Woman, 22, was referred to a specialized service with allegedgum injury that had developed over 7 days. She also reporteddiscomfort, pressure, fever, and facial asymmetry. Intraoral exami-nation detected a firm nodule at the buccal aspect of the rightmandibular region that was attached to the bony tissues and wasreddish purple. It was covered by ulcerated mucosa and measured2 cm in diameter. Imaging studies revealed a radiolucent lesioninvolving the first and second premolars with disruption of corticalbone and bony crest. Incisional biopsy was performed. Histopath-ological and immunohistochemical findings were consistent withundifferentiated high-grade sarcoma. Due to the aggressiveness ofthe disease, the treatment of choice was hemimandibulectomy fol-lowed by mandibular reconstruction, neck dissection, and chemo-therapy. Subsequently, the patient suffered complications andpostponed aesthetic surgical interventions. During clinical follow-up for 10 months no signs of recurrence developed.

Man, 23, had a unilocular radiolucent lesion extending fromthe lower second premolar to the ramus of the mandibule thatinvolved the impacted third molar. After excisional biopsy, thedefinitive diagnosis was unicystic ameloblastoma with luminal andmural tumor growth. The epithelial component showed frequentsquamous metaplasia, resulting in an acanthomatous pattern, aswell as other histological variants, including basal cell and granularcell types. There were also hyaline ring granulomas in the fibrouswall. This granuloma has been described under various names,such as pulse granuloma and oral vegetable granuloma. Hyalinering granuloma is a rare oral lesion with a controversial etiologythat is characterized by the presence of rings of palely eosinophilicstructureless material with foreign body�type multinucleated giantcells around and within the rings. Occasionally the lesions occur inthe wall of inflammatory odontogenic cysts. No other report doc-uments their presence in unicystic ameloblastoma.

Odontogenic keratocyst (OKC) is a locally aggressiveodontogenic lesion lined by parakeratinized stratified squamousepithelium. Foci of calcification in the fibrous capsule of OKChave been reported previously, but are considered rare. Woman,25, had an OKC in the right posterior maxilla. Panoramic ra-diographs revealed a unilocular radiolucency involving uneruptedpermanent tooth #18 and extending to the maxillary sinus.
Computed tomography showed a hypodense lesion circumscribedby a hyperdense halo and content consisting of fluid material andcalcifications. Based on a suspected diagnosis of calcifying cysticodontogenic tumor, incisional biopsy was performed. Micro-scopic examination revealed a cystic cavity lined by parakerati-nized stratified squamous epithelium with a corrugated surface. Inareas of the fibrous wall, scattered irregular eosinophilic masses,dystrophic calcifications with circular shape, and hairpin-shapedstructures were found. With the definitive diagnosis of OKC, totalenucleation of the lesion was performed. The patient remainsunder follow-up.

In Brazil, the Public Health System is deploying an online in-formation system to schedule consultations. A regulatory systemsets the schedule depending on the urgency of the health problem asconveyed by the clinical description. In Florianópolis, SC, insuffi-cient descriptions of oral lesions have been identified as the greatestproblem in determining care priorities. Communication is essentialto the referral process, and failures can delay diagnoses and increasehealth care costs. Tools to guide health professionals to write referralletters can be useful to improve communication. This study evalu-ated the self-assessments of 32 dentists and 64 students of dentistryregarding their ability to describe oral lesions with no guide (“free”description) and supported by two different guide tools. Describingoral lesions using guide tools improves competence, ease, andagility during the process. The most detailed guide was consideredthe more effective support to fulfilling this task.

Verruciform xanthoma is an uncommon benign lesion ofunknown etiology that mainly affects the oral mucosa. Clinically,this painless plaque has a papillary or verrucous surface. Man, 52,was referred for evaluation of an asymptomatic lesion on thepalate. The lesion had appeared 6 years earlier, but had neverbeen treated. Intraoral examination showed a 0.8-cm, well-demarcated, rounded whitish plaque with a verrucous surface andreddish background on the left side of the hard palate. The patientdenied prior local trauma and reported tobacco use. Papillomawas the presumptive clinical diagnosis and an excisional biopsywas performed. Histopathological analysis revealed papillaryepithelial hyperplasia and foamy macrophages in the laminapropria papillary zone. After 1 month of follow-up there is nosign of recurrence. The histopathological characterization ofverruciform xanthoma is essential for the diagnosis, which clin-ically is often mistaken for a papilloma, condyloma, or wart.
