Job Title Support Worker

Accountable to Team Leader

Disclosure Level Enhanced check for Regulated Activity

Job Purpose : To support tenants to live valued lives within the community. This may include provision of personal, emotional, social and physical support on a shiftwork basis. To undertake specific additional responsibilities as part of the supported living team and at all times to work in accordance with Heritage Care’s policies and procedures.

Key Responsibilities

Work closely with staff and tenants in promoting individual packages of care that reflect the person’s chosen lifestyle.

Work as directed by the co-ordinator of the work area, in supporting tenants in their day to day living.

Participate in training opportunities, formal and informal to gain a wide understanding of tenants’ needs and disabilities so as to facilitate their independence and choice.

Work flexibly according to the demands of the service.

Support tenants in leisure, education and employment services inside and outside of the home.

Support tenants as much as possible to undertake household chores, including planning and preparing meals, to involve tenants as much as possible in maintaining the household environment.

To assist tenants in maintaining their personal hygiene where necessary.

Support tenants in developing and maintaining meaningful relationships.

Be experienced and proficient in carrying out (facilitating) all aspects of the keyworker role including Individual Planning.

To take charge of the scheme in the absence of the Team Leader as required.

To deputise for the Team Leader as required.

To complete the Diploma in Health & Social Care Level 2 (which will be supported by Heritage Care) if not already attained (or have an equivalent qualification).

To co-ordinate and deliver training, mentoring, coaching and development activities in the scheme under the direction of the co-ordinator. In particular to develop areas of expertise

Following training and assessment:

Administer medication as required in accordance with individual needs.

To act as a keyworker to a named individual tenant.

Take charge of the home on a shift basis as a designated responsible person.

Liaise with social and health care professionals in supporting tenants’ needs.

Complete written reports and notes as required as part of Heritage Care’s policies and

procedures.

To take responsibility for areas of financial management for the home e.g petty cash.

To take responsibility for the delivery or supervision of complex support issues in the home

and tenants who have special support needs.

Under the Team Leader’s guidance, take responsibility for the supervision of nominated

staff in the home.

Assist the Team Leader with the preparation of rotas.

To assist the Team Leader with the recruitment of staff in the scheme.

To support tenants accessing benefits eg, housing benefit, income support, DLA etc.

To support tenants in their personal financial organisation.

Other responsibilities:

Work a flexible shift pattern working on all days of the week and waking nights (needs led).

Work in accordance with the Care Standards Act 2000, Heritage Care’s policies and

procedures, including Equal Opportunities policy and Health and Safety policies and

guidelines.

Using computers to support individuals in their daily lives, for example using the internet to

book holidays etc. Accessing Heritage Care resources, for example staff website, online

payslips and undergoing e-learning. Using basic software packages to record and access

information.

Accompany tenants on holidays and provide support in planning and booking holidays.

Attend supervisions, staff meetings and other forums as required.

Communicate efficiently and politely with all visitors and callers to the home.

Work a flexible shift pattern including day and night shifts and all days of the week

(including sleep-ins – needs led).

Carry out any other duties deemed appropriate by the supervising manager.

Person Specification

Qualifications/Education/Training

Willing to complete the Care Certificate

No particular qualifications are required but the Diploma in Health & Social Care Level 2 or

related qualification would be helpful.

Experience

Caring/supporting people with mental health support needs in a paid or voluntary capacity.

Experience of working as part of a team at the workplace.

Knowledge & Understanding

Have a basic understanding and knowledge of equal opportunities at the workplace and in

supporting tenants.

Understanding of the nature of the post.

Understanding of people with mental health support needs and how to support them to live

valued lives in the community.

Understanding of Person Centred Planning

The effects of transition on people moving from one home to another

Skills & Aptitudes

Ability to read reports, instructions and policies/procedures in English

Able to write legible reports, care plans and instructions in English

Numeracy skills sufficient to produce petty cash returns, complete monitoring sheets and

perform basic calculations.

Basic computer keyboard skills, and the ability to use computers on a regular basis.

Examples of the level required may include, the use of the internet, emails, and basic soft-

ware packages to record and access information.

Circumstances

Ability to undertake “sleep-ins” or “on call” duties and work flexible shift patterns including

all seven days of the week 365 days per week.

All Heritage Care staff must understand and actively participate in the Safeguarding, Health and

Safety and Equality and Diversity policies and procedures to ensure that service users, other staff

and stakeholders are safe and respected in Heritage Care services.

All Heritage Care staff must ensure that confidentiality is maintained regarding all aspects of care relating to service users and all issues relating to the Scheme and Heritage Care.

Terms & Conditions of Work

Below is a summary of our typical Statement of Main Terms & Conditions of Employment.

Location: Salhouse

Initial work base: 52 Ormonde, Norwich Road, Salhouse, Norfolk, NR13 6QG

Salary Range:

£14,860 per annum (£7.50per hour) (pro rata)

Allowances:

Sleep In Allowance £35.00 per sleep in (if required)

Probationary:

This post shall be subject to a 6 month probationary period during which your performance will

be monitored on a regular basis by your line manager

Annual Leave:

28 days inclusive of all Bank Holidays (Pro rata)

criteria are met for auto enrolment.
