1. About the Document

1.1 Overview

Citizen Login for Issuance of Fitness Certificate of Department of Medical health and Family Welfare, Uttar Pradesh is an online web application which is developed for Citizen. Purpose of development of this web application is to generate Fitness Certificate.

1.2 Scope of the Document

This document provides step by step guide on how to use the Online Web based Software Application through Citizen Login.

1.3 Intended Audience

The Authorized User or Registered User (citizens of Uttar Pradesh) would be intended audience for this Citizen Login of Department of Medical health and Family Welfare, Uttar Pradesh.

1.4 Document Convention

This User Manual has following conventions: a. Fields which have * sign at the end; indicate that those fields are mandatory. b. Error Messages will be displayed in Pop-up box. c. Success Messages will be mentioned in pop-up. d. All the menu links will be mentioned in the side menu.

1.5 System Requirements

To run this application there are some basic requirements such as: a. Window’s Machine b. Firefox 30 or above / Chrome 32 or above c. MS Office (2007 or Above) d. Internet Connectivity (256 Kbps or above)

2. About Death Certificate

2.1 Introduction

A Death Certificate is a document issued by the Government to the nearest relatives of the deceased, stating the date, fact and cause of death. It is essential to register death to prove the time and date of death, to establish the fact of death for relieving the individual from social, legal and official obligations, to enable settlement of property inheritance, and to authorize the family to collect insurance and other benefits.

2.2 User/Applicant Type

Citizens can not apply for Death Certificate through this web application. These rights have been reserved with CMS or CMO login.

2.3 Terms & Conditions / Eligibility

Before you start filling the application form, please keep the following documents handy with you.

Death Person Identity Proof Document Requester Person Identity Proof Document Note: The file size should be in .jpg and .pdf format and maximum file size

should be 2080 KB (2 MB). After submitting the Application form you can check the status of the form

by re-login and click on “View Application Form Status”. Once the application form is accepted by the CHC/DH, you will get the

notification about the Inspection Date on your registered mobile number or you can check the status by login into the account.

On the basis of the Inspection report, CMO will accept/reject the application form.

In case your application is approved, please get in touch with the office of Chief Medical Officer to collect your Certificate. You can also download the Certificate from your Dashboard.

In case your application form is rejected by the Chief Medical Officer, you can contact with the support team.

3. How to Apply

3.1 Accessing Portal

To access the portal, type the address in the address bar as: http://up-health.in. User will be redirected to the homepage of the online application. Once user will click on ‘Link (mentioned below in image)’ he/she will be navigated to Login page.

Click on the Citizen Login link as mentioned in the image after which page will be navigated to Citizen Login page.

3.2 Citizen Login Page (Registered User)

Citizen Login Page will be displayed as shown below:

Enter Registered Mobile Number, Password and Captcha and then click on Login button.

3.3 New Registration Link (New User)

Click on For New Registration link to register as a new user.

Enter Registered Mobile Number, Password and Captcha and then click on Login button.

3.4 Citizen Registration form

Citizen Registration page will be displayed as shown below:

Fill all the details and Click on Register button to get registered as a new user.

Click on Reset button to reset the details.

3.5 Mobile Verification form

After completing the Registration Form an OTP will be sent on mobile number entered by you for mobile verification process. Mobile Verification page will be displayed as shown below:

Fill the details correctly and click on Register button to register as a new user.

Enter the OTP and click on Verify button. Enter OTP and click on Verify button. Click on Resend OTP if no OTP is

received.

Note: User will have to login as mentioned in Point no. 4.2 after mobile verification.

3.6 Dashboard After login user will be redirected to the dashboard. Dashboard will be displayed as below :

Click on the Issuance of Death Certificate tab to proceed.

Click on the Issuance of Death certificate


Enter OTP and click on Verify button. Click on Resend OTP if no OTP is received.

Note: User can download the Death Certificate from this login. He/she can’t

fill the application form for the Issuance of Death Certificate. These rights have been reserved with CMO or CMS login.

a. Death Certificate Dashboard After clicking on Issuance of Death Certificate tab user will be redirected to the

Death Certificate Dashboard. Page will be displayed as shown below:

Click on Download Certificate tab to download the certificate Click on View/Re-Download Certificate tab to perform the particular task. After Clicking on Download Certificate tab user will be redirected to the next step

automatically.

b. Filter Criteria After Clicking on Download Certificate tab user will be redirected to the Filter

Criteria page. Screen will be displayed as shown below:

Select and Fill all the details then click on Search button. Click on Reset button to reset the details. Searched Result will be displayed in grid view structure on the same page. Click on the Select Radio Button and then click on Get Certificate button.

c. Requester Details After Clicking on Get Certificate button user will be redirected to the Requester

Details page. Screen will be displayed as shown below:

Select and fill all the details, Upload the ID Proof and then click on Proceed button.

Click on Reset button to reset the details.

d. Mobile Verification After completing Requester Details user will be redirected to the Mobile

Verification page. Screen will displayed as shown below:


Fill the OTP at the given place then click on Verify button. Click on Resend OTP button to receive OTP again.

Select and fill all the details, Upload the ID Proof and then click on Proceed button.

e. Download Certificate After completing the Mobile Verification, user will be redirected to the

Download Certificate page. Screen will be displayed as shown below :
