Even with the number of companies we accredit increasing, we’re proud to say that the customer service we devote to each and every one of our customers has remained steadfast with our growth. Cus-tomer service is one of our core building blocks and helping our customers succeed is the reason that we’re here. For our 2007-2008 Fiscal Year, we scored a 93% in customer satisfaction. These ratings were based on staff knowledge, meeting time frames, communication and ease of understanding the process, along with other factors. We’re not here to judge you on what you’ve done wrong. We’re here to concentrate on what you’re doing right. That’s why more and more companies keep choosing us for their accreditation needs.

The following chart demonstrates the number of accredited organizations with branch offices that we have across the country. The red pins represent providers accredited by ACHC.

Even with ACHC's continued growth over the past several years, our high level of standards, excellent customer service and values have never been compromised. These are just a few of the reasons that we continue to see such growth. Our customer satisfaction rates and testimoni-als say it all. The next chart demonstrates the amount of growth that we have

experienced over the past several years and this growth is significant!

ACHC has always strived to go above and beyond the boundaries by pushing ourselves to new levels. After all- who accredits the accreditor? What makes us different? For starters, we were the first health care accrediting body in the world to be certified to ISO 9001:2000 stan-dards. And just recently, we learned that we received the North Carolina Awards for Excellence (NCAfE) Commitment (Level 2) Award. This milestone marks ACHC's first step toward applying for the Malcolm Bal-drige National Quality Award. The Baldrige award is the nation’s highest recognition for innovation and performance excel-lence. Receiving this award is one of our “BHAGs”, or in other words, one of our Big Hairy Audacious Goals!

Without our dedicated customers we wouldn't have reached the high level of service and achievement that we have today. We strive for constant improvement, outstanding customer service and the relevant, realistic standards that provider's ask for.

ACHC, like many companies, strives to make quality an integral aspect of its processes and service. To achieve this, it relies on its expert and committed staff. Equally important, ACHC has taken several major steps to reach a higher level of performance. The road to improvement hasn’t always been easy, and this article focuses on some of its experiences along the way.

One milestone for ACHC was its decision in 2003 to develop a formal quality management system using ISO (International Organization for Standardization) standards. Implementing ISO presented a definite advantage because it emphasized items like long-term planning, commitment to provide resources, control of documents, records and processes. Although ACHC had less than ten employees at the time, they learned the standards, worked feverishly with a consultant and successfully passed the first site evaluation in 2004. Every year thereafter, the site visit has taken place and a certificate continues to hang proudly in the lobby.

While ISO helped ACHC develop important systems and practices; its limitations were evident. In 2008 ACHC’s executives received training on the nationally recognized Baldrige Criteria for Performance Excellence, and soon recognized that the Baldrige principles address many important areas that ISO does not. As a result, ACHC decided to implement the Baldrige principles into its operations with the goal to eventually apply for the Malcolm Baldrige National Quality Award.

Because the majority of staff were unfamiliar with Baldrige, it was difficult to get the proverbial ball rolling. Fortunately, the State of North Carolina administers a performance excellence program (NCAfE) based on the Baldrige Criteria, and NCAfE

provides education for applicants upon request. The program divides the criteria into several distinct levels and, at any time, companies can subsequently apply at the national level. This approach was structured and manageable for ACHC.

Once the decision was made to apply through the state, the task of developing the application package became a reality and sometimes seemed overwhelming. Thankfully, an ACHC employee found a book that explained the criteria that helped the staff begin to write the required sections. Some folks used their skills to develop charts and graphs while others participated in teams to

initiate related projects.

Assistance came from outside the office as well. Several board members with Baldrige expertise provided feedback and coached employees to have faith that they could get the job done. The decision to utilize a technical editor to format and streamline the long document also saved an enormous amount of time and frustration. Finally, the folks at NCAfE patiently answered dozens of questions and offered their encouragement.

To set the stage for the required site inspection by Baldrige examiners, ACHC management emphasized that this would be an important educational opportunity where all staff would

participate and gain from the exercise. The site visit was, as predicted, an excellent learning experience and a structured educational session was facilitated by the lead examiner on the last day. Overall, the dialog and post-visit report generated a lot of food for thought and will undoubtedly fuel changes in many areas.

ACHC’s experience with ISO and now Baldrige has been invaluable in its path toward continual improvement. No doubt there will be more challenges for ACHC as the organization continues to learn and grow, but for now it’s definitely on the road toward excellence!

principles of the various reform proposals and thinking about how they will affect home health care in America.

By the time you read this, we still won’t know exactly what the health care reform legislation will look like. Here are a couple of things you can count on:

1. There will be some kind of health care reform legislation.Given all of the pressure by the President and Congress to reform health care, there WILL be some type of legislation. Exactly what remains of the Bill that is currently being discussed is unclear, but there are lots of hidden details that we need to uncover and try to understand.

2. The focus will be on providing coverage for the uninsured.While it is being called “health care reform,” it is really “health insurance reform,” and the focus is on providing coverage for the uninsured and under insured. Most people that I talk with agree that this is a good thing. The questions are: “how to do it” and “how to pay for it.”

3. Money will come out of your pocket to pay for reform.As an executive of a home health agency, you can be sure that you will help pay for this reform in some way. First, you’ll pay through your personal taxes or through increased cost of goods and services produced by those who will pay more taxes. Second, you’ll pay through changes in reimbursement from Medicare and Medicaid that will be diverted from providing home health and hospice to other purposes. The government is already talking about the big savings they plan to wring out of Medicare.

4. Government regulation on home health care will increase as a result of reform.In home health care, we already spend a great deal of time, talent and treasure dealing with government regulation in order to participate in the Medicare and Medicaid programs. Based on the provisions of the current bill that I have read and heard discussed, we can expect more regulation of home health services and other health care.

5. You can and will survive these changes if you get ready now.Over the past 27 years of working in home health care, I’ve observed that we are a very resilient industry. We’ve survived many ups and downs and cross ways crises. We’ll survive this one too. That doesn’t mean it will be easy and some individual agencies will go out of business as a result of the changes.

The agencies that survive and prosper as a result of this reform process will be those that are getting ready now. As you go through your process of being accredited and renewing your accreditation, pay particular attention to those areas that will help your agency learn, grow and adapt to change. You have a great resource at ACHC that can help you adapt to the changes brought about by health care reform. Your accreditation process will be very helpful in preparing your agency for whatever will come.

As executives of highly effective home care companies, there are several things you can do right now to get your organizations ready for the future: 1. Continue to study the information coming out of

Washington .2. Stay involved - or get involved - in your state and national

home care associations.3. Keep your executive team involved in discussing the

issues.4. Develop a clear vision for your agency - a clear mental

picture of your desired future. 5. Focus on growth - now is the time to gain market share so

you're positioned for the fallout that may come. 6. Train your sales and marketing team to bring in more

referrals that turn into admissions.7. Develop your middle managers so they have the

knowledge and skills to adapt to change. 8. Communicate with all of your employees about how

changes will affect them personally.9. Be an encourager - your people need you to lead them from

a position of strength.10. Continue to measure results and give feedback to your

management team.

If you consider these five elements of health care reform that are certain and if you pay attention to the ten tips listed above, I predict that you will be well prepared to adapt to whatever change comes your way as a result of health care reform. Best wishes, and enjoy the journey!

Natalie Carter Sharpe RN, MSN Home Health, Hospice and Private Duty SurveyorNatalie has been a registered nurse for nearly 20 years with 15 of those being dedicated to home care and hospice. She currently serves as the Vice President of Clinical Services for

Community Home Care & Hospice. Natalie is responsible for over-seeing the clinical functions for Community’s 1000 plus patients in locations across North and South Carolina. She plays a major role in policy development, implementation and clinical systems review. Her past experiences in home care include positions as Director of Quality Improvement, Staff Development Coordinator,

Director of Admissions and Referrals as well as regional and local management positions. She has been surveying for ACHC for the past three years.

Natalie received her Masters in Nursing Science from the Universi-ty of Phoenix, Bachelor's in Nursing Science from Fayetteville State University and an Associates Degree in Nursing from the College of the Albemarle. She has a passion for learning and believes that the application of knowledge is the key to success.

Natalie is a devoted wife, mother and grandmother. She lives in Fayetteville, North Carolina with her husband Victor and is very active in her church and community. She is a member of Sigma Theta Tau Honor Society for Nursing and Alpha Kappa Alpha Soror-ity, Incorporated.

Ursula B. Robinson MSW, LCSW, MHA Hospice SurveyorUrsula is the Vice President of Clinical Servic-es at Hospice and Palliative Care of Greens-boro (HPCG) in North Carolina, a position held since 1998. She began her career at HPCG in 1991 as a Medical Social Worker, providing

direct patient care. During her tenure with the organization, the agency has grown tremendously, providing an array of hospice and palliative care services to more than a thousand patients per year. In the role of Vice President, Ursula has been responsible for the clinical operations of the agency, including adherence to federal, state and local laws, regulations and clinical policies. She has overseen the care to adults and children in their private homes, nursing homes, hospitals, Beacon Place (HPCG’s inpatient/resi-dential facility) and community bereavement program.

Ursula worked previously for the Guilford County Department of Social Services and a Long Term Care Facility. She received her

Bachelor of Science degree from the University of North Carolina at Greensboro, Master of Social Work from the University of North Carolina at Chapel Hill, and Master of Health Administration from Pfeiffer University. She is a licensed clinical social worker. In addi-tion to her work at HPCG, Ursula is a Management Development Trainer for the National Hospice and Palliative Care Organization (NHPCO), and faculty member of The Duke Institute on Care at End of Life-APPEAL project. She has made professional presentations on a variety of topics relating to end-of-life care, leadership and management.

Ursula is a Board of Director for the Guilford County Department of Social Services. She is a member of the National Association of Social Workers (NASW) and American College of Health Care Executives (ACHE) and has served on numerous committees, including: Steering Committees for the Veteran Affairs, Long Term Care Committee for the Carolina’s Center on Hospice and End of Life Care and the Guilford County Commission on Aging.

Cindi Hutchinson DME SurveyorCindi has been working in the DME industry since 1986. She attended Western Michigan University and Kalamazoo Valley Community College in Kalamazoo, MI and is a Regis-tered Respiratory Therapist. After leaving the hospital setting as the Director of Cardiopul-monary Services, she started working as a

Respiratory Therapist for a pharmacy-based DME company located in Goshen, IN. As the Director of Clinical Services, she assisted them in obtaining their first accreditation in 1994. In 2001 she assisted her first company in obtaining accreditation with ACHC.

She was so impressed with the ACHC process and their standards, she pursued being a surveyor with ACHC the following year and has been surveying with ACHC since October of 2002. These past years have been a true challenge for everyone in the industry, but almost every company she surveyed has agreed that accreditation has been a positive experience for them and has made them a stronger company.

Cindi currently resides in Elkhart, IN with her husband. She has been busy this past year planning her daughter's wedding, which happened this past June. She has one son in his last year of col-lege. She and her husband enjoy spending time at the lake and playing golf when the time allows.

A Note From Wisdom Health Care Services:Accreditation with ACHC was an exciting experience. The process began with the interpretive guide from ACHC and completing the Preliminary Evidence Report (PER), which is a compilation of ACHC standards and our policies and procedures. For us, the key to passing the survey was the fact that we studied our PER and submitted the appropriate information requested. Familiarizing ourselves with our PER made it possible for our agency staff to know our policies and procedures very well and we were able to answer all the questions during the survey. Our account manager was excellent and was always there to answer our questions. Both our account manager and surveyor made us feel at home during the process. They were very professional and knowledgeable. The atmosphere was not threatening and that helped the staff stay focused. I recommend ACHC to any agency that would like to go through accreditation.

Wisdom Health Care Services

The Lesson Learned:Wisdom Health Care Services applied with ACHC as a new Home Health Agency. They were able to use the interpretive guide, along with ACHC standards, to help write their policies and procedures. This is a method used by several “start up” companies to ensure that they are beginning with a

solid foundation built on the successful principles of ACHC standards. Agatha, Administrator at Wisdom Health Care Services, used the stan-dards as a guideline for writing her policies and procedures, which also helped her to organize her PER.

The purpose of the PER is to ensure that there are policies and procedures or other evidence for each ACHC standard.

With all the consultants available to providers, obtaining policies and procedures or writing their own is only the first step. In addition to hav-ing quality policies and procedures that meet ACHC standards, the providers must also be accomplishing two things: (1) educating the staff and (2) implementing the policies and procedures. Agatha was able to do both, which resulted in a successful accreditation survey with ACHC.

The accreditation survey verifies the provider is meeting the standards through written policies and procedures, interviewing

staff and implementation of policies and procedures.

Wisdom Health Care Services used ACHC standards as a guideline when writing their policies and procedures. They were certain to educate the staff on their policies and procedures, as well as the ACHC standards so that each employee was informed and ready to answer the surveyor’s interview questions. In addition to being informed, the staff was also implementing the policies and procedures, providing quality service to their patients.

The accreditation surveys for sleep labs are scheduled and announced in order to allow observation of an initial patient set-up and are conducted by a sleep specialist.

Since the program was announced earlier this year, ACHC has conducted several beta tests with sleep labs across the country to ensure the standards and the accreditation process would be conducted at the highest level of quality that ACHC accredited providers have come to expect. The initial sleep labs that went through the program found the process to be very user-friendly, with standards that were easy to understand and also promoted better patient care. The labs were very appreciative of ACHC’s focus to complete the accreditation process in a timely fashion.

“The ACHC Sleep Lab Program is an innovative approach to accreditation, focused on patient care and outcomes of the sleep testing process," said Tim Safley, HME and Sleep Manager. "The program began by bringing sleep professionals together to develop an accreditation process that is focused on the patient”.

ACHC’s mission is to help their customers succeed and ultimately improve the quality of care being provided for the patients. ACHC consistently aims to improve its programs and services for

providers through innovation and concern for the entire health care continuum. Due to the growing need of sleep lab regulation and accreditation, a sleep lab program was the next progression for ACHC.

The sleep lab accreditation process includes the Preliminary Evidence Report (PER), which is a compilation of the company’s policies and procedures matched to ACHC’s sleep lab accreditation standards. The company submits their PER, application and deposit, then the survey is arranged in order to allow for actual patient observation. Prior to the survey, the PER Desk Review is sent to the provider so any corrections to policies may be made ahead of time. This ensures the company is not

only compliant, but is also providing excellent patient care.

ACHC is currently accepting applications for Sleep Lab accreditation. The application and standards are available at www.achc.org.

Wal-Mart Chooses ACHC(Continued from Page 1 )

The company’s corporate offices were visited by a team of ACHC surveyors in May and successfully

met all accreditation requirements. In the months ahead there will be unannounced visits to hundreds of retail units around the country to validate system wide compliance with standards.

Tim Safley, HME and Sleep Manager, commented, “The Wal-Mart accreditation team has been serious and energetic to comply with the Medicare Quality Standards and ACHC requirements throughout their preparation phase. For such a large company, they have been impressive in their deployment of information to their workforce”.

ACHC is an independent, private, non-profit 501(c)(3) corporation established in 1986. The demand for ACHC to expand into other health care accrediting arenas has led to the development of standards for home based and alternative site health care services. The core programs include, but are not limited to, home health, pharmacy based programs, home medical equipment and hospice.

Brittany Bouwer, Accreditation SupervisorBrittany began her career with ACHC in 2008 as an account manager, and recently was promoted to Accreditation Supervisor. She oversees the accreditation department and all aspects of the accreditation process. Working together with the account managers, Brittany’s

mission is to provide excellent customer service and exceed expectations throughout the entire process.

“I love coming to work each day knowing that what we do at ACHC has a positive impact on people’s lives," Brittany says. "It is my job to help facilitate the accreditation process for providers, but it ultimately is for the patient”.

Brittany is a graduate of East Carolina University where she received her Bachelor’s Degree in Business Management. Outside of the office, Brittany enjoys boating and spending time with her husband and three dogs.

Aimee Brothers, Account ManagerAimee joined ACHC as an Account Manager in November 2007. Prior to joining the ACHC staff, she served in various positions at the Association for Home & Hospice Care of NC, Inc. She has a passion for working with non-profit health care organizations and strives to uphold the goal of continued excellence in customer service. As the new Sleep Account

Manager, she hopes that her contributions will aid in raising the standards in another field of health care.

Aimee lives in Knightdale, NC with her husband Jason. They have two beautiful daughters, Haley and Lauren. They enjoy outdoor activities such as camping and spending time at the beach.

Kimberly Martin, Account ManagerKimberly joined ACHC as an Account Manager in 2007 working with Home Health and DMEPOS providers to improve patient care and customer satisfaction. In addition to being an account manager, she is now responsible for scheduling all the accreditation surveys for DMEPOS customers. She has successfully

scheduled over 1,200 surveys in just three months, before the CMS deadline in September 2009.

Prior to her employment at ACHC, Kimberly received her Bachelor’s Degree in Visual Communications from Mount Olive College. Her hobbies include spending time with her family, daydreaming of the beach and reading suspense novels.
