PROJECT AIM
Reduce rapid re-hospitalization rates from 14% to 5% on unit 43C.

THE CHANGE - MAY 2011 A peer support specialist was asked to
contact clients within 72 hours of discharge to offer support. Results:
41 clients were discharged 16 clients received follow up phone calls (39%) 8 were re-admitted within 30 days (20%)

THE CHANGE – JULY 2011 43C staff were asked to contact clients within
72 hours of discharge to offer support and/or assistance. Results:
41 clients were discharged 30 clients received follow up phone calls (73%) 7 were re-admitted within 30 days (17%)

NEXT STEP Research shows that follow up after
hospital discharge is proven to be effective in reducing recidivism
The team will continue to ADAPT the change to find an effective technique for recidivism reduction at Milwaukee County Mental Health Complex
Next Change Project: CLASP Program A more intensive follow up program with face-to-face plus
phone follow ups by peers.
