KIDS Safety Net Volume 18, Issue 3Third Quarter Fall 2016 Kansas Infant Death and SIDS Network, Inc. Kansas Infant Mortality Rate Hits Historic Low The Kansas Department of Health and Environment (KDHE) announced a reduction in the state’s infant mortality rate in 2015. This reduction represents an infant mortality rate of 5.9 for every 1,000 live births or a total of 230 reported infant deaths in Kansas. ―This is still too many babies dying before their first birthday,‖ said Schunn. "While we are encouraged by this year's progress, the KIDS Network remains committed to working with our partners across Kansas to continue intervention efforts to decrease infant mortality, reduce racial disparity within birth outcomes and protect the lives of our state’s youngest and most vulnerable citizens." One of the basic indicators of the health of a community is the infant mortality rate. It reflects the association between the causes of infant mortality and other factors that are likely to influence the health status of the whole population such as economic development, general living conditions, social well-being where basic needs are met, rates of illness and quality of the environment.

Kansas Infant Mortality Rate Hits Historic Low

American Academy of Pediatrics Releases New Infant Safe Sleep Guidelines to Protect Against SIDS, Sleep-Related Infant Deaths The American Academy of Pediatrics (AAP) released new safe sleep guidelines for infants in October, the first update since 2011. Infants should sleep in the same bedroom as their parents – but on a separate surface, such as a crib or bassinet, and never on a couch, armchair or soft surface — to decrease the risks of sleep-related deaths, according to the new policy statement.

―These updated guidelines are an important step forward in further reducing infant deaths. It is indicative of the growing need to provide simple and consistent safe sleep messages,‖ said Christy Schunn, Executive Director of the KIDS Network.

The KIDS Network will continue to promote the ABCs of safe sleep which include: baby should always sleep ALONE, on their BACK, in a CLUTTER-FREE CRIB along with the updated recommendations.

AAP recommendations on creating a safe sleep environment include: -Place the baby on his or her back on a firm

sleep surface such as a crib or bassinet with a taut sheet.

-Avoid use of soft bedding, including crib bumpers, blankets, pillows and soft toys. The crib should be bare.

-Share a bedroom with parents, but not the same sleeping surface, preferably until the baby turns one but at least for the first six months. Room-sharing decreases the risk of SIDS by as much as 50 percent.

-Avoid baby’s exposure to smoke, alcohol and illicit drugs.

-Skin-to-skin care is recommended, regardless of feeding or delivery method,

immediately following birth for at least an hour as soon as the mother is medically stable and awake.

Breastfeeding is also recommended as adding protection against SIDS. After feeding, the AAP encourages parents to move the baby to his or her separate sleeping space, preferably a crib or bassinet in the parents’ bedroom.

Other recommendations include: -Offer a pacifier at nap time and bedtime. -Do not use home monitors or commercial

devices, including wedges or positioners, marketed to reduce the risk of SIDS.

-Infants should receive all recommended vaccinations.

-Supervised, awake tummy time is recommended dai ly to facil i tate development.

―Caring for a new baby can be overwhelming, but with the updated recommendations we can help caregivers make safe sleep choices for their newborns,‖ Schunn said.

National Safe Sleep Overview - 1 9 9 2 : A A P r e l e as e d i t s f i r s t

recommendations that infants be placed for sleep in non-prone position.

-1994: Back to Sleep campaign began to educate parents, caregivers, health providers about SIDS and placement of infants on their back to sleep.

-2011: AAP updated safe sleep guidelines to emphasize the importance of breastfeeding, immunizations and stated that bumper pads should not be used in cribs.

-2016: Reduction in the rate of SIDS related deaths since initial Back to Sleep campaign in 1994 is 40%.

-October 24, 2016: American Academy of Pediatrics released new infant safe sleep guidelines.

Exploring the Experience of Life Stress Among Black Women with a History of Fetal or Infant Death Disparate birth outcomes among Black women continue to be a major public health problem. Whereas prior research has investigated the influence of stress on Black women’s birth outcomes, few studies have explored how stress is experienced among Black women across the life course. The objectives of this study were to describe the experience of stress across the life course among Black women who reported a history of fetal or infant death and to identify stressful life events (SLE) that may be represented in the widely used SLE inventory.

Collaborating with Obstetrical providers to Promote Infant Safe Sleep Guidelines The objective of this project was to partner with obstetrical providers to increase promotion of the American Academy of Pediatrics guidelines for infant safe sleep. Specifically, the study evaluates the effectiveness of the Safe Sleep Toolkit during obstetrical visits. Secondary objectives include improving provider and maternal knowledge of safe sleep.

Safe Sleep Instructor Training Thirty-seven individuals representing key communities throughout Kansas participated in Safe Sleep Instructor Training over a two-day, comprehensive seminar in November. The Safe Sleep Instructors (SSI) are the voice of Safe Sleep by promoting Safe Sleep through standardized training and community outreach.

Kansas SSIs will provide training to health professionals in a variety of settings, including hospitals, doctors’ offices, public health, nonprofit organizations, private businesses, schools and colleges within their region. Many will initiate Community Baby Showers for Safe Sleep in their home communities.

The SSI training is funded by a grant from Kansas Department of Health and Environment safe sleep expansion project and is critical to protecting Kansas babies. Each SSI will be tasked with the goal of completing Safe Sleep training with at least 10 professionals to educate their local communities about safe infant sleep practices by March 31, 2017.

“The Safe Sleep Instructors are dedicated to protecting our most vulnerable citizens — Kansas babies. Many who attended training are doing so as volunteers, and they are passionate bringing safe sleep education to their home communities,” said Christy Schunn.

Kansas Research Goes Global

A team of like-minded Wichita area professionals presented their research at the International Conference on SIDS, Stillbirth and Baby Survival in Montevideo, Uruguay in September.

Attending the conference were Dr. Cari Schmidt, Dr. Stephanie Kuhlmann, Dr. Zachary Kuhlmann and Christy Schunn. The team presented on two topics: Baby Talk - Pilot Testing a Collaborative Prenatal Education Program and Developing a State-Wide Infrastructure for Safe Sleep Promotion. Additionally, two research projects were represented: Does Providing Infant Caregivers with a

Wearable Blanket Increase Safe Sleep Practices? A Randomized Controlled Trial and Collaborating with Obstetrical Providers to Promote Infant Safe Sleep. “Being able to attend and present our work and research at the international ISPID conference is an awesome experience. It is exciting to see the passion and efforts that exist worldwide to prevent infant mortality and sleep related deaths. The opportunity to share our local efforts on an international platform helps solidify that our community is actively engaged and contributing to solutions regarding a global healthcare issue.” Dr. Stephanie Kuhlmann, Wesley Children’s Hospital.

Community Baby Shower for Safe Sleep

In collaboration with the Wichita Black Nurses Association, the KIDS Network held a Community Baby Shower for Safe Sleep at St. Mark Church in Wichita on Oct. 1. 130 pregnant women attended and received valuable information about safe sleep and maternal health. In addition, the women received a portable crib and sleep sack to provide their new babies with a safe sleep surface. Many of the moms-to-be reported not having a separate crib for baby, so the one they received at the baby shower could truly be life-saving.
