Class Attendance

• 100% attendance and attention is expected in all tracks and courses. Attendance is documented by instructors. • A maximum of three excused absences are allowed per career track to retain Test Pass Guarantee privileges for

exams. Late arrivals and early leaves are considered full absences. • For an absence to count as excused, the student must contact Student Services via phone or email prior to or on the

date of the absence. In addition, the student must perform all reading and other assignments required to keep up in the course. In the event a student is absent for more than one day (or two evenings) of a class, student must meet with the instructor and Career Services to develop a plan for the student to remain in such class. Until such meeting occurs, student may be removed from the track schedule at the discretion of Centriq.

• Student should review the track schedule frequently to verify the dates of upcoming courses. Student must contact Student Services if unable to attend a scheduled course.

Exam Scheduling and Test Pass Guarantee Policies

• In order to schedule an exam through Test Pass Guarantee, a student must have met the following requirements: • Practice Exams -Completed three practice tests with scores above 90% within the prescribed time period. Copies

of these successful practice exams must be verified in the system at the Front Desk. • Attendance - Only three excused absences are allowed per career track to utilize Test Pass Guarantee. (see

Class Attendance section above to review what qualifies an absences as “excused”). Attendance records are checked before exams are scheduled. If requirements are not met, student is responsible for paying for the any exam retakes.

• Class Performance -Student must complete all projects and assignments assigned by instructors throughout the TechSmart KC. Instructors document this information on a weekly basis. Student Services will confirm this information with the associate at the Front Desk before you utilize Test Pass Guarantee for an exam attempt.

• Exams must be scheduled via email ([email protected]) and at least 24 hours in advance. • If student fails an exam on the first try, he or she must i) complete and turn in three more practice exams above 90%;

and ii) complete all steps on the Exam Retake Process Form (provided by Front Desk upon the exam fail). The mentor reserves the right to approve the student for a retake or require additional steps to be taken. Mentoring sessions are scheduled via Student Services.

Formal Mentoring (Face-to-Face)

• Formal mentoring is to be utilized to meet Test Pass Guarantee requirements or to obtain clarification on course content. It is not to be utilized as a review session for unexcused class absences.

• In order to schedule a mentoring session, students must provide Student Services with the following : o A list of available meeting time. o Copies of all practice tests and failed exams. o Explicit list of topics he or she requests the instructor to cover. (This list must be emailed to the instructor 24

hours in advance of the session or instructor reserves the right to cancel the session.)

1 Centriq Training Overview

1.1 About Centriq Training In August 2005, Centriq, LLC and Foss Training Center joined forces to form Centriq Training - the premier

information technology (IT) training company in Kansas City. Centriq and its predecessor companies provided instructor-led technology training on the latest versions of computer software and network technologies to corporate staffs for over fifteen years. Foss Training had been the area's leading IT training company for individuals preparing for careers in the information technology field. The new Centriq Training blends the best practices of these two leading organizations into a single, full service IT training company. Today Centriq Training provides high-quality IT training for both experienced IT professionals already employed in the technology field (Corporate Program) and for individuals seeking to enter the information technology field or to upgrade existing IT skills (TechSmart KC Program).

1.2 Centriq Training Location Centriq Training operates a training center in Leawood, Kansas. For directions or a map to Centriq Training,

please visit www.centriq.com or call (913) 322-7000.

1.2.1 Leawood Location. The Centriq Training facility is located in Leawood, Kansas. This location also serves as the company’s corporate offices.

Address: 8700 State Line Road, Suite 200, Leawood KS 66206 (located across State Line Road from the Ward Parkway Shopping Center). Phone: (913) 322-7000 Facility: This location has 16 classrooms, including a multi-function room that accommodates seminar seating for up to 50 people. Each classroom is equipped with whiteboards, projector, computers and internet access. Amenities: Coffee, tea, water and snacks are available to students at no additional charge. The facility has a student lounge area furnished with dining tables and a television. There is also a kitchen area which includes a refrigerator and microwave for use by students. There are also soda machines and a snack machine for students’ convenience. Parking: Ample free parking surrounds the building. 1.3 Centriq Training Mission Statement To provide the highest quality technology training designed to prepare students for careers in the technology

industry. 1.4 Ownership of Centriq Training

Centriq Training is owned by Centriq, LLC, a Kansas limited liability company.

2 Industry Affiliations, Governmental Approvals and Academic Partners of Centriq Training

2.1 Industry Affiliations Centriq Training is affiliated with and/or has received approvals from the following industry organizations.

2.1.1 Microsoft Partner Network Gold Partner For Learning Solutions

Microsoft Partner Network Gold Partner for Learning Solutions is the highest level of accreditation provided by Microsoft Corporation to a training partner after meeting strict instructor, delivery, and classroom requirements designated by Microsoft. CPLS qualifies Centriq Training to deliver authorized Microsoft world-class certification training, learning services, and Microsoft Certified Professional exam preparation.

2.1.2 CompTIA Computing Technology Industry Association

The Computing Technology Industry Association (CompTIA) is the leading association representing the international technology community. Its goal is to provide a unified voice, global advocacy and leadership, and to advance industry growth through standards, professional competence, education and business solution. CompTIA has more than 13,000 members in 89 countries.

2.1.3 Cisco Learning Partner Associate (CLPA) The Cisco Learning Partnership authorizes Centriq Training to provide authorized Cisco training. All Cisco Learning Partner Associates are required to meet strict guidelines in instruction, hardware, classroom, and courseware.

2.1.4 EC-Council Accredited Training Providers (ATP).

ATP’s deliver hands-on instruction to e-business and security professionals who seek to become proficient in building, deploying, and maintaining e-Business and security projects. Training Partners are certified to teach each of the authorized curricula for e-Business and security technologies. Centriq Training has met the rigid standards in instructor certification, course delivery, adherence to training facility requirements, and ongoing quality commitment.

2.1.5 Authorized Prometric Testing Center Thomson Prometric is the global leader in technology-enabled testing and assessment services for academic,

professional, government, corporate, and information technology markets. Thomson Prometric delivers standardized tests for 300 clients, in 26 languages, over the Web or through a global network of testing centers in 134 countries.

2.1.6 Authorized VUE Testing Center Pearson VUE provides innovative electronic testing solutions that enhance the performance, reliability and

security of high-stakes testing programs throughout the world. Pearson VUE is the computer-based testing business of Pearson Education, which in turn is a part of Pearson, the international media company.

2.2 Governmental Approvals Centriq Training has received approvals from the following governmental bodies.

2.2.1 Kansas Board of Regents The Kansas Board of Regents authorizes Private and Proprietary Schools offering instruction on

business, trade, technical or industrial occupations to operate in the State of Kansas with a Certificate of Approval. This authorization is renewed annually, and Centriq Training is required

to meet strict standards to be approved by the Kansas Board of Regents.

2.2.2 Kansas Commission on Veterans’ Affairs Centriq Training is approved by the Kansas Commission on Veterans’ Affairs under the

provisions of section 376 of Title 38 U.S. Code as a provider of veterans’ educational benefits in the State of Kansas for specific Centriq Training programs. This approval does not imply that the student is eligible for VA training assistance. Eligibility is determined on an individual basis and is a separate determination from the approval of training programs. Please contact Student Services to learn more about applying for these educational benefits if you believe that you may be eligible.

2.2.3 KansasWorks/Workforce Partnership Centriq Training is a WIA approved Eligible

Training Provider pursuant to the Workforce Investment Act (WIA) and the Local Workforce Investment Board of Local Area III. WIA mandates that training providers and their respective programs be re-certified annually to meet strict requirements on programs and student completion rates. Centriq Training is listed on the KansasWorks.com website as a delivery resource for technology training programs.

2.2.4 Missouri Full Employment Council Centriq Training is approved by the Missouri Full Employment Council as a delivery resource for

technology training programs. 2.3 Academic Partners

Centriq Training has a relationship with the following academic institutions.

2.3.1 Grantham University University of Phoenix Grantham University and the University of Phoenix both offer the opportunity to document the

knowledge you have obtained by completing the Centriq training career track programs with credit. College credit is made available for students who successfully complete Centriq training courses that are instructor-led. This provides the ability for students and employers to take advantage of company tuition reimbursement funds and other tuition assistance resources for coursework that is both in demand and essential for skill development.

3 Centriq TechSmart KC Instructors 3.1 Instructor Qualifications Centriq instructors are rigorously evaluated prior to joining the training staff. They are knowledgeable

experts in their individual fields and have proven their skills as expert trainers. Centriq instructors are dedicated to helping each student succeed through individualized instruction, mentoring, and the creation of class projects than can be used as models for real-world projects after training is complete. All technical instructors are required, as part of their professional development plan, to become Certified Technical Trainers (CTT+) - a highly respected industry certification awarded by CompTIA - a leading association representing the technology community.

3.2 Instructor Staff

Centriq instructors currently include the following individuals - either as full-time or part-time instructors:

Sandy Bentch Sandy is a Certified Technical Trainer (CTT+) with over 30 years of experience as a consultant and trainer. Sandy also has a Masters of Business Administration from UMKC. She has taught a wide variety of classes, including Project Management Essentials, Office 2003/2007/2010/2013, Crystal Reports, ITIL Foundations, ITIL Practitioner- Support and Restore, Java programming, C++, and Object Oriented Analysis and Design. Sandy enjoys learning new technologies, customizing content for clients, and engaging her students with an interactive teaching style.

Eddie Dressler Ed has over 13 years of training experience and 16 years of IT experience. His certifications include CTT+, MCTS, MCSA, MCDBA, MCT and MCP. He has developed applications, administered and designed databases, and managed large projects in enterprise environments. This allows him to bring a real world flavor into his classroom discussions and enables him to communicate with students from varied backgrounds and levels of expertise. He enjoys teaching seasoned professionals, as well as first-time developers and technicians. Andy Duncan Andy teaches CompTIA A+, Network+, and Microsoft server classes within the IT Administrator TechSmart KC program. He earned his Bachelor’s Degree in Technology from DeVry and has worked within the Information Technology field since 1994. His certifications include CompTIA A+, CompTIA Network+, Green ILT and MCP. He has provided IT support to over 250 clients. Andy also volunteers his personal time to community activities, including coaching, boy scouts, and leadership within local associations. Brian Griffiths Brian is a Certified Technical Trainer and teaches A+, I.T. Administrator courses 1-6 and Security + for our TechSmartKC program. Brian is a graduate of our program and has eight years of experience in I.T. and three years in I.T. training, focusing on desktop and network support. He currently runs a PC repair and small business network support company, and previously taught I.T. courses for a technical college. Brian holds certifications in CompTIA A+, MTA in Networking, CompTIA Security+, Polycom CVE and VBrick VNCE. In his spare time he enjoys playing bass guitar and building custom motorcycles. Brian Harsh Brian is a Certified Technical Trainer and teaches I.T. Administrator courses 1-8 and Security + in the TechSmartKC program. Brian is a 2011 graduate of Centriq Training and spent a year and a half providing Desktop Support and I.T. Training for a national law firm. He holds certifications in CompTIA A+, CompTIA Security+, MTA: Networking Fundamentals and is a Microsoft Certified Professional. His expertise lies in Microsoft Server, Windows, Office and Networking. Kevin Hay Kevin teaches I.T. Administrator courses 1-4 in the TechSmartKC program. Kevin graduated from Centriq

in 2010 and then went on to work for Centriq Training for four years as the Systems Administrator and Classroom Set-Up Specialist. Prior to working for Centriq, Kevin ran his own residential PC repair company specializing in virus and malware removal. He also spent over 8 years as a Senior Product Support Specialist for a medical device company where he assisted authorized dealers with hardware installations, product training and troubleshooting. Kevin has earned the following certifications; A+, MCP, MCTS: Vista, MCSA. In Kevin’s spare time he enjoys spending time with his daughter, power lifting and strength training and playing guitar in his band.

Hai Hilvitz Hai has been in the I.T. industry for the past 21 years and has been training for the past 17years. Hai teaches I.T. Administrator courses 1-8 for our Career Track program and provides for a unique learning experience that combines book information and open discussions between students and instructor. His teaching style is rooted in his experience dealing with the product and an understanding of the student’s learning needs. Hai is a Certified Technical Trainer and holds the following certifications: CCA, CCEA, CCI, MSCE, MTA: Networking Fundamentals, MCT, CompTIA A+, and CompTIA Network+. Doug Hoff Doug started computer instruction in 1983 and is a Microsoft Certified Trainer, as well as a CompTIA Certified Trainer. He has been a graphic artist, pre-press manager, webmaster, project manager, marketing manager and trainer. Doug was a leader for both Windows and Macintosh user groups while in California where he taught at a junior college. He has used PHP and Java in building web sites and is a certified Sun Java Programmer. Since being in Kansas City, he has been an instructor at Sprint/Nextel and the Kansas City Art Institute in software development, web design, graphics, and Java courses. At Centriq he teaches a full range of Business Skills, ITIL, .NET, Java, XML, and web courses. Neill Horton Neill has been in the IT training industry for over 15 years. He is experienced in developing .NET and Java applications and transfers his knowledge to students in Centriq’s Application Developer TechSmart KC program. His passion for technology and learning are visible in his always-energetic classroom environments. He currently specializes in teaching .NET/C#, MVC, Entity Framework, ADO.NET, ASP.NET, Windows Forms. HTML5, XHTML, CSS3, XML, XMLSchema, DTD, XSLT, SQL, JavaScript, jQuery and Mobile Development using responsive design and media queries. Kasama Kasemvudhi Kasama is an MBA with 14 years of IT experience and 13 years of technical training experience. His experience has focused upon server/network support, security management, and project implementation. Kasama teaches for our Career Track program as well as our Corporate clients. He is a Certified Technical Trainer and holds additional certifications in CompTIA Security+, CompTIA Network+, CompTIA A+, MCP, MTA: Windows Server Administration, MTA: Windows Operating System, MTA: Networking, MCTS and MCDST. He is passionate about technology and enjoys creating a classroom environment that is open, informative, energetic and fun. Erin Long-Crowell Erin teaches Desktop Application courses on our Corporate side and Business Communications to our I.T. Administrator students for Centriq’s Career Track program. Erin has a background in training and instruction, as well as experience with web design and online and print marketing. She holds two Bachelor of Science degrees and an M.Ed in Adult Education. Erin enjoys gaming, genealogy and photography. She describes her teaching style as engaging, respectful and supportive. Janet Nichols Janet has been in the IT and computer training industry for over 20 years and started teaching in 1984. She has been a Certified Novell Instructor since 1991 and a Microsoft Certified Trainer since 1996. She holds MCITP certifications in Server 2008 Enterprise, Server 2008 Administration, and Windows 7. She has taught, held certifications and authored numerous courses and several certification exams for Hewlett Packard. She has also held certifications from Intel as well as others. Her area of expertise is networking disparate systems. Steve Nichols Steve has been in the IT industry for 32 years and has spent the last 25 in the IT Training industry. He is MCT, MCSE, MCNE, MCTS, and CTT certified. He has travelled across the world learning and teaching technologies he is passionate about. He is a subject matter expert in Novell, CompTIA, Microsoft Server Technologies, Database Technologies, SharePoint 2007/2010, VMware, System Center Configuration Manager, and Cisco. He is adept in breaking down complex concepts so all users can understand.

Jason Presley Jason has earned certifications in MCPD and MCITP, as well as holding his CTT+. He holds a Bachelor of Arts degree in Education and has been a technical trainer for over five years. Jason specializes in HTML/CSS, SQL and ASP.NET. He enjoys spending time with his family, repurposing furniture, doing home improvement projects and spending time with friends around his fire pit. Eric Reid Eric is a dynamic, Certified Technical Instructor who blends his business and technology experience into his classes. He is a Microsoft Certified Professional and holds certifications in MTA-Web Development Fundamentals, Master CIW Designer, ACE Adobe Certified Expert in Dreamweaver, Flash and Photoshop CS3, among several others. Eric has been in the I.T. field for 14 years and has been a trainer 12 of those years. His areas of expertise are C#, HTML, CSS, JavaScript, jQuery, Angular, SQL, Microsoft Office and Adobe Creative Suite Software. Bob Reinsch Bob has been a Microsoft Certified Systems Engineer and Microsoft Certified Trainer since 1995. He began his study of computers in 1977 and started working full-time in the IT field in 1987. He has taught for and held certifications for HP/Compaq and IBM, as well as CompTIA and others. He has been a technical editor on 22 books, as well as a presenter and Technical Learning Guide at Microsoft's TechEd Conference and COMDEX. He also specializes in Exchange, Windows Server, Security, and Windows 7. Mike Rissen Mike has been a programmer for over 19 years in the financial services and customer relationship management (CRM) industries. His is a subject matter expert in C#.NET, VB.NET, HTML, T-SQL, and Microsoft SQL Server. He follows the Microsoft patterns and practices team and has taken part in setting coding standards for both C# and VB.NET. He is passionate about learning new technologies and enjoys sharing his knowledge with his students. Classes taught by Mike are energetic and interactive. Robert Ritter Robert has been in the IT industry for 15 years, and he has spent 11 of them in a technical instructor role. He is MSCE, MCP, MCTS, CCDA, A+, and CTT certified. He has vast experience designing and implementing networks for large organizations in the Kansas City area. He is a subject matter expert in Microsoft Windows, Server 2008, and Exchange. In addition to teaching Microsoft operating system and networking classes for Centriq TechSmart KCs, Robert also teaches Powershell and Server classes on the corporate schedule. Karla Sharp Karla, a Certified Technical Trainer, is a Desktop Applications and SharePoint instructor with 15 years of training experience. She has delivered sales and technical end-user training in her roles at Sprint’s Business Sales Academy, T-Mobile, and O.C. Tanner. She specializes in Microsoft Office 2007/2010/2013, SharePoint End User and Site Owner, and Microsoft CRM. Karla maintains an informative, entertaining learning environment where students are encouraged to stay involved with plenty of hands-on exercises. Keith Smith Keith is a Microsoft Certified Technology Specialist and a Certified Technical Trainer for our Application Developer, TechSmart KC program. Keith graduated from the Application Developer program in 2009. He then was employed as a Developer and a Systems Analyst prior to joining our instructor team in September 2013. She is a graduate of the program and prior to joining the Centriq team she was an Internal Operations Director for a local funeral home. In addition to development, Megan has extensive knowledge of team management skills, Photoshop and Social Media outlets. He is a subject matter expert in Visual Studio, customer service excellence, and project management. He enjoys incorporating enjoyable projects and exercises into the classroom, as well as developing courseware. Jerrod Wilson Jerrod holds certifications in CompTIA A+, Microsoft Technology Associate in Networking and CompTIA Security+. He is a graduate of our I.T. Administrator program and worked at a local architecture firm as a Help Desk Technician prior to joining our instructor team. 5 Centriq Training TechSmart KC Program Objectives 5.1 Network/Systems Administrator Track

Program Length: 65 Days (455 hours) Daytime or 104 (455 hours) Evenings Vendor Certifications Obtained: CompTIA A+, MTA 98-366, Cisco Certified Entry Network Technician (CCENT), and CompTIA Security+.Prerequisites: Interests in computer hardware and networking, fundamental PC & Microsoft Windows skills, basic typing skills, take free aptitude test. Who should consider this training program? Individuals who seek to update their certifications, or a career changer who enjoys computers and operating systems and interacting with people. Program Description: The duties of a Microsoft administrator can include building, fixing and troubleshooting computer hardware, administering Windows Operating and Networking systems. The IT Administrator program at Centriq Training uses lecture, discussions and hands-on labs that will give the students the knowledge, skill-sets and confidence to perform their duties as a Microsoft administrator. The students will leave the program with the knowledge and ability to plan, administer, support and troubleshoot computer hardware, Windows 2012, PowerShell, Active Directory, Windows Networking, and Windows 8 technologies. Additionally, students will learn security fundamentals and how to support Cisco routing devices.

5.2 Application Developer/Web Developer/Programmer Track

Program Length: 65 Days (455 hours) Daytime or 104 Evenings (455 hours) Prerequisites: Interests in programming, fundamental PC & Microsoft Windows skills, basic typing, take free aptitude test. Who should consider this training program? Individuals who wish to update their programming skills or a career changer with symbolic reasoning ability, enjoys math, problem solving and/or puzzles. Those who enjoy learning foreign languages and musicians tend to make good programmers. Program Description: The student will learn to develop websites and leading-edge applications using the Microsoft .Net framework which is utilized by a vast majority of Kansas City employers. Emphasis is in C#.Net, MVC, HTML, CSS, Responsive Web Design, SQL and Web Forms along with additional open-source tools. Candidates will also complete real world projects (web development and software development) and develop a portfolio to showcase those projects. This program includes a laptop computer, carrying case, and software.

5.3 Computer Support Specialist Program Length: 20 days (140 hours) Daytime or 32 Evenings (140 hours)

7 TechSmart KC Financial Assistance and Payment Options

7.1 Payment Options Enrollment in a Centriq Training TechSmart KC must be secured by one of the following payment options:

credit card, student loan, cash, personal check, ACH, direct deposit or verifiable grant approval through VA, VR, or WIA. Enrollment is confirmed upon receipt of payment.

7.2 Financial Assistance Centriq Training is a private, proprietary technical school and, as such, is not eligible to accept traditional

FAFSA aid such as Pell Grants, Stafford Loans, etc. However, there are several government grants and private student loans for which students who enroll in a Centriq Training TechSmart KC may be eligible. Following is a brief summary of grants and student loans for which Centriq Training is an eligible institution:

7.2.1 Department of Veterans Affairs (VA)

Students who are or were active duty or reserve duty military may be eligible for VA grants such as the Montgomery GI Bill. Please consult Student Services to determine your eligibility.

7.2.2 Kansas and Missouri Departments of Vocational Rehabilitation (VR)

These grants are designed for students who have a physical or mental impairment that prevents them from performing their job and requires that they be trained in a new field that is not as physically challenging. To determine eligibility, please consult Student Services.

7.2.3 Workforce Investment Act (WIA) This grant provides training funds for those who live or worked in Kansas that are unemployed or underemployed. For instructions on how to apply for this grant, please consult the Student Services.

7.2.4 Missouri Full Employment Council (FEC) This grant provides training funds for those who live or worked in Missouri that are unemployed or underemployed. For instructions on how to apply for this grant, please consult the Student Services.

7.2.5 Trade Act Funds Trade Act funds are available for displaced workers whose jobs were outsourced overseas. . For instructions on how to apply for this grant, please consult the Student Services.

8 Centriq Training TechSmart KC Cancellation and Refund Policy

A full refund will be provided when Centriq Training receives written cancellation notice from Student more than 14 calendar days prior to the Track Start Date or within 3 business days from the signing of the Track Agreement. Cancellation notices received thereafter but prior to the Track Start Date will result in Centriq Training retaining $250 to cover enrollment expenses and retaining additional funds to cover the cost of any and all materials (e.g., textbooks or laptop) purchased for, and delivered to, Student. Cancellations are not allowed after the Track Start Date. See Withdrawal/Refund Policy below for rules pertaining to Student’s withdrawal after the Track Start Date.

9 Student Success Keys and Student Responsibilities

9.1 Keys to TechSmart KC Student Success As training at Centriq serves as a stepping stone into a professional career, students are expected to treat their

education the same as a professional job. Attendance and hard work are standards for accomplishing goals. Success is also determined by the student’s ability to perform the following:

• Comprehend course material. • Complete section labs, readings, and assigned projects. • Thoroughly review and internalize written resources. • Pass certification exams corresponding with course materials.

Throughout a student’s track, support is offered and provided by Centriq Training instructors, Student Services, and staff. At all times, students will have resources to help them succeed. At the end of each course, students receive a Certificate of Achievement to indicate the successful completion of the course requirements. (Attendance policies apply. See Section 12.1 Attendance)

9.2 TechSmart KC Student Responsibilities Student responsibilities include consistent school attendance, conscientious effort in classroom work and

career development, and compliance with Centriq Training’s Disciplinary Procedure. It is the responsibility of each student to respect the rights of all who are involved in the educational process. Should a student compromise the rights of another student, he or she will be subject to the consequences listed in the Centriq Training Disciplinary Procedure (Section 15).

Students will be asked to read and sign a document agreeing to the following responsibilities:

• Be aware of and exhibit good conduct. • Attend and be on time for all classes. • Notify the school anytime an absence occurs (or is foreseen to occur), as well as provide documentation. • Make up all missed coursework as outlined by instructor or Student Services in order to stay on track. • Follow rules and regulations. • Cooperate with school staff in operating a safe and professional school. • Pursue and complete the course of study. • Express ideas and opinions in a respectful and accurate manner. • Prepare for all classes and complete assignments given by instructor. • Notify instructor and Student Services of learning issues or when assistance is needed. • Exercise proper care when using public facilities and equipment. • Dress and groom in a clean and appropriate manner. Clothing that contains obscene or offensive

messages will not be permitted.

10 TechSmart KC Enrollment Agreement Terms and Conditions 10.1 Entrance Requirements To be admitted to any Centriq Training TechSmart KC program, a prospective student must complete an interview with a Centriq Training Educational Representative. In addition, the following specific requirements must be met regarding age, prior education, and demonstrated proficiency in the basic skills required for study at the post-secondary education level.

• Each applicant must be at least 18 years old by the first day of classes. Documentation of age may be required, as well as a parent/guardian signature for students wishing to enroll who are under age 18.

• Each applicant must be a high school graduate, hold a General Education Development (GED) certificate, or have a degree from an accredited postsecondary institution.

• Each applicant must demonstrate the ability to benefit from the program through testing and an interview. An aptitude test is available and typical for students who have no prior technical experience. A typing assessment is available for students considering the Application Developer track.

10.2 Payment Payment is due prior to the start of the TechSmart KC. Please see Section 7.1 for details. 10.3 Written Notice of Cancellation or Withdrawal In the event a student elects to cancel his or her TechSmart KC Enrollment Agreement prior to the start of

the TechSmart KC or elects to withdraw from the TechSmart KC program after the start of the TechSmart KC, the student must notify Centriq Training in writing in order to receive any refund. Please see Section 8 above for the terms applicable to Cancellation refunds. Please see Section 10.7 for the terms applicable to Withdrawal refunds.

10.4 One Year Expiration of Benefits

Track students are given one year from the start date of the first scheduled course in their track to complete their courses and exams. Any courses not attended or exams not taken within one year from the date of the course shall be forfeited. Other benefits, including, but not limited to, use of practice exams, study guides, computer lab, and other student benefits will also expire at the end of such one year period. Any requests for extending benefits beyond one year will be at the discretion of Centriq Training. Select courses/curriculum and/or certification requirements may change as dictated by the applicable technology vendor. Should curriculum and/or certification exam requirements change during the course of the training track, Centriq Training reserves the right to charge students for additional exams and/or updated curriculum as required by the new vendor certification requirements. Prices charged to students will not exceed the cost incurred by Centriq Training to the vendor.

10.5 Grading Policy Centriq Training is a vocational, job-readiness, and computer training center, but it is not a grading

institution. TechSmart KC students are subject to strict attendance policies in order to receive Certificates of Achievement for courses taken (See Section 12.1). Vendor certifications are earned by passing exams corresponding with courses taken at Centriq Training

10.6 Track Exam Fund Policy

In the event Track Exams are included in the Track Agreement, Centriq Training will designate from the Total Track Agreement Price the appropriate sum to cover these costs. Student has the right to request disbursement of the Track Exam funds to Student for any Track Exam the Student has not taken. Such disbursement of funds requires a written request specifying the Track Exam(s) for which the funds are being withdrawn, will result in the elimination of the Test Pass Guarantee privileges for such Track Exam(s), and will result in a reduction in pre-paid Track Exams to the extent of such disbursement. Student shall have thirty days from the end of the one year term of the Track in which to request disbursement of any unused Track Exam funds or such funds shall be forfeited. All disbursement checks will be made out jointly to Student and the lender to be applied to any educational loans, if applicable.

10.7 Withdrawal/Refund Policy Student may withdraw from the Track at any time after the Track Start Date. All Track withdrawal notices must be made in writing. Should Student elect to withdraw completely from the Track after the Track Start Date, this Track Agreement will be terminated and Student will be refunded the unused portion of the Total Track Agreement Price. The unused portion of the Total Track Agreement Price will be calculated by subtracting the following from the Total Track Agreement Price: i) the regular retail pricing for the Track Courses the Student attended, in part or in full: ii) the cost of Other Items received by Student; iii) any reschedule or other fees incurred by Student; and iv) all Track Exam funds for Track Exams which Student has taken. Student must notify Centriq Training in writing of the election to withdraw completely from the Track at least seven (7) business days prior to their next Track Course or Student will be deemed to have attended such Track Course. Tracks are sold at a substantial discount because they allow Centriq Training to pre-fill course seats in advance, and because an entire bundle of courses is purchased in advance. No refund, except for unused Track Exam funds, will be made after Student has attended, in part or in full, 50% of the Track Courses or if Student withdraws during the last 50% of the one year term of the Track. Other Items include laptops, practice exams, test preparation software, mentoring, PC Pass classes, Career Service Program services, classes and workshops; reschedule fees and other Materials and Services received by Student to enhance the learning experience. Once received as part of the Track, these items will be retained by Student. Student may request a review for an exception to this policy in writing to Centriq Training. The decision of Centriq Training is final.

10.8 Laptop Computer (Developer and Network Administrator Students Only)

Some Centriq Training Tracks include a laptop that meets or exceeds the requirements set by Centriq Training. Laptop models and configuration may vary slightly within a course. Once the laptop is issued and signed off on by the student, all issues with the laptop or its components are at the sole responsibility of the student (including contacting the manufacturer for service or repair). In the event a laptop is sent to the factory for repair during the Track program, a loaner laptop – subject to laptop availability and contingent upon the student signing the Centriq Training Loaner Laptop Policy - will be provided by Centriq Training to the student. Centriq Training is not responsible for the theft of any student laptop. Laptop damage due to neglect or abuse is the responsibility of the student. Centriq Training is also not responsible for support of peripheral hardware and software packages that are distributed with laptops.

Per Centriq’s Laptop Policy (which is signed upon receiving track laptop), students shall not install hardware/software (i.e. Steam, games, new hard drives, SSDs, additional software not related to the track, etc.) unless approved by Centriq Student Services. Such actions will void the laptop’s hardware warranty. If Centriq IT Support or instructors are engaged to fix or troubleshoot issues that are deemed related to any of the examples listed above, students will be charged $50 per hour. In addition, a valid credit card must be presented before work will take place. Centriq is not liable for issues or damages resulting from the attempt to repair.

10.9 No Job Placement Centriq Training is a technical training center and is not a job placement agency. Student Services is available to guide and provide career related training; however, job placement is solely the responsibility of the student.

10.10 Termination Policy

Centriq reserves the right to terminate the Track Agreement and expel Student with no refund for the following reasons:

• Student violates Centriq Training’s drug/alcohol or weapon policy as stated in the Student Handbook. • The Total Track Agreement Price is not paid in the time specified or the funds are insufficient. • Student is acting in a manner that is disruptive and hinders the learning of other students in

Student’s Track or in any other Track or training program of any kind held at Centriq Training or is otherwise in violation of the Student Conduct provisions of the Centriq Training Handbook.

Should the Track Agreement be terminated by Centriq Training for any of the foregoing reasons, Student forfeits all sums paid to Centriq Training. Student may file for an appeal of any such termination with Centriq Training for re-admittance or for a refund. Refunds and re-admittance will be assessed on a case by case basis and will be entirely at the discretion of Centriq Training.

10.11 Acknowledgement of Terms and Conditions of TechSmart KC Contract By signing the contract at the beginning of the TechSmart KC program, the student acknowledged that he or

she read the terms and conditions of the contract, that he or she understands the terms and conditions of the contract, that he or she received a copy of the signed contract, and that no other agreements were made verbally or otherwise to the student. The contract went into to effect once all parties signed and dated the contract.

11 TechSmart KC Course Schedule 11.1 Course Enrollment Students in Centriq Training TechSmart KC programs will be enrolled in their entire series of courses at the

beginning of their track. Start dates may be changed at the discretion of Centriq Training. Centriq Training will make every effort to advise students of this event with a reasonable amount of advance notice and will provide alternate dates as soon as they are available. Track courses are arranged in a consecutive manner, as material from early classes serves as a foundation for classes occurring later in the track. Because of this arrangement, it may be necessary to reschedule an entire track if a student is absent for several classes in his or her program.

11.2 Current Course Schedules Student Services at Centriq Training has published listings of current and future TechSmart KC course

schedules. To request an extra copy of a schedule, inquire about future classes and tracks, or reschedule a course, please contact Student Services.

For non-track students, individual class and corporate class schedules are posted at www.centriq.com.

11.3 Rescheduling of Courses

Student may reschedule to attend any Track Course that is offered by Centriq Training on a different date subject to space availability and Student meeting all prerequisites of such course; however, the Track must be completed within the one-year time period set forth above. In order to reschedule a Track Course, Student must notify Centriq Training in writing at least one week in advance of the start date of such Track Course. Failure to attend a scheduled Track Course without prior notification two times will result Student being placed on a “space availability basis” for the remainder of the Track, meaning that if a course is full, Student will be the first student to be bumped from the course. There is a $250 reschedule fee for each Track Course that is rescheduled by Student. This fee is waived by Centriq Training unless the Student withdraws from the Track as set forth in the Withdrawal/Refund Policy below. Centriq Training reserves the right to reschedule the start of a Track or a Track Course for the following reasons: insufficient student enrollments, instructor availability, or human or natural disaster. All Tracks will start within 3 months from the originally scheduled Track Start Date. Should Centriq Training be the sole cause of a delay exceeding 3 months, Centriq Training will offer Student a refund on interest incurred on any applicable student loans. Should Student’s first Track Course be delayed more than three months, Student has the right to: i) terminate the Track Agreement without penalty; ii) change into a different Track; or iii) wait for the next available Track . Student acknowledges that there may be a cost differentiation for any different Track and accepts full financial responsibility to pay any increase.

12 TechSmart KC Policies 12.1 Attendance In order to successfully complete Centriq Training TechSmart KC programs, 100% attendance and attention

is expected in all classes. TechSmart KCs are meant to prepare students for the professional world; therefore, being present and punctual is standard.

12.1.1 Absence Limit Policy Centriq Training understands that illness and personal emergencies may occur. However, just as in

an employment setting, students are expected to communicate with Centriq Training any time these circumstances occur. In order to earn a complete transcript and utilize the Test Pass Guarantee, only three excused absences are allowed per TechSmart KC program. Late arrivals and early leaves are counted as an absence. For an absence to qualify as “excused”, the student must notify Centriq Training Student Services via email or phone prior to or on the date of the absence, as well as perform all reading and assignments to stay on track with other students in the course. In the event a student is absent for more than one day (or two evenings) of a class, the student may be removed from the track, and required to enroll in the next session, at the discretion of Centriq Training.

12.1.2 Guidelines for Absences beyond Limit

If more than one day (or two evenings) of a single course are missed, the student is responsible for contacting the instructor and Student Services immediately. The student will need to speak with both parties to develop a plan for moving forward in the track and/or obtaining missed course material. If all parties decide student will be able to catch up without resitting the course, documentation of reason for absence (ex- doctor’s note) must be provided to utilize Test Pass Guarantee. If a student misses more than the allowed absences and fails to contact the instructor or Student Services for the duration of the class, the student will need to reschedule the missed class and potentially his or her entire track. If two classes are not attended by a student, he or she may be removed from the track schedule by Centriq Training until communication is made.

12.1.3 Attendance Documentation and Test Pass Guarantee

Attendance is documented by instructors and will be referenced when students schedule exams. Test Pass Guarantee students who miss more than the allowed number of absences for a track will either be required to pay for corresponding test or resit the class. Students are required to keep track of their own absences in order to plan accordingly for testing fees.

If more than the allowed number of absences occur for any track, Test Pass Guarantee remains valid only if the student performs the following:

• Communicated reason behind absence and provided documentation to Centriq Training immediately.

• Completed all coursework and readings assigned through communication with instructor and Student Services to make up for missed class.

12.2 Attention in Class Students are expected to provide undivided attention to instructors during class time. Activities such as

instant messaging, reading non-course related materials, listening to music, sleeping, etc. are distracting and disrespectful to peers and instructors. Centriq Training reserves the right to remove a student from a class or program if disruptive behavior does not cease upon request.

12.3 Class Times Students should plan to arrive at least 15 minutes before class is scheduled to begin. Late arrivals are distracting to both instructors and peers in the classroom. Instructors report those who continuously arrive late and/or leave early to Student Services, and a note is placed in the student file regarding his or her attendance record. In fairness to all students and staff, students who arrive more than 30 minutes late or leave more than 30 minutes early on a continuous basis may need to be rescheduled for another course date. Students are responsible for checking on the exact time their classes start.

Day classes usually begin at 8:30 a.m. and end at 4:00 p.m., with breaks and a lunch period included. Evening usually classes begin at 6 p.m. and end at 10 p.m., with breaks included. 12.4 Computer Usage Policy TechSmart KC students have access to designated Centriq Training computers in classrooms. In addition,

programming students have access to the laptops provided by Centriq Training for their track. Anytime a student is on a Centriq Training computer or has access to a Centriq Training network, the following activities are strictly prohibited:

• Downloading music or videos. • Viewing inappropriate internet websites which includes, but is not limited to, the following:

o Militant/Extremist o Sex Education o Sexual Acts o Full/Partial/Artistic Nudity o Drug Culture o Satanic/Cult o Gross Depictions

• Utilizing password cracking programs, port scanners, sniffers and other such network hacking tools.

• Connecting external storage devices to Centriq Training computers unless approved by the Centriq Training instructor.

• Disabling antivirus software • Disabling services unless approved by the Centriq Training instructor. • Attempts to gain access to other students’ computers, and/or classrooms, and/or Centriq Training

servers or network equipment

Centriq Training shall retain the right to remotely access laptops and desktops for any computer while connected to the Centriq Training network to ensure compliance.

12.5 Grievance Procedure In cases of dispute or disagreement, students are encouraged to voice their concerns with Student Services.

Students have the right to appeal to Centriq Training. If the student does not feel the complaint has been resolved at the institutional level, they may contact the Kansas Board of Regents at (785) 296-4917, or on the KBOR website at http://www.kansasregents.org/private_postsecondary_complaint_process.

12.6 Previous Experience and/or Course Credit Students may receive credit for prerequisite knowledge or certain courses by providing proof of such

knowledge. Acceptable forms of proof include Certificates of Achievement from other approved training centers [Kansas Board of Regents approved institutions, Microsoft Certified Technical Education Centers (CTEC’s), Microsoft Certified Partners for Learning Solutions (CPLSs), and Authorized Training Providers of CIW] and test certifications from official testing centers. Students who believe that have already attained knowledge on portions of a course of study may ask for a one-on-one assessment with an instructor to determine if it is possible to "test-out" of that portion of the course and/or advance to a higher level.

12.7 Scheduling Certification Exams Exams must be scheduled at least 24 hours in advance. To schedule an exam, send an email

[email protected]. All required practice exam scores must be verified by an associate at the Front Desk. In the event of rescheduling a certification exam, students must abide the policy listed below.

12.7.1 Rescheduling Certification Exams (Testing Vendor Policy) Each time a student schedules a certification exam with the Front Desk, Centriq purchases an exam voucher from a testing vendor (Prometric, Vue, etc.). If a student needs to reschedule an exam time, all testing vendors require test takers to provide a minimum of 24 hours notice to the testing location (Centriq). If the student notifies Centriq of a reschedule with less than 24 hours notice, the voucher is lost. In this situation, the student’s Test Pass Guarantee privileges will be lost for that exam only. Student will have to purchase all future attempts to pass that specific exam. No exceptions will be given.

12.8 Retaking a Certification Exam

Students who fail an attempt to pass a certification exam will likely wish to retake the exam. TechSmart KC students without Test Pass Guarantee services will need to contact testing administrators at the Front Desk to learn about the retake process and exam fees. TechSmart KC students on the Test Pass Guarantee program will need to requalify in order to retake the test without any fee. See Section 13.8 entitled “Test Pass Guarantee” for requalification guidelines. All students should be aware that some vendors require a waiting period on retakes after a certain number of attempts. For example, Microsoft typically mandates a 14-day waiting period for all retakes after the second attempt. CompTIA typically mandates a 30-day waiting period for all retakes after the second attempt.

12.9 Rescheduling Courses or Tracks

Student may reschedule to attend any Track Course that is offered by Centriq Training on a different date subject to space availability and Student meeting all prerequisites of such course; however, the Track must be completed within the one-year time period set forth above. In order to reschedule a Track Course, Student must notify Student Services at least one week in advance of his/her next scheduled course. If the track the student wishes to reschedule into is full, the student will be placed on a waiting list and will be contacted if a seat becomes available.

Failure to attend a scheduled Track Course without prior notification two times will result Student being placed on a “space availability basis” for the remainder of the Track, meaning that if a course is full, Student will be the first student to be bumped from the course. There is a $250 reschedule fee for each Track Course that is rescheduled by Student. This fee is waived by Centriq Training unless the Student withdraws from the Track as set forth in the Withdrawal/Refund Policy below.

13 TechSmart KC Program Benefits

The content of each TechSmart KC and the optional benefits selected by the student may vary. All students should review their specific contracts regarding optional benefits they selected and paid for within their program.

13.1 Student Services Student Services should be the first point of contact for TechSmart KC student questions and concerns

regarding most topics, including educational issues, rescheduling classes, attendance notifications, scheduling mentoring sessions, and career search questions. To contact Student Services:

Students may retake any course for free an unlimited number of times for up to one year after their Centriq Training start date (on a space available basis). In such event, students must reuse their original course material. If the courseware or curriculum has changed since the last time the course was taken, the student must purchase another one. Only if the student has passed the certification exam that corresponds to the class (if applicable), will the student be allowed to sit a course without current curriculum. If a class a student wishes to re-sit is full, the student will be placed on a waiting list and will be contacted if a seat opens. On the waiting list, students who have not yet taken the course will take precedence over students re-sitting the course. In addition, all students wishing to re-sit will work with Student Services on a “Student Re-Sit Plan” which must be signed by the student. The “Re-Sit Plan” includes items listed above as well as additional guidelines; such as, mentoring sessions, attendance, participating in Career Services events, and instructor recommendations.

13.3 Practice Exams TechSmart KC students with certification practice exams included in their services can access practice exams

as directed by instructors and Student Services. In order to best utilize practice tests to prepare for exams, students should avoid using their courseware or notes to complete the questions. For students who have the Test Pass Guarantee included in their programs, see Section 13.7 for more information.

13.4 Centriq Training Study Hours:

Monday- Thursday 7:30 AM – 8:00 PM Friday 7:30 AM – 5:00 PM

Centriq Training suggests that students spend regular study time prior to attempting exams. Students may come in and use the facility to study during hours posted above. Remote access to practice testing is available on a limited basis. Please direct all practice testing related questions to Student Services.

13.5 Student Mentoring Centriq Training instructors are available for additional assistance by appointment, email support or such

other method as Centriq Training determines.

13.5.1 Formal Mentoring (Face-to-Face) Students may email formal mentoring requests to Student Services. Face-to-face mentoring is to be utilized only for Test Pass Guarantee students who have failed and wish to retake an exam, as well as for students who would like additional information on course material. Mentoring is not to be utilized as a review session for missed class time due to unexcused absences. To schedule a mentoring session, please contact Student Services with the following information:

1. Dates and times available to schedule mentoring.

2. Course name and number that mentoring will focus upon. 3. Student contact information to confirm session.

Once mentoring is scheduled with an instructor, the student must complete two tasks to prepare for the session:

1. Complete and make copies of practice exams and failed exams for instructor to review. 2. Email an explicit list of items the student wishes the instructor to cover in the session at least 24 hours in advance. Instructor email addresses will be provided by Student Services. If the student does not email the list to the instructor within 24 hours, the instructor reserves the right to cancel the mentoring session.

13.6 Test Pass Guarantee

The Test Pass Guarantee is an optional benefit included in certain Centriq Training programs The terms of the guarantee require that the student be fully prepared prior to taking the exams.

13.6.1 Guidelines for First Attempt at a Certification Exam:

1. Student must have no more than 3 excused absences during their TechSmart KC program. To

find what qualifies an absence as excused, please see section 12.1.1 to review attendance guidelines.

2. Student must complete all projects and assignments assigned by instructors throughout the TechSmart KC. Instructors document this information on a weekly basis. Student Services will confirm this information with the associate at the Front Desk before Test Pass Guarantee is utilized for an exam attempt.

3. Student must demonstrate that they have passed the prescribed practice tests three times at 90% or higher within the prescribed time period. All scores must be verified by an associate at the front desk an exam can be scheduled.

*Note- Test administrators will view the student’s attendance and project/assignment records before an exam is scheduled. If requirements outlined in the steps above have not been met, student is responsible for paying for the exam unless the student chooses to resit the track.

13.6.2 Guidelines for Subsequent Attempts at a Certification Exam:

Each time a student fails the certification exam, he or she will be required to requalify for Test Pass Guarantee as follows:

1. Student must demonstrate that they have passed the prescribed practice tests three times at

90% or higher within the prescribed time period. All scores must be provided to the front desk or [email protected] before an exam can be scheduled.

2. Student must follow the “Certification Exam Retake Process”, which is provided by the front desk associate when an exam is failed. Part of this process requires the student to schedule a face-to-face mentoring session with an instructor to review exam material. The mentor will answer questions regarding the material and make assessments to the readiness of the students to retake the exam. Students will need to follow any prescribed preparation steps assigned by the mentor before retaking the exam. Preparation steps can include, but are not limited to: reviewing specific materials, reworking lab exercises, performing small projects, and retaking specific classes.

As long as the student meets the mentor’s requirements for each attempt at an exam, Centriq Training will pay for the student to take the certification exam an unlimited number of times - for the duration of the student’s contract term - until the student passes. Please note also that certain vendors require that if candidates for certification exams have failed twice or more, they must wait 14 to 30 days prior to their next retake.

13.7 PC Pass

The PC Pass is included in certain Centriq Training programs. TechSmart KC students who have the PC Pass may request to sit Levels 1 and 2 of Microsoft Office classes, including Excel, PowerPoint, Word, and Outlook. Enrollment is available on a space available basis.

13.8 CompTIA Pass

The CompTIA Pass is included in certain Centriq Training programs. TechSmart KC students who have the CompTIA Pass may request to sit corporate Network+ on a space available basis up to a year from their start start date.

13.9 Career Services Program Students who purchase the Career Services Program as part of their TechSmart KC package are eligible for the following:

Career Seminars: Seminars focus on career-related topics, including technical resume development, customer care for IT professionals, IT interview techniques and IT job search strategies.

Careers Team: Full-time members of staff can provide professional career consultations, interview and resume advice, and referrals to KC employers and staffing agencies.

13.9.1 Inside Track Program: Centriq Training will endeavor to provide the following services:

o Make efforts to leverage their credibility and long-standing relationships with Kansas City Employers to find job opportunities for the TechSmart KC students who meet the Employers’ requirements.

o Evaluation Reports created and provided to Employers to support Inside Tracker resumes. o Candidates included on an “Inside Tracker List” circulated to a database of KC Employers. o Resumes circulated and interviews organized upon request from Employers. o Proactive submission of Inside Tracker resumes to companies with or without current vacancies. o Exclusive access to Employers with selective requirements.

Student shall be responsible for maintaining inside track eligibility by:

o Aiming to maintain a 100% attendance record. o Contacting Career Services for any excused absence (sick, family emergency, etc) prior to or on the

day of absence. o Contacting Career Services when expecting to arrive late for training. o Having no more than a total of 3 excused absences, late arrivals, or early leaves per track. o Participating in all Career Skills Seminars and completing all related assignments on time. o Meeting project requirements and deadlines given by instructors. o Participating in presentations assigned by instructors. o Participating in mock interview and obtaining approval from interviewer evaluation. o Attending interviews facilitated by Career Services. o Zero disciplinary meetings during on record. o Receiving at least “Average” ratings for “Professional Approach”, “Participation/Attentiveness”,

and “Problem Solving Initiative” by instructors in all classes. o Attending all scheduled mentoring sessions with instructors. o Attend at least one Mock Interview Boot Camp and participate in both a General and Technical

interview.

14 General Policies

The following policies and procedures apply to ALL students and visitors to Centriq Training:

14.1 Personal Possessions Policy Individuals are responsible for their own personal possessions while at Centriq Training . Centriq Training

will not be held liable for items lost or left unattended on the premises. This includes but is not limited to class materials, food, purses, backpacks, laptops, etc. The front desks staff maintains a lost and found bin at each training location.

14.2 Drug and Alcohol Policy Drugs and alcohol are expressly forbidden. Centriq Training prohibits any use, possession, or distribution

of alcohol and/or drugs on school premises. Students violating this policy will be immediately disciplined according to the Disciplinary Procedure.

14.3 Smoking Policy

Centriq Training is located within a non-smoking building, and smoking shall be prohibited within all enclosed areas. This includes the use of e-cigarettes and vapor cigarettes. A designated smoking area is located outside on the west side of the building.

14.4 Weapons Policy Weapons are expressly forbidden. Individuals are not permitted to possess, carry, or have access to any

weapon of any type at Centriq Training facilities. Students violating this policy will be immediately disciplined according to the Disciplinary Procedure. This includes immediate termination of training and filing of appropriate legal charges.

14.5 Special Assistance

Centriq Training facilities are wheelchair accessible. Students who have special needs addressed by the Americans with Disabilities Act should give Centriq Training as much notification as possible prior to class so that arrangements can be made to accommodate their needs. 15 Student Conduct Code

The purpose of the Disciplinary Procedure is to ensure a healthy, safe, and productive learning environment in our school. The development of self-discipline, professionalism, and the acceptance of personal responsibility are important goals of education. Students help create an environment in which learning can take place by mastering self-discipline, professionalism, and modeling appropriate behavior in the school and community. Good faith and common sense are necessary for a proper interpretation of this Disciplinary Procedure. Questions regarding interpretation may be addressed to Student Services.

15.1 Goals of the Conduct Code

• To communicate the Centriq Training Student Conduct Code to students and staff. • To enforce school policy, local, state and federal laws. • To insure the rights and personal dignity of students and staff. • To emphasize the need for acceptance of personal responsibility, professionalism, and self-discipline. • To assure a fair, reasonable, and consistent approach to the enforcement of the Disciplinary Procedure. • To maintain an effective and safe learning environment. • To maximize learning and minimize disruptions. • To protect and maintain school property. • To identify, diagnose, and remediate students who exhibit inappropriate behavior.

15.2 Four Levels of Discipline and Course of Action

15.2.1 Level 1 Minor misbehavior on the part of the student, which impedes orderly classroom procedures or interferes with the orderly operation of the school.

These misbehaviors should be handled by an individual staff member/Classroom Instructor but sometimes require the intervention of other school support personnel.

Examples, but not limited to:

• Listening to loud, disruptive music during school/class hours • Using tape recorders or video cameras in the classroom without authorization • Throwing any inappropriate or harmful objects • Littering • Verbal harassment of others • Continual classroom tardiness • Inappropriate language • Inappropriate dress and grooming that is disruptive to the educational process (Instructor

and Student Services discretion)

Examples of disciplinary options: • CONDUCT REPORT MUST BE FILLED OUT • Instructor asks for immediate remediation of problem in classroom • Instructor asks for immediate remediation of problem one-on-one

15.2.2 Level 2 Misbehavior whose frequency or seriousness tends to disrupt the learning climate of the school.

These infractions, which usually result from the continuation of Level 1 misbehaviors, require the intervention of personnel on the management level because the execution of Level I disciplinary options has failed to correct the situation. Also included in this level are misbehaviors which do not represent a direct threat to the health and safety of others, but whose educational consequences are serious enough to require corrective action on the part of management personnel.

Examples, but not limited to: • Extreme Level I misconduct • Abusive, obscene, or disrespectful language, writing or gestures • Violation of school attendance policies • Unauthorized possession or use of school property and/or facilities • Open defiance of authority; insubordination

• CONDUCT REPORT MUST BE FILLED OUT • Student Services/Training/General Manager asks for immediate remediation of problem

in a one-on-one meeting with student. • Student will be asked to review the Disciplinary Procedure • Student may be asked to voluntarily leave the program with refund per the Enrollment

Agreement.

15.2.3 Level 3 Acts whose frequency or seriousness tend to disrupt the learning climate of the school and/or acts directed against persons or properties and whose consequences could endanger the health or safety of others in the school.

These infractions require the intervention of personnel on the management level.

Examples, but not limited to: • Continuation of or extreme Level II misconduct • Vandalism • Petty theft • Fighting • Racial slurs or intimidation • Trespassing on or in school property when closed • Refusal to leave school property when ordered to do so • Threatening or intimidating others • Destruction of the property of others • Lewdness or indecent exposure • Possession of drug paraphernalia • Sexual Harassment

Examples of disciplinary options:

CONDUCT REPORT MUST BE FILLED OUT

Student will be expelled from the program with refund in accordance with financial agreement less any costs incurred from damage to personal or company property.

If in the case of damage to personal and/or company property, and replacement or repair costs exceed the refund amount, charges may be filed, and legal action will be taken to recover costs.

Acts directed against persons or property which could or do pose a threat to the health, safety or welfare of others in the school. Such acts will require management/executive action which could result in the immediate removal of the student from school and the possible intervention of law enforcement authorities.

These infractions require the intervention of personnel on the management/executive level.

• Continuation of or extreme Level III misconduct • Extortion or attempted extortion • Bomb threat or threatening phone calls • Possession/use/transfer of dangerous weapons • Aggravated assault/battery • Theft or possession/sale of stolen property • Arson or attempted arson • Transfer/sale/possession/ purchase/procurement/distribution/use or under the

influence of unauthorized substances, e.g., drugs, alcohol or drug paraphernalia.

• Disorderly conduct • Unauthorized use of fire alarm or equipment • Reckless driving or speeding on school property • Other violations of federal, state, or local laws

Examples of disciplinary options:

• CONDUCT REPORT MUST BE FILLED OUT • Expelled from program with no refund • Charges filed • All violations must be reported to Human Resources immediately

16 Acceptance of Student Responsibilities and Centriq Training Policies

Student responsibilities include consistent school attendance, conscientious effort in classroom work and career development, and compliance with Centriq Training policies. Most of all, students share with the management and instructors a responsibility to develop a climate within the school that is conducive to professional work and learning. It is the responsibility of each student to respect the rights of all who are involved in the educational process.

16.1 Student Responsibilities Every student must:

o Be aware of and exhibit good conduct. o Attend and be on time for all classes. o Notify the school when an absence occurs (or is foreseen to occur), as well as provide documentation. o Make up all missed coursework as outlined by instructor or Student Services in order to stay on track. o Cooperate with school staff in operating a safe and professional school. o Pursue and complete the course of study. o Express ideas and opinions in a respectful and accurate manner. o Prepare for all classes and complete assignments given by instructor. o Notify instructor and Student Services of learning issues or when assistance is needed. o Exercise proper care when using public facilities and equipment. o Dress and groom in a clean and appropriate manner. Clothing that contains obscene or offensive

messages will not be permitted. o Follow rules and regulations set forth by Centriq Training. o Utilize the complaints procedure should the situation arise.

16.2 Inside Track Performance Requirements

o I understand that to maintain eligibility for the Inside Track services I am responsible for maintaining performance standards outlined in section (13.9.1).

o I understand that should I not maintain the performance standards outlined in (13.9) that I will no longer be eligible for the Inside Track services (13.9.1) but can still make full use of the Career Services program outlined in (13.9)

□ I wish to be included in the Inside Track program and will endeavor to

maintain the performance standards outlined above (16.2) in addition to all other TechSmart KC responsibilities.

Acceptance by Student

I have read and understand in entirety the Attendance Policy, Student Conduct Code, and Student Responsibilities and other policies and regulations as set forth herein.
