1. Introduction

Services of the technical supervision of the construction of a residential complex with built-in

trading and office premises, situated on the 100-A Vasylkivska street, in the Holosiyivsky district of

Kyiv according to the contract "About the provision of technical supervision services, respectively" to

the requirements of the current legislation of Ukraine.

2. Provided service

The process of execution of work on the construction site was planned on the basis of the schedule of

execution of works (Appendix No. 1 to the contract of the general contract, developed in аugust 2017).

2.1.1. Control of quality. Control over violation of the requirements of the agreed and approved

project documentation, construction norms, rules, standards and specifications, approved project, the use

of substandard building materials, structures, products.

2.1.2. The Construction readiness degree and determination of degree of backlog of the planned deadlines

for the completion of the relevant construction stage specified in the project documentation and

construction schedule, by providing the Customer with conclusions.

2.1.4. Control over the compliance of safety regulations during the construction, provided by the State

construction regulations and rules of Ukraine.

2.1.5. Technical supervision of the construction in accordance with the requirements of the approved

design documentation;

2.1.7. Verification of the availability of documents which confirm qualitative characteristics of structures,

products, materials and equipment used during the construction - technical passports, certificates,

documents that reflect the results of laboratory tests, devices and technologies in accordance with the

design documentation, legislation and boundary indicators of works cost.

2.1.9. Controlling measurements, carried out at the time of inspection by the state supervisory bodies and

departmental inspections, as well as the independent conduct of control measurements of performed

work;

P.2.1.7. Verification of the presence of documents which confirm qualitative characteristics of a

structure, products, materials and equipment, which are used during building of the facility, such

as technical passports, certificates, documents that reflect the results of laboratory tests, devices

and technologies in accordance with the design documentation, legislation and boundary indicators

of works cost. The quality of reinforcing products and concrete mix is controlled constantly as well as

laboratory analyses of materials. The quality of materials and products meets all necessary regulatory

acts, state standards passports, certificates.

P.2.1.10. Control over the maintenance of the general log of the construction works of the standard

sample. Verification of the necessary records in works logs, author's supervision monitoring ,

availability of necessary licenses;

The logs of author's supervision,production works, concrete, welding and joint works are being

conducted, there are no remarks.

P.2.1.4. Control over the compliance of safety regulations during the construction, provided by the

State construction regulations and rules of Ukraine.

The perimeter of the building's site, the area of production (floors), the work of the tower crane and

the assembly area of the reinforcement frame are fenced. The passages of the elevator's shafts are closed,

the overlap is installed in the mines.

All builders work in protective helmets, vests, belts and overalls. All floors are cleaned, staircases

are fenced, the construction site is in good condition. Toilets are installed in the construction camp and on

the upper floors of building; containers for household and solid construction waste are provided as well as

their regular export.

Logs of operational control over the state of occupational safety, registration of instructions on the

safety of work in the workplace, instructions for the right of movement of goods by a base and a screw

crane, the issuance of safety devices and personal protective equipment, periodic inspection of removable

load grip devices and containers, are kept. Logs of operational control over the state of occupational

are conducted. There is no remarks.

Fire safety of the construction camp. The register of instructions about fire safety is conducted.

Warehouses are equipped with extinguishers, fire-prevention shields are completed, "Instructions about

fire safety in office and household premises" are present. The condition of electrical networks in premises

which were mentioned earlier is normal. All house hold premises are grounded.

Conclusions of the section: In general, the quality of performed works corresponds to the

working project and operating norms, measures for the protection of labor and industrial

sanitation on the construction site in proper condition.

P.2.1.2. The construction readiness degree and determination of degree of back log of the

planned deadlines for the completion of the relevant construction stage, specified in the project

documentation and constructions chedule, providing the customer with conclusions.
