UNIT OVERVIEW This section corresponds with Administrative Unit Assessment Document 1

Unit Mission Providing quality outpatient health, psychological, and prevention services is the primary focus of the Hazen

Center for Integrated Care. The staff of the Health Center, Counseling Center, and Prevention and Outreach

Services desire to enable a diverse student population to pursue the primary goal of acquiring an education with

minimal lost time or distraction due to physical, emotional, or social issues. The collaborative nature of these

programs enhances the overall health and development of the College at Brockport student.

Outcomes for this goal were met. 100 % of Hazen Health Center staff were successful in better utilizing the features of the Medicat system, specifically to document phone calls for prescription refills, send lab results via secure messaging and enter immunizations into Medicat.

CLOSING THE LOOP

Resources Used: describe what resources (human, financial, etc.) were used to pursue this goal

Human capital was needed to achieve all steps in the action plan of this goal. Lynne Maier and Cheryl Van Lare

distributed and scored pre and posttest taken by all Hazen Health Center staff. Lynne and Cheryl conducted follow

up conversations with staff on strengths, areas of improvement, and strategies identified to increase proficiency with

Medicat.

Key Findings: list key findings related to unit goal and outcomes (5)

100% of staff were successful in reviewing record - ability to refill Rx, updating medication allergies,

changing pharmacy, updating medication in Manage meds, ordering Rx in Rcopia, coding, use of ICD-10,

reviewing patient summary including allergies, medication lists, and secure messaging.

Additional practice is needed for use of favorite note and summary section in note section of patient chart.

Dissemination/Discussion of Key Findings: how and with whom were key findings shared and what were the

results of these discussions (6)

Results from the pre and posttest were shared shortly after they were graded. Lynne and Cheryl followed up with all

staff who took the pre and posttest to provide feedback and areas of growth. During these meetings it was

identified what areas still needed to be improved upon and how they can support staff who needed additional

training or practice with the software.

survey and collected responses. Sara Gleisle analyzed the data.

88% of respondents felt satisfied or very satisfied with the friendliness, courtesy, and helpfulness show by the staff

Findings: Overall students were pleased with their interactions and time spent at the Health Center. The respondents provided important information for ways to improve and better our services. Themes: Please list 1-3 themes you identified which emerged from the focus group dialogue(s).

Key Performance Indicators and Benchmarks Analysis

In addition to accommodating multi-complaint visits, we have increased the number of higher acuity visits resulting in visits that require a longer appointment and reduce the number of available appointments each day. For example:

Visits for Initial MH and the ongoing management of MH patients have increased, especially following the loss of Psych NP

Screening lower acuity illness visits via telephone and therefore illness visits seen in the HC are more acute and require longer visits

As a result of participating in the SafeZone training, 95% of Hazen staff will feel more confident in their ability to support students who identify within the LGBTQIA+ community.

Opioid usage continues to be a growing issue and concern. The Hazen Center for Integrated Care aspires to be

prepared in the event that an individual is suffering from an opioid overdose while also furthering the information

we currently have regarding usage trends among the college aged population and society at large.

Rationale description

The College Diabetes Network (CDN) is a non-profit organization whose mission is to provide innovative

peer based programs which connect and empower students and young professionals to thrive with diabetes.

Students coming to college with diabetes have a significant health concern that may be better managed by

working with other students in similar situations.

At Brockport, we want to work with students wanting to provide a network that helps students adjust to

living on their own, manage their diabetes, and make a healthy adjustment to college life.
