General Consent for Years 3-6

In order to comply with the latest data protection laws and to ensure access to a variety of services, we need your consent.

1. Data Sharing for Additional Services Consent – For all pupils. We need to share information with business and Welsh Government to allow us to provide services such as homework apps, online working, revision apps, library services. We need your permission to share this information.

2. NPT Library Membership Consent – For pupils in Years 3-11. We would like to encourage use of NPT libraries to develop reading habits. We need your permission to allow us to share information with the library and for them to set up membership.

3. Offsite Activities Consent – For all pupils. We want to ensure that we can offer a broad range of educational visits and trips. Pupils to take part in school trips and other activities that take place off school premises and can be given first aid or urgent medical treatment during any school trip or activity. We need your consent for this.

4. Photographic Consent – For all pupils. We take photographs of the children at our school. We may use these images in our school prospectus or in other printed publications that we produce, in displays and on our website. We may also make video or webcam recordings for school-to-school conferences, monitoring or other educational use. We allow 3rd party companies to take school and prom photos to sell to you. We need your consent for this.

If any choices are left blank, then the current consent will not be changed.

I do not want my child to have access to additional services.

2. NPT Library MembershipPlease choose one of the following:I give permission for the school to share information with NPT Library Servicesand for my child to become a member of NPT Library.

I do not want my child to be a member of NPT Libraries.

3. Offsite ActivitiesPlease choose one of the following:I give permission for my child to take part in school trips and other activities that take place off school premises and to be given first aid or urgent medical treatment during any school trip or activity.

I do not give consent for my child to take part in school trips.

4. Photographic ConsentPlease choose one of the following:I give permission for my child’s image to be taken and used within the school database systems. I may wish to purchase school photos. Photos may also be used in publicity material for the school, including printed and electronic publications, video and webcam recordings and on websites and social media. I give permission for images of my child to be used by the news media in printed and/or electronic form and stored in their archives.
