KAREN T. BRAKE, MD, FAAFP Dr. Brake is a board-certified physician who specializes in family practice and a Fellow of the American Academy of Family Practice. She earned her medical degree from the University of Illinois and completed her family medicine residency in Sylvania, Ohio. Dr. Brake’s personal philosophy

is “We should be learning something new every day of our lives.”

GENA LATouCHE, FNP-BC Virginia “Gena” Latouche is a board-certified family nurse practitioner who earned her bachelor’s and master’s degrees in nursing from the University of Southern Indiana. Gena is from Valencia, Venezuela, and believes “You can’t sit around waiting for something to happen to you. Make things happen for you!”

Activate Healthcare is excited to partner with Old National to provide access to Health & Wellness Centers throughout the ONB footprint. Activate currently operates 14 locations in Indiana with plans for continued expansion.

Activate’s facilities provide associates and dependents (over the age of 3), who participate under an Old National medical plan, access to convenient comprehensive primary and acute health care and wellness services delivered by experienced physicians and advanced practice providers.

ACTIVATE’S SERVICES INCLuDE:

+ Primary/Acute Health Care – Illnesses, condition management (asthma, diabetes, etc.), lab/bloodwork, allergies, and other medical needs for which you’d normally consult a doctor or medical clinic

+ Preventive Care – Physicals, flu shots, health coaching, health profiles

+ Pharmaceuticals – Generic prescription medications dispensed on site

ACTIVATE’S SERVICES ARE:

+ Affordable: Services are free for PPO medical plan participants. Due to IRS rules, High Deductible Health Plan (HDHP) participants will incur a small cost ($20/visit and $5/filled prescription) for non-preventive services until plan deductibles are met.

+ Completely confidential as required by law. Information can be shared with your personal physician based on your direction and authorization.

+ Convenient: More time with the doctor and less time waiting.

Your Activate Health & Wellness Center is the place where you can go for a wide range of health services. You will be able to use the center for: + Primary health care services, including sick visits at

low or no cost*

+ Care for chronic illnesses

+ Low- or no-cost medicines prescribed by the clinic providers and available at the clinic*

+ Provider can serve as your primary care provider

+ Free lab tests when done at the clinic

+ Orders for bloodwork needed by other providers, performed at no charge

+ Health education

HoW To uSE THE CLINIC

+ Call to schedule an appointment.

+ You can usually get an appointment on the same day.

+ Go to the clinic at the time of your scheduled appointment.

+ You will be seen by the doctor or another medical provider who can assist you.

WHEN To uSE THE CLINIC

Go to the Activate Health & Wellness Center if you:

+ Want regular primary care, including annual physicals

+ Are sick

+ Have a condition that needs to be checked regularly

+ Have a child that needs a sports physical

+ Take prescription medicine

+ Need lab work or a blood sample taken

+ Would like personal wellness coaching

PRESCRIPTIoN REFILLS

+ If you are prescribed medicine by the clinic provider and you need a refill, call the clinic and ask for a refill over the phone.

+ It should be ready for you to pick up at the clinic within 24 hours.

* Participants on the high-deductible plan who have not met their deductible will be charged $20 for office visits and $5 per prescription for non-wellness appointments.
