WHAT IS HAIR?

The main constituent of hair is the protein keratin. Keratin is a structurally strong and remarkable protein that is resistant to wear and tear. It can be found in feathers, claws, nails and more importantly hair. Similar to other proteins, keratin is comprised of large molecules, consisting of much smaller units called amino acids, which are held together in chains under strong chemical bonds. Hair comprises of 10-13% of water, fats, the pigment melanin, vitamins and traces of zinc and other metals, all of which are essential components. Each strand of hair consists of three vital layers.

1) An innermost layer or medulla which is only present in large thick hairs.

2) The middle layer known as the cortex, which

provides strength, colour and the texture of hair.

3) The outermost layer is known as the cuticle. The cuticle is thin and colourless and serves as a protector of the cortex.

The hair follicle

The Hair follicle is a tiny cup-shaped skin organ deep in the fat of the scalp that produces hair. Attached to the follicle is a sebaceous gland, a tiny sebum-producing gland found everywhere except on the palms, lips and soles of the feet. The follicle is supplied with vital nutrients from the minute blood vessels.

The temperature around the follicle is regulated to normal body temperature and is not affected by external changes. The hair follicle can be divided into two important regions.

a) The hair bulb

The hair bulb lies inside the hair follicle. It is a structure of actively growing cells, which eventually produce the long fine cylinder of a hair. New cells are continuously produced in the lower section of the bulb. As they grow and develop they steadily push the previously formed cells upwards. When the cells reach the upper part of the bulb they begin to arrange themselves into six cylindrical layers, one inside the other. The inner three layers of cells become the actual hair. The outer three layers become the lining of the hair follicle.

At the base of the hair follicle is the dermal papilla which contains receptors for male hormones and is feed by the nutrient rich bloodstream to produce new hair. It is the dermal papilla (DP) which directs and dictates the embryonic generation of a hair follicle and it also retains this instructive ability throughout the life of the hair follicle. The bigger the DP, the more cells it has and the thicker the hair fibre becomes.

Special cells in the hair bulb produce the pigment that colours the hair. The pigment is called melanin, and these cells are known as melanocytes. As the developing hair moves upwards in the follicle the melanin is carried with it.

b) The hair shaft

This is the part of the hair that can be seen above the scalp. It consists mainly of dead cells that have turned into keratins and binding material, together with small amounts of water.

Terminal hairs on the head are lubricated by a natural oil (sebum) produced by the sebaceous glands of the follicles. How much natural oil your glands produce is mostly determined by your genetic inheritance. In many teenagers, a massive surge in hormone levels leads to raised grease production, thus producing greasy hair.

THE PHASES OF THE HAIR GROWTH CYCLE

Each hair follicle undergoes a cycle of activity in three distinct phases. The hair grows to a maximum length, then ceases growth and the hair is shed and replaced. At any particular time we only have around 85% of our hair on our head as the remainder is in the resting stages. These three key stages are:~

1. Anagen: the period of active growth 2. Catagen: the period of breakdown and change 3. Telogen: the resting stage

Anagen Phase – Growth Phase The anagen phase of a new hair starts the moment it begins to grow in the active region of the hair bulb. This usually lasts for some years, generally between three and seven, without interruption. Since human hairs grow approximately 1 centimetre a month, hairs potentially can extend to a length of a metre or so.

Pigment (melanin) is produced in the hair bulb throughout this phase of the hair cycle. Reduced pigment production occurs in the hair of older people. This is why white hairs appear, even though the hair itself may still be growing strongly. With age the hair cycle becomes shorter, the follicles gradually give up producing long, strong hair, and the hairs become thinner and shorter. The result may be a general thinning of the hair, or even a degree of baldness. Catagen Phase – transitional or intermediate phase This marks the end of the active growth period, as changes occur in the follicle. The hair stops growing and becomes detached from the base of the follicle forming a club hair which begins to break down, resulting in the follicle becoming shorter. A small section of the outer root sheath remains in contact with the group of cells that formed the papilla. This period of breakdown or change lasts about three weeks. As the inner root sheath breaks down, the hair remains in the follicle due to its shape. On average, 1% of follicles are held in the catagen stage.

Telogen Phase – resting or the shedding phase The resting phase follows the catagen phase and normally lasts about 5-6 weeks. During this time the hair does not grow but stays attached to the follicle while the dermal papilla stays in a resting phase below. Approximately 10-15 percent of all hairs are in this phase at any one time.

After the telogen stage the cycle returns to anagen and the root germ begins to grow downwards and forms a new bulb around the dermal papilla. It is the lower end of the germ that forms the new bulb, producing a new hair. The upper part of the germ forms the new cells that lengthen the follicle below the club hair. Shedding is part of the normal process of the replacement of old hair with new. At any one time, around

one in ten of the follicles on an individual’s head are in the shedding phase.

What controls hair growth?

General health and nutritional factors are increasingly believed to be important factors for healthy hair growth. Malnutrition as a result of alcoholism, starvation or anaemia has been proven to affect hair growth, leading to loss of hair in many instances. Minerals and vitamins are particularly important for hair growth, for example, a lack of zinc in the diet is known to produce fine, sparse hairs and even loss of hair. Vitamin B, also known as panthenol, plays a vital role in hair growth by improving the physical properties, such as; elasticity, strength and glossy appearance of the hair shaft. It is essential to therefore maintain a healthy diet and insure a decent vitamin intake to maintain a healthy head of hair

HAIR TYPES AND COLOUR

The type of hair you possess is inherited from your parents, more specifically the mixture of specific genes you have inherited from your parents. Scientists have identified three basic types of hair in today’s human population, and have related them to three early races: Asian, Caucasian and African. Asian These are people from the Orient, for example from China and Japan. Their hair is very straight, and always black in colour. Caucasian Caucasian is extremely varied; they range from the fair-skinned people of north-west Europe to the widely different people of the Indian subcontinent. Their hair may be either wavy or straight with great variations in width and usually from black to a pale blond in colour. African African people originated in Africa. Their hair is black and tightly curled. It tends to be woolly and dry, and is extremely easily damaged by heat or chemicals.

Variations in hair structure

Factors affecting variations include:

o Arrangement of the large bundles of keratin within each hair shaft.

o Position of the hair bulb in the hair follicle. o Irregular growth in the hair bulb. o Twisting of the hair shaft. o Shape of the hair follicle.

All hair undergoes twisting as it grows. The greater the number of twists in a given length of hair, the curlier the hair strand will become.

Some African hair has up to 12 times more twists per centimetre as than a Caucasian hair. Caucasian people tend to

have oval shaped shafts and the differing keratin bundles produce combinations of straight and wavy hair. In Asian people the keratin bundles in the hair are all straight and the hair shaft tends to be thick, and almost completely round.

WHAT IS MALE PATTERN BALDNESS?

A progressive, diffuse loss of scalp hair in men, dependant on the presence of the androgenic hormone testosterone, and is caused by a combination of genetic and hormonal factors. Also calledalopecia hereditaria, androgenetic alopecia, male pattern alopecia, patterned alopecia. Male pattern baldness is the common type or cause of hair loss that develops in most men at some stage in their lives. On average it usually takes 15-25 years for the entire cycle to produce total baldness, however, some unfortunate men can be completely and utterly bald in fewer than five years. Male Pattern Baldness (MPB) is believed to be hereditary and hair loss can be aggravated by hormonal imbalances. One aspect that is certainly apparent across all ages, gender and ethnic grouping is that hair loss diminishes confidence and self-esteem in the individual. The Hamilton-Norwood scale is a way of measuring male pattern baldness. It was introduced by Dr. James Hamilton in the 1950s and later revised and updated by Dr. O’Tar Norwood in the 1970s.

The main types of balding based on this scale are anterior (front) and vertex (back). Typically, the hair begins to recede or regress at the front above the temples, together with a noticeable thinning of the hair on top of the head. A bald patch gradually develops in the middle of the scalp. The receding front areas, and the bald patch on the top (the crown) gradually enlarge and converge together. A rim of hair is often left around the back and sides of the scalp. The specific stages of hair loss are illustrated below.

It is important to note that not all cases are identical so an accurate system of measuring baldness is a difficult task. The gradual changes that occur in the scalp skin however, are the same whether describing male or female baldness.

The Norwood scale provides one of the most accurate current classifications. There are seven levels of loss in the Norwood scale:

1) A normal head of hair, with no visible signs of hair loss.

2) The hair is receding at the temples forming a widow’s peak.

3) The temples recede more and the frontal hairline thins.

4) The bald spot appears on the crown and the frontal area

recedes further.

5) The hair in the frontal area and the crow thin out further and

start to join up.

6) The final strip of hair which separates the crown from the

frontal area disappears.

7) The classic “horseshoe” look – hair no longer exists on the

frontal area and the crown and existing hair recedes towards

the neck and the ears.

An alternative……. Norwood’s classification neglected to integrate disparities amongst different ethnic groups, displaying various patterns of hair loss. “Alvi Armani Hair Loss Scale” is a new classification system developed by Dr. Antonio Armani.

Dr. Armani’s 5 different patterns of balding depict global diversity in receding patterns.

Pattern 1: Describes Norwood’s older classification. Hair loss from

the front and back regions eventually converge in the middle.

Pattern 2: Frontal hair loss progresses towards the crown.

Pattern 3: Hair recedes from the crown progressing to the front

hairline.

Pattern 4: (Ludwig’s Pattern) Centrally based balding, progressing

outwards. Common with women, the pattern also targets a vast

majority of men in the Mediterranean and Middle Eastern regions.

Pattern 5: A crescent shape progressing around a small tuft of hair,

commonly found in men of Mediterranean, Middle Eastern, and

Asian regions.

Who is affected by male pattern baldness?

Nearly all men experience some form of baldness by the time they reach their 60s, with well over half the male population experiencing hair loss by the age of 50. The age at which hair loss begins differs from individual to individual. Many men in their twenties or early thirties can develop male pattern baldness and it is not uncommon to discover a 30 year old male with complete baldness. Most affected sufferers desire some form of treatment or solution to their condition. Modern applications can prevent further hair loss often promote new growth. Some women can develop a type of hair loss, mainly at the crown, which differs from male pattern baldness. Loss of hair in women is much more common after the menopause and is measured or assessed using the Ludwig Scale.

Physiological Aspects of Male Pattern Baldness

There are two key areas in understanding the emotional impact of hair loss:

1) The reaction and interaction of people suffering from hair loss. These are often influenced by the negative stereotypes of baldness and the importance we place on appearance.

2) The individual’s own perception of body image and self-esteem. Unfortunately hair loss has historically been regarded by others as a negative trait, with countless unfavourable stereotyped images of bald man or woman being used to illustrate this. Think Doctor Evil!!!

There are numerous explanations for why many people suffer from hair loss during their life time. We have attempted to outline all the current theories pertaining to Androgenetic Alopecia and general balding or loss of hair, from an unbiased prospective.

Heredity Factors

In the 1940s Dr James Hamilton concluded that the presence of the male hormone androgen and a genetic tendency where the factors that caused the development of Androgenetic Alopecia. More precisely, the hair follicles of men who are afflicted with male pattern baldness are genetically predetermined to be more susceptible to the harmful effects of a male hormone called dihydrotestosterone (DHT) which is converted from the enzyme testosterone by the enzymes 5 alpha reductase. It is understood that this hereditary weakness of the hair follicle is passed down genetically through the DNA from generation to generation. Interestingly enough individuals with a deficiency in 5 alpha reductase do not develop male pattern baldness, and the hair growth cycle is unaffected. By studying the tendency or occurrence of baldness in the families of both parents, you will be able to predict

your predisposition for hair loss and be able to fully prepare for the eventuality.

Hormonal Imbalance

Hair follicles in the scalp are stimulated into growth, shedding and resting phases by a multitude of hormones and enzymes. However, the most important hormones in the hair growth cycle are testosterone and a derivative called dihydrotestosterone (DHT). During puberty, testosterone and DHT are responsible for simulating the enlargement and growth of hair follicles on the scalp, underarms, facial areas and pubic area. This stimulation causes the increased hair growth associated with puberty. However, it is believed these same hormones may cause the shrinking of hair follicles later in life. When a hair is shed the DHT present in the surface sebum, will enter the follicle and react on a chemical level. This produces shrinkage of the follicle and narrowing of the hair shaft. With each new growth cycle the subsequent hairs will be smaller, weaker and finer until eventually the hairs become transparent and stop emerging.

Nervous Disorders/ Stress

Stress can be contributed to an array of health problems and afflictions across the globe, including hair loss in men and women. As more individuals report hair loss as a symptom while going through periods of high stress (such as divorce, bereavement, unemployment and even childbirth) more doctors are looking into the medical connection of these two conditions. Losing your hair can be extremely embarrassing. It can make you feel depressed, undesirable and drained of any self-esteem and motivation. Often is the case with hair loss that the individual begins to lack confidence and seeks to hide their problem, even themselves, from others. When an individual experiences intense stress, chemicals in the body will transmit signals to the hair follicles, which cause them to enter a resting phase. During this phase there is no new hair growth. In the following few months hair will be shed normally but new growth will not occur to replace lost hair. This uneven pattern can cause hair to appear thinner and more sporadic. Stress levels can even increase as a result of verbal bullying and hurtful comments, which leave you feeling deflated and susceptible to health issues and disorders, notably hair loss. For many men, baldness can be hugely distressing, and detrimental to their lives, particularly if it is excessive or occurs early in life. Many hair loss treatments do exist however, to combat this problem.

The symptoms can even be compounded if you have a predisposition to male pattern baldness. Around the time of surgical operations when we feel anxious and stressed, our bodies as a threat induced reaction, diverting nutrients to our major organs; the heart, lungs and muscles. This can result in the hair becoming weakened and hair loss occurs. In severe cases, hair follicles can stop producing new hair altogether.

The Ageing Process

Hair loss is a perfectly normal component of the hair growth cycle; it is common to lose between 50 and 150 hairs daily from hair shedding however these hairs are usually replaced by the growth of new hairs. As we age, replacement of hair slows down as the rate at which hair follicles replenish themselves starts to dwindle. Protein is synthesised less in the body since less growth and changes are taking place, this could lead to hair falling out and slow rejuvenation of new hair. It is not uncommon to experience loss of pigmentation from hair as you age. The individual hairs also become thinner and smaller.

NATURAL TREATMENTS

For the majority of men hair loss or gradual baldness is a

natural occurrence and a normal consequence of the ageing

process.

However, baldness can be particularly distressing, especially if

the condition is excessive or occurs early in life. Here are just

a few references to natural aids.

H2O

Water is undeniably the most essential component of all of us.

Drinking at least two litres of water a day as part of a healthy

diet, will flush the build-up of toxins out of the body,

decreasing the risk of health problems and hair issues.

Exercise

Any form of exercise for at least 20 minutes a day increases

heart rate, strengthens body and mind plus is extremely good

for you.

Increasing blood circulation means all your organs are being

fed nutrients on a more efficient level; this includes your scalp

and hair follicles.

Many clubs and societies exist to allow you to get involved and

exercise, alternatively, throw on some trainers and go for a

run, dust off the old bike or simply walk to the shops. The key

is just to get started and begin de-toxing your body.

Saw Palmetto

Is a natural remedy that has been used for centuries to

successfully combat hair loss and baldness. This extremely

potent herb contains powerful agents that decrease DHT in

the body and block 5 alpha-reductase; this in turn blocks the

receptor sites on cell membranes preventing the absorption of

dihydrotestosterone (DHT).

Arginine

This essential amino acid is important for hair growth as the

metabolic precursor for Nitric Oxide, which is a messenger

substance that stimulates hair growth by opening Potassium

channels.

Arginine in large doses (3g-12g) releases Growth Hormones

which may help in hairloss (along with anti-aging and body

development). Since arginine stimulates Nitric Oxide, it also

enhances sexual performance.

Not tested directly for hair loss, but has been shown to

indirectly help other substances against hair loss.

Emu Oil

This superb prized oil helps reduce inflammation on the scalp

and reduce the loss of hair. A small amount added to the

affected areas can ease the inflammation and help maintain

strong and healthy hair.

TRADITIONAL HAIR LOSS METHODS

Wigs

The use of wigs is one of the oldest and best known methods

of disguising hair loss. Historically, their use was to convey

social status/power by the representations of European royalty

and also British judges and lawyers, who continue to use wigs

today.

Hair loss is commonly interpreted as a sign of weakness or

immorality in many cultures around the world, therefore the

use of wigs can instantly disguise ones baldness. In Great

Britain alone, approximately one and a half million wigs are

sold each year, indicating a huge demand for this traditional

method of concealing baldness.

An expensive well fitted wig can almost pass undetected.

Noticeable wigs are usually of poorer construction and not

custom made or colour matched to the individual. As time

passes the wearer’s hair colour can alter, allowing the wig to

become more apparent. This problem is further increased by

the natural bleaching effects of the sun.

The most common form of attachment is to attach a wig to the

scalp with tape or glue. This method is far from bulletproof with

many wigs becoming detached at inconvenient moments

causing huge embarrassment in public. Many wearers are

restricted from engaging in many sports and activities in case

the wig dislodges, which would be humiliating.

Toupees

Toupees are very similar to wigs but generally cover a much

smaller area. The main problems associated are fitting and

cleanliness.

You can actually swim and shower with a wig on, but these

activities will quickly cause permanent damage to the toupee,

as water affects the colour and styling. Soap, sea water and

chlorine in swimming pools will quickly shorten the life span of

a wig or a toupee.

New toupees can be expensive to purchase and to maintain. It

is often advised to have three wigs/toupees; one to wear while

one is being cleaned and a spare for emergencies.

Hair Weaving

A technique used to knot hair to an artificial base which is then

woven into whatever hair remains on the scalp of the wearer.

The frequent adjustment or tightening of the wig can produce

additional permanent hair loss, because as hair grows the

weave will need to be re-tightened about once a month.

After around six months of wearing the wig permanent hair

loss around the attachment point is evident, known as traction

alopecia.

Artificial Hair Implants

Hair implants are synthetic (acrylic) strands of filament with

hooks on the end, which are inserted into anesthetised scalp

tissue one by one with a needle, at a rate up to 500 implants

per hour.

The immediate results are evident, although a sustained

thatch is extremely unlikely.

This procedure is extremely painful and can cause serious

infections and permanent scarring to the scalp.

Around 25 % of all implants are rejected by the scalp tissue so

require replacement. Furthermore, within a year the implants

can often snap near the base leaving tufts which are extremely

unsightly. The broken strands can only be removed surgically

but this inevitably scars the scalp.

Hair Extensions

Hair extensions are woven into you own hair and are often

favoured by women more so than men.

Extensions can produce excellent results but are only really

suitable for people with a great deal of existing hair. Some

retightening on a regular basis is also recommended.

Artificial Hair Concealers

Hair thickeners or concealers are suitable for thinning or fine

hair in alopecia sufferers, a patient with a hair transplant or

men with male pattern baldness. After an application the hair

can be combed or styled in the normal way and the thickener

can be removed by shampooing.

Solid Concealers: These products are coloured powders that

coat and thicken the hair without clogging the pores or

affecting natural hair growth. It allows the skin to breathe freely

and is safe to wear all day long.

Spray Thickeners: Effective and advanced aerosolized

polymer for thickening hair, which covers bald spots or

thinning hair. These products are sprayed onto the hair, but do

require practice to get the desired result.

A large range of colours mean almost everyone would be able

to find a match. Hair thickener are not suitable for large areas

of baldness and certainly should not be applied directly to the

scalp, they should be sprayed onto existing hairs to be

effective.

Particle Application: A method to eliminate the appearance

of baldness and thinning hair by sprinkling thousands of tiny

colour matched hair fibers on to the scalp. The fibres bond

with the existing hair by static electricity and will remain in

place even in strong winds or torrential rain.
