Introduction The health and care transition programme was established in September 2010 and ran until 31 March 2013. It affected 45,000 staff and more than 400 organisations across the NHS, ALBs, Local Government, Public Health and the Department of Health. The transition programme provided a governance framework to co-ordinate a number of distinct programmes of work across the health and care system to deliver the Government's vision to reshape healthcare so that it is more patient-centred, led by health professionals and focused on delivering world-class health outcomes. The transition programme plan – also known as the “transition roadmap” – provided an at-a-glance summary of the programme’s milestones. The “roadmap” recorded and plotted the key and critical milestones for delivery of the health and care reforms in the final year of transition following the Health and Social Care Act receiving Royal Assent in March 2012. The milestones plotted were identified from the plans of the distinct programmes of work which came together to form the health and care transition programme. The “roadmap” enabled progress to be tracked on the key activities involved in the implementation of the reforms and allowed interdependencies to be easily identified and managed. It was part of a core set of documents informing discussions at each of the transition governance boards. In May 2012, the Health and Social Care Reforms: Transition Programme Scheme for Publication included a commitment to publish the programme plan or “roadmap” when the transition programme closed in 2013. What follows is the “roadmap” as it was at the closure of the transition programme on 31 March 2013.
