DANKWOORD

Het ene moment was ik nog vol overgave aan het werk om deze thesis te voltooien en de eerste patiënten met COVID-19 te behandelen, het volgende moment zag de wereld er heel anders uit. Ineens was ik zelf een intensive care patiënt met een ernstige COVID-19 infectie, die geconfronteerd werd met alle onderwerpen zoals in deze thesis beschreven. Zelfs de roze olifantjes kwamen voor mij dansen. Hoewel ongevraagd, heeft het mijn blik op diverse zaken verruimd. Gezien de ernst van de aandoening, ben ik enorm dankbaar dat ik dit ziektebeeld heb overleefd. Daarom is dit een iets ander dankwoord dan gebruikelijk in een thesis.

Prof. dr. M.J. Schultz, beste Marcus, mijn promotor, onze kennismaking stamt uit 1995, tijdens onze stage longziekten in het Longfunctielaboratorium van het AMC. Ook de opleiding tot intensivist hebben wij gelijktijdig doorlopen, eerst bij prof. dr. Chris Stoutenbeek, vervolgens bij prof. dr. Jozef Kesecioglu. De basis voor deze thesis ligt in een ontmoeting in Wenen, waar wij aan de praat raakten over mijn wens om onderzoek te doen naar onder andere longschade bij patiënten die een slokdarmverwijdering ondergingen. Voor mijn interesse in pijn en delier, bracht je me in contact met Peter Spronk en dat bleek een schot in de roos te zijn.Afstand was geen obstakel dankzij de moderne media. We zagen elkaar regelmatig voor overleg op zeer bijzondere locaties zoals Barcelona, maar ook in Kerkrade en Maastricht. Je reageerde vrijwel altijd direct op mijn mails en ondanks het verschil in tijdzones tussen ons, corrigeerde je mijn manuscripten meestal al binnen 24 uur. Dat was heel prettig, zeker omdat mijn tijd erg beperkt was. Jouw perfectionisme, tot op het moment van de deadline, zorgde ervoor dat dit een thesis is geworden waarop ik heel trots ben. Ook de afgelopen periode, zowel voor, tijdens, als na mijn intensive care opname, heb je mij intensief gesteund en een hart onder de riem gestoken. Ik heb dat heel erg gewaardeerd.

Dr. P.J. Spronk, Peter, mijn copromotor, ik heb jou leren kennen in Wenen en al snel zijn wij begonnen aan ons eerste onderzoek. Als je een poosje niets van mij hoorde, stuurde je altijd een appje om even bij te praten via FaceTime of kwam je ‘even’ langs in het verre Zuiden. Jouw nuchtere benadering van de onderwerpen zorgde voor een mooie samenwerking en ik ben erg blij dat je mijn copromotor wilde zijn. Ook jij hebt me gesteund tijdens de afgelopen periode, wat heel fijn was. De gesprekken met jou leverden mij altijd weer stof tot nadenken en nieuwe ideeën op.

Laura Bormans, paranimf, onze ‘intense’ samenwerking begon tijdens de eerste studies. Als research nurse had jij, door je enorme georganiseerdheid, een voorbeeldfunctie voor mij. Dankzij jou kon ik altijd onze data eenvoudig terugvinden. Al snel hadden we een klik en je werd voor mij een goede vriendin, bij wie ik altijd mijn verhalen kwijt kon. Samen zetten we de schouders eronder als we weer eens de dossiers in moesten duiken omdat er aanvullende gegevens opgezocht moesten worden. Ook wanneer het mij af en toe een beetje te veel werd, was je er en samen kwamen we er wel uit. Dank voor al je hulp!

Coby Heij, paranimf, we hebben elkaar leren kennen op 1 oktober 1993, onze eerste dag van de opleiding tot anesthesioloog in het AMC. We waren vanaf het begin twee handen op een buik. Samen met onze collega Julia ten Cate hadden we onze wekelijkse studie-avonden waarop we, na uitgebreide culinaire hoogstandjes, meestal nog wel een uurtje vrij konden maken om de kookpunten van de volatiele anesthetica te bespreken en het fysiologieboek door te nemen. Ook de opleiding tot intensivist hebben wij samen doorlopen. Na afronding van de opleidingen anesthesie en intensive care hebben we elkaar, ondanks de afstand Amsterdam-Limburg, vaak gezien. Je was altijd weer even enthousiast over en betrokken bij mijn promotie-traject en ik kon altijd op je terugvallen. Dank voor al je steun!

Robert-Harm Bremer, jou wil ik graag bedanken voor de talloze tips en samenwerking bij onze gezamenlijke ‘Delirium’ WESP-studenten. Het was altijd heel plezierig om mijn verhalen bij je kwijt te kunnen en met jou samen te werken bij gezamenlijke projecten. Ook na mijn IC-opname was jouw ondersteuning van onschatbare waarde.

Hugo van der Kuy en Dennis Japink, jullie waren onmisbaar bij de ‘bevalling’ van mijn eerste manuscript. Dennis, dank voor je praktische hulp met de afbeeldingen en mijn eerste Macbook. Je hulp hierbij op de parkeerplaats bij Hoeve de Aar in verband met autopech, blijft me altijd bij.

Hugo, dank voor de deal die je met me maakte. Het was een eer (en een boel stress) om je anesthesie te mogen geven voor je eigen studie naar de ‘trachvent’, in ruil voor hulp van een statisticus. Volgens mij hebben we je een negative-pressure longoedeem bezorgd op je eerste vakantiedag.

Vakgroep Anesthesiologie Zuyderland, dank voor de mogelijkheden om het onderzoek te kunnen verrichten. Harry, Johan en Peter, dank voor het vaak op korte termijn aanpassen van het rooster, waardoor ik op het operatieschema ingedeeld kon worden bij patiënten die een operatie aan hun slokdarm ondergingen. Dank ook voor al jullie steun die wij als familie hebben gekregen tijdens mijn ziek-zijn en voor de bezoekjes na mijn IC-opname, waarbij jullie de tijd namen om even samen met mij op de bedrand te zitten.

Vakgroep Intensive Care Zuyderland, dank voor jullie steun en de mogelijkheden voor het uitvoeren van de studies. Fijn dat jullie mij in de gelegenheid stelden om, in drukke tijden, tijd vrij te nemen om deze thesis af te ronden. Daarnaast hebben jullie natuurlijk, noodgedwongen, ook de zorg voor mij op jullie genomen tijdens mijn intensive care opname. Dat moet emotioneel enorm zwaar geweest zijn en ik wil dan ook mijn bewondering voor jullie uitspreken. Dank ook voor al jullie steun aan mijn familie en aan mij.

Yvo en Raf, dank voor al jullie hulp bij de broncho-alvelaire lavages op de OK. Ik kon altijd op jullie rekenen en alle benodigdheden werden stipt op tijd door jullie bij de OK bezorgd. Dank ook dat jullie mij leerden om te assisteren tijdens een BAL. Dat kan wellicht nog vaker van pas komen.

Toekomstige intensive care patiënten, ik weet nu zelf hoe het voelt tijdens en na een IC-opname. Ik beloof jullie dat ik deze ervaringen in positieve zin ga gebruiken.

Petra Teunissen, jouw indrukwekkende proefschrift gaf mij de impuls om ook mijn thesis te voltooien. Dank voor al je support.

Mam en Meep, Pap en Maria, heel hartelijk dank voor al jullie interesse en enthousiasme over mijn werk en het maken van deze thesis. Anne, Liza, Laura en Emiel. Ik ben blij dat ik jullie heb leren kennen en wil ook jullie heel hartelijk danken voor alle support tijdens mijn ziek zijn.

Cathelijn, Aspelin en Quinten, jullie zijn mijn alles. De uitdrukking ‘je bent net zo gelukkig als je ongelukkigste kind’ gaat helaas op voor ouders en heeft natuurlijk ook zijn impact op jullie gehad. Maar na regen komt zonneschijn en gelukkig zijn we daar allemaal sterker uitgekomen. We waren alleen niet voorbereid op dit tweede life-event en jullie werden geconfronteerd met een doodzieke moeder. Iets wat ik jullie liever bespaard had. Ik heb bewondering voor hoe jullie je hier doorheen geslagen hebben. De toekomst zal er vast beter uitzien.

Rob, je bent de liefde van mijn leven. Zonder jou was dit proefschrift er wellicht niet geweest. Vanaf dat ik je leerde kennen, heb je mij aan alle kanten gestimuleerd om het beste uit mezelf te halen. Deze thesis is een zichtbaar resultaat van je steun. Ik realiseer me terdege hoe stressvol het voor je was om mij zo ziek mee te maken. Dank voor al je support en het wegnemen van mijn angsten tijdens en na mijn ziekte. Betere tijden zullen komen.

BIOGRAFIE

Margaretha Christina Elisabeth (Meta) van der Woude werd op 25 oktober 1965 geboren in Raalte. Na de middelbare school, het Gymnasium Erasmianum te Rotterdam, volgde zij de studie geneeskunde aan de rijksuniversiteit te Leiden (artsexamen cum laude, 1992). Zij won een Faculteitsprijs voor het ‘beste afstudeerproject’ met een onderzoek naar de praktische waarden van bio-electrische impedantiemeting ter bepaling van de voedingstoestand bij beademde patienten op de intensive care onder leiding van Giel van Berkel, internist-intensivist (1989) . Vanaf dat moment was ‘intensivist worden’ haar grote wens.

Na haar eerste baan als arts-assistent op de intensive care afdeling in het toenmalige De Wever ziekenhuis te Heerlen (1992), volgde zij vanaf 1993 de opleiding tot anesthesioloog in het Academisch Medisch Centrum te Amsterdam gevolgd door de opleiding tot intensivist in hetzelfde centrum. Aansluitend werd de Europese aantekening Intensive Care behaald (EDIC I en II). Vanaf 2000 tot heden werkt Meta van der Woude als anesthesioloog-intensivist in het Zuyderland medisch centrum te Heerlen.

Meta was langere tijd actief als instructeur voor de ATLS, de MOET en de FCCS cursus. Daarnaast was zij ook bestuurlijk actief, onder meer als lid van de protocollencommissie van de NVIC en Medisch hoofd operatiekamers in het Atrium medisch centrum (2011). Aan het INSEAD in Fontainebleau in Frankrijk, volgde zij het European Health Leadership programma. Na de fusie tussen de zorgorganisaties Atrium medisch centrum in Heerlen en Orbis in Sittard-Geleen in januari 2015, was zij 5 jaar lang medisch manager RVE Verpleegcentrum van het Zuyderland medisch centrum. Ze is lid van diverse ziekenhuiscommissies en was verantwoordelijk voor de implementatie van de veiligheidsthema’s ‘behandeling van sepsis’, ‘Vroege herkenning en behandeling van de vitaal bedreigde patiënt’ en ‘Vroege herkenning en behandeling van pijn’ binnen het ziekenhuis. Zij geeft leiding aan de Nazorgpolikliniek binnen het Zuyderland medisch centrum voor patiënten die op de intensive care hebben gelegen en neemt namens de Nederlandse Vereniging voor Anesthesiologie deel aan de landelijke Richtlijncommissie ‘Nazorg voor Intensive Care patiënten’. Vanaf 2013 begon zij naast de dagelijkse werkzaamheden binnen de anesthesie en intensive care aan een promotietraject onder leiding van prof. dr. Marcus J. Schultz en copromotor dr. Peter E. Spronk.

Meta is getrouwd met Rob Slappendel. Ze heeft 2 dochters, Cathelijn en Aspelin, en een zoon, Quinten.
