Texas Ophthalmological Association

Dear Friends:

The Texas Ophthalmological Association’s 2017 Annual Meeting will be held in conjunction with the Texas Medical Association’s TexMed 2017 and will take place Friday and Saturday, May 5-6, at the George R. Brown Conference Center, Hall B3, Houston Texas. We will have exhibit opportunities for industry leaders at this conference.

This year, TOA offers 10 x 10 exhibit spaces in our Ophthalmology Pavilion. Ophthalmologists will come to this area for food and beverage during their breaks.

Ophthalmology is the second largest group of attendees at TexMed after family physicians, representing one out of every ten physicians in attendance. The TOA Annual Meeting attracts approximately 150 ophthalmologists over the two-day span, with approximately 100 individual ophthalmologists attending various portions of the meeting on each of the two days. Other specialists attending TexMed may tour the ophthalmology exhibit area and attend the CME. The 2017 program will feature a wide range of clinical and practical topics, and will again feature the resident competition.

Our Friday program is followed by a reception at 5:30 pm to which TOA members, their spouses, and TOA exhibitors are invited.

Space is limited, so we will only be able to accommodate the exhibitors on a first-come-first-served basis, based on date of payment. Please complete the enclosed registration form or register online at www.TexasEyes.org.

Texas Ophthalmological AssociationAPPLICATION FOR EXHIBIT SPACE

Exhibits: Exhibits are provided by the Texas Ophthalmological Association (TOA) in conjunction with the Texas Medical Association (TMA).

Payment Deadline: Exhibits are available on a first come, first served basis based upon date payment is received. Once the exhibit area is sold out, companies have the option of being on a wait list in the event that another company cancels. Upon acceptance, TOA will provide exhibit space as outlined in the Exhibitor Regulations. Payment in full is due by March 31, 2017, or TOA has the right to cancel exhibitor’s application. It is not necessary to send payment with this application form, but payment must be received by March 31, 2017.

Cancellation Deadline: Cancellations received prior to March 31, 2017 shall be eligible for a 50 percent refund of exhibit fees. Cancellations received on or after March 31, 2017 will be non-refundable. All exhibit cancellations must be made in writing to TOA. No-shows will not be refunded. TOA and TMA do not guarantee number of attendees.

If after the contract is entered into, the TOA fails or is unable to provide an Exhibitor with the opportunity to exhibit at the TOA Annual Meeting, due to fire, strikes, authority of the law, act of God or any other cause or reason, and the Exhibitor is not responsible for such failure, the Exhibitor’s sole and exclusive remedy shall be the return of monies that it has paid in connection with the Application/Contract. In such case, the TOA shall bear no further liability or responsibility under such agreement.

EXHIBITOR AGREES TO BE RESPONSIBLE FOR HIS/HER OWN PROPERTY. EXHIBITOR SHALL RELEASE AND HOLD HARMLESS AND INDEMNIFY THE TEXAS OPHTHALMOLOGICAL ASSOCIATION AND THE TEXAS MEDICAL ASSOCIATION FROM ANY AND ALL CLAIMS, OBLIGATIONS, LIABILITIES, CAUSES OF ACTION, LAWSUITS, DAMAGES, AND ASSESSMENTS, INCLUDING LEGAL FEES THAT RESULT FROM AN ALLEGATION OF NEGLIGENCE ON THE PART OF THE EXHIBITOR OR TEXAS OPHTHALMOLOGICAL ASSOCIATION OR TEXAS MEDICAL ASSOCIATION OR THIRD PARTIES IN THE USE OF THE EXHIBIT SPACE OR ACTIVITIES IN CONNECTION WITH THE USE OF EXHIBIT SPACE.

Acceptance of Application: Acceptance of this application as an agreement between the Exhibitor and the Texas Ophthalmological Association will occur only when an official confirmation has been sent by the TOA. It is understood that disapproval of an Exhibitor, per the terms outlined in the Exhibitor Regulations, will result in termination of the agreement without penalty to either party.

Exhibitor Regulations Agreement: The undersigned acknowledges and agrees to the Exhibitor Regulations as published by the Texas Ophthalmological Association (available upon request and at www.TexasEyes.org) to be part of this agreement.

Warrant of Authority: The Exhibitor and person signing this application on its behalf represent and warrant that the undersigned person is a duly authorized and appointed agent of the Exhibitor, is fully empowered to bind the Exhibitor to all provisions contained in this agreement.
