vsc -.!1- Investigation report on a Fire Incident at Tirunelveli IRD on 22/09/14. ED O&D has constituted a Committee consisting of DGM O&D, HQO, Chief Manager HSE Corporate (Operations) and Chief Manager HSE,SZ to enquire into the Fire incident at Tiruneveli IRD on 22-09-2014.The Team visited the location on to enquire into the Incident. Incident Description: Reproduced as reported By Location As a part of MBLCRIP ,Works for fixing ofROSOV's on Tanks was taken up by the location from August 31st 2014. Tank No 2 for HSD was fitted with ROSOV and MOV. Subsequently Tank no 3 for SKO was being emptied to handover to M/s. Nexigen for cleaning. After product was removed through normal Gantry Tank truck filling, the residual product below the suction level was taken out after opening the manhole cover using hose connection into Tank Trucks for filling. Loaded tank trucks were checked for QC, invoices were made and released. On 19/09/14, one TT (TN74E7530) was filled with SKO from tank and on checking the sample, it was found that the product in 2 compartments were OK and in the third compartment, Product was found mixed with water and sludge. Another TT no TN39L1062 was also filled with product from same SKO Tank and it was observed that product in 2 compartments were good and product in third compartment was found mixed with water and sludge. In order to release Truck no TN74E7530 with good product, that product from Compartment No.3 of the truck no TN74E7530 was transferred to Truck no TN 72Z1500 and it was filled into 2 compartments partially. Subsequently good product from the TN39L1062 was filled i'nto the 3rd compartment of the truck no TN74E7530 . Samples were tested and invoiced and sent outside on 19/09/14. Further removal of SKO product could not be carried out on 20/09/14 due to rake decantation, it was decided to empty out Truck No TN72Z1500 for filling on 22/09/2014. Since one compartment was empty in the TN39L1062, it was decided to transfer the contaminated product from TT No. TN72Z1500 to Truck no TN39L1062. At 11:30 AM on 22/09/14, the arrangement was made for taking the oil into half open drum and pumping product from the cut drum to TN39L1062. On starting the pump, within a matter of 1 min., a fire was noticed on the delivery side of the pump and the fire was immediately extinguished with water & foam by the Depot Officials. Investigation report on a Fire Incident at Tirunelveli IRD on 22/09/14.

ED O&D has constituted a Committee consisting of DGM O&D, HQO, Chief Manager HSE Corporate (Operations) and Chief Manager HSE,SZ to enquire into the Fire incident at Tiruneveli IRD on 22-09-2014.The Team visited the location on 23~09-2014 to enquire into the Incident.

Incident Description: Reproduced as reported By Location

As a part of MBLCRIP ,Works for fixing ofROSOV's on Tanks was taken up by the location from August 31st 2014. Tank No 2 for HSD was fitted with ROSOV and MOV. Subsequently Tank no 3 for SKO was being emptied to handover to M/s. Nexigen for cleaning. After product was removed through normal Gantry Tank truck filling, the residual product below the suction level was taken out after opening the manhole cover using hose connection into Tank Trucks for filling. Loaded tank trucks were checked for QC, invoices were made and released.

On 19/09/14, one TT (TN74E7530) was filled with SKO from th~ tank and on checking the sample, it was found that the product in 2 compartments were OK and in the third compartment, Product was found mixed with water and sludge.

Another TT no TN39L1062 was also filled with product from same SKO Tank and it was observed that product in 2 compartments were good and product in third compartment was found mixed with water and sludge. In order to release Truck no TN74E7530 with good product, that product from Compartment No.3 of the truck no TN74E7530 was transferred to Truck no TN 72Z1500 and it was filled into 2 compartments partially. Subsequently good product from the TN39L1062 was filled i'nto the 3rd compartment of the truck no TN74E7530 . Samples were tested and invoiced and sent outside on 19/09/14.

Further removal of SKO product could not be carried out on 20/09/14 due to rake decantation, it was decided to empty out Truck No TN72Z1500 for filling on 22/09/2014. Since one compartment was empty in the TN39L1062, it was decided to transfer the contaminated product from TT No. TN72Z1500 to Truck no TN39L1062.

At 11:30 AM on 22/09/14, the arrangement was made for taking the oil into half open drum and pumping product from the cut drum to TN39L1062. On starting the pump, within a matter of 1 min., a fire was noticed on the delivery side of the pump and the fire was immediately extinguished with water & foam by the Depot Officials. The person who was holding the delivery hose (Mr.Manikandan) left the hose down to run away. However·, the fire caught him and he ran with the fire for

help and in panic. His own contractor who was available on the. spot extinguished the fire with water and removed the cloth. He was immediately shifted to City hospital .During this time, the person who was standing on the TT holding the delivery hose jumped down and fractured his leg, who was also taken to hospital and his condition is stable now.

Depot officials handled the situation promptly and averted major incident.

Methodology adopted by Investigating team :

1) Visit to Accident site along with the Depot Officers. 2) Verification of all Documents related to the incident 3) One to One Interaction with Officers and stakeholders present during the

Incident. 4) Probable cause of leak and fire 5) Conclusion.

SITE Observations:

1. The trucks involved in the incident were removed from the actual site and parked behind the TT loading Gantry' area. It was observed that Company Tank truck no TN39L1062 Tank body was found to be covered with black soot on its left side , Rear Left Tyre and mud flaps found partially burned.

2. Kirloskar make,1987 model Old Mud pump with 1.25 inch dia discharge faucet with Cable connection drawn from nearby Junction Box .opp to Tank no 2(HSD).The glands used were oversized and loosely fitted to the FLP housing.

3. One six Meter long 2" Dia partially burnt Plastic hose with clamps and L bend Fittings with quick shut off Valve. One abandoned MS Spanner.

4. Another plastic hose of app.8 mts length. which was used for suction was found lying near the Tank trucks parked behind the gantry area, partially burned at one end.

5. One pair of Oil soaked Chappals.

6. 14 drums of sludge stored opposite the incident spot 3 meters away from the spot.

7. Burnt shoes and Burnt strands of clothing's 10 meters form the site of incident towards the TWD pump house.

8. No presence of DCP extinguishers nearby the incident site.

9. Cut drum used for Transferring SKO was found shifted and kept at the Gantry.

10.Water gel blanket could not be produced on 23-09-14 But it was subsequently brought and shown in the morning of 241h Sept 2014. The sealed packet containing the blanket which was kept in its container was found unopened.

Document verification: 1) Hot work Permit 2) Job safety analysis 3) Scope of PO placed on Handling Contractor, Arumuga Nainar and MRR with

attachments 4) Security Gate Records. 5) Fire Engine Log book. 6) Training

Observations on Document verification:

- There was no work permit made for the specified job of Transferring product form One Tank truck to another tank truck which caused this incident.

- Available Permits no 0086 -00 signed and verified by Depot manager for M/s Nexigen on 19-09-14 for the cleaning of TK 03 was extended on 20-09-14 vide permit no 0086-01. This was received by Nithin kochuraman but not approved by depot manager, but was utilized by Handling Contractor, Arumuga nainar.

This has been done through the online Hot work system . The same Permit was extended on 22-09-14 manually by the Location in charge with a comment to enter the permit in the system without giving reasons.

- No Electrical permit was issued by the depot though temporary power connection was taken from JB by the Contractor.

- JSA made on 19-09-14 has been made casually and not on the specific Job being undertaken. There was no JSA prepared for the job being undertaken on 22-09-14.

- The gate records showed that this contract employee who sustained Burn injuries has been shown as Mani instead of Manikandan who has been coming to HPCL for One year.

- There has been no record of payment to Manikanttan by the contractor in the wage register submitted alongwith the Bill .It has also been seen that there is no witnessing of the disbursement of wages to contract employee by HPCL officers.

- There is no Photo identity card issued to the above person even though he was working inside the premises for more than a year.

Interactions with stakeholders:

- Electrician has confirmed that he has tripped the power on seeing the incident from MCC room.

- Electrician has also confirmed that he has noticed that there was a fuse blow out in all three phases in MCC room on the cable which has been connected to the Motor of Mud pump utilised by depot officials for Tank truck to Tank truck transfer.

- Nexigen personnel has informed that their Mud pump which was kept inside the dyke of TK 03 was found shifted to the Incident spot without their knowledge or permission.

- SOP's for MBLCRI project were not readily available with the depot or the Officer handling MBLCRI jobs.

- Safety officer has confirmed that they have connected a 2.5" diaPlastic Hosepipe to the pump discharge faucet of 1.25" Dia using Teflon packing and a Clamp. The hose was physically held by the contract worker Shri.Manikandan to prevent it from sagging. Contractor along with another contract worker Shri.V.Murugan were standing on the Truck holding the delivery end of the Hose.

- Safety officer has also confirmed that neither the Tank trucks nor the pump set was earthed.

Safety officer who was supervismg the activity has informed that he has immediately pulled out the suction hose form the cut drum on noticing the fire.

Video footage could not be seen due to the non-functioning of 3 cameras m incident areas.

- Our Non management employees have confirmed that Fire fighting operations were carried out by them and could extinguish the fire within 2/3 minutes.

Probable Causes of leaks and fire:

- The cause of leak evidently was due to the imperfect match between the oversized Plastic hose and the pump discharge faucet.

- The clamped end most probably could not stand the discharge pressure making the hose to get detached fl·om the discharge faucet resulting in the product get sprayed outside.

- Sparking that ignited the product vapor might have occurred due to discharge of Static charges generated due to use of Plastic hoses for the transfer of product mixed w·ith sludge and water.

- Since the product being transferred contained water and sludge mix, presence of pyrophoric substances like Iron sulphide in the mixture can not be ruled out which might have got ignited when came into contact with air.

Conclusion:

The investigating team after going through the above Process has arrived at the following conclusions:

1) The location was successful in containing the fire immediately with Participation of all personnel of the depot, which should be noted.

2) The location has not followed Standard Operating procedures in carrying out the above job, which led to the Incident.

3) The Location has failed to identify the hazards dearly and did not analyse the risks involved in the activity that was being undertaken. More so because the job, was of non-routine nature where special f(.)cus should have been there.

4) The team found that the quality of JSA's made by the Location is very poor. The one liner JSAs do not meaningfully capture the hazards involved and the associated and probable risks. ·

5) The type of Materials used by the location for the above operation showed that the location was careless in its approach towards doing this job, maybe out of lack of knowledge or awareness.

6) The presence of MS spanner at the site indicated that non-Sparking tools were not used while carrying out the above activity, though non-sparking tools were available with the location.

7) The presence of the abandoned chappels at the incident spot proved that Implementation of PPE' s was not strictly being enforced.

8) T'here is lack of Knowledge on First Aid Procedures, \Vhich has been revealed by the non-usage of the water gel Blanket on the Vktim by tht: ·ream.

9) Work permit system was bypassed while doing this job, which led to this incident.

1 0) They have used the Hot work permit with a specific job requirement to be clone by Nexigen, to another contractor for doing a different job requirement which is totally wrong and incorrect.

11) We have found in our interactions with the officers that there is lack of awareness among young officers on the job hazards connected with their jobs.

12) The Investigation members felt that there is no cohesion between the Location in charge and the Otlicers in the Depot.

13) We noticed that there was total lack of involvement or the Location in charge in the day to day operations.

Recommendations:

I ) Depot can start Normal Operations .

2) MBLCRlP work can be resumed only after the Zonal Operations and HSE team make SOP's required for working at depot and ensure strict compliance of the SOP's and all safety requirements.

3) There is a Training calendar available at the depot and trainings are held as per the same. Ilowever the effectiveness of all the Training program's

• including First Aid, usage of PPE etc need to revisited.

4) Zone shall arrange training on Work permit system and JSA immediately to all the officers. ·,,

5) Zonal Operation shall monitor proper implementation of site specific SOP's at th<: Depot.

2 Labourers Hurt in. Fire Accident· at HPCL Express News Service

Tinmelveli: Two contract labourers were injured in a fire accident at the Hindu­stan Petroleum Corporation Limited (HPCL), here on Monday while pumping kerosene sludge from one yehicle to another.

Sources said, Mani alias Manikandan (2.8) from Thenpathu, and. Murugan (25) from Rajavallipuram work as contract labourers at HPCL, Thatcbanallur in Tirun.elvel'i. On :Monday, · when they were involved in pumping kerosene sludge from one lorry to another, they received burn injuries due to a fire accident that took place following a techni­cal snag in the motor pump used to transfer kerosene. Following this, the HPCL administration doused the
