Overview – PD HomeCare

Field test of electronic documentation for home peritoneal dialysis. (In peritoneal dialysis (PD), the peritoneum acts as the dialysis filter) Daily documentation of home PD improves the quality of dialysis. Daily documentation of medication allows for timely adjustment by the attending physician.


Goals

• Electronic communication between physician and patient • Local acquisition of PD data (Cost reduction as manual entry by dialysis center nursing staff is eliminated) •  Blood pressure, blood sugar and weight automatically acquired from the respective monitoring devices (Multiple manual data entry avoided) •  Daily transfer of dialysis data to the dialysis center (Patient usually visits the dialysis center and consults their physician only once a month) •  Documentation of medication (Transparency for the attending physician) • Regimen guidelines, drug plans (Treatment guidelines) • Electronic feedback
