ROOT CAUSE ANALYSIS
Root cause analysis is an approach for identifying the
underlying causes of why an incident occurred so that
the most effective solutions can be identified and
implemented. Generally it is a tool designed to help
identify not only what and how an event occurred, but
also why it happened. Only when the investigators are
able to determine why an event or failure occurred,
will they be able to specify workable corrective
measures that prevent future events of the type
observed. The investigator’s goal should be to identify
specific underlying causes. The more specific the
investigator can be about, why an event occurred, the
easier it will be to arrive at recommendations that will
prevent recurrence. Occurrence investigations must be
cost beneficial. It is not practical to keep the valuable
manpower occupied indefinitely searching for the root
causes of the occurrences. Recommendations should
directly address the root causes identified during the
investigation. If the analyst arrives at vague
recommendations, then they probably have not found a
basic and specific, enough cause and need to expend
more effort in the analysis process.A root cause
analysis should be performed as soon as possible after
the error or variance occurs. Otherwise, important
details may be missedGreat responsibility lies on the
shoulders of the managers while conducting the
analysis. Managers must explain that the purpose of
the analysis is not to assign the blame. The comfort
level with the technique increases with use, but the
analysis wil always be subjective. It differs from
troubleshooting problem solving and the failure
analysis. The process however is not very simple.
Determining the reasons for problem, designing a
solution, implementing the solution and evaluating the
success of the solution are the major steps in root cause
analysis. Once the problem is defined, the team will
brainstorm potential reasons why the problem exists.
These reasons may be human, organizational or
physical. The process of brainstorming requires a bit of
research. Once the real, underlying root cause of the
problem is understood, a solution can be designed.
Implementing and evaluating the solution are relatively
straight forward. The key to implementation is
planning and ensuring buy-in at all levels. Evaluation
can be done in many ways. Apart from this clear
communication is required for solving problems
effectively. Even a simple analysis can be eye-opening
to the people who deal with the problem on a regular
basis. Managers are sometimes unaware of the details
that those closest to the work view as common
knowledge.Root cause analysis is a valuable
management tool that can be readily learned by
managers as well as frontline personnel. It can be
conducted at several levels of depth and complexity.
Root cause analysis is a very popular business tool,
and as a result many different business theorists have
developed tools to get the job done.

Amount: Rs. 7 Lakhs
Period: 1 year
PI: Ms. Veda Sandeep Nagaraja, Assoc. Prof, E&CE
Dept
Co-PI: Mr. Girish G.K., Asst.Prof, E&CE Dept
Objective
The main objective of the proposal is to design a
MEMS based array of microphones and signal
processing system. The project is to design a system to
identify the vehicle (tanker) in a given radius in a land
mine scenario.
The incoming acoustic signal will be captured by the
MEMS microphone and further enhanced using a
signal processing circuitry. The signal processing
circuitry will improve the directionality of the MEMS
microphone array subsystem whereby the overall
efficiency of the system will improve. The tiny
directional microphone array can easily be mounted on
a PCB and integrated with the signal conditioning
circuitry.
The proposal will also look into the various aspects of
signal attenuation due to the underground burial of the
overall system. The signal conditioning circuitry will
also filter the noise signals and sense the direction of
the acoustic signal emitted by a vehicle in the battle
field scenario.
