Coordinated School Health Program Proven
Successes• Planned, sequential health education
resulted in a 37% reduction in the onset of smoking among seventh-grade students.
• The prevalence of obesity decreased among girls in grades 6–8 who participated in a school-based intervention program.
• Students who participated in a school-based life skills training program were less likely to use tobacco, alcohol, or marijuana than were students not enrolled in the program.

How is Terrier Care Funded?
• Terrier Care will operate at CCHS, but will be administered with staff and funds that are independent of the school.
Affordability• Terrier Care will provide services without
the ability to pay, as Terrier Care offers a financial assistance program based on family size and income
• Terrier Care will bill insurance, Medicaid, Kidcare, etc. If your insurance program requires a co-pay, you will also have a co-pay at Terrier Care
Services for Students• In order for a student to be seen
and treated by Terrier Care the child must be enrolled in the Terrier Care program
• Enrollment forms and parental consents are being mailed to each child’s parent or guardian
Services for Students• Services are age-appropriate and address
the most important health needs of CCHS students.
• Services will include but are not limited to:– Primary care for acute and chronic health
conditions– Sports injury assessment and treatment– School and sports physicals
Services for Students• Social Services• Substance abuse
services• Case
management
• Nutrition education
• Health education • Health promotion
Services for Students by Referral Only
• Pregnancy testing• Sexually Transmitted Disease
testing and treatment• Family Planning • Case Management procedures will
be monitored for these referrals
Student Access to Services
• Student can access Terrier Care by making an appointment or on a walk-in basis
• Student services will only be provided with parental consent
• If a student is not enrolled and presents at Terrier Care the school nurse will see the student following the current policies in place
Services for Staff and Faculty
• Staff and faculty will receive comprehensive services including:– Pregnancy testing– Sexually Transmitted Disease testing
and treatment– Family Planning
Access to Services for Staff and Faculty
• Staff and Faculty can access Terrier Care by making an appointment or on a walk-in basis.
• Staff and Faculty may register on site for services
Confidentially• All services at Terrier Care will remain
confidential. Terrier Care will act in compliance to the Health Insurance Portability and Accountability Act (HIPAA)
• Your protected information means health information, including demographic information, any information collected from you and created or received by the provider, another provider, a health plan, employer or a health clearing house.
