Registration form

Special discount: For every three registrants from your agency, get a fourth for half price.The discount will be applied to the lowest fee and must come in the sameenvelope from the same office address.

Members of Wisconsin Professional Society on the Abuseof Children: May subtract $10 from your registration fee. Your membership will beverified against a WIPSAC member list.

Members of Prevent Child Abuse Wisconsin:May subtract five percent from your registration fee. Your membership willbe verified against a PCAW member list.

Cancellations:Full refunds will be received upon receipt of written request received byFriday, March 16. Refunds after March 16 will be assessed a $25processing fee. No refunds after Friday, March 30. Substitutes accepted.

No-shows are responsible for the full fee.

Scholarships: Submit completed scholarship application form to PCAW. Request anapplication form by calling (800) CHILDREN. Deadline is Thursday,March 1.
