นิยามโรคผ่ืนภูมิแพผ้วิหนัง (atopic dermatitis / atopic eczema) เป็นโรค

ผวิหนังอกัเสบเร้ือรังที่พบไดบ้อ่ยในวยัเดก็ มลีกัษณะทางคลนิิกที่ส าคญั คือ มอีาการ

คนัมาก ผวิหนังแหง้ อกัเสบ และมกีารก าเริบเป็นระยะ ๆ

ผูป่้วยอาจมรีะดบั IgE ในเลือดสูง และสว่นใหญม่ปีระวตัสิว่นตวัหรือ

ประวตัคิรอบครัวเป็นโรคภูมิแพ ้เชน่ โรคหืด (asthma) โรคจมูกอกัเสบภูมิแพ ้

(allergic rhinitis) เป็นตน้

Epidemiology

• Affects 15-30% of children, 2-10% of adult

• 60% begin during the first yr.

• 45% begin within the first 6 mo.

• 85% begin before 5 yrs.

• Up to 70%: spontaneous remission before adolescence

: อุบติัการณ์

โรคน้ีสว่นใหญพ่บในเดก็อายุต ่ากวา่ 5 ปี

จากการศึกษาความชุก (prevalence) ของโรคผ่ืนภูมิแพผ้วิหนังในประเทศไทย

พบวา่

o ในกลุม่เดก็อายุระหวา่ง 6-7 ปี (ISAAC Phase III) ในกรุงเทพมหานคร

พบรอ้ยละ 16.7

o ในกลุม่เดก็อายุ 13-14 ปี พบรอ้ยละ 9.6

o ความชุกในผูใ้หญโ่ดยการศึกษาในกลุม่นักศึกษามหาวิทยาลยั ใน

กรุงเทพมหานคร เม่ือปี พ.ศ. 2518 พบรอ้ยละ 15.2 และจากรายงาน

ลา่สุดในปีพ.ศ.2540-2541 ซ่ึงศึกษาในกลุม่นักศึกษาเชน่เดยีวกนั พบ

รอ้ยละ 9.4

- ส่ิงแวดลอ้มที่อาจมสีว่นท าใหเ้กดิผ่ืน

การซกัประวตัแิละตรวจร่างกายทัว่ไปโดยละเอยีด

ประวตัภูิมิแพข้องตนเอง และครอบครัว เชน่ อาการคนัจมูก จาม น ้ามูกไหล หอบ

เหน่ือย หายใจมเีสยีงวี้ด เป็นตน้

ประวตักิารแพอ้าหาร ประวตักิารแพส้ารกอ่ภูมิแพใ้นอากาศ (airborne allergen)

โรคและภาวะอื่น ๆ ที่มสีว่นท าใหผ่ื้นมคีวามรุนแรงมากข้ึน เชน่ การตดิเช้ือที่ผวิหนัง

การกระจายของผ่ืน และอาการอ่ืน ๆอยา่ง

ละเอยีด อาจใชเ้กณฑก์ารวินิจฉัยของ

Hanifin (2001)

ตรวจดูวา่มผ่ืีนผวิหนังอยา่งอ่ืนที่อาจพบร่วม

ดว้ย เชน่ ผวิแหง้ (xerosis) กลากน ้านม

(pityriasis alba) เป็นตน้

Acute Atopic Dermatitis

• Intensely pruritic, erythematous papule associated with excoriations , vesiculation, and serous exudate

Chronic Atopic Dermatitis

• Dry skin

• Lichenification : Thickening of skin with accentuated surface markings and fibrotic papules

Genetics of Atopic Dermatitis

• The concordance rate for atopic dermatitis is higher among monozygotic twins (77%) than among dizygotic twins (15%)

• atopic dermatitis–related loci on chromosomes 3q21,11 1q21, 16q, 17q25, 20p,12 and 3p26

• 1q21, which harbors a family of epithelium-related genes called the epidermal differentiation complex

Patients with Atopic Dermatitis have Impaired Barrier FunctionTransepidermal water loss (A) increase not only in lesional, but also in non-lesional skin of patients normal controls, and hydration (B) is reduced.1

• The skin produces the cathelicidin LL-37; the human β-defensins HBD-1, HBD-2, and HBD-3; and dermcidin. The inflammatory micromilieuinitiated by interleukin-4, interleukin-13, and interleukin- 10 down-regulates these antimicrobial peptides in the skin of patients with atopic dermatitis.
