TRANSFERS

Air: The handiest airports for this stretch of the Italian Riviera are Genoa and Pisa both towns are connected by rail to the coast, and both are on budget airline routes from the UK.

Train: Santa Margherita Ligure's train station is fairly central, and provides good connections with the rest of Italy. On the main railway line between Pisa and Genoa, access is easy from Rome and other big cities. You may need to change to a 'stopping' train at a nearby station such as Chiavari. Local buses run from outside the station; the seafront and several hotels are just a short walk away.

Car: 30mins from Genoa, 2.5hr From Milan airport, Malpensa, and from Pisa airport it is a 2 hour trip.

The Ethics of Restorative and Esthetic Dentistry in a Cheater’s WorldDr. Richard Simonsen, D.D.S., M.S.Dr. Richard Simonsen is presently the Founding Dean of Midwestern University, College of Dental Medicine, in Glendale, Arizona, one of the newest dental schools in the US. Dr. Simonsen was born in England and graduated from the University of Minnesota School of Dentistry in 1971(DDS), and from the U of M School of Public Health in 1981(MS). He was Professor and Chairman of the Department of General Dentistry at the University of Tennessee, Memphis from 1985-1989 and from 1989 - 1999, Dr. Simonsen was Global Professional Services Manager for the 3M Company. He was the Editor-in-Chief of Quintessence International from 1984 to 1996. Dr. Simonsen has authored three textbooks and numerous scientific publications, has lectured in over 30 countries, and is the developer of the Preventive Resin Restoration procedure and the etching of porcelain for esthetic and restorative procedures. He has been awarded a Lifetime Achievement Award from the World Congress of Minimally Invasive Dentistry and he received the 1999 Distinguished Service Award from the American Society of Dentistry for Children.

Lecture Synopsis:

Dentists in practice have a difficult balance with one foot in the business world and one foot in the healthcare world. At some time, every clinician has to make a choice whether to pursue the path towards excellence in clinical care with concomitant health benefits for patients; or the path towards maximising practice income, sometimes at the expense of the ethical standards of the clinician and the profession. This presentation will discuss the recent trend towards creating the perfect ‘smile,’ sometimes with full mouth reconstruction based on shaky science. Is it appropriate for general practitioners to treat full-mouth reconstruction cases without additional training beyond a weekend course? When is referral to a specialist ethically required? What constitutes appropriate training and which unaccredited institutions practice evidence-based treatment? Is it a surprise that we are seeing such ethical lapses in our profession when society is experiencing so many cheaters among our supposed role models. The ethical responsibility of the profession towards patient treatment will be discussed.

Esthetic Implant Therapy – is the new paradigm shift justified?Dr. Markus HürzelerDr. Hürzeler received his dental degree from the University of Zurich, his certificate as a specialist in Periodontics from the Swiss Society of Periodontology, the Docent (Associate Professor) degree from the Department of Prosthodontics at Albert-Ludwigs University in Freiburg, Germany, and his certificate in P r o s t h o d o n t i c s f r o m t h e G e r m a n S o c i e t y o f Prosthodontics. He is Clinical Associate Professor at the Albert-Ludwigs University of Freiburg, Department of the Operative Dentistry and Periodontology and Clinical Associate Professor at the University of Texas in Houston, Texas. Dr. Hürzeler has produced more than 100 scientific publications within the field of implants, periodontology and tissue regeneration and is a regular national and international lecturer. He maintains a private practice focused on periodontics and implant dentistry in Munich, Germany.

Lecture Synopsis:

Modern treatment concepts shifted implant dentistry away from the traditional two stage surgical protocol favouring immediate implant placement and immediate provisionalization. Many clinicians stated that “immediate implants in the esthetic zone have demonstrated the same level of success as those placed under more traditional protocols”. Therefore both patients and clinicians have reframed their treatment approaches and expectations to this new development. However the parameters used to define the level of success remain unclear. In particular in the esthetic zone a successful reconstruction of chewing function cannot be regarded as the ultimate treatment goal. In this context the functional and the esthetic outcome should also be perfect. Yet the critical issue is to define a ‘perfect esthetic’ result. In the future, common esthetic parameters need to be used to evaluate the esthetic result of an implant in the anterior area.This presentation addresses the mentioned problems and shortfalls when dealing with esthetic implant therapy. The participants should understand the advantages and disadvantages of different treatment approaches and be able to discriminate between ‘high risk’ and ‘low risk’ cases.

Endodontics and Cone Beam Computed Tomography – The way ahead?Dr Simon Cunnington, BDS, MSc

Simon Cunnington has been in endodontic practice for nearly 25 years and has been full time since leaving the Eastman Dental Hospital in 1994. He is one of only a few UK based endodontists to receive their specialty training in the USA and was the first European endodontist to become a Diplomate of the American Board of Endodontics. He was extremely fortunate to have his training supervised by Dr Edward Osetek , who was recognised as one of the foremost endodontic educators in America. He has always been a strong advocate of the speciality and served time on both the British Endodontic Society Committee as well as the European Society of Endodontology and is still involved on the ESE specialist training subcommittee. He is due to become President of the British Endodontic Society in 2012. With the introduction of continuing profession development he was responsible for the creation of the Langham Endodontic Study Group, a professional organisation dedicated to the continuing education of endodontic specialists. He continues to lecture throughout Europe and present practical training courses for the profession.

Lecture Synopsis:

Cone Beam Computed Tomography is a very attractive technology allowing three dimensional assessment of both odontogenic and non-odontogenic lesions, canal morphology, osseous response to root fractures, resorptive lesions and anatomical assessment prior to surgery. This presentation will review the basic principles underlying this technology and the best types of machine that could be incorporated into endodontic practice. The advantage of the additional views, cross sections and reconstructions will be discussed and whether they can change the delivery of care or the outcome. Reference will be made to Image interpretation, assessment of artefacts and disadvantages of such systems.

Learning Objectives:

At the conclusion of the presentation the participants should be able to:1. Understand the principles of Cone Beam-Computed Tomography.2. Be better able to interpret and manipulate CBCT images.3. Describe the benefits and responsibilities in utilising this technology.

Mesenchymal Stem Cells and Tissue Regeneration in the Craniofacial Complex.Dr. John E. Davies BDS PhD DSc

Dr. Davies, who trained as an oro-maxillo-facial surgeon and subsequently read for a PhD in the areas of solid-sate and surface physics applied to calcium apatite, is a Professor of Dentistry and Biomaterials at the University of Toronto. He received the degree of Doctor of Science (D.Sc.) from the University of London, in 1998, for his sustained contributions, over a period of 20 years, to the field of Biomaterials, was elected a Fellow of Biomaterials Science and Engineering (FBSE) in 2000, and was the 2002 recipient of the Society for Biomaterials Clemson Award for Basic Science. In the mid-80’s, he devised the first in vitro biological methods to study the mechanisms of bone bonding to bioactive ceramics. His work has led to a new understanding of the mechanisms by which bone grows on implant surfaces and within tissue engineering scaffolds. He has developed ceramics that can be resorbed by osteoclasts, calcium phosphates that stimulate increase in local bone mass, scaffolding materials for bone regeneration, and human umbilical cord derived mesenchymal stem cells as a potential source of cell-based therapies.

Lecture Synopsis:

Mesenchymal stem cells (MSC) provide all the cells that give rise to the musculoskeletal system. They reside in normal tissue around blood vessels, as either perivascular cells, or pericytes. It is these cells that migrate into a bony wound site, whether an extraction socket, a fracture, or an augmentation site. One character of MSC is that they do not provoke an immune response if transplanted into an unmatched host. Furthermore, such allogeneic MSC are also immunomodulatory - that is they calm down a host immune or inflammatory reaction. For these reasons MSC are currently being employed as ‘drugs’ in the treatment of scores of medical indications including those of tissue engineering. This presentation will provide an overview of the contributions my laboratory is making to the fields of MSC biology and MSC therapy and will cover topics as broad as the recruitment of such cells in peri-implant healing, and bone regeneration to the sourcing of such cells from tissue regarded as medical waste!

Dento-facial Architecture and Fixed ProsthodonticsDr. Aris Tripodakis DDS, MS, Dr.Odont.

Aris Petros Tripodakis graduated from the National and Kapodistrian University of Athens in 1973, and Tufts University, Boston, USA in 1978 where he got a Certificate of Prosthodontics and a Master of Science in 1981. In 1985 he got a Doctorate Degree and currently he is an Associate Professor in the University of Athens. In 1994 he was visiting Assistant Professor in Albert-Ludwigs University, Freiburg, Germany. He has participated in several research projects in the fields of Dental Materials, Prosthodontics, and Implantology. He was honored with research awards from the Academy of Osseointegration in 1995 and 1998. He has published several articles, written one monograph, contributed to six textbooks and lectured extensively as an invited speaker internationally. He is an Associate Editor of the Journal of Japan Prosthodontic Research & Practice and the Ti International Scientific Journal of Dental Implants and Biomaterials. He has served as President of the Eastern Mediterranean Association for Osseointegration, President of the American Dental Society of Europe, President of the International College of Prosthodontists and President of the International College of Dentists-European Section. Currently he is the Vice President of the European Academy of Esthetic Dentistry.

Lecture Synopsis:Fixed Prosthodontics aspires to position the artificial teeth within the oral cavity in the appropriate three-dimensional position, irrespective of the location and the contour of the residual ridge. The total volume of the missing structures is taken into account and the full curve of the dental arch is restored. The required support is drawn upon fixation of the prosthesis, either on natural remaining teeth or on the jaw bone by the insertion of endosseous implants. Likewise, an effort is made to rebuild and reinstate all the hard and soft tissues that are missing. Extensive deficiencies of the surrounding structures are restored either prosthetically by artificial mimicking, or by combining a surgical reconstructive intervention. Consequently, the adequate support of the lips and the facial structures is provided. The proper vertical dimension of the lower third of the face is restored and the achieved result positively influences the face of the patient comprehensively.

missing teeth but also of the supporting hard and soft tissue structures.2.Reestablishment of the full curve of the dental arch as it relates to the proper support of the facial

Recognition, Management & Prevention of Dental ErosionDr. Terry Donovan DDSHAROLD HILLENBRAND MEMORIAL LECTURE

Dr. Terry Donovan received his D.D.S. from the University of Alberta in 1967, and practiced full time in Regina, Saskatchewan for 13 years. He received his Certificate in Advanced Prosthodontics from the University of Southern California in 1981. He was Professor and Director of the Advanced Education in Prosthodontics Program at USC from 1988 - 2006. He has been Associate Dean for General Practice and Executive Associate Dean for Academic Affairs at the University of Southern California, as well as the Chairman of the Department of Restorative Dentistry from 1984 – 1991, and 1994-2001.Dr. Donovan is currently Professor and Section Head of Biomaterials in the Department of Operative Dentistry at the University of North Carolina, School of Dentistry at Chapel Hill. He is also Director of Fixed Prosthodontics at the Scottsdale Center for Dentistry. He has published extensively, and has lectured world wide on the topics of restorative dentistry and materials science. He is a member of the ADA, CDA, Los Angeles Dental Society, CAIC, American Academy of Restorative Dentistry, American Academy of Esthetic Dentistry, the American College of Prosthodontists, a Fellow of the American College of Dentists, and an Honorary Life Member of the College of Dental Surgeons of Saskatchewan, The Association of Prosthodontists of Canada, and the Western Canada Dental Society. Dr. Donovan is past Chairman of the American Dental Association's Council on Dental Materials, Instruments, and Equipment.

Lecture Synopsis:The number of patients presenting in dental offices with extensive loss of tooth structure is increasing. The etiology of loss of this tooth structure in most cases is multifactorial, and chemical erosion plays a major role in most patients.This presentation will introduce the concept of erosive tooth wear and address the diagnosis, treatment and prevention of extensive tooth structure loss due to erosion and bruxism.

Learning Objectives:1.Recognise the clinical signs of chemical erosion at an early stage and implement the

appropriate preventive strategies.2.Understand that the location and pattern of erosive tooth wear provides important clues

regarding the specific aetiology.3.Implement specific prevention and treatment protocols for patients with bulimia and

Dental Management of Snoring and Obstructive Sleep ApneaDr. Massimiliano di Giosia, DDS, Cert. in Orofacial Pain (USA), ABOPMassimiliano Di Giosia graduated from the University “G.d’Annunzio” School of Dentistry, Chieti, Italy. He completed a three years residency program in Orofacial Pain at the University of Kentucky Orofacial Pain Center, directed by Dr. Jeffrey Okeson, in 2007. Mike worked in territory sales and in sales management. He developed training programs for teaching dental products and training programs for sales people in the dental industry. Diagnosis is all that matters. Professor Dr. Sandro PallaProfessor Sandro Palla just retired from his position at the University of Zurich, where he has been chairman of the Clinic of Masticatory Disorders, Removable Prosthodontics, and Special Care Dentistry between 1981 and 2009. 2007 – 2009 he served as Dean of the Center for Oral Medicine, Dental and Maxillo-Facial Surgery. He has authored numerous research papers on c r a n i o m a n d i b u l a r d i s o r d e r s , o r o f a c i a l p a i n , temporomandibular joint biomechanics/imaging and removable prosthodontics and has lectured throughout the world on these topics. He is an active member of several international pain and dental associations in some of which he served as president. At present, Dr. Palla is Associate Editor of the Journal of Orofacial Pain and Odontology.Lecture Synopsis:It is not unusual to encounter a patient with orofacial pain in a daily practice, since the prevalence of orofacial pain in the population about 25%. In the majority of cases, the non-odontogenic pain is caused by a myoarthropathy of the masticatory system, i.e. by a craniomandibular disorder. Nevertheless, it is important to keep in mind that several pathologies / disorders may cause orofacial pain. Indeed, this may be the manifestation for example of a referred pain from the cervical structures, of a persistent idiopathic facial pain, of an ectopic e.g. orofacial primary headache or of a neuralgia. Despite the belief of many clinicians, the diagnosis of orofacial pain does not rely on a thorough clinical examination, but on a comprehensive pain history (localization, quality, intensity, course and presence of accompanying neuro-vegetative signs), lacking the majority of the orofacial pain conditions neurological and/or somatic signs. Furthermore, tenderness to palpation is not always a sign of a tissue lesion, for example of a muscle or temporomandibular joint inflammation, but of morphological and functional changes within the pain system itself, eg of central sensitization. Thus, in the pain diagnostic process the history must lead to a diagnostic hypothesis that is successively confirmed / denied by the clinical examination or other diagnostic tests.As far the therapy of a myoarthropathic pain, it is mandatory to differentiate between an acute and a chronic pain condition, where the difference between these two conditions is not solely based on the time criterion (pain lasting for more than 3 or 6 months) but in first place on the presence of psychosocial distress. This difference is important because both conditions need a different therapeutic approach and only the acute condition can be treated in a dental office. The chronic condition needs a multimodal therapy that addresses also the affective, emotional, cognitive and behavioral pain component.Learning Objectives:1. Elucidate the diagnostic process in case of orofacial pain.2. Provide the clinician with a few, simple strategies to manage patients with a

myoarthropathy of the masticatory system in a daily practice.
