Background & Purpose• One of the deliverables of the project is to:

‘To develop an innovative and culturally appropriate Model of Care (to be shared provincially) serving the needs of diverse populations in the Mississauga Halton and Central West LHINs for independent modalities and dialysis access.’

• The goal of the Model of Care:‘To provide language and culturally diverse supports towards improving the number of individuals choosing to go on independent/home dialysis’

• The vision of the Model of Care:

‘Our Commitment: To provide Language and Culturally Appropriate tools for an Excellent and Equitable Health Care Experience’

The CKD Patient’s Journey through a Diversity Lens

• This Model of Care provides a culturally competent framework for a comprehensive approach to the prevention, early detection and management of CKD across the continuum of care

• Culturally and linguistically tools/resources have been developed to enhance education and awareness of the language and cultural needs of both healthcare providers and patients at each stage of the CKD journey

• The CKD patient’s journey across the continuum includes:• Education across the continuum

• Early Detection and Management• Pre Dialysis• Body Access• Home Dialysis• Facility Based Dialysis

• Conservative Management across the continuum

What is Cultural Competence?

• Cultural awareness – health care providers become aware of, appreciative of and become sensitive to the values, beliefs and ways of life in other cultures

• Cultural knowledge – health care providers seek out and become familiar with culturally/ethnically diverse groups, beliefs and practices

• Cultural skill – for example, learning how to conduct a culturally competent assessment

• Cultural encounter – experiencing cultural encounters with people from culturally diverse backgrounds
