By signing and submitting this Team Beachbody Independent Coach Transfer Form, I am transferring my Team

Beachbody Coach business to the individual/business appointed above and am permanently abandoning my position

within the Team Beachbody genealogy. I understand and agree that by transferring, the following will occur:

• I will lose the right to sell Beachbody products.

• I will no longer be eligible to earn commissions under the Team Beachbody Compensation Plan.

• I will permanently lose my position in the Team Beachbody genealogy.

• I may not re-enroll as a Team Beachbody Coach again until six calendar months from the date of my

transfer.

Beachbody On Demand Members (check one)

Although I am transferring my Coach position and my Team Beachbody business, I still want to remain a member

of Beachbody On Demand. I understand that Team Beachbody will continue to charge my Beachbody On Demand

membership fees to my credit card until I cancel.

Please cancel my Beachbody On Demand membership. I understand that unless I check this box, I will remain an active

Beachbody On Demand member and will be responsible for the Beachbody On Demand membership fees as noted

immediately above.
