By anyone’s measure, we are entering a new and exciting era in health care.

The passage of the Patient Protection and Affordable Health Care

Act in March set in motion reforms that will influence the entire health care industry. We at the Vineyard Nursing Association are most interested in how the legislation will affect home health care on Martha’s Vineyard. We not only are committed to understand-ing the new model and adapting our business practices accordingly, but also to capitalizing on opportunities - created by the legislation - to expand our programs and enhance the services we provide.

The national law also comes at a time of shifting demographics for our community. We are all living longer, and the segment of the Vineyard population over the age of 60 is also growing due to the number of retirees choosing to move here. In less than a decade, almost a quarter of the island population will be senior citizens.

It is clear that the Vineyard will face sharp increases in the demand for home health that require action now.

As we look to the future, we see three priorities for the VNA: investing in staff development, enhancing programs and services, and purchasing our current headquarters.

Our reputation stands on the shoulders of our nurses, therapists, and health aides. The healthcare field changes fast, and finding and retaining professional staff on the Vineyard is difficult. We must shore up our recruitment efforts, as well as develop the skills of current staff by connecting them to professional

education and training opportunities. To this end, with the support of Our Island Club, we are helping a new nurse with the educational costs associated with obtaining her RN. We need to do more of this.

We must also make sure our employees are armed with the best tools and the latest technologies. Already, we have equipped our nurses with laptop computers so they can report and chart patients’ health changes. These reports are also available electronically to physicians, enabling access to the most current patient information possible. Most recently, we invested in automated machines that help in the daily distribution of medication to patients, greatly reducing problems associated with medication mismanagement.

Additionally, we need to develop new programs to meet the growing needs of an aging Vineyard population-expanding our patient and caregiver support groups and potentially implementing an Adult Day Health program for people needing special medical support.

Lastly, few people realize that the VNA does not have a permanent home. In the past five years we have occupied three different locations. Fortunately, we have an opportunity to create a permanent location at our current address. We have a one-time option to purchase our current headquarters. An investment in this location will enable us to invest more than $100,000 a year in savings from rent into new programs and staff that will be required to serve the needs of the Vineyard community.

OUR SERVICES

The Vineyard Nursing Association is the only certified home health care agency on Martha’s Vineyard. We offer a full range of nursing, home care and other services, with specialties from pediatrics to geriatrics. As such, we are committed to providing the highest quality home health care and assisted living services and health education to the community. All of our compassionate caregivers are trained, CORI checked, insured, licensed and/or certified by the state of Massachusetts. With our expertise in continuous care nursing, we are able to transition even the most complex cases, from the hospital to home. Our primary services are:

Nursing ServicesOur skilled nurses act as case managers coordinating with physicians to implement an individualized care plan for our patients. Specialized and advanced care programs are provided by professionals with extensive home care experience and advanced training which include: wound care management, home IV therapy, cardiac and respiratory care programs, diabetes management, maternal and child health, geriatrics, orthopedics, oncology, osteoporosis screening and management, tele-monitoring, pain management and palliative care.

Home Health AidesSupervised by Registered Nurses, our home health aides, homemakers and companions lead our patients through activities of daily living to help them live in the comfort of their own homes. Some of our services:• Help with bathing, dressing, medication reminders, ambulation, meal preparation and simple physical

exercises.• Light house keeping and transportation services, including transportation to off-island medical appointments.

Rehabilitation ServicesPhysical Therapists Trained therapists provide general therapeutic exercises for ortho/neuromuscular development and improving balance, gait, and transfers.

Occupational TherapistsOccupational therapists provide general therapeutic exercises for patients with an injury or illness affecting perceptual motor skills or the ability to perform activities of daily living such as dressing, eating, or cooking.

Medical Social WorkLicensed Social Workers assess the needs of patients and families and present them with emotional support strategies and appropriate referrals. Coping skills, mental health issues, safety, financial concerns, housing options, counseling, long-term planning, and more are common issues addressed.

OUR PROGRAMS

The above services are offered to patients through four primary ‘Programs’ - the programs are categorized by patient needs and/or type of financial support. In addition to Insured Care (Medicare and Private Insurance) the VNA offers the following programs:

Private Care ProgramThis program provides any and all services designed to offer an “extra layer” of security for those seeking additional help that is not covered by Medicare or when Medicare coverage has ceased.

Elder Services & Disabled Services ProgramElder ServicesThe VNA takes referrals from The Elder Services of Cape Cod & the Islands to provide eligible elders with the supportive services that they may need to live safely and independently at home. Services may include meal preparation, shopping, and homemaking. Financially qualified individuals 60 and over may be eligible for services.

Disabled ServicesOur Home Health Aides provide homemaking services to eligible adults with disabilities who are functionally limited in meeting their own nutritional and environmental needs. The VNA provides these services to people ages 18-60 in a contract with the Massachusetts Rehabilitation Commission through its Homecare Assistance Program. Services we provide enable disabled individuals to maintain their independence while living in the community.

Public Health ProgramThe VNA provides community nursing services for Martha’s Vineyard through contracts with each town’s Board of Health. Community nursing services include:

Nursing Clinics The VNA community nurse holds regularly scheduled nursing clinics at convenient locations across the five towns we serve. The town nurse assesses an individual’s health, provides public health information aimed at improving health, and directs patients to appropriate providers if issues are recognized that need further investigation and diagnosis.

Blood Pressure ScreeningEvery month, the community nurse offers free blood pressure screenings in each town.

New Baby WellnessAfter the birth of a child, within two weeks of being at home, families are offered a home visit by a VNA nurse. Services include education, referrals and basic newborn and new mom support.

Influenza ClinicsThe VNA is part of the Island’s coalition of health care agents and healthcare providers who conduct an Island-wide seasonal flu clinic. Additional clinics are offered in the towns and in local elementary schools as needed.

Community Health EducationThe VNA community nurse establishes an annual education plan addressing health issues facing Vineyard residents and visitors. Topics range from diabetes to Lyme disease to breast cancer and more. The nurse will also prepare workshops for children.

Emergency Response TeamIn addition to the contracts with the towns to support community health needs, the VNA is also an associate member of the Medical Reserve Corps of Martha’s Vineyard. The VNA participates in the planning for responses to emergency situations that might arise on the Island. Such emergencies might include pandemics similar to the recent H1N1 influenza outbreak, or medical support during disasters such as hurricanes.

The VNA is responsible for tracking and reporting any outbreak of communicable disease on the Vineyard. Utilizing Department of Public Health software applications, the VNA reports regularly on status information regarding communicable disease. The software also allows VNA to be aware of communicable disease outbreaks around the state.

The Vineyard Nursing Association’s financial statements are reflective of a stable and well-run nonprofit organization. The complete financial statements for the fiscal year ending December 31, 2009 were audited by independent accountants Alexander, Aronson, Finning & Co., P.C. who expressed an unqualified, or “clean” opinion on the statements.

Our Mission

Vineyard Nursing Association is a nonprofit, premier provider whose mission is to provide compassionate, high quality and state of the art care for patients and families with acute, chronic, or advanced illnesses. We are also committed to the interdisciplinary management of palliative pain and symptom control issues through every stage of illness.

Purpose

Vineyard Nursing Association’s purpose is:• To provide skilled nursing, restorative and other therapeutic services and homemaker/home

care aide services on a part time intermittent basis.• To assist in evaluating, promoting, educating, and maintaining the health and welfare of

individuals in the community of Martha’s Vineyard.• To promote cooperation and collaboration among agencies and providers in improving the

The VNA’s mission is not only to care for our community in the present, but also to anticipate the needs of the future. By all accounts, in ten years the island population will have both grown in size and shifted in demographics. It is estimated that by 2020 more than 20% of people living on the Vineyard will be senior citizens. We also know that 70% of people aged 65 and older require long-term health care services at some point in their lives. In other words, the need for home health care services on Martha’s Vineyard is only going to dramatically increase as time passes, and it is crucial that we expand both our staff and our services in order to meet that need.

The VNA is poised to grow services to meet the needs of our community. We hope to pursue programs such as:

An Adult Day Health Program that will provide social and medical help for seniors who are currently unable to participate in senior programs.

Use of advanced technology such as telehealth & telemonitoring of patients for more regular health monitoring.

Medically related support groups for patients and caregivers, i.e. stroke and diabetes.

Community Interest Health Workshops, i.e. Lyme disease & seasonal influenza.

The Vineyard Nursing Association is prepared to initiate a capital campaign in 2010 to purchase the building in which our headquarters are currently located. By purchasing the building, the VNA will be able to reduce overhead costs and expand services and staff to meet the needs of the community and be equipped for the future growth.
