Käesolevasse kogumikku on koondatud peamine osa Euroopa Liidu ja Eesti tervishoiu-
valdkonna statistikast. Kogumiku esimeses osas võrreldakse omavahel Euroopa
Liidu liikmesriikide näitajaid. Teises osas keskendutakse Eesti tervisestatistika üksik-
asjalikumale esitamisele. Kogumik annab lühiülevaate rahvastikust, haigestumusest,
tervishoiuteenuste kasutamisest ja tervishoiuressurssidest 2007. aastal. Kogumiku
koostamisel on kasutatud erinevaid andmeallikaid. Rahvusvahelised võrdlused on
tehtud peamiselt Eurostati andmebaasi põhjal. Teatmiku Eesti osas kajastatakse
Tervise Arengu Instituudi tervisestatistika osakonna kogutud andmeid.
Loodame, et kogumik aitab paremini mõista Eesti positsiooni võrdluses ülejäänud
Euroopa Liidu liikmesriikide rahvastiku terviseseisundi ja tervishoiusüsteemidega.
Kogumik on mõeldud poliitikakujundajatele, analüütikutele, tervishoiutöötajatele
ning kõigile neile, kes soovivad osaleda arutelus Eesti tervisevaldkonna arengu üle.

Dear reader!
This publication contains the core of the Estonian and European Union health care
statistics. The publication is divided into two parts. The fi rst part compares the
indicators of the European Union Member States. The second part focuses on more
detailed presentation of Estonian health statistics. The publication gives an overview
of the population, morbidity, use of health care services, and health care resources
in 2007. The publication is based on various sources of information. International
comparisons have been made mainly on the basis of the Eurostat database. The
Estonian part of the guide covers the data gathered by the Department of Health
Statistics of the National Institute for Health Development.
We hope that the publication will help you better understand Estonia’s position terms
of population health status and health care system compared to other Member States
of the European Union. The publication is aimed at policy-makers, analysts, health
care professionals and everyone interested in participating in the discussion over the
development of health care in Estonia.

Rahvastiku ülevaade
Euroopas on iga seitsmes inimene 65-aastane või vanem. Vanemeaealiste osakaal
rahvastikus kasvab ning laste osakaal väheneb. Hinnanguliselt võib Euroopa Liidus
(EL) 2050. aastaks üle 65-aastaste inimeste arv võrreldes tööealise elanikkonnaga
kahekordistuda. Euroopa rahvastiku vananemist mõjutab madal sündimus, kasvanud
keskmine eeldatav eluiga ja Lääne-Euroopa beebibuumi aegse põlvkonna peatne
jõudmine pensioniikka.
Euroopas on summaarne sündimuskordaja langenud peaaegu kaks korda — kui 1964.
aastal oli EL-i keskmine näitaja 2,7 last naise kohta, siis 1999. aastal 1,4 last naise
kohta. Mõnes riigis viimastel aastatel kasvanud sündimus on aga kaasa toonud EL-i
keskmise näitaja tõusu (2007. aastal 1,5 last naise kohta). Sünnitaja keskmine vanus
arenenud riikides kasvab. Nooremas eas sünnitamine on tavapärasem Euroopa idaosas.
Lääne-Euroopa riikides seevastu on suurem nende naiste osakaal, kes sünnitavad
35-aastaselt või vanematena. Möödunud kümnendi jooksul kasvas sünnitaja keskmine
vanus kõige kiiremini Tšehhi Vabariigis, Balti riikides, Ungaris ja Sloveenias – vähemalt
kaks aastat.
Viimase 50 aasta jooksul on EL-i keskmine imikusuremuskordaja langenud enam kui
36 surmalt 1 000 elussünni kohta viiele. Riigiti on näitaja väärtus erinev, kõrgem on
imikusuremus Euroopa idaosas.
EL-i rahvastiku keskmine eeldatav eluiga sünnimomendil on poole sajandi jooksul
kasvanud kaheksa aasta võrra. 2006. aastal oli meeste keskmine eluiga veidi üle kuue
aasta lühem (75,8) kui naistel (82,0). Meeste ja naiste eluea vahe sünnimomendil ja
hilisemas vanuses on hakanud vähenema enamikus EL-i liikmesriikides. Aastal 2006
oli 65-aastaste keskmine eeldatav eluiga naistel 20,4 ja meestel 16,8 aastat.
Pikem eluiga, meditsiini areng ja inimeste teadlikkus tervislikest eluviisidest on kaasa
toonud tervelt elatud eluaastate kasvu. Mehed elavad võrreldes naistega suurema osa
elust tervisepiiranguteta. Seevastu naised, kelle keskmine eeldatav eluiga on meestest
pikem, elavad pikema perioodi oma elust tervisest tingitud piirangutega. Naiste
eeldatav eluiga oli 2007. aastal 6,2 aasta võrra kõrgem kui meestel. Kuid tervelt elatud
aastatelt edestavad nad mehi vaid 0,7 aastaga. 2007. aasta andmetel elavad naised
EL-s tervelt keskmiselt 62,3 ja mehed 61,6 aastat.
Surmapõhjused erinevad vanuseti. Hingamiselundite haigused on lapseeas
surmapõhjusena juhtival kohal, asendudes teisme- ja varases täiskasvanueas õnnetuste
ja muude välispõhjuste tagajärjel saabunud surmadega. Vähktõbi (pahaloomulised
kasvajad) ja vereringeelundkonna haigused on peamiseks surmapõhjuseks vanemas
eas.

EL-is on kõige levinumad surmapõhjused nii meeste kui naiste hulgas vähktõbi ja
südame isheemiatõbi. Lääne-Euroopa riikides on suremus vähktõppe kõrge vanemas
eas, Euroopa ida- ja lõunaosas aga nii nooremas kui vanemas eas. Suremus südame-
veresoonkonna haigustesse väheneb Lääne-Euroopas kiiresti. Samas erinevus
südame-veresoonkonna haigustesse suremuses on Lääne-Euroopa ning Kesk-ja
Ida-Euroopa riikide vahel kuni kümnekordne.
Vigastustest tingitud surmad on surmapõhjustest Euroopas kolmandal kohal.
Ligikaudu 800 000 inimest kaotab igal aastal vigastuste tõttu elu. Vigastused
moodustavad kõigist surmapõhjustest aastas keskmiselt 9% ja haiguste tõttu kaotatud
eluaastatest 14%. Enesetappude arv on Euroopas kõrge – keskmiselt 15,1 suitsiidi
100 000 elaniku kohta. Liiklusõnnetustes kaotab Euroopas igal aastal elu 127 000
inimest, sealhulgas 32 000 alla 25-aastast noort. Liiklusõnnetustest põhjustatud
enneaegsete surmade ja töövõimetuse tõttu kaotab Euroopa rahvastik aastas 3,6
miljonit tervena elatud eluaastat.
Mittenakkushaigused ja nakkushaigused
Euroopas on suurim haiguskoormus tingitud mittenakkuslikest haigustest nagu
südameveresoonkonna haigused, vähktõbi, vaimse tervise häired, suhkurtõbi,
kroonilised hingamisteede haigused ja lihasluukonna haigused. Vereringeelundite
haigused olid 2007. aastal suure osa EL-i riikide puhul peamiseks haiglaravi
põhjuseks.
Jätkuvalt on tuberkuloos tõsine rahvatervise probleem. Eesti, Läti, Leedu ja Bulgaaria
on kõrgema haigestumusega EL riigid.
HIV esinemissageduse osas on EL-i liikmesriikide seas suured erinevused. Kõige
kõrgema näitaja on Eestis (1,3% rahvastikust). Ka kõrgeim uute registreeritud juhtude
arv oli Eestis (472 juhtu ühe miljoni elaniku kohta).
Hinnangud tervisele ja arstiabi kättesaadavusele
Euroopa sissetulekuid ja elamistingimusi käsitleva uuringuga (EU-SILC) kogutakse
muuhulgas infot täiskasvanud elanikkonna üldise terviseseisundi ja arstiabi
kättesaadavuse kohta. Tulemused näitavad, et Skandinaavia riikide ja anglosaksi
maade elanikud hindasid oma tervist üldiselt paremaks kui Lõuna- ja Ida-Euroopa
riikides elavad inimesed.
Tervishoiu ressursid ja kogukulud
Haiglavoodite arv 100 000 elaniku kohta on vähenemas üle kogu Euroopa, kuid
võrreldes Lääne-Euroopaga on Ida- ja Lõuna-Euroopa näitajad kõrgemad. 2007.

aastal oli EL-is keskmiselt 590 ravivoodit 100 000 elaniku kohta. Kümne aasta jooksul
on ravivoodite arv 100 000 elaniku kohta vähenenud ligi 15% võrra ja kasvanud
on ambulatoorse tervishoiuteenuse, sealhulgas päevaravi, osakaal kõigist osutatud
tervishoiuteenustest.
Oluline ressurss tervishoiusüsteemis on tervishoiutöötajad. Praktiseerivate arstide
arvu kasv ühe elaniku kohta on Lääne-Euroopas viimastel aastatel pidurdunud.
Ida-Euroopa riikides hakkas arstide arv vähenema 1990. aastate esimesel poolel ja
stabiliseerus 2000. aastate alguseks. Õdede arv on Lääne-Euroopa riikides järsult
kasvanud, kuid Ida-Euroopas vastupidiselt hakkas 1990. aastast õdede arv langema.
Langus kestis käimasoleva kümnendi alguseni. Õdede arv 100 000 elaniku kohta
on üldiselt suurem Lääne-Euroopas ja erinevus Ida- ning Lääne-Euroopa vahel on
aeglaselt kasvanud alates 1990. aastate keskelt.
Tervishoiule tehtud kulutused, mida avaldatakse protsendina sisemajanduse
koguproduktis (SKP) ja summana ühe elaniku kohta, erinevad riikide vahel
märkimisväärselt. Kulutused on Lääne-Euroopas kiiremini kasvanud kui Ida-Euroopa
riikides.

Population overview
In European countries, every seventh person is aged 65 years or more, and this
proportion is growing while the proportion of children is declining. Predictions are
that the ratio of elderly (aged over 65 years) to people of working age could more than
double by 2050 in the EU. There are three factors that are at the heart of Europe’s
ageing society: persistently low fertility rates, increasing life expectancy, and baby-
boom generations that reach soon retirement age.
The EU fertility rate fell almost twice from an average of 2.7 children per woman
in 1964 to 1.4 by 1999. However, the recent increase in the number of births in
several Member States led to a modest rise in average EU fertility rates (1.5 in 2007).
In developed countries women have tended to have their children later in life. The trend
for postponing birth was, in the last decade, most prevalent in the Czech Republic, the
Baltic countries, Hungary and Slovenia, where the average age of women giving birth
rose by at least two years. In general, births to young mothers are most common in
the eastern part of the Europe and births to mothers aged over 35 years are more
frequent in western countries.
In the course of the last fi ve decades the average infant mortality rate in the EU
fell from over 36 deaths per 1,000 live births to just fi ve. There, however, remain
differences in rates across Member States. Infant mortality is higher in the eastern
part of the region.
The EU’s population is characterised by a relatively high life expectancy. This statistic
has increased by eight years for both men and women during the last half of the
century. Although life expectancy is six years higher for women (82.0) than for men
(75.8) in 2006 due to persistently higher male mortality throughout the entire life
cycle, the gap between the sexes has narrowed in recent years in the majority of the
Member States. In 2006, the average man of age 65 years in the EU could be expected
to live an additional 16.8 years and woman 20.4 years.
Rising life expectancy, medical progress, and greater public awareness as regards
healthy lifestyle choices may all explain the increasing trend in the number of healthy
life years. Men are likely to spend the largest proportion of their lives free from
limitations in everyday activities caused by health problems. Women could expect to
live a slightly lower proportion of their lives free from limitations caused by health,
although their overall life expectancy at birth is higher than this for men. In 2007, life
expectancy for women was 6.2 years higher than for men. However, women have only
for 0.7 years longer healthy life than men. On average, women live healthy for 62.3
and men for 61.6 years in EU.
The importance of specifi c causes of death differs signifi cantly in the different phases

of the lifespan. Respiratory diseases are leading causes of death in childhood, replaced
by accidents and other external causes of death among adolescents and young adults.
Cancer (malignant neoplasm) and diseases of the circulatory system become leading
causes of death in older age groups.
The most important causes of death among men and women in the EU are cancer and
ischaemic heart diseases. The average cancer mortality in western countries is relatively
high among the older population. In the eastern and southern part of the Europe; cancer
mortality is high in both younger and older age groups.
CVD mortality rates have been decreasing in Western Europe in recent decades.
However, there is an up to 10-fold difference in premature CVD mortality between
Western Europe and countries in central and eastern part of Europe, with the highest
rates in the east.
Injuries and other external causes are the third leading cause of death in EU. About
800,000 people losing their lives to injury-related causes each year. Injuries and
violence account for 9% of all causes of death and are responsible for 14% of all the
disability adjusted life-years lost in Europe.
Suicide rates are relatively high in the Europe, the average suicide prevalence rate is 15.1
per 100,000 inhabitants.
Each year 127,000 people, including 32,000 people younger than 25 years, in Europe
lose their lives to road traffi c injuries. Road traffi c injuries in Europe result in an
annual loss of 3.6 million years of healthy life due to premature death or disability.
Non-communicable and communicable diseases
The greatest disease burden in Europe comes from non-communicable diseases,
a group of conditions that includes cardiovascular disease (CVD), cancer, mental
health problems, diabetes mellitus, chronic respiratory disease and musculoskeletal
conditions. Diseases of the circulatory system accounted for the highest number of
hospital discharges in 2007 in the vast majority of EU countries.
Tuberculosis remains an important public health problem in Europe. Estonia, Latvia,
Lithuania and Bulgaria are EU Member States that are among the high-priority
countries.
There are huge disparities of HIV prevalence rates among EU Member States. Estonia
is a country with the highest rates – 1.3%. Also Estonia had the highest incidence (472
cases per 1 million inhabitants).

Self-perceived health and access to health care
The European Statistics of Income and Living Condition (EU-SILC) survey contains
a module on general health status of adult population as well as questions on the
unmet needs of health care. Results show that people in Scandinavian and Anglo-
Saxon countries were observed to have better self-perceived health in comparison to
those live in Southern or Eastern European Member States.
Health care resources and total expenditure on health
The number of hospital beds per 100,000 population is declining throughout the
Europe, but levels in eastern and southern part remain higher than those in western
part. In 2007 there was an average of 590 hospital beds per 100,000 inhabitants
in EU. Within past ten years the overall reduction in total number of hospital beds
have been about 15 % and among all health care services proportion of out-patient
treatment, including day care, has increased.
Health care personnel is a signifi cant resource of health care system. The number of
physicians per capita in Western European countries steadily rose until recently. The
previously high and increasing rates in Eastern countries declined in the fi rst half
of the 1990s and then stabilised by the beginning of 2000. The rate of nurses per
100,000 inhabitants in Western Europe grew sharply; eastern part showed a much
higher rate before 1990, which sharply declined from the early 1990s into the 2000s
and then became stable. Thus, the gap between average rates for the two groups of
countries has steadily grown since the mid—1990s.
Health expenditure, both as a percentage of gross domestic product (GDP) and per
capita, varies signifi cantly between countries. Expenditure is increasing faster in
Western Europe than in Eastern Europe, thus further increasing the east—west gap.

EESTI TERVISESTATISTIKA
Käesolev osa annab ülevaate haigestumuse, haiglaravi, ambulatoorse arstiabi ja
tervishoiuressursside statistikast 2007. aasta andmete põhjal.
Esmashaigestumuses ei ole viimasel kümnendil toimunud suuri muutusi. Erinevate
haigusrühmade osakaal on püsinud samal tasemel. Lastel1 esineb kõige sagedamani
hingamiselundkonna haigusi, millele järgnevad vigastused ja mürgistused, nakkus-
haigused, nahahaigused ja kõrvahaigused. Täiskasvanute2 hulgas moodustavad
samuti suurima osakaalu hingamiselundite haigused, seejärel tulevad lihasluukonna
haigused, vigastused ja mürgistused ning kuse- ja suguelundite haigused. Vanemas
eas kasvab silma- ning vereringeelundite haiguste osakaal.
Rohkem kui pool kõigist vigastustest on toimunud meestega. Vaid uppumist ja
tahtlikku enesekahjustamist esineb meeste ja naiste hulgas enam-vähem võrdselt.
Peamiselt on vigastused põhjustatud kukkumisest ja mehhaanilise jõu toimest.
Vigastuste arv on aastatel 2000–2007 kahekordistunud 10 896 juhult 100 000
elaniku kohta 21 396 juhule.
Psüühika- ja käitumishäireid esineb poistel oluliselt rohkem kui tüdrukutel. Meeste
psüühika- ja käitumishäired on kõige sagedamini tingitud psühhoaktiivsete ainete
tarvitamisest, millele järgnevad neurootilised, stressiga seotud ja somatoformsed
häired. Naiste puhul on aga somatoformsed häired esikohal. Sageduselt järgmisena
esinevad naistel meeleoluhäired. Läbi aegade on psühhoaktiivsete ainete
tarvitamisest tingitud psüühika- ja käitumishäired põhjustatud valdavalt alkoholist.
Rasedate arvelevõtmise ja rasedusaegsete uuringute arv on viimaste aastate jooksul
pidevalt tõusnud. 1992. aastal võttis end enne 12. nädalat rasedusega arvele
50% ja 2007. aastal 80% naistest. Nende osakaal, kes end rasedusega arvele ei
võta, moodustab ligikaudu ühe protsendi kõigist sünnitajatest ja see näitaja pole
oluliselt muutunud. 1992. aastal tehti ultraheliuuring enne 21. rasedusnädalat
30%-le ja 2007. aastal juba 94%-le sünnitanutest. Looteveeuuringute maht on
samal perioodil kasvanud 0,2%-lt 6,8%-le. Kardiotokograafi a osakaal on kümne
aasta jooksul kahekordistunud, tõustes 30%-lt 1998. aastal 67%-ni 2007. aastal.
Vastsündinutel on sagenenud mitmete haiguste esinemine. Ajavahemikul
2001-2006 on kahekordistunud sünniperioodile iseloomulikud nakkused, mis
on tõusnud 33-lt juhult 1000 elussünni kohta 64-le. Vastsündinu kollatõbi on
kasvanud 16-lt juhult 1000 elussünni kohta 37-le. Lisaks näitab ühtlast kasvutrendi
vastsündinu respiratoorne distress: 2000. aastal 16 ja 2007. aastal 26 juhtu 1000
elussünni kohta. Kaasasündinud väärarendite puhul on kasv 25 diagnoositud juhult
2000. aastal 36 juhule 2007. aastal.

Täiskasvanutel on peamiseks statsionaarse ravi põhjuseks olnud vereringeelundite
haigused ja lastel hingamiselundite haigused. Haiglaravi kestus on aastate jooksul
kahanenud. Pikima keskmise haiglaravil viibitud ajaga on täiskasvanute hulgas
jätkuvalt psüühika- ja käitumishäirete ning nakkushaigustega patsiendid. Laste hulgas
on pikim ravikestus samuti psüühika- ja käitumishäirete korral, millele järgnevad
sünniperioodis tekkivad seisundid.
Haiglast väljakirjutatute arv 1000 elaniku kohta ei ole täiskasvanute osas viimastel
aastatel oluliselt muutunud. Meestel jääb see näitaja 170 ja naistel 200 piiresse.
Laste puhul on vastav näitaja 2007. aastaks võrreldes 2000. aastaga langenud:
poistel 190-lt 170-le, tüdrukutel 160-lt 149-le. Languse peapõhjuseks on haiglaravil
viibimise vähenemine hingamiselundite haiguste tõttu. Haiglaravi põhjustest on
aastatel 2000–2007 meeste puhul veidi tõusnud kasvajate ja naistel puhul langenud
kuse- ja suguelundite haiguse osakaal. Ülejäänud haigusrühmade puhul on haiglaravil
viibinute arv 1000 elaniku kohta olnud suhteliselt samal tasemel.
Viimase seitsme aasta jooksul on märgatavalt kasvanud päevaravi saanute arv:
3,7 meest 2000. aastal ja 24 meest 2007. aastal 1000 mehe kohta, naiste puhul
vastavalt 14,9 ja 56,2 naist 1000 kohta. Laste hulgas on päevaravi saanute arvus
toimunud veelgi suuremad muutused. Kui 2000. aastal viibis päevaravil 4,8 poissi
1000 kohta, siis 2007. aastal 30,6. Tüdrukute puhul vastavalt 4,5 ning 26,1 tüdrukut
1000 kohta. Naiste peamiseks päevaravi põhjuseks on rasedus ja sünnitus, millele
varasematel aastatel järgnesid kuse- ja suguelundite haigused, viimastel aastatel on
aga teisele kohale tõusnud silmahaigused. Meestel oli varem peamiseks päevaravi
põhjuseks lihasluukonna haigused, millele järgnesid silmahaigused. Viimastel aastatel
on need põhjused omavahel kohad vahetanud. Laste puhul on sõltumata soost pidevalt
olnud päevaravi peamiseks põhjuseks hingamisteede haigused.
Kirurgiliste protseduuride osas on silmaoperatsioonid ning naha ja nahaaluskoe
operatsioonid endiselt valdavalt teostatud ambulatoorselt ja päevaravis (~90%).
Ainult statsionaaris teostatakse seevastu keerukamaid kirurgilisi protseduure, näiteks
südame- ja suurte torakaalveresoonte operatsioone.
Arsti ambulatoorsed vastuvõtud on aastatel 2000–2007 enamike erialade puhul
kasvanud. Eriti märgatav tõus on olnud perearstide vastuvõttude arvus. Perearstil oli
2000. aastal 1000 täiskasvanu kohta 1388 ja 2007. aastal 3085 vastuvõttu. Tuhande
lapse kohta oli perearstivastuvõtte 2000. aastal 1678 ja seitse aastat hiljem 3978.
Täiskasvanute hambaravi külastuste arv on aastatel 2000–2007 peaaegu viiendiku
võrra langenud, täpsemalt 1314 külastuselt 1000 täiskasvanu kohta 1064-le.
Laste hambaarstikülastuste arv on vastupidiselt täiskasvanutele umbes 20% võrra
kasvanud, 1684 külastuselt 2029-le. Ligikaudu viiendiku võrra on hambaravis
tõusnud röntgenuuringute arv - 153 uuringult 1000 elaniku kohta 2000. aastal
181-le 2007. aastal.

Kiirabi väljakutsete arv on kümne aasta jooksul kasvanud 9%. Samal ajal vähenevad
inimeste endi pöördumised kiirabiasutustesse. Alates 2004. aastast on umbes
kolmandiku võrra vähenenud haigete, rasedate ja sünnitajate vedu. Samuti väheneb
väljakutsete arv traumade tõttu.
Tervishoiuteenuse osutajate arv on aastate jooksul vähe muutunud. Suuremad
muutused on toimunud haiglate ja iseseisvalt õendusabi osutavate asutuste arvus.
Võrreldes 2004. aastaga on 2007. aasta lõpus haiglaid kuue võrra rohkem. Iseseisvat
õendusabi osutavate asutuste arv on nelja aastaga peaaegu kolmekordistunud –
kaheksa asutust 2004. aastal ja 22 asutust 2007. aastal.
Haiglaravi osutamist saab muuhulgas hinnata ravivoodite kasutamise järgi.
Voodikoormus ja sellega seotud voodihõive ning voodikäive on tasapisi tõusnud.
Keskmine ravikestus haiglates on viimasel viiel aastal püsinud kaheksa päeva juures.
Haiglaliigiti on keskmine ravil viibitud aeg kõige lühem eri- ning keskhaiglates.
Voodikäive on madalaim hooldushaiglates, kus tulenevalt haiglas pakutavate
tervishoiuteenuste eripärast on ravikestus kõige pikem. Voodikäive on kõrge
aktiivravihaiglates, neist kõige kõrgem keskhaiglates.
Alates 2004. aastast, kui jõudis lõpule tervishoiutöötajate ümberregistreerimine
Tervishoiuametis (alates 01.01.2010 Terviseamet), on suurel osal erialadest arstide
ja õendusalatöötajate arv vähe muutunud. Arstide, õendusalatöötajate ja hooldajate
keskmised palgad aastate jooksul tõusid. Samas oli nende tervishoiutöötajate
gruppide keskmise palga suhe riigi keskmise palgaga 2007. aastal võrreldes 2006.
aastaga praktiliselt muutumatu. Arstide palk oli 2007. aasta märtsis esimese
kvartali riigi keskmisest palgast kaks korda suurem. Õendusalatöötajate töötasu jäi
riigi keskmisele tasemele. Hooldajate palk moodustas riigi keskmisest 58%.
Tervishoiuvaldkonna peamiseks rahastajaks on Eesti Haigekassa. Võrreldes 2004.
aastaga on 2007. aastal Eesti Haigekassa osakaal valdkonna rahastamisel natuke
langenud 66%-lt 64%-le. Seevastu riigieelarvest tulev osa on kasvanud 8,5%-lt
9,7-le. Ja inimeste omaosalus jäi peaaegu samaks 21%-lt 22%-le.
Tervishoiukulutustest suurima osa moodustavad raviteenused. Raviteenuste osa-
kaal kõigist kuludest oli 1999. aastal 63% ja 2004. aastaks langesid need 52%-le,
mis on samas suurusjärgus 2007. aasta kulutustega. Kulutused meditsiinitoodetele
on selle perioodi jooksul 5% võrra kasvanud, moodustades 30% kõigist kuludest.
Ülejäänud tervishoiuteenuste rahastamise osas on muutused väiksemad.

ESTONIAN HEALTH CARE STATISTICS
The second part of the guide gives an overview of the statistics of incidence,
in-patient, out-patient health services and health care resources in Estonia. The charts
contain the data of 2007.
No signifi cant changes have taken place in incidence over the past decade. The
share of different disease groups has remained at the same level. Children3 suffer
most frequently from diseases of the respiratory system, followed by injury and
poisoning, infectious diseases, diseases of the skin and ear. Diseases of the respiratory
system also account for the largest share among adults,4 followed by diseases of the
musculoskeletal system, injury and poisoning, and diseases of the genitourinary
system. At an older age the rates of eye and circulatory system diseases rise.
Over a half of all injuries happen to men. Only drowning and willful self-injury is more
or less equal among men and women. The injuries are mostly caused by falling and
mechanical forces. The number of injuries has doubled through 2000-2007 from
10,896 cases per 100,000 inhabitants to 21,396 cases.
Boys have considerably more mental and behavioural disorders than girls. Mental
and behavioural disorders among men are most frequently caused by consumption
of psychoactive substances followed by neurotic, stress-related and somatoform
disorders. Somatoform disorders are the most frequent among women. The next in
line in terms of frequency among women is mood (affective) disorders. Throughout
history the mental and behavioural disorders caused by consumption of psychoactive
substances can be attributed mainly to alcohol.
The number of registration of pregnant women and pregnancy examinations has
constantly risen in recent years. 50% of women in 1992 and 80% in 2007 registered
themselves before the 12th week of pregnancy. The share of those who do not register
their pregnancy amounts to approximately 1% of all childbirths and this indicator has
not changed much. In 1992 ultrasound examinations were made before the 21st week of
pregnancy to 30% and in 2007 to 94% of the childbearing women. The rate of amnion
fl uid examinations has risen from 0.2% to 6.8% over the same period. The share of
cardiotocography has doubled in ten years, rising from 30% in 1998 to 67% in 2007.
Occurrence of various diseases has increased among the newborn. Through
2001-2006 infections specifi c to the perinatal period have doubled from 33 cases
per 1,000 live births to 64 cases. Neonatal jaundice has risen from 16 cases per
1,000 live births to 37 cases. In addition, respiratory distress of newborn is showing a
constant growth trend: 16 cases per 1,000 live births in 2000 and 26 cases in 2007.
In the case of congenital malformation the number of diagnosed cases has risen from
25 in 2000 to 36 in 2007.

For adults, the main reasons for in-patient treatment have been diseases of the
circulatory system and for children, diseases of the respiratory system. The duration
of hospitalisation has decreased over the years. Among adults, patients with mental
and behavioural disorders and patients with infectious diseases are hospitalised
for the longest time. Among children the longest time of hospitalisation is also
related to mental and behavioural disorders, followed by conditions originating in
the perinatal period.
The number of discharges per 1,000 residents has not changed much among adults
in recent years, being about 170 for men and 200 for women. In the case of children
the respective fi gures have fallen by 2007 in comparison with 2000 from 190 to
170 for boys and from 160 to 149 for girls. The main reason for the decrease is
the reduction of hospitalisation caused by diseases of the respiratory system.
As for the reasons of hospitalisation, through 2000 -2007 the share of tumours
among men has slightly risen and the share of diseases of the genitourinary system
among women has slightly fallen. In the case of other disease groups the number of
hospitalised people has remained at the same level per 1,000 inhabitants.
Over the last seven years the number of people who have received day care has
risen substantially from 3.7 to 24 per 1,000 men and from 14.9 to 56.2 per 1,000
women. Among children the number of those who have received day care has
changed ever more. In 2000, 4.8 boys received day care per 1,000 boys and the
rate has risen to 30.6 by 2007. In the case of girls the rate of day care receivers per
1,000 girls has risen from 4.5 to 26.1 over the same period. In the case of women
the main reasons for day care are pregnancy and childbirth, followed by diseases
of the genitourinary system in previous years and in recent years the second rank
has been taken by diseases of the eye. In the past the main reason of day care for
men was diseases of the musculoskeletal system, followed by diseases of the eye.
In recent years these reasons have changed places. In the case of children the main
reason for day car regardless of the gender has been diseases of the respiratory
system.
As for surgical procedures, surgeries of the eye, skin and subcutaneous tissue
are being carried out mainly in out-patient and day care (approximately 90%).
More complicated surgical procedures such as cardiovascular surgeries and
surgeries of major thoracic vessels are carried out only in in-patient care.
The number of out-patient visits has risen across most specialties through
2000-2007. The fi gures have risen the most in the case of family doctors. A family
doctor had 1,388 visits per 1,000 adults in 2000 and 3,085 visits in 2007. In 2000
the number of family doctor visits per 1,000 children was 1,678 and seven years
later the fi gure was 3,978.
The number of adults’ dental visits has decreased nearly by a fi fth through
2000-2007, i.e. from 1,314 visits to 1,064 visits per 1,000 adults. The number of
children’s dental visits has risen by approximately 20%, i.e. from 1,684 to 2,029.

The number of calls to emergency medical care has risen by 9% over ten years.
At the same time the number of people’s direct visits to emergency medical care
institutions is dropping. As of 2004 the transport of the ill, pregnant and women giving
birth has fallen by about a third. Also, the number of calls attributable to injuries is
falling as well.
The number of health care service providers has changed little over the years. Major
changes have taken place in the number of hospitals and institutions providing
independent nursing assistance. Compared to 2004 there are six more hospitals
in 2007. The number of institutions providing independent nursing assistance has
nearly tripled in four years: 8 institutions in 2004 and 22 institutions in 2007.
Hospitalisation can, among other things, be evaluated based on bed utilisation.
Bed occupancy and the related bed occupancy rate as well as bed turnover have
slowly increased. The average length of stay in hospitals has remained around eight
days in the last fi ve years. Time spent in hospital was the shortest in special and
central hospitals. Bed turnover is the lowest in long-term care hospitals where, due to
the characteristics of the health care services provided in the hospital, the length of
stay is the longest. Bed turnover is high in active treatment hospitals and the highest
in central hospitals.
As of 2004 when the re-registration of health care professionals in the Health Care
Board (since 01.01.2010 Health Board) was completed, the number of doctors and
nurses has changed little in most of the specialties. The average wages of doctors,
nurses and caregivers rose over the years. At the same time the ratio of the average
wages of these health care professional groups to the average wage of the state in
2007 did not virtually change in comparison with 2006. In March 2007 the wage of
physicians was twice as high as state average in the fi rst quarter of 2007. Nurses’ and
midwives’ wages were on the same level as the state average and the wage received
by caring personnel amounted 58% of the state average.
The main fi nancier of the health care is the Estonian Health Insurance Fund. The
Fund’s share of total health expenditure has a little bit fallen from 66% in 2004 to
64% in 2007. On the other hand the state budget portion has increased from 8.5%
to 9.7%. And out-of-pocket expenditure has remained almost at the same level: from
21% to 22%.
Curative care services account for most of the health care expenditure. The share of
curative care services in all expenditure in 1999 was 63%. By 2004 it had fallen to
52%, which is in the same range as the expenditure in 2007. Expenses of medical
products have risen by 5% over the period, amounting to 30% of all expenses.
Changes are smaller in fi nancing the remaining health care services.

DEFINITSIOONID JA SELETUSED
Haiglast lahkunud — kalendriaasta jooksul haiglaravi lõpetanud patsientide arv,
kes kirjutati välja või surid, lõpliku põhidiagnoosi järgi. Ei ole arvestatud teise haiglasse
üleviiduid.
Imikusuremuskordaja — alla ühe aastaste laste surmade arv 1000 sama aasta
elussündinu kohta.
Igapäevategevuste piiratus — pikaajalise haiguse või tervisehäire tõttu igapäeva-
tegevuse piiratus vähemalt viimase 6 kuu jooksul.
Lapsed — tervisestatistikas 0–14 aastased.
Päevaravi — plaanilises korras patsiendile osutatav tervishoiuteenus(ed), mille puhul
patsient viibib tervishoiuteenuse osutaja juures ühel päeval üle 6 tunni ning lahkub
ravilt samal päeval.
Ravikestus — kalendripäevade arv hospitaliseerimise ja haiglast lahkumise vahel.
Samal kuupäeval lahkumine loetakse üheks päevaks, hospitaliseerimise ja haiglast
lahkumise päev samuti üheks.
Sisemajanduse koguprodukt (SKP) — residentide toodetud lisandväärtuse summa
kogurahvamajanduse ulatuses, millele on lisatud netotootemaksud.
Tervishoiu kogukulud (THKK) — residentide tervisega seotud kaupade ja teenuste
lõpptarbimise kulud, millele on lisatud tervishoiuteenuse pakkujate kapitalikulud.
Täiskasvanud — tervisestatistikas 15 -aastased ja vanemad.
Voodihõive — päevade osakaal aastast, mille jooksul ravivoodit kasutatakse.
Voodikoormus — päevade arv aastas, mille jooksul ravivoodit kasutatakse.
Voodikäive — keskmiselt ravitud haigete arv ühe ravivoodi kohta aastas.

DEFINITIONS AND EXPLANATIONS
Adults — in health statistics people aged 15 and over.
Average length of stay — total number of occupied hospital bed-days for
a discharged inpatients within a given period divided by the total number of
discharges in the period by the cause of hospital care.
Bed occupancy — number of days per one year when a bed is occupied.
Bed occupancy rate — number of days when hospital bed is occupied as
percentage of all year.
Bed turnover — average number of patients per one hospital bed in a year.
Children — in health statistics people aged 0-14.
Day care — health care services provided by an in-patient or out-patient health
care provider, within the patient has to stay for treatment or examination more than
6 hours, but do not stay overnight.
Gross domestic product (GDP) — the sum of the gross value added of all
resident producers (institutional sector or industries) at basic prices, plus taxes less
subsides on products (which are not allocated to sectors and industries).
Hospital discharges — total number of patients discharged from hospitals during
the given calendar year by the principal diagnosis. Discharge is the conclusion of
a period of in-patient care, whether the patient returned to his home or died, but
without patients transferred to another hospital.
Infant mortality rate — deaths of children less than one year of age per 1,000 live
births of the same year.
Limitations in everyday life - activity restriction due to illness or health problem
for at least the past 6months.
Total health care expenditures — expenditures on health care services and
goods for the country’s residents where added health care providers’ capital
formation expenses.
