September 4, 2015 Dear Parents, We are planning the “How to Be a Wise Guy” children’s retreat on October 2 & 3 at Lake Salla- teeska in Pinkneyville, IL and we would like for your 3rd, 4th, or 5th grader to join us. You will find enclosed in this packet all the information you need to know for your son or daugh- ter to sign up and have a great time of Bible study, recreation, food, and fun. A tentative sched- ule is enclosed so that you will know departure and return times as well as what we will be doing throughout the weekend. However we are always flexible!! To register your son or daughter please read all of the enclosed information, answer all the questions on the enclosed forms, including the Parental Consent Form and the Camper’s Cove- nant (children may not attend without these). Send or bring them to the office with your payment of $40.00 on or before, September 20. Children may sign up after this date but in order to guar- antee correct T shirt sizes we need reservations early. Space is limited so don’t delay. Pay- ment and forms turned in guarantees your spot. We are encouraging children to bring their friends (children and friends must be in 3rd, 4th or 5th grade to attend) and additional packets of information are available in the office. One thing to consider, parents, is whether or not your child is ready for an overnight away from home experience. Some children are ready to stay away from home at this age, and others are not; some children are willing to join in the camp activities and follow the rules and others resist doing it. Since we can only take a limited number of campers, please be sure that your child is both ready and willing to be a part of camp. I know that this camp can be a great experience for your child, offering opportunity for spiritual growth as well as fun and fellowship. We are planning the “How to Be a Wise Guy” children’s retreat on October 2 & 3 at Lake Salla-teeska in Pinkneyville, IL and we would like for your 3rd, 4th, or 5th grader to join us.

Here We Go!!! What You Need to Know!!!

We are going to Lake Salleteeska, one of our Illinois State Convention Camps, in Pinckneyville, IL.

Please feed campers before coming to the church or at least give them a snack. We will eat about 6:45 p.m.

We will leave the church parking lot at 4:30 P.M. on Friday October 2, and arrive at our retreat destination at about 5:45 P.M. (see the enclosed schedule).

We will leave Lake Salleteeska at 3:21 P.M., on Saturday, October 3, and arrive at the church at about 4:57 P.M. (see the enclosed schedule).

Please make sure that your child brings his or her Bible. There will be lots of fun, fellowship, and yes, food.

We will follow our schedule as closely as possible realizing that flexibility is the key word for the weekend.

There will not be a registered lifeguard on duty this year when the children are swimming. There will be adults who can swim present at all times.

We will provide dinner on Friday night around the campfire, breakfast and lunch on Saturday, and several snacks throughout the retreat. Children are welcome and encouraged to bring snacks that they can share with the group. Please no peanuts or peanut butter products!!

We will not be making any stops on the way to or from the retreat so extra money is not needed.

All participants will receive a t-shirt on Saturday.

A covenant is an agreement between two people or two groups. By signing this form, you are promising to follow the guidelines while you are at camp. Each person must sign a Camper's Covenant to attend. Failure to follow the covenant completely can and will result in the camper being sent home at parent’s ex-pense. As a participant of Calvary’s Children’s Camp, I agree to obey the following rules of cooperation in order to allow a good experience for all campers.

1. I will abide by all requests and instructions of the Camp Director and counselors, and follow all rules under “Camp Guidelines”.

2. I will follow the camp schedule faithfully, being on time, in the right

place, and ready to participate as the schedule indicates. This includes wake-up and lights-out time, meal times, recreation, and worship times.

3. I will participate in camp activities.

4. I will respect the property of the camp and other campers, and will pay

for any damage that I cause.

The undersigned does hereby give permission for our (my) child _______________________________,

to attend and participate in the “How to Be A Wise Guy” Children’s Retreat sponsored by Calvary Baptist Church’s Childhood Ministry on Friday & Saturday, October 2 & 3, 2015 at Lake Sallateeska in Pinkneyville, IL. We will leave the church on Friday, October 2 at 4:30 p.m. and return on Saturday, October 3, no later than 4:57 p.m.

We (I) authorize an adult, in whose care the minor had been entrusted, to consent to any X-ray examination, anes-thetic, medical, surgical or dental diagnosis or treatment, and hospital care, to be rendered to the minor under the general or special supervision and on the advice of any physician or dentist licensed under the provisions of the Medical Practice Act on the medical staff of a licensed hospital, whether such diagnosis or treatment is rendered at the office of said physician or at said hospital.

The undersigned shall be liable and agree(s) to pay all costs and expenses incurred in connection with such medi-cal and dental service rendered to the aforementioned child pursuant to this authorization.

Should it be necessary for our (my) child to return home due to medical reasons or otherwise, the undersigned shall assume all transportation costs.

The undersigned does also hereby give permission for our (my) child to ride in any vehicle designated by the adult whose care the minor has been entrusted while attending and participating in activities sponsored by Calvary Bap-tist Church.

We (I) do hereby release, forever and agree to hold harmless Calvary Baptist Church and the directors, employees, and agents from any and all liability, claims or demands for personal injury, sickness or death, as well as property damage and expenses, of any nature whatsoever which may be incurred by the undersigned and the child-participant that occur while said child is participating in the above described trip or activity.
