Mr. and Mrs. Jones, I wanted to thank you for coming in to our offices. I think it is important that we get together to review your financial and estate

planning. How has the family been? That’s Great…Glad to hear things are going good for you.

I would like to take some time to review with both of you so we are up to date.

•First, I would like to review the fixed annuity that you have with our firm. The current value of this annuity is $213,345.00 and the issue date was 2/09/2004. We have done very well with this annuity and I just wanted to take the time that we have the correct beneficiaries on them. The Good News is that we have $ 21,334.00 that is under the Free withdrawal provision…meaning these funds are not subject to any penalties. We can talk about these funds later.

•Your Medicare supplement seems to be meeting your needs. Do you have any questions at this time on your Medicare supplement plan?

•Did you ever meet with the attorney to complete your legal documents on your estate planning?•It shows in my file that you don’t have Long Term Care Insurance. Have you considered the benefits of long term care insurance?
