EDUCATIONAL ACTIVITIES

1. Curriculum /Course Development Digital Media Education in Teaching

Exploring digital audiovisual technology applications to dermatology surgery training, resident education, and general patient education. We aim to use digital capture of audio and video to create an interactive web-based format for education. These menu-driven sites will be accessible via the internal Mayo Web. Physician oriented sites will have a virtual reality classroom where medical and surgical topics may be learned. Patient oriented sites will have a variety of condition specific education topics. The key features will be its interactive format, accessibility from any computer site, and of course clarity with digital signals.

Dermatology Surgery Video Series Developed in collaboration with Mayo Audio-Visual, a portable broadcast quality video

recording unit. This unit will record a diverse and comprehensive range of dermatologic procedures. These may include but are not limited to Mohs micrographic skin cancer surgery, basic excisions to complex reconstruction, dermabrasion, laser therapy for vascular lesions, tattoos, photoaging, pigmented lesions, hair removal, scar revisions. Other surgical techniques such as liposuction, hair transplantation, ambulatory phlebectomy, sclerotherapy will also be included. Once edited and organized, these tapes will greatly augment resident and medical student education in dermatology surgery. The Dermatology Surgery Fellow will be an integral part of producing these series. Potentially, these tapes may also be marketable nationally and applicable to other specialties such as Plastic Surgery, ENT, and Oral Surgery. This project is ongoing.

Pig Flank Utility in Dermatology Surgery Education Background : Training our dermatology residents in excisions flaps, and reconstruction

occurs during their 2 month core surgery rotation as a 2nd year resident. Didactic lectures also supplement these topics. Other practical experiences include pigs’ feet sessions. Pigs’ feet, however, are sub-optimal as the adherent skin and lack of subcutaneous tissue does not simulate well human tissue movement. Fresh frozen cadaver heads are the best medium but are prohibitively expensive for routine use.

Innovation: The Cardiology research labs regularly sacrifice pigs in their experiments. After some inquiry, pig flanks are now obtained without cost after these sacrifices for use in

practical sessions. Pig flanks are superior tissue for surgical training as they include large areas with bone, muscle, subcutaneous fat, and overlying epidermis and appendages. Normally discarded materials are now an important part of our residents’ surgical training.

Cosmetic Surgery Curriculum, Dermatology Residency Education Developing and or co-directing “after hours (after 5 pm)” practical sessions for resident

education. These sessions involve recruiting patient volunteers for some sessions, or acquiring pig flanks for other sessions.

The sessions are preceded by didactic education on the related topic, followed by a supervised hands-on session. Sessions usually are 2-3 hours each depending on the subject. Below are the after hours sessions throughout the academic year.

Surgery Core Curriculum. Department of Dermatology. Residency curriculum presentations Preoperative evaluation and local anesthesia . Understanding of local anesthesia and nerve

blocks, evaluating patients preoperatively.1 hour didactic

Cutaneous laser surgery . Laser physics and tissue-light interactions as well as therapeutic applications of laser surgery are presented. 1 hour didactic. Supplemented by practical session using various lasers on fruits and volunteer patients (1-2 hours additional)

Hair transplantation . Art and science of hair transplantation for androgenetic alopecia. 1 hour didactic

Liposuction and sclerotherapy . Evolution and current practice of liposuction and sclerotherapy. Risks, benefits, and complications of both procedures are presented. 1 hour didactic. Supplemented by both a liposuction and sclerotherapy practical session (2-5 hours additional)

Flaps in reconstruction . Complex closures using flaps to reconstruct skin cancer defects. Principles of tissue movement and flap designs. 1 hour didactic. Supplemented by practical session using pig flanks (2-3 hours additional)

Androgenic agents and drugs affecting hair growth in dermatology . Discussion of agents affecting androgen cycles as used in dermatology. 1 hour didactic

3. Assessment of Learner Performance Performance exams in dermatology surgery

Background : Given the expanding scope of surgery in Dermatology, residencies are increasingly asked to recommend their graduates for surgical privileges once they leave the training program. A blanket endorsement of their surgical skills is inappropriate and recollections of what residents were or were not capable of performing well are inaccurate, especially if months/years pass before such recommendations are solicited.

Innovation : Development of performance exams for a wide range of surgical procedures. It is not sufficient that residents log their names on the surgical dictation. Using guidelines from the Residency Review Committee (RRC), examinations for key procedures that require performance must be documented if residents are to graduate or request an endorsement letter. These procedures include excisions, electrosurgery, cryotherapy, flaps and grafts, nail surgery, etc. Other procedures require “knowledge” but not necessarily “performance” according to the RRC. These include liposuction, hair transplantation, laser resurfacing, sclerotherapy, soft tissue augmentation, etc. Residents may also schedule performance examinations in these procedures if the cases are appropriate. For all examinations, residents are required to perform the procedures with staff directly supervising and completing a formal evaluation form. These records will be filed and part of the resident’s permanent record for future reference. In addition to ensuring comprehensiveness and consistency in surgical skills, these exams will enable the Department of Dermatology to certify competency in specific surgical procedures.

Foundation for International Dermatology Education (FIDE)- non-profit foundation providing educational scholarships to dermatologists abroad (usually in underserved nations) to attend the AAD national meeting. At the 2002 AAD convention, FIDE granted scholarships to 13 international dermatologists

Dermatology Education for Vietnam: Each year, I personally sponsor a Vietnamese dermatologist to the United States for further dermatologic education. Their experience with me varies from 1-6 months during which they learn general dermatology, dermatologic surgery, and dermatopathology
