Ms. Sheehy was present on the conference call and accompanied by her Monitor/Supervisor,

Ms. Chaya PG. Posner, MSOT, and not represented by legal counsel


Ms. Sheehy’s license is currently on Probation, is active and expires February 28, 2019. Ms.

Sheehy requested her probation be terminated and appearance before the Board be acceptable

as required by her final order filed May 1, 2017.

MOTION: After discussion, Mr. Spafford made a motion to accept/approve Ms. Sheehy’s

appearance. The motion was seconded by Dr. Banta and carried with a 4/0 vote.

PROSECUTOR’S REPORT

TAB 2 Eric Fryson, Prosecuting Attorney

Mr. Fryson presented the Prosecutor’s Report outlining the current status of 19

open/active disciplinary cases.

INDIVIDUAL CONSIDERATION

TAB 3 Nicolas Ordonez-Rocha, OTA by Endorsement, File # 9676

Mr. Ordonez-Rocha was present on the conference call and not represented by legal counsel.

Mr. Ordonez-Rocha’s OTA by Endorsement application was presented to the Board as an

Individual Consideration due to an affirmative response to previous criminal history question on the

application.


MOTION: After discussion, Mr. Spafford made a 04/30/2018- motion to table/continue

Mr. Ordonez-Rocha’s application for licensure requiring a Personal Appearance after the

resolution of his current open April 2018 Criminal cases. The motion included the option

that Mr. Ordonez-Rocha could seek a Professionals Resource Network (PRN) evaluation

and come before the Board with PRN’s resolve (evaluation and/or recommendation) prior

to the resolution of the closed criminal cases at the earliest meeting of the Board (which

ever happens first) if he chooses to ensue. The motion was seconded by Ms. German

and carried with a 4/0 vote.

TAB 4 Ann Reuter, OT by Endorsement, File # 19194

Ms. Reuter was present on the conference call and not represented by legal counsel.

Ms. Reuter’s OT by Endorsement application was presented to the Board as an Individual

Consideration due to an affirmative response to previous disciplinary history question on the

application.

MOTION: After discussion, Mr. Spafford made a motion to approve Ms. Reuter’s

application for licensure. The motion was seconded by Dr. Banta and carried with a 4/0

vote.

EXEMPTION FROM DISQUALIFICATION APPLICATION

TAB 5 Lisa B. Rust, OTA 16355, File # 9605

Ms. Rust was present on the conference call and not represented by legal counsel.

Ms. Rust’s AHCA Exemption from disqualification from employment application was before the

Board due to a previous background level II screening reflecting previous criminal history.

MOTION: After discussion, Ms. German made a motion to grant an exemption for

disqualification from employment. The motion was seconded by Dr. Banta and carried with a 4/0

vote.

APPLICANT RATIFICATION LISTS

TAB 6 Licensed Occupational Therapists & Assistants

MOTION: Mr. Spafford moved to approve the ratification list of 284 Occupational

Therapists & 194 Occupational Therapy Assistants, totaling 478 applicants for licensure.

(APPENDIX 1). The motion was seconded by Dr. Arthur and carried with a 4/0 vote.

ACTION TAKEN: 478 names ratified for licensure.


TAB 7 Continuing Education Provider Applications

MOTION: Mr. Spafford moved to approve 8 continuing education providers, 5 Prevention

of Medical Errors course providers, and 1 Florida Laws and Rules course providers.

(APPENDIX 2) The motion was seconded by Dr. Arthur and carried with a 4/0 vote.

TAB 11 Board Chair, Jim Spafford

2018 NBCOT OT State Regulatory Leadership Forum

The Board Chair stated his pleasure in attending the NBCOT OT State Regulatory Leadership

Forum, saying there were many presentations provided by NBCOT on the scope of practice,

from the FTC, state surveys and admirable networking information. Mr. Spafford advised he

would like to send some materials provided to him, for Board staff, and to further distribute to the

other members. Mr. Spafford stated in his observation in listening to the opinions of many other

state representatives, he feels very fortunate and professed his great appreciation for the level of

dedication, involvement, and work afforded to the state of Florida by this Board’s excellent staff

and attorneys.

TAB 13 Staff Recognition

Mr. Spafford commended Board Staff stating his awareness that the office has been operating

short staffed and yet the kudos and recognition are proof again of the exemplary and stellar work

consistently provided. Mr. Hall also publicly thanked Ms. King and Ms. Boutwell for the

wonderful job they have continued to do, in maintaining the work flow and receiving great

comments from customers, even though they have both been under stressful workloads, being

short staffed for over three months now, in Ms. Horton’s absence.

Dr. Calvo’s Resignation

Dr. Calvo addressed the Board stating this would be his last meeting with the Board of

Occupational Therapy Practice as he is resigning his position, due to his resent acceptance to

the American Occupational Therapy Association Board of Directors. He thanked the Board

members and staff for the opportunity and experience, wished the Board well, and stated it has

been “his pleasure and professional highlight to serve the Board”.

MOTION: There being no further business, Dr. Banta made a motion to adjourn the meeting at

10:01 a.m. The motion was seconded by Ms. German and carried with a 4/0 vote.
