1 – Preface

ThisProgramhasbeenwrittenanddevelopedbytheCSPI(CorrugatedSteelPipeInstitute)tosupportmembersandstakeholdersandhasbeenauditedbyCWB.UseoutsideofCSPImembershipisstrictlyprohibited.

TheCorrugatedSteelPipeInstitute,orCSPI,isafederallycharteredindustrialtradeassociationofcorrugatedsteelpipe(CSP)andstructuralplatecorrugatedsteelpipe(SPCSP)manufacturers,steelproducers,andsupplierstotheCSPindustry.

ThisisthefirstversionoftheProgramforSPCSP,toreflectthe2014versionofCSAstandardG401andtheMTOPerformanceBasedSpecificationforDesignandConstructionofStructuralCulverts.

2 – Scope

2.1 ThisProgramprescribesrequirementsforthecertificationtoCSAG401-CorrugatedSteelProductsof: • StructuralPlateCorrugatedSteelPipeproductsandDeepCorrugatedStructuralPlateproducts.

2.2 CertificationofSPCSPandDCSPtothisProgramisplantandproductspecific.

2.3 ThisProgramislimitedto: SPCSPandDCSPofallshapesandsizes.

3 – Definitions

5 – General

5.1 CSPIperformsanoversightroleintheadministrationoftheprograms,CB’sandManufacturers.

5.2 CSPIshallmaintainalistofManufacturers’certifiedProducts,approvedCB’sandtheCBMark(s).CSPIshallpromptlyadvisetheparticipatingManufacturerswhenrevisionsorchangeshavebeenmadetotheProgram,andtheeffectivedateofimplementationunderthisProgram.

5.3 A diligent effort has beenmade to select appropriate standards. However, CSPImakes no representation,warrantyorguaranteeinconnectionwiththestandardortheprogramandexpresslydisclaimsanyliabilityorresponsibilityforlossordamageresultingfromparticipation,foranyviolationoffederal,provincial,ormunicipalregulationwithwhichtheunderlyingCSAstandardmayconflict,orfortheinfringementofanypatentresultingfromtheuseoftheCSAstandard.

6 – Manufacturer requirements

6.1 General TheManufacturermustbeaCSPImemberandshallestablish,implementandmaintainaqualitysystemthat

conformstothisProgramandmanufacturecertifiedproduct(s)totherequirementsofCSAG401-14(seeAppendixA).

6.2 TheManufacturershallestablishandmaintainadocumentedQualityControlPlan(QCP) toassurecertifiedproduct(s)meetorexceedtherequirementsofG401-14.

6.3 TheQCPshalldetailtheprocessesfor(seeAppendixA): a) Materialscontrol(suchasmaterialspecifications,purchasingpractices,sourcing,receiving,storage,inventory,

andstockingmethods) b) Fabricationensuringconsistentqualityofproduct c) Identificationandtraceability d) Handling,StorageandTransportation e) Inspection f) Testing(ProductTestReports–AppendixB) g) ControlofRecords h) Self-AssessmentofPlantStandards i) Nonconformingproductandprocesses j) ControlanduseofMarks

Note: 1 Testingincludesthemethodsandfrequencyofsamplingandtestingforallrawmaterialsandproductspur-

chasedormanufacturedatthatlocation. 2 QualityControlandQualityAssurancedatashallberetainedbytheManufacturerfornotlessthantwoyears

6.4 TheManufacturershalllabelconforming,fullyassembledstructurewiththecertificationMarkandManufacturernameorlogo,traceabletothefacilityofmanufacture.Labelingoptionsincludeeitheranameplateontheentirestructureatbothends,orindividualmarksoneachplate.

6.5 TheManufacturerisresponsibletocontactandarrangefortheCBauditandtestingfortheinitialandannual

plant(s)andproductaudits.TheCBshallexecuteavalidAgreementwithaCertificationBodyunderthisPro-graminordertoconductsuchauditsandtesting.

6.6 TheManufacturershallprovidetheQCPtotheCBforreview.Thefollowinginformationshallbeincludedwiththisqualitycontrolplan:

a) alistofmanufacturingfacilitiesandlocationofplants; b) alistoftheapplicableProduct(s)producedateachplant;and c) thenameandtitleoftheindividualresponsibleforthequalitycontrolprogramateachplant.

6.7 ChangesattheManufacturer TheManufacturershallnotifytheCBofanysubsequentchangeinmanufacturingconditionsthatimpactonthe

qualityofthesystemorproduct.TheCBmayrequirepartialorcompleterequalificationbasedontheevidencesubmittedtoit,astothenatureandeffectofthechanges.

6.8 Anychangeinthepointofmanufactureshallrequirere-certification.

6.9 ModificationofCertifiedProducts

6.9.1 WhendesignchangesaremadeinacertifiedProductandtheManufacturerbelievesthatthischangewillstillresultinaProductequivalenttothecertifiedProduct(s),ManufacturershallnotifytheCBinwritingpriortoshipment.ManufacturershallsubmitasummaryofpropertiestotheCBtodemonstratecompliance.

6.9.2 ThisprocessalsoappliestochangesinProductformulation(rawmaterials).Equivalencycanbeestablishedbysuppliertechnicaldata,plateproducertesting,orgenerallyrecognizedindustrypractices.

6.9.3 ThelegalresponsibilityfortheauthenticityofsubmitteddatarestsontheManufacturer.

6.9.4 ThisprocedureisonlyintendedforuseincasesofdesignchangesorProductformulationchangesdeemednottoaffectcompliance.

6.9.5 TheCBmayrequirepartialorcompleterequalificationbasedontheevidencesubmittedtoit,astothenatureandeffectofthechanges.

6.10 Ifanonconformityisidentified,theManufacturershall: a) identifyandimplementdispositionandcorrectiveaction(s), b) determinetheextentofnonconformityandtheimpactonProduct(s)ininventorytodeterminewhataction(s)

shouldbetaken, c) reportshipmentsofpossiblenonconformingproductstotheirclients, d) documentthenonconformity,dispositionandcorrectiveactions(s).

7 – Certification Body Requirements

7.1 TheCBprovidesCertificationtothisProgram. 7.2 InitialCertificationActivities TheCBshall: a) Performinitialplantinspection/auditbasedonthelistofplateproductstobecertified. b) Identifyanddocumentnonconformity,whererequired. c) Issueaproductcertificationtotheplantuponsuccessfulcompletionoftheinvestigation.

7.3 Inspectiontestingshallbeperformedonasampleorsamples,selectedatrandom,byCBfromtheProductlineeitherinproductionorininventoryattheManufacturer’sfacility.

7.4 CB’s inspectorshallselect testsamples,appropriatelymarkthem,andseetheyarepreparedfor testingwithoutalteration.

Note: ProductswithsimilarattributesthatcanlogicallybeplacedinaProductattributegroupmaybecertifiedbasedonthetestingofarepresentativeProductorProductsfromtheProductattributegrouping.PlatesamplesshallbemadeavailableuponrequesttotheCBinasufficientquantitytoconductalltesting.

7.5 TheProductsample(s)selectedmaybe transported to theCB’s laboratory. IfManufacturerhas itsown testfacilitiesthatareacceptabletoCB,testingofManufacturer’splateatitsownfacilityispermissibleprovidedthatCB’sinspectororrepresentativewitnessesit.

7.6 ThecostofallinspectiontestswillbebornebyManufacturer.

7.7 AnnualCertificationActivities Asurveillanceinspectionoftheplantoperationswillbeperformedannually.Typicallythesurveillanceauditwill

randomlysampletheelementscoveredintheinitialcertification.Eachproductwillbetestedatleasttwicewithinthecertificationperiod(onceatinitialandatleastonceinyears1to4).

7.8 Re-CertificationActivities: Priortoexpirationofthecertification(fiveyearsfromthedateofgrantingcertification),oratthediscretionofthe

CBaninspectionsimilartotheinitialshallbeperformed.

7.9 Additionalinspectionsandtesting,ifrequired,torenewtheplant’scertificationstatusisattheCB’sdiscretion.

7.10 TheCBwillmakepublicallyavailablethelistofcertifiedproducts.

7.11 Inaddition,theCBwillnotifyprogramparticipantsastohowitwillmanagere-testingofcertifiedProduct(s)andvalidationofnewProduct(s).

7.12 NoticeofProductCertification

7.12.1 AnoticeshallbeissuedtoManufacturerstatingthedateonwhichcertificationhasbeengrantedandtheProductdesignation.

7.12.2 TheNoticeofProductCertificationshallcontainthefollowinginformation: a) manufacturer’sname,plantlocation,platetypeandallappropriateclasses, b) reportnumberandcertificationandexpirydates, c) certificationProgram

7.13 NoticeofProductFailuretoCertify:

7.13.1 IfthetestedProduct(s)doesnotcomplywithalltherequirementsoftheProgram,awrittennoticewillbesenttoManufacturerstatingthatitsProduct(s)didnotcertifytothisProgram.De-certificationbasedonunsatisfactoryinspectionisaddressedseparately.

7.13.2 Thenoticewillinclude: a) manufacturer’sname,plantlocation,platetypeandallappropriateclasses, b) reportnumberanddate;and, c) thespecifictestfailureorfailuresonwhichnonconformityisbased.

7.14 RecognitionofPriorTests AtthediscretionofCB,resultsfromtestsconductedbeforethisProgrambecameeffective,ortestsconducted

beforearevision toCSAG401-14 ispublished,maybedeemedtosatisfy theapplicable testingrequirementprovidedthat:

a) ThetestsandreportsfullycomplywiththeProductCertificationRequirementsofAppendixBandprovidetheinformationneededbyCBtovalidateManufacturer’scertificationunderCSAG401-14;

b) CBdeemsthetestinglaboratorytobecertifiedtoperformthetestsconducted; c) ManufacturercertifiesthattherehasbeennochangeintheProduct(s)orproductionprocessesthatwould

affecttheProduct’scompliance,and; d) TheCBmayrequestadditionalinformationorevidencesupportingtherequestforrecognitionofpriortestresults.

7.15 NonconformityandCorrectiveAction: CBwillcompletelydescribeanddocumenttheclassofnonconformityandevidencefornonconformityand

informtheManufacturer.

7.16 IftheCBdeterminesthataProductisnonconformingduringaplantinspection,theManufacturerwillbegiventheopportunitytocorrectitimmediately(dispositionandcorrectiveaction).

7.17 ForaMinornonconformitythatcannotbeimmediatelycorrected: a) TheManufacturershallidentifydispositionanddeterminecorrectiveactionandsubmittotheCBwithin

thirtydaysoftheinspection. b) IftheCBdoesnotapprovetheresponse,theCBwillnotifyanddiscusswiththeManufacturer. c) IftheManufacturerdoesnotnotifytheCBwithinthirty(30)days,theCBwillescalatethenonconformityto

aMajor.

7.18 ForaMajornonconformity, a) TheManufacturershall takeactionwithoutunduedelay toprevent theproductionofproduct thatmay

beaffectedbythenonconformityandshallprovidetheCBwithevidenceofimplementationofthisactionwithinseven(7)daysofthenonconformitybeingraised.

b) IftheCBdoesnotapprovetheresponse,theCBwillnotifyanddiscusswiththeManufacturer.

7.19 IfaMajornonconformityisnotresolvedtothesatisfactionoftheCBwithinthirtydays,orwithinsuchtimeastheManufacturerandCBagreetoextendtheperiodoftheresponsetime,

a) TheManufacturershallimmediatelyceaseapplyingMark(s)totheProductlistedinthenonconformity,asapplicable. b) TheManufacturershallsuspendtheuseofliteraturethatrepresentstheProduct(s)ascertifiedunderthisProgram. c) Toresumeparticipationintheprogram,theManufacturershallconfirmtotheCBthatcorrectiveactionhas

beenimplemented. d) Inaddition,a retest forProduct(s) identifiedshallbescheduledwithin thirty (30)dayssubmissionof the

correctiveaction.AllcostsforthisextraProduct(s)testingwillbebornebytheManufacturer. e) TheManufacturershallnotresumemarkingProduct(s)ascertifiedunderthisProgramuntiltheCBapproves

thecorrectiveactionandtheretestisincompliance.AnyProductmanufacturedbeforethisoccursshallnotbemarkedascertified.

f) Ifanyoftherequiredtimelinesarenotmet,ortheCBdoesnotapprovethedispositionandcorrectiveaction,orifthereteststillresultsinnonconformity,theCBshallproceedwithde-certification.

g) TheCBshallinformtheManufacturerandCSPIofde-certification.CSPIwillremovethatProduct(s)fromthecertifiedProduct(s)list.

7.20 TheManufacturermayappealadecisionoftheCBinaccordancetotheCB’sappealprocess.

7.21 Re-certification Tore-certifyProduct(s)followingdecertificationundertherequirementsofsection7.19,theManufacturershall

reapplyandsubmittotheInitialCertificationprocess.

7.22 InspectionReports: CBshallcommunicatewithManufacturer regardinganymatters requiringclarificationorotheractionon the

partofManufacturer.AllofficialcommentsordecisionswithrespecttoconformityofcertifiedProduct(s)willbeconfirmedinwritingfromtheCBwithinthirty(30)daysofthesitevisit.

TheinspectionreportisconfidentialandissubmittedonlytotheManufacturerordesignatedrepresentative.

7.23 General TheCBshall: a) Invoke certification limitations or cancellations on a plantwhere an inspection has noted deficiencies,

whichhascompromisedthequalityofplateproduction.Limitationswouldapplytocertainplateproductswhilecancellationswouldapplytoallplateproductsproducedattheplant.Thelimitationsorcancellationswillbeinplacewhiletheplantundertakeseffortstocorrectthenoteddeficiencies;

b) Issueinterimcertificates,unlesseachplateproductproducediscoveredunderonecertificate,totheplantandonlyforthosepipeproductsnotaffectedbytheplantoperationaldeficiencies;

c) Revokeorcancelplantcertificateswheretheplantisnotincompliancewiththecertificaterequirementsandhasfailedtodemonstratesatisfactorycorrectionofthedeficienciesnotedduringaninspection.

d) Applytheconditionsforre-instatementofcertificationduetocancellationofcertificateorlapsingofcertificateasoutlinedintheircertificationproceduresorprogram.

e) InformCSPIofaplateproductionplant’slimitedorcancelledcertificationstatus. f) Provideinspection/auditreportstotheManufactureraftereachcertificationactivity.

7.24 TheManufacturershall,priortotheinstallationofaculvertprovidethecustomer’sContractAdministrationwithacopyoftheCertificateprovidedbytheCB.

8 – Revisions to Program

8.1 WhenCSApublishesarevisiontotheProgram,theCBshallnotifyprogramparticipantsofhowtheywillhandletestingandvalidationofProducts.

8.2 TheManufacturershallreviseitsqualitysystemandprocessesforcompliancewithanyrevisiontotheProgram.

8.3 Testingtothepreviousspecificationsormostrecentpriorrevisionwillbeacceptedforaperiodofninety(90)daysafterpublicationoftherevision.AllcertifyingProductsshallbeproducedincompliancewiththelatestrevisionoftheProgramwithinsix(6)monthsofthepublicationoftherevisionunlesstheCBnotifiesparticipantsthatalongerperiodisneededfortesting.TheCBcanwaivere-testingundertherevisedProgramwhenprevioustestresultsadequatelydemonstratecompliancewiththerevisedProgramoriftherevisedProgramestablisheslessstringentcriteria.

9 – Certification Mark

9.1 TheManufacturershallusetheMarkprovidedbytheCBwithitscertifiedproduct(s).SeeAppendixC.

9.2 TheCertificationMarkrepresentsthattheplateManufacturerisproducingaproductthatisafaithfulrepresenta-tionofthetestedproduct.TheCBdoesnotrepresent,warrantorguaranteethatproductsbearingtheProgramMarkdoinfactconformtoCSAG401requirements.

9.3 TheCertificationMarkshallbeinformtothatnotedinAppendixC,ExhibitA.TheMarkshallbelegibleandaffixedtotheproductstructureorindividualplateinamannerthatisresistanttoenvironmentalconditionsfortwoyearsbeyondthepointofitsinstallation.

9.4 TheCertificationMarkmaynotbemodifiedandshallnotbeusedorplacedinsuchamannerastoimplyanyotherendorsementsorcertificationsbytheCB.

9.5 TheplateManufacturershallnotknowinglyreleaseaproductforsalewiththeCertificationMarkaffixedtoanorderthatdoesnotmeettherequirementsofCSAG401-14.IfaManufacturerknowinglyreleasesanorderthatdoesnotcomplywiththerequirementsofCSAG401-14,allpreviouslycertifiedproductsinthenon-compliantfacilitywillbeautomaticallyde-listedpendinginspectionandre-certification.

9.6 PlateManufacturersarepermittedtousetheCertificationMarkintheirpromotionalmaterialsandliteratureonlyafteranappropriateagreementoftheCB.

9.7 Toavoidmisunderstanding,referencestocertificationinManufacturerliteratureshallspecifytheparticularproductsthatarecertified,unlessalloftheproductsmentionedintheliteratureoradvertisingarecertifiedunderthisProgram.

9.8 MarkingontheplateshallincludetheapprovedlogosofthecertificationbodyandofCSPI.

10.1 UseofNon-participatingProducts:Thisprogramisnotintendedinanywaytoinhibitthesale,purchaseoruseofProductsnotapprovedtousetheProgramMark.

Appendix A: Plate Product Manufacturing Process QC/QA Fundamentals

The following information isprovidedasanaid to thePlateProductManufacturers indevelopingQA/QC (QAP)procedurestoobtaincertificationstatusfromanacceptedCertificationBody(CB).Theinformationprovidedshouldnotbeconstruedasbeingallinclusivetomeettheexpectationsofthecertifyingbodyorgroup.Theactualinformationalrequirements,qualitydocumentationneeds,testsandothercertificationaspectsrequiredisdependentontheplateproductbeingmanufactured.AllactualrequirementsshallbedictatedbytheacceptedCertificationBodyundertakingtheauditoftheproductionplantforthepurposeofcertifyingplateproduct(s).Thefollowingisalistofconsiderationareasintheproductionofplateproductsbeginningwiththerawmaterials,throughoutthemanufacturingoperationsandfinallyendingwith thefinishedproductproducedconforming tospecification. 1. RawMaterials 1.1ReceivingConsiderations: Checksand/orteststodetermineiftherawmaterialcompositionisasrequiredtomanufacturethespecified

product.Checksand/orteststoassessmayinclude,butarenotlimitedto: • Platequality(chemistryreport,gaugereport,measurementsverification); • ConformanceofnutsandboltstoqualityanddimensionalrequirementsofG401-14, • Documentmatchingtoproductmarking(milltestcertificates).
