Instructions Transfer/Transient/Former Students

Ø A “transient” student is one who enrolls temporarily for a single course or one term only. Ø Submit either a paper or an electronic application (www.applytexas.org), NOT BOTH. We encourage you to apply online. Ø Read page 6 before you start this application. Ø Majors and codes are available on the Internet at http://www.applytexas.org.; and in the Code Book, available at

http://www.collegefortexans.com, in the admissions office of public universities and colleges, and in high school counseling offices.

Part I.1. Enter your Social Security number. The Social Security

number is important to match the application with your grade transcript and test scores. The Social Security number is required to give (disburse) financial aid. NOTE: While no specific legal authority requires you to list your Social Security number for the purpose of applying for admission to the university, not providing the information will hold up further processes, such as financial aid.

2. Enter your date of birth (month, day, year). Do not leave

blank; this information is required. 3. Enter your full, legal last, first, and middle name. Do not

use nicknames or abbreviations because this information will be used for your official record if you enroll. Use your full, legal name on all documents sent to the institution to which you are applying.

4. Enter other names, if any, under which your transcripts or

other credentials may be listed. 5. Designate whether you are male or female. 6. Enter the city, state, and country where you were born. 7. Provide the information regarding your ethnic background.

The information will be used for federal and/or state law reporting purposes and may be used by some institutions in admission or scholarship decisions.

8. Enter your permanent address and phone number. See the

Texas county codes and the two-letter state abbreviations listed in the Code Book (refer to “Instructions,” above). Use the county code if your permanent address is in Texas; if it is not, you do not need to provide a code. If you do not have access to the county code information, you may leave this item blank.

9. Enter your current address and phone number only if it is

different from your permanent address. Enter the date this address expires.

10. Enter the name and phone number of an emergency contact

person.

11. Enter your E-mail address. The inclusion of your E-mail address may allow institutional representatives to contact you with admissions information.

12. Check Fall, Spring, Summer I or Summer II, and enter the year you plan to begin attending. “Mini or short semester” may not apply to all universities.

13. Check if you are applying as a transfer student, transient

student, or former (re-entry) student. Enter the appropriate information for each category. Check all that apply.

14. If you plan to apply to more than three universities, fill out the application, leaving item #14 blank, and make copies. Then enter the name of each university to which you are applying. List your first and second choice of major and the appropriate major code(s)/abbreviations for each. Majors and major codes may be found in the Code Book (refer to “Instructions,” above). Send a copy of your completed application to the admissions office of each university. Keep the original application for your files. Refer to page viii for helpful instruction tips.

15. Enter the complete name of the high school from which

you graduated, as well as the city and state where it is located. Enter the month and year you graduated. Indicate if you were home-schooled. Have an official transcript sent to each university to which you apply.

16. If you did not graduate from high school, indicate whether

or not you have a GED and whether it is the English or Spanish version.

17. Indicate whether you plan to pursue a pre-professional

program; e.g., pre-law, pre-medicine, nursing, pre-dental, pre-veterinary, physical therapy, etc.

18. Check “Yes” if you plan to seek a teaching certificate in

addition to your degree and “No” if you do not. If “Yes,” indicate if you will be seeking elementary (Early Childhood-Grade 4), middle/junior high level (Grades 4-8); high school level (Grades 8-12); or all-level certification.

19. List all college or university courses in which you are

presently enrolled, the institution you are attending, the department abbreviation, course numbers, dates taken, and course credit you expect to receive for these courses.

20. List all colleges or universities you have attended.

Include their locations, the dates you were enrolled, any degrees you received, and the date the degree was conferred. If in Texas, include their codes (see the Code Book -- refer to “Instructions,” above). Have an official transcript sent to each university to which you apply.

21. Indicate whether or not you will have earned a bachelor’s

or associate’s degree by the time you plan to enroll. List the college or university from which you have received or will receive the highest degree and the date conferred.

22. Indicate whether you are currently on academic

suspension from any college or university. 23. Indicate if you are a member of Phi Theta Kappa, the

honor society at community/junior colleges. 24. Indicate the date you took or plan to take the TOEFL (for

international students) test. 25. (a) Check “Yes” if you are a U. S. citizen. If you are not,

check “No” and indicate the country of your citizenship. If your response is “No,” in (b) indicate if you hold Permanent Residence Status (valid I-551). If “Yes,” enter the date it was issued and your alien number. Return a photocopy of both sides of your Permanent Resident card along with the Texas Common Application for Admission of Transfer/Transient/Former Students. If you are an international student, do not use this application. Contact the institution to which you are applying for the proper application. (c) If you are not a citizen or permanent resident, check “yes” if you have an application for permanent residence pending with the Bureau of Citizenship and Immigration Services and “no” if you have no application for permanent residence pending with the BCIS. (d) Check “Yes” if you are not a citizen or permanent resident or have no application for permanent residence pending but have lived in Texas for three years, at least part of the time with a parent or court-appointed legal guardian, and graduated from a Texas high school or received a GED. Check “No” if this does not apply to you. (e) If you are not a permanent resident and have not filed for permanent residency, indicate what type of visa you hold.

26. Enter your address or addresses for the last three years.

Include the street address, city and state, and length of time you resided in each location.

27. If you have been employed full time (40 hours per week)

during the last two years, enter your employer’s name, the location of the business, and the dates you worked there.

28. Check “Yes” if you are a Texas resident and “No” if you

are not. If “No,” you may skip questions 29-31 and proceed directly to item 32. Note: By answering “No,” you are stating that you are not a Texas resident, and you will be charged the out-of-state tuition rate.

29. Indicate if your parent or court-appointed legal guardian

claimed you as a dependent for U. S. federal income tax purposes for the tax year preceding your registration. If you are self-supporting and your parent or court-appointed legal guardian will not claim you as a dependent for the current tax year, answer question

Application Fee Application fees are listed on pages ii through iv. Include the student’s name and Social Security number on your check or money order. Some universities also accept credit cards. Check with the institution(s) to which you are applying for information concerning a possible waiver of the application fee; i.e., college application fee waiver. Part II. Additional Information See pages ii through iv to determine if the university(s) to which you are applying requires this section. If it is required, provide complete information on employment, internships, and/or summer activities you have had that indicate your special interests, abilities, challenges, or achievements. You may also submit a résumé in addition to this list. Attach it to your application and make a note that you have done so. Spell out names of organizations, clubs, honors, awards, etc.; do not use acronyms. Essays Most universities in Texas do not require essays as part of the admission process. Check the list on pages ii through iv or the web site at http://www.applytexas.org. Procedure for Applying to More Than One College or University

The Texas Common Application was created to help make applying to a Texas institution of highereducation easier. You are strongly encouraged to apply online at www.applytexas.org.

The following procedure is recommended for students who wish to apply to more than one college oruniversity and who complete a paper application:

1. Each institution has its own unique admission instructions and required credentials. Beforemailing your application, refer to the web site or contact the admissions office of each college oruniversity to which you apply for additional institutional information.

      2. Complete Parts I and II of the Texas Common Application. Leave item #14 blank as you are

completing the application.

3. Photocopy the original completed application and make one copy for each college or university towhich you are applying. Keep the original application for your files in the event you wish toto other institutions at a later time.

4. On each copy of the application, complete item #14 by entering the name of the college oruniversity to which you are applying and your first and second choice of major. Follow theinstructions for completing theTexas Common Application to ensure proper listing of major(s).

5. Submit one completed application to each college or university to which you are applying,including Part II, essays and application fee if these items are required by the institution.
