Agenda Item 1 – Welcome and Introductions Action

Becca Faulkner, Co-Chair welcomed everyone and introductions were made.

Agenda Item 2 - Minutes from previous Learning Disabilities Partnership Board 19 March 2018 - Already sent for comment – Outstanding Issues

Traffic Light System – Page 3Obtaining ‘key rings’ is on-going due to supplier problems. Frazer Kirk apologised for this and chase with supplier.

Merging of LD Partnership Board and LD SIFChris Sterry said that he isn’t happy with the decision to merge. He queried the comment in the previous board minutes of 19 March 2018 where it was stated that the Chair and Vice Chair of the LD SIF agreed to the merger, however, they only went along with this decision, at a separate meeting with them and the Practice Development Team, due to not having a choice, they were told only one

could continue the LDPB or LDSIF and they both thought it should be the LDPB, reluctantly through no effective choice. Chris Sterry feels that merging the LD Partnership Board (Strategic) and LD SIF (Operational) would be too much to cope with. Another member of the board said that key people were missing from the meeting in March 2018 which made this decision.

The LD Board and LD SIF met every 2 months; with a couple of weeks in between each meeting. Chris Sterry thought the extra half an hour given to the LD Board to merge the LD SIF and LD Board wasn’t enough. Michelle Glossop said that the decision was made by a majority vote through work which was carried out at the Workshop at St. Mary’s.

Christina Herbert suggested that it might be of benefit to go back and look at what was agreed on this day i.e. the action plans etc. The day was very well supported.

Andrew Wheawall said that he didn’t feel that the last LD Partnership board was productive, lacked focus and enjoyment. Members put effort and commitment coming to the meeting and he didn’t want anyone feeling disheartened and losing commitment. Bringing the SIF into the LD Partnership at this time is the best thing to do. A clear agenda is needed for the rest of the year which will contribute in an operational and strategic way.

Chris Sterry proposed that the merging of the LD Partnership Board and LD SIF will be looked at in 9 months where it could be revised.

What happens to the minutes of this meeting?Patricia Edney said that she was concerned about where the minutes and conclusions of the board go. And wanted to know if the minutes are shared with other groups and organisations to help plan services? Andrew Wheawall said that members of the board take away the views and opinions to promote in other

areas of service for discussion.

Terms of Reference/WebsiteMichelle Glossop said that website update is on-going – all updates are on a waiting list ready for putting on the Webpage.Action: Practice Development Team to give update at the next LD Board.

Agenda item 3 - Treat Me Well Campaign

Anita Winter, Christina Herbert, and Una Cunningham presented the ‘Treat Me Well Campaign’.

‘Treat me Well’ is a new health campaign from Mencap which will be promoted through the Learning Disability Week. The aim of this campaign is to help ensure that the NHS Hospital Trust treat people with learning disabilities appropriately upon admittance and during their stay in hospital.

Una Cunningham emphasised the importance of reporting concerns during hospital visits.

Points raised during the presentation

Hospital Case Register/red flagUna Cunningham said there are about 3,500 people on the case register. NHS staff need to be sensitive to everyone who walks through the door and assist appropriately depending on person’s needs if they are flagged on the register or not.

Not everyone is on the registerIn some cases it is because the person might have a hidden disability such as Autism or the person might not want to be on the register. Una Cunningham said approximate 20% of people aren’t on the case register.

Becca Faulkner said that some people are reluctant to be on the case register because some of the questions asked are patronising. Anita Winter said the reason for this is because the data set is large. This is to capture as much information as possible on the person. Anita Winter said that questions need to be asked in a way which is appropriate for the person concerned. Anita Winter said that this may need looking at.

Adam Butcher said that he had attended a meeting where NHS Digital was discussed.This system gathers data from wider health and social care spectrums to help the NHS deliver better care to people. It would be a massive step forward if this was rolled out nationally. Anita Winter said that this is starting to happen in Care Trust and across service areas.

TrainingIt was highlighted that in some cases NHS staff seem to be apprehensive about supporting someone with learning disabilities – due to lack of understanding/training. In the past training was given to staff in groups. Una Cunningham said that group training has decreased. Most NHS training taking place now is E-Learning. The E-Learning is produced by Yorkshire and Humberside Education. This training isn’t mandatory.

Una Cunningham highlighted that the NHS has 16,500 staff and now delivery training to all levels of staff. E-Learning training is a way of reaching as many staff as possible.

Michelle Glossop said training is delivered differently in each area of service. Sheffield City Council conducts 1:1 training which is co-produced and co-delivered. This training is mandatory for providers of service (who work in Andrew Wheawall’s area). Training is rolled out to social workers. Trish Edney thought it would be a good idea for training to take place in schools to reinforce understanding.

Una Cunningham said now nurses get a lot training but could not confirm what training the GP’s receive.

Chris Sterry said that the ‘Carers Project Group’ have done a lot of good work in

getting carers recognised. This helps NHS staff communicate with a person when there is only a short period of time to get to know the person.

LeDeR (Learning Disabilities Mortality Review Programme)Anita Winter said that she would like to have a session about the LeDeR on the work which has happened so far.

Anita Winter is the local area contact in Sheffield LeDeR. This means that she gets all the information on all the deaths of people with learning disabilities in the city. A team of reviewers review the deaths – this is about learning about the person and how that person has died to help make improvements for the future.

Anita Winter said that Sheffield is one of the biggest reported numbers across nationally – this is because people with learning disabilities are flagged consistently.

Hospital PassportChristina Herbert said that she isn’t entirely happy with the current passport. Anita Winter said that people need to be aware that there is more than one passport available. If ‘Hospital Passport’ is googled on the internet there are loads passport formats. This makes it difficult to identify one specifically for use across the City.

The Mencap passport format has been used. Christina Herbert said the authority do not own the copyright which means changes cannot be made. The Safeguarding Operational Board have been asked to pull together a ‘Task and Finish Group’ to look at the passport to see if there is anything that can be recommended to Mencap to improve the format.

Una Cunningham said it doesn’t really matter what the format is as long as the

person’s needs are identified. When a person is first admitted to hospital they are always asked if they have a passport.Christina Herbert said that the Hospital Passport should be known as a Health Passport because the passport isn’t only used when someone attends hospital; it is used to access all kinds of health and social care services.

Adam Butcher said that the National Mencap and Gateway are supporting setting up a local ‘Campaign Group’. Anita Winter said it would be good to give the Care Trust a link to this group.Action: Adam Butcher to send link to Anita Winter.

Andrew Wheawall said that the ‘Treat Me Well Campaign’ has been circulated to all social workers.

Action:Andrew Wheawall requested an update in 3 to 6 months.

Agenda item 4 – Transforming Care - WorkplanThe draft Workplan documentation was circulated to the board members.

Action: amend ‘Transferring’ to ‘Transforming’ on Workplan.

Andrew Wheawall read through the titles of the Workplan. Michelle Glossop said that the titles were pulled out from previous meetings and welcomed everyone’s comments and contributions.

Anita Winter – thought LeDeR (Learning Disabilities Mortality Review Programme) should go under ‘Treat me Well’. Action: Anita Winter to send LeDeR document to members.Frazer Kirk raised concerns that there were too many headings for the board to cover realistically.

Anita Winter said there had already started a number of work streams in relation to LeDeR which would lead to actions.Andrew Wheawall said that the following would need to be identified:

Decision makers in each area. Who sits on different boards? Check for duplications. Trish Edney said that the aims and objectives would be needed in each

topic. Plans from outside organisations could be fed into the work plan. Feed into the CCG objectives and get the feedback from their progress. Timescales and updates required on time. Timescales not met should be

queried. Quality Assurance – Andrew Wheawall said collectively the board could

carry out QA checks within difference types of organisations. Providers should welcome valued judgement to improve services.

ActionIt was agreed the board would look at the Workplan outside the meeting and send contributions to Frazer Kirk who will collate the information received.

ChairAndrew Wheawall said he is hoping that Councillor Chris Peace will chair the next meeting. The board will need to decide if they want a councillor as chair on the board.Chris Sterry would like to monitor the attendance of the board.ActionThis will be part on the Agenda for the next meeting.

Agenda item 5 – Mental Health StrategyDavid Luck (Commissioning Officer for Mental Health). Leads on a number of mental health projects for Sheffield City Council.

David Luck is in the process of developing a new Mental Health Strategy for the City.

This is important work for everyone because one in four people will develop a mental condition at some time in their life. As the strategy has developed in Sheffield, two things are starting to stand out:

1. Mental Health Services need to provide and delivery services in the right way.

2. Sheffield wants to be a City where well-being is pursued for mental health.

David Luck said that a ‘Consultation’ should be on line on Friday 25.5.2018 or soon after this date. Council’s - Consultation space, Citizen space – What do you think?The consultation will run for six weeks.

Adam Butcher informed David Luck that he knows of various forums and meetings who could contribute during the Consultation period. And advised David Luck to contact Dawn Shearwood or Gillian Hallas who would be able to help. Other groups who may be able to contribute are: The Carers Support Group, Teaching Hospitals has a mental health committee and Service User Engagement Group. Andrew Wheawall said there would contact David Luck if they thought of any more contacts.

The survey will be available soon and will be sent out to known networks. David Luck asked the board if other groups would be interested in receiving the survey.

Action: David Luck to send a link to Frazer Kirk who will distribute to board members.

David Luck said that he would ensure that the draft consultation feedback is sent to the LD Board. The aim is for the work to be signed of in the Autumn.

Andrew Wheawall asked David Luck if the board could help due to similarities of LD/MH. David Luck said Mental Health can learn from LD and bring information back to the board. The relationship between the two will be developed over time.

Agenda item 6 – Local AccountMichelle Glossop said that late last summer Elaine Dutton came to talk about the local account. Elaine Dutton’s approached the board again and has asked how the board would like to contribute.

Michelle Glossop asked the board if they want Elaine Dutton to attend a meeting and talk about the local account or would it be preferable to distribute previous documents to look at to see if there is anything missing. Michelle Glossop said Elaine Dutton hasn’t given a timescale however last year the timescale was September 2017.

Agenda item 7 - Learning Disability Week 18th – 24 June 2018Adam Butcher explained that every year Mencap have a Learning Disability week. This year the focus will be on the ‘Treat Me Well Campaign’. A lot of events are happening in hospitals.

National Mencap have a website which informs people how they can contribute to the event. Adam Butcher said that he has been tasked to gather data on people’s hospital experiences. The hospitals involved are the Hallamshire and Northern General.

Any Other BusinessChanging Places

Chris Sterry raised concerns about the ‘Changing Places’ facilities at ‘Ponds Forge’. During a recent visit with his daughter he noticed:

The hoist wasn’t working. Chris Sterry thought it was because people weren’t putting the hoist on charge after use.

The changing bed was down to the floor and wasn’t working. A member of staff didn’t know that Ponds Forge had ‘Change Places’

facilities.

Chris Sterry said he has written to Ian Hamilton the manager at Ponds Forge asking that:

Staff should be informed that Ponds Forge has ‘Changing Places’ facilities. Guidance signs asking people to put the hoist and changing bed on charge

after use.

In addition Chris Sterry asked if a variety of commodes could be supplied because the hoist, on the visit in question, only worked to some degree at the charging point which is in a position where it is difficult for people to be toileted. Chris Sterry has requested a timescale for completion of this work from Ponds Forge.

Chris Sterry informed the board that when he accessed the Council’s website he was directed to out of date webpages. He was also amazed that the Disabled Go website only recorded two ‘Changing Places’ toilets across the whole of the city. It was highlighted that the board had carried out a lot of work to ensure more ‘Changing Places’ toilets are available across the city. Chris Sterry thought that after the ‘Special Olympics 2017’ more facilities would be in place.

Anita Winter said that further investigation would be needed to find out exactly how many ‘Changing Places’ toilets are available.

LeDeR Report - Learning Disabilities Mortality Review ProgrammeChris Sterry said the LeDeR report wasn’t very good reading and that the North is coming out of it very bad. Anita Winter stated that the report focusses on 2016/17. Sheffield didn’t start the process until December 2016 (not all reports were received) so the LeDeR Report will not give a fair reflection of Sheffield. The next annual report will focus on all the information which has come from Sheffield.

Annual Health CheckChris Sterry said that Heather Burns used to attend the board and present the Annual Health Checks.Action: Anita Winter to bring the CCG Annual Health Checks/ Adam Butcher to bring the Public Health England health checks to the next meeting.

CarefirstChris Sterry raised concerns that he thought that documents can be left open in Carefirst, which could lead to someone being able to make changes to the document after it has been completed. He had expected a document to be sent to CCG in March 2017. However, when Chris Sterry received a copy it was dated

April 2018.

Andrew Wheawall said that when a document is printed off from carefirst it gives the date that the document has been asked for and would be very surprised if the document had been altered. Andrew Wheawall said Chris Sterry could Question this Legitimately and then an investigation would take place to track the document’s process. A new system called ‘Liquid Logic’ will be in place by the end of this year which should be better.

Carers VoiceChris Sterry – On the 28.6.2018 will be attending CCG event where he will launch the Carer Voice documents, he will update the meeting next time.
