Acute Respiratory Infection (both Upper and Lower)  patient consultations have been reported to DEWS from the disaster affected districts of 4 provinces in Pakistan.   

 In the graph at right, DEWS reports receiving in 2011 are compared to the reports of 2010 and 2009 from disaster‐affected districts.  This week, DEWS received 262,166 reports of Acute Respiratory Infection, 31% of total consultations, 1% lower than the proportional morbidity reported by DEWS in week‐6/2010 but the same as week‐5/2009.

0  0  0  1 During a routine visit to DHQ, a 5 year old girl suspected case of Tetanus was found. TIG was provided by WHO but patient became LAMA before receiving TIG. Later patient was located in a hospital run by an INGO. Improved outreach vaccination

1  0  0  0 Upon field investigation, 1 suspected case was found. Sample was found positive for Measles. Patient was missed during last campaign. No other unvaccinated children were present. 

7‐Feb‐11  Pertussis  Sindh  Ghotki MCH Centre (UC II and UC Qadirpur) 

0  1  0  1 Two Pertussis cases were reported by pediatrician were identified and are improv‐ing. Their UCs are under‐immunized so outreach session are being planned to provide two doses of pentavalent to children at risk.. 

7‐Feb‐11  ARI  FATA Mohmand Agency 

IDP Camp Nahaqi  2  0  1  0 An alert was reported from IDP camp regarding ARI. Upon field investigation 3 deaths were reported from the IDP camp; these cases had not reported for medical treatment. Awareness session planned for families in the camp. 

7‐Feb‐11  Measles  ICT  Islamabad Masjid Alfurqan ‐ I/8‐3  

2  0  0  0 Upon field investigation 2 post measles cases were found. During active search no new cases were found.  All children were immunized already. 

7‐Feb‐11  Measles  Sindh  Badin Photo Machi Village‐UC Talhar 

1  0  0  0 Upon field investigation 1 suspected cases was found. 1 sample was collected and sent to NIH. Vit. A doses were administered to all patients. Health education was imparted. Any unimmunized children in the community were vaccinated.

1  0  0  0 Health facility reported the suspected measles. Patient identified, sample collected, two doses vitamin A provided. Any unimmunized children in the community were vaccinated. 

8‐Feb‐11  H1N1  ICT  Islamabad Govt. Hostel‐Near Serena Hotel 

0  1  0  0 Alert was reported from BHU Kotla Haripur and the said alert was patients contact. Sample was collected and found positive for Influenza H1N1 (2009). Oseltamivir was provided by WHO. 

8‐Feb‐11  NNT  Balochistan  Nasirabad Nursery ward, Town area  

1  0  0  0 During a routine visit to DHQ, case of NNT was found. There was no history of TT vaccination of the mother. Patients got treatment (TIG) and his condition improved.

Upon field investigation 34 cases of Pertussis were found. 9 Samples were col‐lected and sent to NIH. Investigations are under process. This UC is under‐immunized so outreach sessions are being planned to provide two doses of penta‐valent to children at risk.

1  0  0  0 1 Suspected case of mumps was reported in ARI center at RHC Talli. No history of immunization. Upon field investigation no more cases were found in the vicinity.

Probable rabies death was reported by the health facility from dog bite 2 months ago. Upon investigation 13 other persons were noted to have been bitten or scratched by the suspect dog before the dog was killed. 5 started ARV but found it expensive. Awareness raising for families conducted. Anti‐rabies vaccines will be distributed by the district health authorities to each case.

0  0  1  1 Upon field investigation 2 suspected cases of Chickenpox were found from the camp. Isolation of the patients and symptomatic treatment were advised.

Upon investigation 2 suspected cases with whooping cough symptoms were found. Patients were already Tx with antibiotics so no sample was collected. This village is  under‐immunized so outreach sessions are being planned to provide two doses of pentavalent to children at risk.

Epidemiological Bulletin: Flood Response in Pakistan

Province Khyber Pakhtunkhwa

• This week 13 districts reported to DEWS from KP province, 495 health centers reported 116,482 patients consultations to DEWS.

• ARI is the leading disease in KP province. ARI cases reported 46,959 (40%), 1% less propor-tional morbidity as compared with last week.

• 9 alerts were reported in this Bulletin, of which 7 were for Measles, 1 was for H1N1, and 1 was for ARI from FATA.

Province Punjab • 11 districts reported data to DEWS from Punjab

province, 757 fixed health centers reported a total of 176,911 patient consultations during this reporting period

• ARI cases reported were 47,421 (27%), the same proportional morbidity percentage as compared with last week.

• 2 alerts were reported in this bulletin, both were for Measles.

• Malaria cases are still reported from flood af-fected districts. This week a total of 4,169 ma-laria samples were tested of which 150 were posi-tive (slide positivity rate 3.5%), while falciparum rate was 61.3%.

Province Sindh

• This week 22 districts reported to DEWS from Sindh province, and 693 health centers re-ported a total of 480,933 patient consultations during the reporting period of week 6, 2011. 

 

• In Sindh, ARI cases reported were 144,585 (30%), same proportional morbidity percentage as compared with last week

• 25 alerts are reported in this Bulletin for weeks 6 and 7: 13 were for suspected Measles; 3 were for chickenpox, 2 each were for Pertussis and Influenza, while 1 each was for meningitis, Neonatal Tetanus, Tetanus, ARI, and Rabies.

Province Balochistan

 

• In this week, 12 districts reported to DEWS from Balochistan province. 302 fixed and 8 mobile medical outreach centers reported a total of 67,875 patient consultations.  

• ARI cases reported a total of 21,731 (32%) cases, which is 2% higher proportional morbidity as com-pared with last week.  

• 6 alerts were reported for week 6 in this Bulletin: 2 alerts were for Neonatal Tetanus, while 1 each for Mumps, Chickenpox, Pertussis, and Measles-Rubella.

• Malaria cases are still reported from Jaffarabad, Kech and Jhal Magsi districts. This week a total of 498 malaria samples were tested of which 158 were posi-tive and slide positivity rate was 31.7%, while falcipa-rum rate was 67.1%.

Distribution of Wild Polio Virus cases Pakistan 2010 and 2011

Year 2010: The total number of polio cases in 2010 so far is 144 includ-ing 120 type-1 cases and 24 type-3 cases and the total number of infected districts/towns/agencies is 40 to date.

Year 2011: In week-6 one new type-1 polio case was confirmed by the laboratory from district Tando Allayarm Sindh. The total number of infected districts/towns/agencies is 4 to date.
