Who we are and what we do

We are the CCGs in Norfolk and Norfolk

County Council. We plan and pay for

health and social care services in Norfolk.

Norfolk, Great Yarmouth and Waveney

has formed a Transforming Care

partnership in Norfolk. A partnership is

when different groups and people join

together.

We want to change the way health and

care services for adults, young people and

children with learning disabilities and

autism happen.

Some people with autism and learning

disabilities find it hard to communicate

and make friends. They might also have

mental health problems.

We have brought the Transforming Care

partnership group together to make sure

health and social care is run well in

Norfolk, Great Yarmouth and Waveney.

We have made a plan with the group to

make some changes to the way people

with learning disabilities and autism get

care and support.

The plan will happen over the next 3 years

– from 2016 until 2019.

What the plan is about

Making better care plans for people

with learning disabilities.

Giving people with learning

disabilities more choice in how they

get help and support.

Having a plan means that people with

learning disabilities and autism can get

better care and support closer to home.

For example, it means they can get care

in the community where they live, instead

of being in hospital.

Some people might still need to be in

hospital but we think it is better if people

can also get care at home or in the

community where they live.

Our plan says how

We are working towards keeping

more hospital beds free for people

who need them.

Services will change so that people

with learning disabilities and their

families get the care and support they

need in the community.

We want people with learning disabilities

to

Get the right care in the right place

– we want them to be able to speak

out if they want to leave hospital and

get care closer to where they live

Get the best care possible – we will

make regular checks that care

services are looking after people in

the best way

We will make sure that

Services who fail people with learning

disabilities have to say why.

We have the best staff and train them

to know more about how to care for

people with learning disabilities and

mental health problems.

We collect more information about

people with learning disabilities so we

know more about what they need.

Making the plan work

We have done a lot of work this year to

make changes to healthcare, but there is

more to do.

We are setting up a strong group to take

charge of this work. This group will meet

once a month to check that what is in the

plan happens.

We will ask other groups and services to

make sure they think our plan is working

well too.

These groups will be made up of different

people who know a lot about caring for

people with learning disabilities and

autism.

We are working with NHS England to

make sure our plan works well and the

changes to health and social care happen.
