Four-step application process:1. Complete and submit the pre-approval application.
Submit a pre-approval application and all required documentation beforemaking any purchases or starting any work on the project.
� General information (page 4) and appropriate pages for your project.
� Prescriptive (pages 6-20)
� Custom (page 22)
� An itemized quote from selected vendor that includes the quantity, size,type, make and model of proposed items and labor costs, if applicable.Internal labor cannot be included in project cost.
� The manufacturer (OEM) specification sheets for all eligible measures perprogram guidelines.
� A copy of one months’ utility bill for prescriptive projects or 12 consecutivemonths’ of utility bills for custom projects. The first two pages aresufficient if they include a summary of energy use and charges.
� Incomplete applications will result in delays.
2. Obtain pre-approval notice.Upon verifying that the project meets the program requirements, the customerlisted on this application will receive a letter via email (or standard mail ifemail is unavailable) indicating the funds have been reserved for this project.
� This letter must be signed and returned within 10 days.
� From the date of issuance of this letter, you will have 90 days oruntil November 30, 2019 to complete the project and submit all finaldocumentation. Applicant may submit a written request for one 30-dayproject completion extension.
� After 90 days or November 30, 2019, this application will expire and thereserved funds will be forfeited.
3. Install your project.After signing and returning pre-approval notification, install the newequipment and remove the old equipment, Incomplete applications will resultin delays.
� Only new products, which are exact product types listed in this application,are eligible for Prescriptive incentives.
4. Complete and submit the final application.� Applicants must participate in an exit interview with Efficiency United staff
to identify incentive opportunities and document a Joint Energy EfficiencyPlan (JEEP).
� Submit the final application (page 23), including all project invoices, afterthe project is completed.
� Incomplete applications cannot be processed for payment.
Eligibility:1. Your business must be a
current commercial customer ofUpper Peninsula Power Companyto participate.
2. Project must be installed andcompleted between January 1, 2019and November 30, 2019.
3. Application documents must bereceived within 60 days of purchaseand/or installation of measures.
4. Equipment must meet all the statedeligibility requirements in the catalogto qualify for a rebate.
5. Rebate cannot exceed 75% of the totalcost of the project.

ELIGIBILITY: Efficiency United energy efficiency incentives are offered to qualifying electric customers only while funding is available. Projects must be implemented (completed) by November 30, 2019. All application documentation must be submitted within 60 days of project completion or December 1, 2019. Projects must reduce electric usage through system efficiency improvements, control upgrades may also qualify. The following measures will not qualify: peak shaving, demand reductions, fuel switching, power generation, renewable energy, or operating schedule changes. Equipment must meet the minimum number of annual operating hours. Customer must participate in an exit interview with Efficiency United staff to identify incentive opportunities and document a Joint Energy Efficiency Plan (JEEP).
COMPLIANCE: All projects must comply with applicable federal, state, and local laws and building codes. All equipment must be new or retrofitted with new components per program specification. Used or rebuilt equipment is not eligible for incentives. Existing equipment must be removed or permanently disconnected. New equipment must meet specification requirements and existing equipment must be operational when the application is submitted. Only one incentive will be granted for each project. Customers can submit multiple projects in a calendar year; however, the incentive totals cannot exceed the incentive limit cap per year. If the project is in a leased building, the lease term must be at least three (3) years and a copy of the lease may be requested. Up to 24 months of utility usage information may be requested.
INCENTIVE CAPS: Incentive caps may not exceed 75% of the total projectcost, including materials, external labor, permits, equipment rental ordisposal. Custom incentives will not be provided for projects with lessthan a 1-year simple payback or greater than an 8-year simple payback.Incentives may be limited by a tiered cap system depending on theenergy savings of the project(s), or while Program funding is available.
INSPECTION: Program staff reserves the right to conduct pre-inspections and post inspections of proposed and installed projects.
PUBLICITY: Efficiency United reserves the right to publicize your participation in this program, unless you specifically request otherwise.
LOGO USE: Customers or allies may not use the Efficiency United name or logo nor that of any participating utilities in any marketing, advertising, or promotional material without written permission.
PROGRAM DISCRETION: Incentives are available on a first-come, first-served basis. Incentive amounts and offerings are subject to change or termination without notice at the discretion of Efficiency United.
PAYMENT: Once completed paperwork is submitted, incentive payments are usually made within 6 to 8 weeks. Incomplete applications will delay payments or result in denial of application approval. Efficiency United reserves the right to refuse payment and participation if the customer or contractor violates program terms and conditions. Efficiency United must receive 100% of the energy savings for the rated life of the product(s) or for a period of three (3) years from the receipt of incentive, whichever is less. If you do not provide the energy savings, the facility in which the installed projects are located closes or ceases operation within three (3) years from receipt of incentive, or you cease to be a customer of a participating Efficiency United utility during the three (3) years, you shall refund a prorated amount of the incentive.
RELEASE/INDEMNIFICATION: Payment of rebates under the Program and/or evaluation of applications for rebates shall not deem Efficiency United or any of its affiliates, employees, contractors or agents (“Efficiency United Parties”) to be responsible for any work completed in connection herewith. Applicant fully releases Efficiency United Parties from any and all claims it may have against Efficiency United Parties in connection with this application, the rebates or the work performed in connection with them. In addition, applicant agrees to defend, indemnify and hold Efficiency United Parties harmless from and against any and all claims, losses, demands or lawsuits by any third parties arising in connection with this application, the payment or non-payment of rebates, or any work performed in connection with them. The customer hereby releases Efficiency United and participating utilities from any and all liability arising from or connected with releasing the information to Efficiency United set forth herein.
Efficiency United is a program administered and implemented by CLEAResult and the Michigan Community Action Agency Association pursuant to a contract with the State of Michigan, Department of Energy, Labor & Economic Growth, and in compliance with PA 342 of 2016.

The intent of this document is to assist customers in identifying future energy reduction projects within the next three years. Doing so gives Efficiency United the opportunity to assist in the development and implementation of a Joint Energy Efficiency Plan (JEEP) with customers. The goal is for the customer to develop an implementation plan with the help of Efficiency United for the identified projects.
The customer has set a goal to reduce its existing electric usage by _______________% and/or natural gas use by _______________% within the next three years.
Commencement DateBy choosing to develop a JEEP, the date of this Agreement will become the commencement date for the projects identified. This commencement date for projects will be indicative of when Efficiency United and the customer acknowledged the projects were under consideration and verified the incorporation of energy efficiency options into equipment purchases.
This can serve as a conditional pre-approval date for technical opportunities that may be considered for Efficiency United customized rebates. This eliminates a situation in which the pre-approval occurs after equipment is ordered or purchased, thus avoiding the possibility of not receiving an incentive. This however does not guarantee the project will qualify for a rebate, nor does it guarantee implementing the project will result in energy savings. All projects identified in this Agreement will still need to go through the pre-approval process to determine if the project will qualify for an incentive, the incentive amount and to reserve appropriate funding. Technical opportunities identified and scoped through work performed as part of one of the studies included in this plan will be subject to this date of influence.
Access to Utility DataIn order to assist the customer with the development and implementation of a JEEP, customer agrees to provide Efficiency United with the 12 most recent months of electric and natural gas cost and consumption information. This information can be provided in the form of copies of utility bills or in a spreadsheet summary.

Commercial and Industrial Energy Efficiency Program
ELIGIBILITY: Efficiency United energy efficiency incentives are offered to qualifying electric customers only while funding is available. Projects must be implemented (completed) by November 30, 2019. All application documentation must be submitted within 60 days of project completion or December 1, 2019. Projects must reduce electric usage through system efficiency improvements, control upgrades may also qualify. The following measures will not qualify: peak shaving, demand reductions, fuel switching, power generation, renewable energy, or operating schedule changes. Equipment must meet the minimum number of annual operating hours. Customer must participate in an exit interview with Efficiency United staff to identify incentive opportunities and document a Joint Energy Efficiency Plan (JEEP).
COMPLIANCE: All projects must comply with applicable federal, state, and local laws and building codes. All equipment must be new or retrofitted with new components per program specification. Used or rebuilt equipment is not eligible for incentives. Existing equipment must be removed or permanently disconnected. New equipment must meet specification requirements and existing equipment must be operational when the application is submitted. Only one incentive will be granted for each project. Customers can submit multiple projects in a calendar year; however, the incentive totals cannot exceed the incentive limit cap per year. If the project is in a leased building, the lease term must be at least three (3) years and a copy of the lease may be requested. Up to 24 months of utility usage information may be requested.
INCENTIVE CAPS: Incentive caps may not exceed 75% of the total projectcost, including materials, external labor, permits, equipment rental ordisposal. Custom incentives will not be provided for projects with lessthan a 1-year simple payback or greater than an 8-year simple payback.Incentives may be limited by a tiered cap system depending on theenergy savings of the project(s), or while Program funding is available.
INSPECTION: Program staff reserves the right to conduct pre-inspections and post inspections of proposed and installed projects.
PUBLICITY: Efficiency United reserves the right to publicize your participation in this program, unless you specifically request otherwise.
LOGO USE: Customers or allies may not use the Efficiency United name or logo nor that of any participating utilities in any marketing, advertising, or promotional material without written permission.
PROGRAM DISCRETION: Incentives are available on a first-come, first-served basis. Incentive amounts and offerings are subject to change or termination without notice at the discretion of Efficiency United.
PAYMENT: Once completed paperwork is submitted, incentive payments are usually made within 6 to 8 weeks. Incomplete applications will delay payments or result in denial of application approval. Efficiency United reserves the right to refuse payment and participation if the customer or contractor violates program terms and conditions. Efficiency United must receive 100% of the energy savings for the rated life of the product(s) or for a period of three (3) years from the receipt of incentive, whichever is less. If you do not provide the energy savings, the facility in which the installed projects are located closes or ceases operation within three (3) years from receipt of incentive, or you cease to be a customer of a participating Efficiency United utility during the three (3) years, you shall refund a prorated amount of the incentive.
RELEASE/INDEMNIFICATION: Payment of rebates under the Program and/or evaluation of applications for rebates shall not deem Efficiency United or any of its affiliates, employees, contractors or agents (“Efficiency United Parties”) to be responsible for any work completed in connection herewith. Applicant fully releases Efficiency United Parties from any and all claims it may have against Efficiency United Parties in connection with this application, the rebates or the work performed in connection with them. In addition, applicant agrees to defend, indemnify and hold Efficiency United Parties harmless from and against any and all claims, losses, demands or lawsuits by any third parties arising in connection with this application, the payment or non-payment of rebates, or any work performed in connection with them. The customer hereby releases Efficiency United and participating utilities from any and all liability arising from or connected with releasing the information to Efficiency United set forth herein.
Efficiency United is a program administered and implemented by CLEAResult and the Michigan Community Action Agency Association pursuant to a contract with the State of Michigan, Department of Energy, Labor & Economic Growth, and in compliance with PA 342 of 2016.

2018 Custom Project Specification

� All custom projects must be facility improvements that result in a permanent reduction in electric energy usage due to an increase in systemefficiency. Projects that result in reduction of energy consumption without an improvement in system efficiency are not eligible for a customincentive. However, projects involving automated control technology may be eligible for incentives. All equipment purchased for customprojects must be new. Projects that entail measures covered by the prescriptive rebate program are not eligible for custom incentives.
� The annual electric savings must be calculated for all custom projects using industry accepted engineering algorithms and/or simulationmodels. Calculations must be completed for both the existing and proposed equipment/system based on the current operation of thefacility. If the equipment has reached the end of its useful life, the existing system must be substituted with equipment that would meet theapplicable federal and local energy codes when calculating the annual energy savings.
� New lighting fixture change-outs and/or retrofits that do not fall into one of the prescriptive lighting categories are eligible for customincentives. Each area receiving upgrades must be assessed separately and, if applicable, all proposed technologies must meet therequirements for any prescriptive measure featuring those same technologies. Technologies that feature certification and/or approval fromENERGY STAR, DLC or CEE must include products approved by the appropriate certifying body or meet minimum requirements for theapplication. Retrofit of existing T12 fluorescent lighting systems with T5 fluorescent lighting systems and/or installation of T8 equipment notapproved by CEE as HP or RW do not qualify.
� All calculations, metered data, equations and assumptions must be submitted with the application along with their sources, if applicable.Efficiency United is solely responsible for the final determination of the annual energy savings to be used in calculating the rebate amount.Preliminary and post inspections are required to verify equipment and operation conditions. Efficiency United reserves the right to requirespecific measurements and verification of measures, including monitoring both before and after the completion of the project. The rebatepayment will be based on the result of the above-mentioned activities.
� Project payback is equal to the ratio of the project cost divided by the annual energy bill savings. To qualify for a custom project, the projectpayback must be at least one year and no more than eight years. A pre-application is required for all custom projects while the existingequipment is still in operation in order to allow Efficiency United the opportunity to verify the existing equipment.
� The following types of projects do not qualify for energy efficiency rebates� On-site electricity generation� Renewable energy� Fuel switching (i.e. natural gas to electric, electric to natural gas)� Changes in operational and or/maintenance practices, or simple control modification that do not involve capital cost� Projects that involve peak-shifting and not actual kWh reduction
