What is The Journal of Lifestyle Medicine?

The Journal of Lifestyle Medicine is produced by The S’eclairer Center for Behavioral Health and Wellness, to provide the latest research in healthcare sciences. This Journal is written for all those who aspire to have a healthful life. We all can have a joyful life by embracing scientific knowledge of current and ancient wisdoms.

The Journal of Lifestyle Medicine is partnering with multidisciplinary providers rendering services for psychiatric, addictive, medical and lifestyle disorders, thus sharing trusted resources of such providers in Greater Pittsburgh and tri-state region to it’s readership.

We were instructed to carry a love for one another, and show a great respect for all the beings of this Earth. We are shown that our life exists with the tree life, that our well-being depends on the well-being of the vegetable life, as well as being close relatives of the four-legged creatures. In these ways, spiritual consciousness arises and can be the highest form of politics. (Akwesasne).

We have the option of getting intoxicated by life or feeling miserable and liable to addictions of entertainment and

I was trained to treat depression, PTSD, anxiety states, severe psychiatric disorders as well as many addictive disorders. After observing the burden of many lifestyle diseases, I have now embarked on providing lifestyle medical care. It is playful to live in wisdom of knowing the sciences of health—that changing lifestyle changes genes, down regulating (“turning off”) genes that promote cancer, heart disease, inflammation, and other illnesses.

AVICENNA (IBN SINA) (C. 980—1037)

Abu ‘Ali al-Husayn ibn Sina is better known in Europe by the Latinized name “Avicenna.” He is probably the most significant philosopher in the Islamic tradition and arguably the most influential philosopher of the pre-modern era. Born in Afshana near Bukhara in Central Asia in about 980, he is best known as a physician whose major work is The Canon (al-Qanun fi’l-Tibb) that continued to be used as a medical textbook in Europe and in the Islamic world until the early modern

period. As a philosopher, whose major summa, The Cure (al-Shifa’), had a decisive impact upon European scholasticism and especially upon

Thomas Aquinas (d. 1274). The philosophy of Avicenna articulates that God is the Necessary Existence that lays the foundation for his theories of the soul,

intellect and cosmos. Apart from philosophy, Avicenna’s other contributions are in the fields of medicine, the natural sciences, musical theory, and mathematics. His influence in medieval Europe spread through the translations of his works first undertaken in Spain.

The Science and Management of Obesity

In recent years, overweight and obesity has reached epi-demic proportions in the United States. According to a study in The Journal of the American Medical Association

(JAMA), in 2008, the obesity rate among adult Americans was estimated at 32.2% for men and 35.5% for women. All indi-cations are that Americans will keep getting obese in the next two decades.

Obesity is defined as an excess proportion of total body fat. Sustained positive energy balance results in excessive ac-cumulation of adipocytes, which, in the abdomen, leads to chronic inflammation. Nearly 72 million adults in the United States can be defined as obese. There are several indexes used to assess obesity. The most common measure of obesity is the body mass index BMI:

Obesity BMI > 30Severe Obesity BMI > 35Morbid Obesity BMI > 40

Health Risks of Obesity are many including Type 2 diabetes, heart diseases, high blood pressure, stroke, nonalcoholic fatty liver disease, osteoarthritis, some cancers like breast, colon, endometrial, and kidney.

Gut Microbial Factors: Evidence that gut microbes composition can differ between obese and lean humans and speculation is that gut micro biota can participate in the pathophysiology of obesity. The first mechanism consists in the role of gut micro biota to increase energy extraction from indigestible dietary polysaccharides. However, many questions are still unanswered. TREATMENT OF OVERWEIGHT AND OBESITY There is no single approach that can help prevent or treat overweight and obesity. Treatment may include a mix of behavior adjustments, diet, exercise, and sometimes weight-loss drugs like:

In some cases of extreme obesity, weight-loss surgery may be an option.

BALANCING THE SCALEObesity results from an energy imbalance. The body needs a certain amount of energy (calories) from food to keep up basic life functions. Body weight tends to remain the same when the number of calories eaten equals the number of calories the body uses or “burns.” Over time, when people eat and drink more calories than they burn, the energy balance tips toward weight gain, overweight, and obesity. Many factors can lead to energy imbalance and weight gain. They include genes, environment, nutrition and eating habits, how and where people live, attitudes and emotions, life habits, and gut microbes.

Genetic Factors: Genetic obesity tends to run in families, suggesting that it may have a genetic cause, but with lifestyle changes, many people genetically predisposed to obesity do not become obese or are able to lose weight and keep it off.

Environmental Factors: Environment includes lifestyle behaviors such as more driving and less walking, the overuse of TV-Computer-Video games which promote a sedentary way of life.

Nutritional Factors: Nutritionally, Americans tend to have high-fat diets, often putting taste and convenience ahead of nutritional value when choosing meals. This includes, the introduction of processed food, unstructured meal times, fast-food, snacking, and consuming large portions at restaurants. Meals now are 4 times larger than they were in the 1950’s.

Psychological Factors: Psychological factors influence eating habits. Many people eat in response to negative emotions such as boredom, sadness, or anger.

Talking About Pain: Naturally

Donald L. ValentineDC, MSW Whether a pain is a mild irritation lasting just

moments or severe and lasting for weeks, months, or years, there is an inner instinct to

seek a remedy for pain symptoms. As a natural healthcare practitioner, I must keep in mind that pain is an indicator, not a condition in itself. Think about it! The pain is the signal of some problem in one or more tissues in the body. The solution is determining what tissues, organs, and/or body systems are at fault and finding the most appropriate way to help it heal. If pain itself is managed alone, then the problem is never addressed.

Medical management of pain plays an important role when the pain affects the quality of life—basic self care, temperament, and regular daily activities. In this light, treating pain as a component of the condition is straight-forward.

As a chiropractor, I seek to solve the underlying prob-lem causing the pain by understanding the source. Once that is understood then, hands-on, manual therapies can safely begin. Treatment is an organized approach that can include one or more modalities that facilitate many levels of motion and healing processes, such as, traction, stretch-ing, trigger point therapy, deep tissue, myofascial, mobi-lization/ manipulation, exercise, and nutritional support.

As practiced at S’eclairer, Dialectical Behavioral Theory (DBT) complements individual therapy with group meetings to develop the skills necessary for a healthy emotional and cognitive life.

Among these skills, are mindfulness, effective stress responses, emotion-al management, and interpersonal engagement.

In this therapeutic context, the practice of yoga can develop several skills that are powerful tools for healthier feelings and thinking. While all eight branches of hatha yoga can advance this skill building, the two that are used foremost at S’eclairer are pranayama (deep breathing) and the asanas (poses and movements).

Deep-breathing—breathing from the diaphragm—is a practice that is done within DBT. Although deep-breathing is not labeled as pranayama within DBT, the theory does cite deep-breathing as a powerful skill in response to stress and for emotional management. And there is increasing medical evidence that deep-breathing works.

In additional Pranayama is an essential component of yoga practices that focus on the asanas, the aligned poses and movements. In fact, a practice featuring the asanas, extends the development of mindful breath-ing into the development of a mindful body. To develop a mindful body, the yoga practitioners learn to “listen to their body” that is, to monitor the orientation and alignment of their bones and joints as well as the sensa-tions within their muscles. Yoga asserts its ability to link our body, mind and spirit.

How does movement change the mind?

Dance/movement therapy (DMT) is a psycho-therapeutic tool that uses body movement as the modality for both assessment and treatment.

DMT is founded on the principle that body movement patterns correlate with psychological states. Although we may have devised ways to mask our true thoughts and feelings from ourselves and others, the body never lies. Our memories and emotions are not housed purely in our minds, but rather lived through (and often held in the musculature of) our bodies. Therefore, body awareness gives us the most direct access to our emotions and memories.

Psychological and neurological patterns become physical patterns (and vice-versa) and we need to literally “move through” these feelings and make physical adjustments in order to truly heal. Our bodies are naturally wired with these physical release mechanisms—the release of having a good cry or a hearty laugh are ones we can all relate to.

Dancing, moving, changing our posture or movement patterns are also our inborn healing mechanisms, but ones that most of us have long forgotten how to utilize. Since outer movement reflects inner mental and emotional states, changing movement behavior can lead to change in the psyche and spirit. Dance/movement therapy aims to reconnect people with their own bodies—as the body is the most direct vehicle of expression and connection to others and to our environment.

How does movement change the mind?

In this day and age it is very hard to balance health and wellness. This can be especially difficult for women. In our society today, women are expected

to “do it all”. We continue to instinctively care for our children, pets, friends and family while at the same time having to work outside of the home. Due to the variety of demands that are placed on us, it is too common for women to put themselves last and not take the time to care and nurture themselves. When we neglect ourselves, we begin to feel the effects not only in our body but also mentally. The question then is, “What can women do to reduce the mental stress that comes with day-to-day life?”

As I continue to be exposed to health related information, there seems to be a common theme. The four things that can help keep us all healthy are: 1) Regular Exercise, 2) Proper Nutrition, 3) Proper Sleep, and 4) Regular practice of Mindfulness Meditation.

As a counselor, I often hear women say, “I don’t have time for any of that!” I believe that all small changes count! It just takes a commitment of doing one thing differently that starts the process. Avoiding fast food, committing to a regular bedtime, or sitting and breathing for 10 minutes may be all that it takes to get the ball rolling. I challenge you to take time to nurture and care for yourself. It can be uplifting and you will be glad you did!

Why Homeopathy Works!

The Webster’s Dictionary defines homeopa-thy as the treatment of disease by the ad-ministration of very small doses of drugs

which would produce in a healthy person effects similar to the symptoms of the disease. I some-what agree, but the definition falls short of the current research and effectiveness of the treat-ment. Homeopathy (used for over 250 years) is a specific science, and not to be confused with herbal or drug therapy.

Many people think homeopathy is not based on science, but actually homeopathic research designs are far more scientific than any pharma-ceutical research practices. Modern medicine including drug therapy is based on symptoms and double-blind studies (done mainly on ani-mals), and focuses on the physiological effect looking for disease outside the body. homeopathy practitioners believe that disease comes from inside the body, and people who change their insides (a healthier diet, positive intent, less emotional stress and/or a healthy life style) will cause their outsides to change. In other words, they look at the entire human picture—mental, physical and emotional.

Homeopathy respects the wisdom of the body and is based the ‘law of similar.’ The assumption is that the body wants to be in balance or ‘homeosta-sis.’ This law states that substance that causes a symptom or disease in a healthy person will cure the same symptoms or disease in an unhealthy or sick person. For instance, a person with diarrhea or a high fever would be treated with a homeopathic remedy that causes diarrhea or a high fever, leading to recovery. When I want to rid a bacteria like Streptococcus from the body, I recommend a homeopathic de-rived from the same bacteria. In most cases it works effectively within weeks. This treatment is inconsistent with the conventional medical model that there is a drug for each type of symptoms.

Dr. Thomas Rau of the Paracelsus Klinic in Switzerland (www.paracel-sus.ch) believes that the theory of homeopathy is particularly fascinat-ing. He states that homeopathy deals with a relationship between the ele-ments of the earth, plants, metals and human beings. All of these must be present in a harmonious way. Further-more, the great merit of experienced homeopaths is to recognize that ex-cesses that may cause a specific ill-ness may be effectively treated with micro-doses of the same substance. ‘Like is treated with like’ but, depend-ing on the emphasis of expression of the compliant, treatment may neces-sitate different potency or different dosage. Dr. Rau has used this wisdom to further his practice of BioMedicine.

Homeopathy encourages the be-lief that a person with abnormal vi-brations or frequencies can lead to a state of disharmony. This is regardless of the type or number of symptoms reported by the patient. This explains why homeopathic treatments are so congruent with new therapies in ‘energy’ medicine. At NutriFarmacy,

This study had a tightly controlled population of seasoned body builders and athletes, tracking how they built muscle and experienced the same rates of repair and soreness. In the ‘big’ pictures, Dr. Rau and I recommend supportive measures including: antibiotic use for only 2-3 weeks limit, detoxing the viral and heavy metals, repair the gut and immu-nity and build up vital forces (vitamins, homeopathics, herbs, fatty acids).

Q: I HAVE BEEN SUFFERING with an autoimmune disease for over eight years. My doctor says that we don’t know the cause, but he insists I need both Humira and Prednisone as maintenance therapies to con-trol my disease. A: DEAR RUTHThis is an excellent questions and in my practice I am committed to inves-tigate the potential causes of autoim-mune disease (over 85 of them). To look into the newest and most rel-evant research one has to follow the Europeans who are way out front of the American medical community on non-drug treatments and therapies.

When looking at potential ‘causes’ we must recognize that your physician will say that they don’t know the cause. Not because they are disingenuous, but because it was never studied in medical school. According to the non-con-ventional expert, the three most likely culprits include: heavy met-als and pollutants, parasites, and molds and fungus. These are criti-cal areas where conventional West-ern medicine and Big Pharma has contributed nary a dollar to further investigate potential ‘causes.’

In my experience after testing hun-

Defining Chinese Medicine and TCM

There is a great deal of curios-ity about Chinese medicine and also much confusion about what

it is. It is definitely more than acupunc-ture and herbal therapy. Chinese Med-icine is a synthesis, an evolution–an empirical build of information span-ning 10,000 years that has to come to ask some of the most definitive ques-tions of mankind. Classical Chinese medicine asked the same questions that we do today, but hidden some-times in metaphoric language. Ques-tions of life and death and what ingre-dients lead to an optimal life were the hallmark before the stethoscope was invented or Antonie Philips van Leeu-wenhoek (1632-1723) first observed microorganisms with a microscope.

The big difference between Chinese Medicine and Western medicine is that western medicine is based on fact. A fact has an inability to grow and evolve–not very humanistic. Chinese Medicine is based on empirical experience–not facts. priest, an oncologist, a hematologist, a registered dietician and a psychologist all in the same room discussing your health care and presenting a collabora-tive assessment. In my practice, one of the most challenging things reported to me to this day is getting all the people on the patient’s health care team to talk to one another.

In classical Chinese Medicine, (NOT Traditional Chinese Medicine or TCM), this concept has remained intact to this day via oral transmis-sion. In fact, in my training model, you specialize first to become ac-quainted with your topic, wide and deep as we say, and then you become the general practitioner. When per-mitted to practice, one was known as a Zhong Yi sometimes translated as Chinese Physician. Zhong accurately translated means China or to hit the mark or center and Yi means whole or one medicine or doctor.

THE POWER OF MAOTraditional TCM (Traditional Chi-

nese Medicine) is a system devised by Mao Zedong. While he never believed in Chinese medicine him-self, he knew it was a great political strategy. Chairman Mao brought over

several physicians (consultants) from France to build a system that could be marketed to the west via President Nixon to various U.S. universities such as Harvard. It was a clever politi-cal move to market the uniqueness of China. TCM uses basic Chinese medi-cine concepts eliminating much of the QiGong, Taiji, internal practices of Qi as well as removing anything remotely spiritual in nature to appeal to the sci-entific (fact) mind of the West.

However in the last 10 years or so, the acupuncture schools are opening up to the Classical Chinese medicine to alert the students to the depth of their art. This inclusion gives hope that the collective wisdom of all we have discovered about the human condition will be put to use in the future of inte-grative medicine.

Defining Energy Medicine AND HOW IT IS DIFFERENT THAN TRADITIONAL MEDICINE

Energy Medicine is an ancient art and science of working with the subtle energies of the body. For optimum health, our body’s energy needs to be balanced, flowing in the

correct direction, without any blockages. It is the oldest form of medicine there is—older than both western and eastern medicine. It is a complete system and is a great compliment to traditional medical treatments in and out of the hospital.

Energy Healing works on the physical level, and can ad-dress both chronic and acute situations. It is excellent for strengthening and balancing the immune system, healing ill-ness or disease, as well as, relieving pain. Measuring Visual Performance THE ROLE OF DEVELOPMENTAL OPTOMETRY

There is a little known but crucial science of vision therapy that is the domain of the behavioral, functional or developmental optom-

etrist. Our job is to help you understand what vision actually is, and how visual performance is measured.

As an optometrist, optometric vision therapist, Fellow in the College of Optometrists in Vision De-velopment, and Somatic Movement Educator, I’ve worked therapeutically with patients of all ages in the area of soft neurological signs. Beyond the eye health and visual acuity part of vision measured in a conventional eye doctor’s office, we look at human potential through the lens of vision. In addition to measuring visual acuity, or how clearly a person can see a chart at distance or near, we measure a per-son’s binocular vision, the ability to use the two eyes together as a team. My goal as a facilitator in opto-metric vision therapy is to help people re-develop, or possibly develop for the first time, their ability to achieve single, simultaneous binocular vision.

The totality of the visual process includes visual acuity, peripheral awareness, depth perception, track-ing ability, focusing ability from near to far, conver-gence and divergence with ease, and visual percep-tion, all combined.
