SWORN STATEMENT

I, _____________, of legal age and resident of No. 22 Katarungan St. Mabuhay Village, Sta. Mesa, Manila under oath declare that:

1. I am employed at Westpark International School, Manila;2. I am the art teacher/adviser of Scarlett Cruz Batumbakal, the daughter of Mr. and Mrs. Frank Batumbakal;3. Last September 10, 2006, during our art class at about 2 oclock in the afternoon, Ms. Melanie Cruz Batumbakal, Scarletts mother, excused Scarlett from the class for an alleged emergency;4. On that same day, Scarlett did not return from the class and missed her other subjects for that day;5. Since then, Scarlett, without any excuse letter from either of her parents, never attended school;6. Scarletts continuous non-attendance in school resulted in her name being removed from the list of Westpark International Schools scholars.
