Task 1: Observe, report and document changes For this task, you must demonstrate that you have observed, reported and documented changes in the condition of a person you support. These changes can be physical, behavioural, environmental or emotional.
You will be observed demonstrating this task and the verifier or assessor may also ask questions to clarify your understanding of the tasks you are carrying out. Your observation, report and documentation must comply with your organisation’s policies and procedures.
