TECHNICAL ENQUIRY DOCUMENT FOR DESIGN, ENGINEERING, AND CONSTRUCTION OF RCC PLATFORM AND EXTENSION OF EXISTING SHED OF CAST HOUSE-3

1.0 INTENT OF SPECIFICATION:

01.01 General

The Aluminium Complex of Bharat Aluminium Company Ltd. (BALCO) is situated at Korba in the State of Chhattisgarh, India.

01.02 Intent of Specification

The intent of specification is to enable BIDDER to submit a detailed and comprehensive offer on EPC basis for the Design, engineering, for construction of RCC platform along with extension of existing shed of Cast House-3 taking into account all the technological/ technical requirements specified in this specification.

01.03 Site Information

The site lies at 2224’08” N latitude and at 8244’00” E longitude. The average elevation of the Plant area is at about 300 m above mean sea level. All plant, material and equipment shall be designed to suit the climatic conditions prevailing in this area. The meteorological data pertaining to Korba site is furnished below:-

02.01 SCOPE OF WORK (including but not limited to):-

The scope of work and service of bidder shall cover the following activities

1) Design, Engineering and construction of an open RCC platform approx. 5500 sqm and extending the existing shed of 84 m length and 13 m width for storage of Finished Goods inclusive of civil, structural, sheeting and electrical

2) Visits and surveys at site to assess the actual requirement and capture the required data for detailed design

3) Detail layout of the area to be constructed and incorporating the same in Plant Layout. 4) Providing of detail execution drawings after detail survey and Engineering. 5) Identification of existing facilities and rerouting of the same with proper plan for

required shutdowns. 6) The pre-project activities will include area cleaning, earth removal, rerouting of

existing facilities etc. which are required to be done before start of foundation works. 7) Open RCC platform should be of load bearing capacity of an axle load of 80T/Sqm. 8) Grade Slab of the Shed should be of load carrying capacity of an axle load of

60T/Sqm. 9) All structural members should be pre-fabricated outside plant premises and to be

dispatched for erection only after dispatch clearance from BALCO. 10) Adequate lighting (110 Lux during night) as per standard including supply of

materials. 11) One lighting High mast to be supplied and erected for open RCC platform area and

another high mast to be supplied by BALCO for erection in the designated place. Bidder has to arrange cables and necessary auxiliaries for the same.

12) Earthing and lightning conductors for the sheds as per Indian electricity standards 13) Manufacture / fabrication/ procurement of bought out items. 14) Shop Assembly 15) Inspection & testing at manufacture’s work/site. 16) Shop painting

17) Packing 18) Transportation to site 19) Unloading, storage and handling at site. 20) Relevant IS codes have to be followed for all design 21) Brick wall shall be of height 1 meter. Cladding will be above the brick walls. 22) Lighting supply and installation shall be in scope of bidder and minimum 110 lux

Illumination shall be maintained during night. 23) The purpose of constructing the shed will be for storage of Finished Goods and the

clear height for the shed to be 8m. 24) Before construction of the shed the existing facilities such as fire hydrant, existing

drains needed to be re-routed. 25) Openings for hydra entrance shall be 3 nos.in the shed. 26) Standard staircase with handrails shall be provided at both end of sheds to allow

access of Maintenance crew on top of roof

27) Erection of the complete mechanical and auxiliaries under the scope including the Electrical and controls, interconnecting cables & trays, control panel for lighting, interconnecting piping, safety guards, trench covers and all civil works will be in contractor scope and contractor has to provide the civil design and drawing, as applicable.

28) Final painting 29) Completion of sheds in all respects and handing over the sheds to the purchaser. 30) BALCO will make one construction power source available at site and the

distribution panel within 500 m radius of periphery of construction area. The contractor will make his own provision for tapping power from this panel to his distribution panel with MCBs/ MCCBs/ ELCBs etc. The cable for such distribution is included in contractor scope. The distribution panel of the contractor shall be equipped with all necessary protective relays as desired by BALCO. The maintenance and breakdown of cable and panel is the responsibility of the contractor. All site job/ maintenance jobs must be executed at site through "certified 'quality' electricians" only.

31) Temporary lighting in working area shall be arranged by the contractor. Illumination level must be sufficient or as directed by EIC. Any welding/ cutting job/ enclosed area welding/ cutting job must be carried out with suitable lights of 12V DC.

32) Contractor shall make its own arrangement of compressed air for construction purpose.

33) Temporary approach road for loading, unloading, fabrication and erection shall be made by the contractor as per the site requirement and as directed by EIC.

34) Arrangement of all tools and tackles (including power tools) for the construction is in contractor's scope.

35) The contractor shall mobilize at least the following equipment ‘but not limited to’ within 2 weeks from the date of LOI:-

1. 60 Ton crane, telescopic boom and capable of marching with counterweight- 01 no.

2. Cherry pickers (Man –lift) capable of lifting man basket to a height of 15 meters- 02 no.

3. Hydra, capacity 12 T/14T- 4 nos. 4. Electrical winch- 2 nos. 5. Arc welding machines- 20 nos. 6. Trailers-2 nos. 7. MIG welding machines- 02 nos. 8. Vertical drilling machines- 02 nos. 9. Chain blocks- 40 nos. 10. Plate bending machine- 01 no. 11. Adequate scaffolding material of international standard

36) Mobilization of Manpower- minimum 200 nos. with adequate no. of skilled fitters and welders

37) Mobilization of Adequate number of qualified engineers, quality engineers, supervisors (one supervisor per 15 workers), safety officers and safety stewards ( one safety steward per 40 workers)

PAINTING PROCEDURE

1 Painting the primed structural steel

A Cleaning the surfaces of structural steel thoroughly with sand blasting/shot blasting

B Painting the erected structural steel surfaces with two coats of Red Oxide Zinc Chromate primer conforming to IS 2074. (Total 60 microns).

C Painting the primer coated structural steel surfaces with Acrylic polyurethane finish paint (RAL-5012 Phiroza blue) of approved make as specified. (Total DFT 110 microns- 60 micron primer+ 50 micron blue paint)

2 Painting of Handrails

A Cleaning the surfaces of handrails thoroughly with sand blasting/shot blasting

B Painting the Handrails surfaces with two coats of Red Oxide Zinc Chromate primer confirming to IS 2074 (total 60 microns).

C Painting the Handrails surfaces with Acrylic polyurethane finish paint (RAL-1004, Golden yellow) of approved make as specified. (Total DFT 110 microns- 60 microns primer+50 micron yellow paint)

02.01.01 Any other auxiliary / accessories not specially mentioned but considered essential for completion of the sheds shall be in scope of supply and erection of the bidder.

02.01.02 Quality guideline:

a) Contractor shall adhere to definite QAP (approved by BALCO) for civil, structural, sheeting, utility and electrical jobs as well as test reports, test procedures.

b) NDT, wherever applicable, shall be in the scope of bidder. c) Only qualified welders shall be employed at site. d) All the steel shall be procured from reputed primary producers only viz. SAIL,

TATA, Jindal e) TMT bars for civil construction should be of make TATA, SAIL, JINDAL. f) Approved cement brands for construction only viz. Ultratech, ACC, Ambuja. g) Bidder to set up / arrange laboratory facilities for periodic testing of the

construction materials, concrete etc. h) Vendor to make necessary provision for concrete mixing by installing required

capacity of concrete batching plant with batch print facility. i) TMT bars testing to be carried out by the vendor before use. j) Bidder to rectify (if any) all construction deformities observed with approval from

BALCO by referring necessary IS. k) Bidder shall offer all the items for inspection before dispatch. All dispatches shall

follow only after a formal dispatch clearance certificate from the Purchaser. l) The above mentioned sheets shall be corrugated Profile sheet made out of 0.50

mm TCT, YS-550 mpa,AZ-150 Gsm Zinc Aluminium Color Coated Galvalume sheet with Regular Modified Polyester Coating of 20 microns over 5 microns primer and 6-7 backup epoxy coat on the bottom over 5 microns primer. As per standard ASTM A 792 / AS1397. Color:- Off White (RAL 9002) . Make –The steel should be from any primary producer only viz. SAIL, TATA, Jindal.

m) The FRP sheet shall be 2mm thick and shall be able to be used and overlapped with the galvalum sheets. The sheets shall be supplied by the supplier in exactly the sizes and numbers described in the table above. The packing of the sheets shall be such that they can be easily unloaded and transported at site using mobile cranes without causing damage to the sheets.

Any other particular item/ specs, necessary for the completeness of the system shall be mentioned/ recommended by Contractor to be implemented in this project

The Tentative dimensions for FG Storage Shed shall be as follows:-

a) Length = 84 meters b) Width = 13 meters c) Height- sufficient enough to allow the entry, exit, Hydras, trucks etc.

The design should be economical and safe. Light structural design shall be preferable.

Contractor shall be deemed to have carefully examined and to have knowledge of all the related documents, drawings and other documents forming part of the contract and also have satisfied itself as to the nature and character of the work to be executed. Any information Contractor has, or otherwise obtained from Owner, shall not in any way relieve Contractor from its responsibility for completion of total scope mentioned in the contract.

3. MANPOWER:

Contractor shall ensure to keep the required and competent manpower to carry-out the job. Contractor shall at all times exercise reasonable skill, care and diligence in the discharge of our professional services.

4. CONFIDENTIALITY:

4.1) Contractor must maintain strict confidentiality regarding BALCO’s confidential information and shall not divulge all or any aspect of BALCO confidential information to any person other than as strictly allowed by BALCO.

4.2) Contractor must not use BALCO confidential information other than for this Project.

4.3) CONTRACTOR shall have the right to disclose BALCO confidential information to its officers, employees and agents, and those of its related entities whose duties in relation to the Engineering Services for this Project shall reasonably require such disclosure. BALCO’s Confidential Information shall at all times remain the property of BALCO.

4.4) Before any disclosure is made to any contractor or third party, CONTRACTOR must ensure that the person or entity to whom the disclosure is made agrees to keep the BALCO Confidential Information confidential and not to disclose or use it except as permitted under the Contract. CONTRACTOR must require such a person or entity to execute (and in the case of a corporation, the additional execution by its directors) the Confidentiality Contract.

4.5) If CONTRACTOR is uncertain whether information comprises part of the BALCO Confidential Information or not, then CONTRACTOR must seek direction from BALCO before divulging information to any other person.

4.6) CONTRACTOR shall acknowledge the necessity of protecting the BALCO’s Confidential Information and agrees that any disclosure in breach of the Contract may cause damage to BALCO and accordingly, CONTRACTOR agrees that all of the provisions of this Contract are reasonable in all the circumstances.

5. OWNERSHIP:

The Owner shall acquire the sole and exclusive right to the use of any or all, drawings, documents, presentations, analysis, know-how achieved by the CONTRACTOR during the execution of this Contract

6. PROGRESS REPORT:

The Contractor shall provide progress reports on request of Engineer-in-charge whenever desired.

10. DRAWINGS & DOCUMENTS:

The Contractor shall prepare basic engineering which shall be furnished to owner for approval. However any such approval by the owner shall not absolve the contractor from its obligation to complete the sheds. After approval the contractor shall prepare detailed engineering and submit to the owner in 6 copies in hard copy (A0 size) as well as soft copy in AutoCAD format.

The reports submitted by the Contractor shall become the Property of the Owner and the Owner may use the same in any Work as may deem fit. However, the Contractor shall in all times maintain confidentiality of the data.

11. SUB-CONTRACTING / ASSIGNMENT:

CONTRACTOR shall not sublet, transfer or assign the whole or any part of this Contract without the previous written approval of BALCO.

CONTRACTOR shall be responsible for all the losses suffered by BALCO in case of their failure to abide by this condition. Notwithstanding such approvals, CONTRACTOR shall be responsible for the work carried out by their Sub-Contractor if any.

12. CODES & LANGUAGE TO BE USED:

All technical services to be rendered under the Contract shall be executed in the manner set out in the Contract and in accordance with the trade/ engineering practices judged by the International standards. Wherever the codes are not mentioned, International Standards shall be followed with the approval of Owner.

Any deviation from standard shall need the approval in writing by Owner, and in such matters, Owner shall be the sole authority. English language shall be the language to be used in all correspondence, instructions, drawings, specifications, catalogues, brochures, pamphlets, documents and any other data to be given.

13. CHANGES/ VARIATIONS:

a) The Owner shall have the right, during the performance of the Contract, to change the scope and/or technical character of the Project and/or of the services stipulated in the Contract. Owner may by a notice in writing to the Contractor request for changes in the scope of work. Within 3(three) days of receipt of such notice, the Contractor shall inform the owner, in writing of the effect of such change would have on the price, schedule, and other provisions of the contract and within 3 (three) days of receipt of such information, Owner shall advise the Contractor in writing of its acceptance or otherwise. On the same being accepted by the owner such changes shall be deemed to form part of this agreement..

b) If any changes are required for completeness of the work for the scope of work as mentioned, the Contractor shall not be entitled to extra price or time. Any other item/service required for the completeness of the sheds, but not mentioned in this document and not expressly excluded in this document, shall be supplied and installed by the bidder at no extra cost to purchaser.

c) CONTRACTOR may make changes and/or improvements in the work without any extra cost to Owner as far as it is to the advantage of Owner, provided the CONTRACTOR gives satisfactory reasons to Owner and obtain Owner’s prior approval in writing. However, Contractor’s obligation under this Contract shall not change in any circumstances.

14. SAFETY AND SECURITY:

CONTRACTOR is responsible for the safety and security of all CONTRACTOR’s personnel, assets and equipment and is responsible for the provision and issuing of all necessary health and safety equipment CONTRACTOR shall ensure adequate insurance cover in respect of injuries at work for his personnel are maintained during the Contract period. CONTRACTOR shall indemnify the Company and its officers against any claim, dispute and litigations arising in this regard.

15. LEGAL COMPLIANCE AND STATUTORY REQUIREMENTS:

Contractor shall be solely responsible for Statuary Compliance in respect of all applicable laws of land to ensure that the Statutory Compliance in respect of all applicable laws of land which includes Central, State, Labour laws and Regulations/Rules made there under. The Contractor/Contractor shall be solely

responsible for maintenance of records and filing of various forms/ returns prescribed under all applicable Central, State, Labour laws and Regulations/Rules made there under in respect of Workmen employed by it.

16. Custody

Contractor shall take custody of all the materials pertaining to the two sheds and shall be responsible for any theft, damage etc. and shall re-supply such stolen/damaged materials at no extra cost to purchaser.

General

1.0. Scope:

1.1. This document defines the principle requirements of BALCO on Safety, Health and Environment (SHE) associated with CONTRACTOR/ sub-contractor and any other agency to be practiced at construction worksites at all time.

1.2. This document applies to all aspects of CONTRACTOR’s scope of work, including all aspects conducted by sub contractor of CONTRACTOR’s and all other agencies. There shall be no activity associated to the contract, which is exempted from the purview of this document.

1.3. The objective of these guidelines stipulated herein is to ensure that adequate precautions are taken to avoid accidents, occupational illness and harmful effects on the environment during construction.

1.4. This document:

1) Describes the SHE interfaces between BALCO and CONTRACTOR,

2) Details the processes by which CONTRACTOR shall manage SHE issues while carrying out the work under the contract,

3) Describe by reference, the practices and procedures for best SHE performance

1.5. These requirements shall be read together with OHSAS 18001-1999, Occupational Health and Safety Management System and ISO 14001: 2004

Environmental Management Systems.

2.0 ‘SHE’ Target and Goals

The SHE targets, goals and aim for the Construction work site are to achieve:

i) Zero total recordable injuries.

ii) Zero reportable environmental incidents.

iii) All personnel inducted in accordance with the approved SHE plan.

iv) Total compliance of conducting inspections and audits as per approved SHE plans.

v) 100% incident recording and reporting.

vi) 100% adherence of usage of appropriate PPE’s at work.

vii) Executing construction work with least disturbance to the environment, adjoining road users and traffic.

3.0 Compliance

3.1 Indian Statutory Regulations

CONTRACTOR shall develop thorough understanding about various local, state government and central government acts, rules & regulations with regard to Building and other Construction workers regulations their welfare not only to satisfy the perspective but use legislation as strong tool for effective SHE management at construction worksites. CONTRACTOR is strongly advised to practice the principle of voluntary compliance.

3.2 In addition, the construction works shall be undertaken in accordance with all applicable legislation and Indian statutory requirements listed below and their amendments time to time but not limiting to:

i) Indian Electricity Act 2003 and Rules 1956.

ii) National Building Code 2005.

iii) Factories Act for the state government

iv) Motor Vehicles Act as amended in 1994 and The Central Motor Vehicles

Rules, 1999.

v) Indian Road Congress IRC: SP: 55-2001 ‘Guidelines on Safety in Road

Construction Zones.

vi) The Petroleum Act, 1934 and Rules 1976.

vii) Gas Cylinder Rules, 2003.

viii) Indian Explosives Act, 1884, along with the Explosives Substance Act

1908 and The Explosives Rules 1983.

ix) The (Indian) Boilers Act 1923.

x) The Public Liability Insurance Act 1991 and Rules 1991.

xi) Minimum Wages Act 1948 and Rules 1950.

xii) Contract Labour Act, 1970 and Rules 1971.

xiii) Child Labour (Prohibitions & Regulations) Act 1986 and Rules 1950. xiv) Environment Protection Act, 1986 and Rules 1986.

xxv) Batteries (Management & Handling) Rules.

xxvi) Fly Ash Utilization Notification, September 1999 as amended in August

2003.

3.3 Workmen Compensation Act 1923 along with Allied Rules.

The CONTRACTOR shall ensure that all his employees/workmen are covered under ‘Workmen Compensation Act’ and shall pay compensation to his workmen as and when the eventuality for the same arises.

3.4 Notwithstanding the above Acts/Rules, there is nothing in those to exempt CONTRACTOR from the purview of any other Act or Rule in Republic of India for the safety of men and materials.

4.0 CONTRACTOR SHE Policy and Plan

4.1 Within 4 weeks prior to site mobilization, the CONTRACTOR shall submit a detailed and comprehensive Contract Specific SHE Plan. The SHE plan shall include detailed policies, procedures and regulations which, when implemented, will ensure compliance of the contract provisions. The SHE plan shall include the following but not restricted to:

i) A statement of CONTRACTOR’s policy, organization, and arrangements for SHE.

ii) The name(s) and experience of the person(s) within CONTRACTOR’s proposed management who shall be responsible for co-ordinating and monitoring CONTRACTOR’s SHE performance.

iii) The number of SHE staff who shall be employed on the works, their responsibilities, authority and line of communication with the proposed CONTRACTOR’s agent.

iv) A statement of CONTRACTOR’s policy and procedures for identifying and estimating hazards, and the measures for addressing the same.

v) A list of SHE hazards anticipated for this contract and sufficient

information to demonstrate CONTRACTOR’s proposal for achieving effective and efficient health and safety procedures.

vi) A description of the SHE training courses and emergency drills which shall be provided by CONTRACTOR, with an outline of the syllabus to be followed.

vii) Details of the safety equipment which shall be provided by CONTRACTOR, including personal protective equipment.

viii) A statement of CONTRACTOR’s policy and procedures for ensuring that CONTRACTOR’s Equipment used on the Project site are maintained in a safe condition and are operated in a safe manner.

ix) A statement of CONTRACTOR’s policy and procedures for ensuring that sub-contracors comply with CONTRACTOR’s safety plan.

x) A statement of CONTRACTOR’s disciplinary procedures with respect to SHE related matters.

xi) A statement of CONTRACTOR’s procedure for reporting and investigating accidents, dangerous occurrences or occupational illnesses.

xii) All safety standards will be shared with the bidder before planning of execution of the project.

4.2 The CONTRACTOR shall, from time to time and as necessary are required by BALCO to produce supplements to the SHE plan such that it is at all times a detailed, comprehensive and contemporaneous statement by CONTRACTOR of his site safety, industrial health and environment obligations, responsibilities, policies and procedures relating to work on site. Any and all submissions of supplements to the SHE plan shall be made to BALCO in accordance with the agreed procedures.

4.3 If at any time the SHE plan is in, BALCO’s opinion, insufficient or requires revision or modification to ensure the security of the works and the safety of all the workmen upon and visitors to the Site, BALCO may instruct CONTRACTOR to revise the SHE Plan and CONTRACTOR shall within 7 days submit the revised plan to BALCO for review.

4.4 Any omissions, inconsistencies and errors in the SHE Plan or the BALCO’s acceptance or rejection of the SHE Plan and/or supplements thereto shall be without prejudice to CONTRACTOR’s obligations with respect to site safety, industrial health and environment and shall not excuse any failure by CONTRACTOR to adopt proper and recognized safety practices throughout the execution of the Work.

4.5 CONTRACTOR shall adhere to the SHE Plan and shall ensure, as far as practically possible, that all it’s sub-contractors of all tiers require that contracting parties each have a copy of the Site SHE Plan and comply with its provisions.

4.6 CONTRACTOR must follow Minimum requirements at all the time:

a) To depute qualified full time Safety Officer from day one along with Project

Manager

b) To depute trained Safety stewards for every type & every job area

c) 100% labors must have Photo Gate Pass, Safety shoes & Safety helmets and Other PPE’s.

d) All labours working at height should have Safety belts, height permit and medical certificate from qualified Doctor.

e) Safety induction training on first day for every new labour joined.

5.0 CONTRACTOR SHE Organisation

5.1 Education & Experience

5.1.1 CONTRACTOR shall appoint the required SHE personnel based upon the statutory requirement and establish the safety organization. The minimum educational qualification and the work experience should be adequate to cover the responsibilities.

5.1.2 In order to effectively interact on labour welfare matters with BALCO and the statutory authorities enforcing the labour welfare legislations, CONTRACTOR shall employ a full time Labour Welfare Officer duly qualified and experienced.

5.2 Conduct & Competency

The CONTRACTOR shall ensure that all personnel are competent to perform the job assigned to them. In the event that the CONTRACTOR is unable to demonstrate the competency of any person whose activities can directly impact the on the Works’ SHE performance, the BALCO shall remove the person from the site without any procedural formalities.

5.3 Responsibility of SHE personnel

For all works carried out by the CONTRACTOR and his sub-contractors, the responsibility of ensuring the required SHE manpower lies with the CONTRACTOR only. The minimum required manpower indicated by BALCO includes the sub-contractors work also. It shall be the responsibility of the CONTRACTOR to provide required SHE manpower for all works executed by CONTRACTOR and its subcontractors. Necessary conditions shall be included in all sub-contract documents executed by CONTRACTOR.

5.4 Reporting of SHE personnel

All SHE personnel are to report to the Chief SHE Manager who shall report directly to the Chief Project Manager. BALCO shall monitor adherence to this procedure at all times. In case of non-adherence penalty shall be levied as indicated in the penalty clause.

6.0 CONTRACTOR SHE Committee

6.1 All employees should be able to participate in the making and monitoring of arrangements for safety, industrial health and environment at their place of work. The establishment of site SHE committees in which employees and CONTRACTOR and sub-contractors’ management are represented can increase the involvement and commitment of employees. The CONTRACTOR shall ensure the formation and monitor the functioning of CONTRACTOR SHE committees.

6.2 Terms of Reference

6.2.1 The terms of reference for the committee shall be as follows:

i) To establish construction site safety policies and practices.

ii) To monitor the adequacy of CONTRACTOR’s site SHE Plan and ensure its implementation.

iii) To review SHE training.

iv) To review CONTRACTOR’s monthly SHE report.

v) To identify probable causes of accident and unsafe practices in building or other construction work and to suggest to remedial measures.

vi) To stimulate interest of BALCO and building workers in safety by organizing safety week, safety competitions, talks and film shows on safety, preparing posters or taking similar other measures as and when necessary.

vii) To go around the construction site with a view to check unsafe practices and detect unsafe conditions and to recommend remedial measures for their rectifications including first-aid medical and welfare facilities.

viii) Committee members should perform a site inspection before every committee meeting and to monitor SHE inspection reports.

ix) To bring to the notice of the BALCO the hazards associated with use, handling and maintenance of the equipment used during the course of building and other construction work.

x) To suggest measures for improving welfare amenities in the construction site and other miscellaneous aspects of safety, health and welfare in building or other construction works.

xi) To look into the health hazards associated with handling different types of explosives, chemicals and other construction materials and to suggest remedial measures including personal protective equipment.

xii) To review the last safety committee meeting minutes and to take action against persons/sub-contractors for non compliance if any.

6.3 Within 14 days after start of site mobilization, the SHE committee shall be constituted and notification regarding the same shall be communicated to the members and employees.

6.4 Co-contractor’s participation

In case of depot, station, and other contiguous areas where more than one main contractors are working together, BALCO shall instruct the other contractors

to join for the monthly SHE committee meeting of the main civil contractor, so as

to discuss and decide about the common provision of security, lighting, toilet, drinking water, etc and sharing the maintenance cost of the same.

7.0 ID Card and First day at work, SHE orientation training

7.1 CONTRACTOR shall ensure that all personnel working on site receive an induction SHE training explaining the nature of the work, the hazards that may be encountered during the site work and the particular hazards attached to their own function within the operation. The training shall cover the contents as given in the General Instruction.

7.2 All personnel shall be issued a photo identity card of size 85mm x 55mm duly

signed by the authorized persons before they are engaged for any work as per the format.

7.3 CONTRACTOR shall issue a personnel SHE handbook in a language known to the workers, which provided information on SHE and emergency procedures that all personnel working on contract are required to know and the need to follow. CONTRACTOR shall ensure that this is distributed and its content introduced to all personnel working at the site.

8.0 SHE Training

8.1 The CONTRACTOR shall organize quality SHE training to engage managers, supervisors and other personnel in behavioral change and improve safety performance.

8.2 CONTRACTOR shall analyze the training requirements for all the employees and initiate a training program to demonstrate that all persons employed, including sub-contractors are suitably qualified, competent and fit. This will include:

i) Detailed job descriptions for all personnel, to include their specific SHE

responsibilities.

ii) Specification of qualifications, competency and training requirements for all personnel.

iii) Assessment and recording of training needs for all personnel, including sub-contractor’s employees in the work force, vendor representatives and site visitors.

iv) A system for assessing new hirer’s e.g. previous training.

v) A means of confirming that the system is effective.

vi) A matrix and schedule of training requirements, covering general, task specific and SHE related training, showing the training frequency and interval between refresher courses.

vii) Timely, competent delivery of training courses.

8.3 The CONTRACTOR shall arrange behavioral-based training programs for all the executives to identify recognize and eliminate unsafe act and unsafe conditions.

8.4 The refresher training program to all employees shall be conducted once in six months.

8.5 Toolbox talk and Project SHE manual shall be conducted to all high risk workmen every day.

8.6 On the spot practical skill development training on height safety including scaffold safety, crane safety, welding safety, electrical safety, traffic safety for marshals shall also be conducted to all foremen/workmen who were associated to the concerned jobs.

8.7 Daily safety oath as given in the Project SHE manual shall be taken by every employee including workmen without fail.

8.8 All vehicle drivers including Hydra operators shall be trained on defensive driving at local Institute for Driver Training. All vehicle drivers shall also undergo refresher training on defensive training provided by the same institute once in six months.

9.0 SHE Inspection

9.1 The CONTRACTOR shall evolve and administer a system of conducting SHE inspections and other risk management analysis on a periodical basis.

9.2 The purpose of SHE is to identify any variation in construction activities and operations, machineries, plant and equipment and processes against the SHE plan and its supplementary procedures and programs.

9.3 Following SHE inspection program shall be adopted.

i) Planned general inspection.

ii) Routine inspection.

iii) Specific inspection.

iv) Other inspection.

10.0 SHE Audit

10.1 General

10.1.1 The purpose and scope of SHE audit is to assess potential risk, liabilities and the degree of compliance of construction Safety, Health and Environmental plan and its supplementary procedures and programs against applicable and current

10.2 Monthly Electrical Safety Audit

A team comprising of CONTRACTOR’s senior SHE (Electrical) engineer and BALCO’s representative shall conduct monthly electrical safety audit covering the following and submit the report to BALCO.

i) Electrical accidents investigation findings and remedy.

ii) Adequacy of power generation and power requirements. iii) Power distribution and transmission system in place.

iv) Updated electrical single line diagram showing the current condition of power source and distribution including the IP44 DBs arrangement.

v) Electrical protection devices- selection, installation and maintenance.

vi) Earth or ground connection and earth pit maintenance details.

vii) Education and training of electrical personnel undertaken.

viii) Routine electrical inspection details.

ix) Electrical maintenance system and register.

x) Name plate details of major electrical quipment.

xi) Classified zones in the site if any.

10.3 External SHE Audit

External SHE audits are to be conducted by external agencies that are competent with ISO qualified auditors with prior approval of BALCO.

11.0 SHE Communication

11.1 The CONTRACTOR shall take every effort to communicate the Safety, Occupational Health and Environment management measures through posters/campaigns/ billboards/banners/glow signs being displayed around the work site as part of the effort to raise safety awareness amongst the work force. Posters should be in Hindi, English and other suitable language deemed appropriate. Poster/billboards/banners/glow-signs should be changed at least once in a month to maintain impact.

11.2 CONTRACTOR shall also observe important safety days as listed by government/local bodies and printing and displaying safety signage and posters.

11.3 These are the minimum requirements of the BALCO and the CONTRACTOR is encouraged to further the SHE communication activities by formulating suitable reward schemes for safety performers and any other activities, which deem fit for the purpose.

12.0 SHE Submittals to BALCO

12.1 CONTRACTOR’s SHE management should send the following reports to the

BALCO periodically:

i) Daily reporting of total no. of workmen

ii) Monthly SHE report

iv) SHE Committee Meeting Minutes

iv) SHE inspection reports

v) SHE audit reports

a) Monthly Audit Rating Score (MARS) report

b) External SHE audit, if any

c) Electrical safety audit

12.2 Daily reporting of total no. of workmen

CONTRACTOR shall report to BALCO the total no. of workmen engaged by

all including sub-contractors within 2 hours of starting any shift in any day. This reporting shall be the primary duty of the Chief SHE Manager of CONTRACTOR. The onus of checking the receipt of the same by the BALCO lies with CONTRACTOR. If the information is not received or received more than 2 hours of starting the shift, penalty shall be levied.

12.3 Monthly SHE Report

CONTRACTOR shall prepare a monthly SHE report consisting of the following and submit 3 copies within 5th of next month to BALCO as specified in the Project SHE Manual.

13.0 Accident reporting and investigation

13.1 Reporting to BALCO

13.1.1 All accidents and dangerous occurrences shall immediately be informed verbally to the BALCO. This will enable the BALCO to reach to the scene of accident/dangerous occurrence to monitor/assist any rescue work and/or start conducting the investigation process so that the evidences are not lost.

13.1.2 Reports of all accidents (fatal/injury) and dangerous occurrences shall also be sent within 24 hours as per format to BALCO.

13.1.3 No accident/dangerous occurrence is exempted from reporting to BALCO.

13.1.4 Any willful delay in verbal and written reporting to the BALCO shall be penalized.

13.2.1 Investigations should be conducted in an open and positive atmosphere that encourages the witnesses to talk freely. The primary objective is to ascertain the facts with a view to prevent future and possibly more serious occurrences.

13.2.2 Accidents and dangerous occurrences which result in death, serious injury or serious damage must be investigated by the CONTRACTOR immediately to find out the cause of the accident/occurrence so that measures can be formulated to prevent any recurrence.

13.2.3 Near misses and minor accidents should also be investigated by CONTRACTOR as soon as possible as they are signals that there are inadequacies in the safety management system.

14.0 Emergency Preparedness Plan

CONTRACTOR shall prepare an Emergency Response Plan for all work sites as part of CONTRACTOR SHE Plan. The plan shall integrate the emergency response plans of CONTRACTOR and all other sub-contractors. The Emergency Response Plan shall detail the CONTRACTOR’s procedures, including detailed communication arrangements for dealing with all emergencies that could affect the site. This include where applicable, injury, sickness, evacuation, fire, chemical spillage, severe weather and rescue.

15.0 In order to have utmost compliance to Safety in all the activities undertaken, further strict observance of the guidelines stipulated, BALCO may impose penalties on the parties defaulted or issue prizes for best compliances.

16.0 Relevant Vedanta standards of safety are enclosed in separate files with this technical annexure. These standards have to be strictly adhered to by the contractor.
