PROFESSIONAL ACTIVITIES CLINICAL Director, Obstetrical Anesthesia, Moffitt-Long (Children’s) Hospital Birthing Center: I provide clinical care and faculty supervision for anesthesia residents and medical students in the UCSF Birthing Center about 3 days per week. My academic career has been focused on anesthetic care for pregnant women; I have authored many chapters and co-edited a textbook about anesthesia for obstetrics. I am the primary teacher of obstetrical anesthesia at UCSF; all anesthesia residents spend at least one month on my clinical service and I am responsible for their education in this important clinical subspecialty of anesthesiology. Fetal Treatment Center: I have been a member of the world-renowned UCSF fetal surgery enterprise since its inception, and a charter member of the UCSF Fetal Treatment Center. I pioneered methods to provide anesthesia for both mother and fetus undergoing these bold, complex procedures, demonstrating success in non-human, primate animal models before the UCSF team considered operating on pregnant women. I pioneered and provided anesthesia for the first human fetal surgical procedure to correct a simple, congenital anatomical fetal malformation that, if untreated, would have resulted in fetal or neonatal death. For the past 25 years, I have provided anesthesia for the vast majority of the hundreds of fetal procedures performed at UCSF for a broad diversity of fetal conditions. I have taught anesthesia for fetal procedures to faculty colleagues and residents at UCSF, as well as nationally and internationally through chapters in many textbooks, invited review articles, invited public lectures, clinical consultation and teaching visitors. The techniques I pioneered have become utilized worldwide.

Moffitt-Long Hospital Operating Room & Post-Anesthesia Care Unit (PACU): I provide clinical care and faculty supervision for anesthesia residents in the operating rooms and numerous other places in which anesthesia care is required for late running and emergency procedures during in-hospital, 24-hour clinical shifts about 1.5 days per month. During these shifts, I also supervise patient care in the PACU and provide emergent care for patients in the intensive care units and those who suffer cardiac or respiratory arrest (‘code blue’’). Mt. Zion Hospital Operating Room & Intensive Care Unit (ICU): I provide faculty supervision for anesthesia residents in the operating room, clinical care in the ICU and emergent care for patients who suffer cardiac or respiratory arrest (‘code blue’') during in-hospital overnight shifts about 1-2 days per month. SUMMARY OF CLINICAL ACTIVITIES My clinical activities are focused on anesthetic care for pregnant women undergoing labor, vaginal or operative delivery, fetal surgery or other surgical procedures during pregnancy. I specialize in anesthetic care for high-risk pregnancies, pregnancies complicated by co-existing diseases and anesthetic care for fetal surgeries. I have been the primary teacher of obstetrical anesthesia at the University for more than a decade. I also provide clinical care in the operating rooms, post anesthesia care units, intensive care units numerous other places in which anesthesia care is required in Moffitt-Long and Mt. Zion hospitals.

SUMMARY OF SERVICE ACTIVITIES I have been engaged in many University and Public service activities throughout my career at UCSF. Currently I serve as a Medical Student Career Advisor and Chair the Academic Standards Committee for the School of Medicine. Medical Student Career Advisors assist medical students with choices of specialization and the applications process for residency training. The Academic Standards Committee assesses medical students whose academic progress falls short of established standards, reviews the details of their progress and problems, and explores potential confounding personal problems or other problems that can contribute to impairment of academic progress. The Committee meets with the referred student and makes recommendations to the Dean about whether to offer special assistance, special leniency, some interventional therapy, or whether to proceed to dismissal. I also serve on the Dean’s Advisory Committee on Postdoctoral Education and have served on the admissions committee, ad hoc promotions committees and special grievance committees for the School of Medicine. At the Departmental level, I serve in a leadership role by directing the residency-training program, participating as a member of the Chairman’s Executive Committee and recently having organized a faculty retreat, at which I led a major session to educate the Department’s faculty about new teaching mandates for residency training from the Accreditation Council for Graduate Medical Education (ACGME), and now oversee a major re-engineering of our residency training program. As the residency director, I serve on numerous committees related to resident education and training. Serving in the primary role for the Department’s resident selection and recruitment process alone occupies the majority of my time about two months per year. I have given freely of my time to teach at local Bay Area and Northern California hospitals, speaking at their Grand Rounds, participating in their quality assurance program reviews and serving as a consultant, both formally and informally. I also participate with the UCSF Perinatal Outreach program to provide education at facilities that refer their complicated patients to the University and I teach for the Great Expectations Program for Parents about four times a year. I have provided substantial professional service to the American Board of Anesthesiology in its examination process for certification since 1983, for both the written and oral examinations. I served as a question writer, member of the editorial board, and council member for the examination as well as an examiner for the oral examinations, which occupies one week per year.

Total hours of teaching next fiscal year: >1120 hours Teaching Description Anesthesia 110 – Two-week Core Clerkship in anesthesia for all medical students in their 3rd or 4th year of medical school. This is an introductory course in anesthesia, focused on preoperative evaluation, intraoperative and postoperative management. Students spend the majority of the two weeks in the surgical operating rooms and spend one day in various anesthesia subspecialty venues. I teach the students who elect to spend their day in obstetrical anesthesia at Moffitt-Long Hospital (Children’s Hospital of UCSF). Anesthesia 140.07 - Twelve-week Advanced Clerkship in general anesthesia for medical students who are in the last three months of the medical school phase of the integrated training program in Oral and Maxillofacial Surgery. During this three-month period, students become familiar with administration of general anesthesia for basic surgical procedures. Anesthesia 140.08 - Four-week, advanced elective in obstetrical anesthesia for medical students. The objectives for this course include learning physiologic and anatomic changes that take place during pregnancy, placental physiology and transfer of drugs, their effects on the fetus and newborn, newborn assessment and resuscitation, management of labor analgesia, cesarean section and management of high risk and emergency obstetric patients.

Anesthesia 400 - Didactic lectures in sciences basic to the specialty of anesthesia, as well as case reviews, clinical discussions, and seminars on current medical literature. These are conducted in small group seminars on a, nearly daily basis with residents on my clinical service. Anesthesia 450 - Direct supervision of residents in the management of clinical anesthesia care of patients in the labor and delivery suites (UCSF Birthing Center), operating rooms, recovery rooms and other locations in which anesthesia is provided (cardiac catheter lab, neuroradiology suite, interventional radiology suite, MRI and CT scanners, in vitro fertilization lab, etc.) Obstetrics and Gynecology 400 - Daily teaching rounds with residents in obstetrics, gynecology and reproductive sciences and anesthesia residents on their obstetrical anesthesia rotation. Weekly one-hour participation in perinatal morbidity and mortality conferences attended by faculty, fellows and residents in pediatrics (neonatalogy), obstetrics (perinatology) and anesthesia (obstetrics). ADDITIONAL TEACHING FORMAL TEACHING POSTGRADUATE AND OTHER COURSES I co-organize and direct an annual continuing medical education conference and teach at numerous other continuing education courses offered by the Department of Anesthesia and Perioperative Care, the Department of Obstetrics, Gynecology and Reproductive Sciences, as well as by invitation at many other national conferences. This has been an important avenue for my teaching and updating the practice of anesthesia for fetal surgery, obstetrics, and surgery during pregnancy, newborn resuscitation and many related topics. By teaching in venues that have large audiences, as well as by writing for textbooks, I fulfill an important personal commitment to teaching and widespread dissemination of information. INFORMAL TEACHING CONSULTANT ACTIVITY International, national and local consultation for clinical care -- I am frequently contacted for assistance by clinicians confronted with complex issues pertaining to anesthetic management of pregnant women for labor and delivery, surgery during pregnancy, fetal surgery or other fetal procedures. FACULTY MENTORING A majority of the Anesthesia Department’s faculty are former UCSF residents. For many, as their Program Director during residency training, I spent considerable time as a mentor, and it has been a natural evolution that I informally mentor and counsel many of the faculty members of our Department for professional, as well as personal, issues. (20 hours / year) RESIDENT MENTORING I spend a considerable amount of time mentoring residents. As the Residency Director, I have 72 trainees under my supervision, 24 residents per year for a three-year program and I mentor the majority of our residents for a variety of matters, including educational and academic assistance to help improve their overall academic achievement, career mentoring and/or assistance with securing employment after residency training, as well as personal development mentoring to support residents during times of personal and academic stress or pressure, or for guidance in decision making about career and life issues. I approach mentoring as a guide to residents through a productive thinking, learning, and problem-solving process. Our Residency has a well-established program of planned mentoring that occurs through a structured program in which mentors and participants are selected and matched through a formal process in which a formalized faculty mentor is identified for each resident to facilitate closer faculty-resident relationships and individual counsel for each resident. For the most part, my involvement with residents can be characterized as a more natural mentoring that occurs through friendship, collegiality, teaching, coaching, and counseling. My role as the Residency Director positions me as a contact for many private practices across the country, and therefore as an established and knowledgeable career advisor. Additionally, as the Residency Director I have counseled residents in academic difficulty, residents with attitude or behavioral problems, confronted residents with concern for substance abuse and I’ve counseled residents with personal issues or problems ranging from pregnancy to dying, death of family member, marital discord, depression, angst about career choice, and for numerous other reasons. (80 hours / year) MEDICAL STUDENT MENTORING Each year, about a dozen UCSF senior medical students are interested in applying for anesthesia residency-training programs. Through the Department’s Medical Student Anesthesia Interest Group, I meet with a group of these interested medical students twice per year to discuss a career in anesthesia and guide them about the application process. Further, I meet with most of the students individually to help them assess their qualifications as an applicant, assist them in preparing an application, and guide them in selecting suitable places for them to apply, based on their academic and professional achievements, goals as well as the many other personal factors involved in such a decision, such as location. I spend about an hour with each student. I directly assist most of the students plan and write their personal statements (I edit many of them), and help them select appropriate faculty for letters of reference. I also see many students from other medical schools that come to UCSF for student externships, counseling them about applications in a similar, but more abbreviated fashion. (20 hours / year)

I have collaborated with Dr. Michael Harrison (UCSF Department of Surgery) in this innovational UCSF program since its inception. We demonstrated the crucial importance of skillful and inventive management of anesthesia and intra-operative uterine muscle relaxation for fetal survival in non-human primates, before embarking on our first human clinical fetal surgery. I've provided creative solutions for clinical problems of adequate intra-operative fetal monitoring and intense uterine muscle relaxation (atony; tocolysis) that has been necessary for surgical success. I have been principal in providing anesthesia care to mothers and their unborn fetuses for a variety of congenital abnormalities, many undergoing procedures in a variety of NIH funded trials, including congenital diaphragmatic hernia (CDH) repair, tracheal occlusion for CDH, and presently for myelomeningocele repair and late-gestation tracheal occlusion for CDH. I'm internationally recognized as the pioneer in anesthesia for fetal surgical procedures. Commercial fetal pulse oximetry, recently introduced in the United States resulted from my early work with the Nellcor Company (early 1980's) and my designs of reflectance (rather than transmittance) pulse oximeters. My work has involved significant refinement of anesthetic and intraoperative tocolytic techniques for treatment of congenital diaphragmatic hernia (CDH) in utero, and development of maternal and fetal anesthetic techniques to accommodate EX-utero Intrapartum Treatment (EXIT procedures) for fetuses that undergo in utero tracheal occlusion. Success of the tracheal occlusive procedures required successful modifications of conventional anesthetic techniques for cesarean delivery to maintain the fetal-placental circulation for as long as two hours after 'birth' to ensure the fetal airway is surgically established and secured. Funded Research Participant in NIH funded fetal surgery trials for myelomeningocele repair and management of twin-twin transfusion syndrome. Most Recent Publications Rosen MA: Anesthesia for fetal procedures and surgery. Yonsei Med J 2001; 42:669-680 This invited review article allowed me to summarize the anesthetic achievements for fetal surgery, including techniques developed to provide sufficient time for surgical establishment of a neonatal airway while oxygenation for the 'newborn' is maintained by umbilical blood flow and gas exchange in the placenta. This required anesthetic techniques to prevent the uterus from contracting (which is the normal response to delivery), yet avoiding excessive bleeding from uterine atony associated with halogenated anesthetic agent administration. Techniques for fetal monitoring and anesthetic management of the mother are summarized. Rosen MA. Nitrous oxide for relief of labor pain: A systematic review. Am J Obstet Gynecol 2002; 186:S110-26 This was a Cochrane style review of the world’s literature about use of nitrous oxide for labor analgesia, a technique widely utilized in the United Kingdom, Scandinavia, Canada, and Australia but not in the United States. The author uses nitrous oxide in his practice at UCSF. Rosen MA. Paracervical block for labor analgesia: A brief historic review. Am J Obstet Gynecol 2002; 186:S27-30 This was a review of the use, efficacy and side effects of paracervical block techniques for labor analgesia, a technique that is rarely used in the United States, but much more popular in other parts of the world. By invitation, these two articles were prepared in a Cochrane review format for oral presentation at the New York Academy of Medicine in a symposium co-sponsored by the Maternity Association of America. Since publication, these systematic literature reviews have generated considerable national discussion, consultation and media attention (print, radio and television). Lee SJ, Ralston HJP, Drey EA, Partridge JC, Rosen MA. Fetal Pain: A Multidisciplinary Review of the Evidence. JAMA 2005; 294:947-54 Proposed federal and state legislation would require physicians to inform women seeking abortions at 20 or more weeks after fertilization that the fetus feels pain and to offer anesthesia directly to the fetus. We assembled a multidisciplinary team to examine the evidence about whether a fetus feels pain and if so, whether safe and effective techniques exist for providing direct fetal anesthesia or analgesia in the context of abortion. This paper has generated widespread, international media attention and has contributed to a national political debate about abortion. This paper represents an area of interest I’ve long held and addresses clinical questions I’ve long considered. I have consulted to numerous national organizations about the issues discussed in this paper, and presently I am being sought to testify before many state legislatures, and possibly the US Congress about issues addressed in this paper.

Golombeck K, Ball RH, Lee H, Farrell JA, Farmer DL, Jacobs VR, Rosen MA, Filly RA, Harrison MR. Maternal morbidity after maternal-fetal surgery. Am J Obstet Gynecol 2006;194:834-9. There is a paucity of published data on the maternal risks of fetal surgical interventions. We analyzed maternal morbidity and mortality that were associated with different types of fetal intervention (open hysterotomy, various endoscopic procedures and percutaneous techniques) to quantify this risk. Short-term morbidities include increased rates of cesarean birth, treatment in intensive care, prolonged hospitalization, and blood transfusion, all of which were more common with hysterotomy compared with other techniques. Maternal-fetal surgery can be performed without maternal death. Results from this study provide helpful data for counseling prospective patients.

Educational Administration: Program Director, Anesthesia Residency Training Program The program director is responsible for the program and accountable for the operation of the program, general administration of the program, and for the establishment and maintenance of a stable educational environment. The program director must possess faculty experience, leadership, organizational and administrative qualifications, and the ability to function effectively within the institutional governance. The program director must have significant academic achievements in anesthesiology, such as publications, the development of educational programs, or the conduct of research. The program director must oversee and organize the activities of the educational program in all institutions that participate in the program. This includes selecting and supervising the faculty and other program personnel at each participating institution, appointing a local site director, and monitoring appropriate resident supervision at all participating institutions. The program director is responsible for preparing an accurate statistical and narrative description of the program as requested by the RRC, as well as updating annually both program and resident records through the ACGME's Accreditation Data System. The program director must ensure the implementation of fair policies, grievance procedures, and due process, as established by the sponsoring institution and in compliance with the Institutional Requirements. The program director must seek the prior approval of the RRC for any changes in the program that may significantly alter the educational experience of the residents. In the past several years, with increasing educational mandates and the institution of competency-based assessments by the Accreditation Committee on Graduate Medical Education (ACGME), much more of my time has been focused on restructuring and reorganization of the Department’s residency training program. I have been developing educational goals and objectives for our residency training program, as well as competency-based criteria for measuring outcomes to define more specifically our success in clinical training, and the desirable volume and distribution of clinical work. This has resulted in less productivity in creative activities in other areas of my career. Since 1983, I have been directly responsible for resident selection and residency co-ordination in the Department of Anesthesia, and for more than 15 years I have formally served as the Director of the Residency Training Program in the Department of Anesthesia and Perioperative Care at UCSF. This involves overseeing the selection, employment and education of residents, overall program design and organization of all clinical and didactic activities of the educational program, including clinical rotation development, resident clinical assignments, and the logistic organization of 72 residents at 5 institutions (UCSF, SFGH, SFVAH, Mt. Zion Medical Center,

Oakland Children’s Hospital), in thirty different clinical rotations over a three-year training period. I oversee all the requisite institutional affiliation agreements, resident contracts, resident employment duty hour compliance and resident welfare, required reporting to the ACGME, RRC and American Board of Anesthesiology, and the logistics and supervision of a non-academic staff of three full-time administrative assistants. I also oversee other faculty members who have primary responsibilities for chairing committees for resident didactic education, evaluation and welfare. Program Design & Leadership in Residency Teaching and Evaluation: Most recently, I organized a faculty retreat (Fall 2004) and led a full, half-day session to educate and update faculty regarding the Accreditation Council for Graduate Medical Education (ACGME) Clinical Competencies and new teaching mandates. I assigned faculty to committees to address these mandates and I have sustained an overall leadership role in their implementation and a major restructuring of the residency-training program. I have attended national conferences devoted to these topics for the past two years. Resident Selection: I read about 600 applications per year for our residency program and select about 100 applicants to invite for personal interviews. I coordinate all applicant interview-day activities and personally interview every invited applicant (about 14 full-days of interviewing per year). At the conclusion of the interview season, I create the final rank order list submitted to the National Resident Matching Program. Resident Training Rotations: I oversee thirty different resident rotations and coordinate their sequencing and the logistics of resident scheduling for each resident at each rotation over their three-year residency. I create an annual resident rotation schedule and meet with rotation directors to create and modify teaching goals and objectives, orientation guides and resident evaluation in clinical competency areas of patient care, medical knowledge, practice-based learning and improvement, professionalism, interpersonal and communication skills, and systems-based practice. Resident Logistics: I create and oversee an annual rotation assignment schedule and a monthly resident call schedule for each of the four teaching institutions and coordinate all resident vacations, meetings and absences. Resident Mentoring: described in the Mentoring section Resident Assessment: I serve as an ex officio member of the resident advising and evaluation committee. Prior to ACGME guidelines prohibitions, I served as the Chair of this committee. ACGME Re-Accreditation: The Accreditation Council for Graduate Medical Education (ACGME) is an organization responsible for the accreditation of residency education programs. Completion of an accredited residency program is a prerequisite for primary board certification and for certification in the majority of subspecialty boards. To maintain accreditation, residency programs are expected to comply substantially with the Program Requirements for their specialty, and the institutions sponsoring them are expected to comply with a set of Institutional Requirements. Compliance is measured through periodic review. The formal periodic review of programs involves an on-site inspection. It is based on a Program Information Form (PIF), a comprehensive self-study document prepared by the program being reviewed. The PIF and the site visitor’s report form the basis of the RRC’s review and accreditation decision. The site visit

consists of interviews with the program director, members of the teaching faculty, resident physicians, and often administrators and other key personnel. In preparation for site visitation by a representative of the Residency Review Committee (RRC) of the ACGME and re-accreditation, I oversee the massive undertaking of comprehensively chronicling our residency-training program in a program information form. The writing, data collection and collation involved in this task now occupy a major portion of my non-clinical time. Additionally, as the site visit date approaches, faculty and residents will need to be educated and prepared for meeting with the site visitor. Although these activities have not resulted in peer-reviewed publications, my experience and expertise have resulted in many consultations from colleagues across the country, counterparts at UCSF, one formal invited lecture in England, and widespread national recognition. I’m proud of the contributions I’ve made to the UCSF anesthesia residency program, which has sustained its international reputation as one of the finest training programs in the world. UCSF is one of the most highly sought and highly regarded residency training programs in the United States.
