The M&T Charitable Foundation Charitable Contributions Application Instructions
MISSION STATEMENT At M&T Bank, we recognize that our corporate success is directly related to the health and vitality of the communities we serve. So, as a corporate citizen, we believe in providing resources to not-for-profit organizations that make our communities better places to live and work.
Through our philanthropic arm, The M&T Charitable Foundation, we support a diverse range of civic, cultural, education, health care, human service and youth organizations with financial grants, employee volunteerism and in-kind services.
PROCESS All requests received by The M&T Charitable Foundation are reviewed by local Charitable Contributions Committees. Monthly meeting dates vary from region to region; please contact the appropriate region for the scheduled meetings and deadlines (contact names and addresses can be located on page 3).
* * * * * * * * * * * * * * * * * * * If your organization would like to be considered for a charitable contribution for over $1,000, please submit the following:
1. A completed Charitable Contributions Application located on pages 4 and 5. 2. Board member list and their affiliations. 3. Copy of the IRS 501(c)(3) federal tax exemption letter of determination. 4. A brief proposal of 1 to 2 pages that includes the following:
a) Mission statement / purpose of your organization b) Detailed description of the need for funding c) Description of how the contribution will benefit the quality of life in your community d) The nature and sources of permanent funding e) Annual budget and / or project budget
5. If the request is for $10,000 or more, please complete the Supplemental Questionnaire located on page 6 6. If applying for funding in the Buffalo area, all proposals must be submitted through an M&T Bank sponsor
(employee). A sponsor can be one of the following: a) If your organization banks with M&T, submit your proposal through your banker (branch manager,
relationship manager, trust officer, etc.). b) If you have an M&T employee serving on your board, submit your proposal through the board
member. c) If you have an M&T employee working as a volunteer for your organization, submit your proposal
through the volunteer. d) If your organization does not have any of the above contacts, submit your proposal to the manager
of the M&T branch closest to your organization.
The sponsor will then complete the internal piece of the Application and submit to the Buffalo charitable contributions coordinator. Please note that the committee will not address any proposal unless a sponsor recommendation is received.
7. If requesting the participation of M&T volunteers in conjunction with or without a funding proposal, please complete The M&T Volunteers Program Application Form located on pages 7 and 8.
8. If applicable and applying for funding in the Rochester area, please complete The Logic Model located on pages 9 and 10.
9. Please forward the completed package to the appropriate charitable contributions coordinator located on page 3, or your sponsor (if applying for funding from the Buffalo region).

The M&T Charitable Foundation Charitable Contributions Application Instructions (continued) FUNDING LIMITATIONS In general, The M&T Charitable Foundation does not make grants to:
• Individuals • Organizations which lack the 501(c)(3) tax-exempt status • Political organizations, candidates or lobbying efforts • Fraternal or veterans organizations • Sports teams • National or international organizations, unless their programs have significant local impact • Religious or sectarian organizations, except when they are conducting programs secular in nature
and have wide public impact
ARTS AND CULTURAL REQUESTS OVER $10,000 M&T Bank is a partner in the Cultural Data Project (CDP), an online system for collecting and standardizing historical financial and organizational data. All arts and cultural organizations applying for contributions of $10,000 or more in Maryland, New York and Pennsylvania should submit a Data Profile annually through the CDP website (www.culturaldata.org) (select your state from the drop-down on the left). If you are not yet registered with CDP, please log onto your state at the above site for information on how to participate.

The M&T Charitable Foundation Guidelines for Completing the Logic Model To Be Completed For Submission to Rochester Office Only
A logic model is a valuable organizational planning, implementation, funding and quality improvement process tool. It describes how your programs or capital projects are expected to work in achieving desired outcomes, while identifying the need for adjustments and improvements. The Logic Model was developed by a representative group of health and human service providers and private and public funders to create a common format. This outline is intended to guide you in using the Logic Model. We have outlined the five areas included in the model in the order we recommend you use when planning a program or capital project; the actual model outlines the areas in reverse order (see Logic Model on page 10). Here are the five areas: WRITING YOUR LOGIC MODEL:
1. The goal gives a one-sentence overview of what the program or capital project is designed to accomplish and for whom.
2. Projected outcomes are benefits or changes for individuals or populations during or after participating in
activities. They show effects on knowledge, attitudes, skills, behavior, condition or status; examples include increased reading levels, improved parental management skills and increasing home ownership. The following are not outcomes: number of participants served, participant satisfaction, reports completed. Timeframes for short-term versus long-term outcomes will vary based on the type of program or activities. Short-term outcomes should occur within a timeframe that allows you to measure them. You may be able to measure some long-term outcomes as well; others may go beyond the scope or timeframe that you can measure, but are expected to occur and may be related to short-term outcomes (when completing the model, note with an asterisk (*) outcomes you plan on measuring.
3. Activities show what you do with the inputs/resources to achieve your outcomes. Examples include
support groups, job clubs, individual counseling, structured recreation programs, etc. Activities should be quantified to show information such as frequency, duration and participation (including specific client populations, e.g., school children).
4. Inputs are the resources needed to carry out your activities and accomplish your outcomes. Staff, funds,
volunteers, and materials are all examples of inputs. Inputs should be quantified whenever possible (e.g., number of full-time equivalent [FTE] social workers).
5. Relationships between inputs, activities, short and long-term outcomes should be shown to the best of
your ability, e.g., through numbering, blocking, arrows, etc.
